[Folic acid levels in patients with compensated renal failure and during long-term haemodialysis (author's transl)].

Folic acid concentration in plasma and erythrocytes and vitamin B12 concentration in plasma were determined by radioassay in 56 patients with chronic renal failure. Compensated retention was present in 31, while 25 were haemodialysed at regular intervals, 14 at home, and 11 in hospital. One patient had megaloblastic anaemia. Mean folic acid concentration in plasma for the whole group was 4.60 microgram/l (normal 8.30 +/- 2.57 microgram/l), in RBC 385 microgram/l (normal 459 +/- 143 microgram/l. Vitamin B12 levels were normal. These was no significant difference in folic acid levels betwwen patients on home or on hospital dialysis. During dialysis there was a fall in folic acid concentration in plasma which was proportional to the intial levels. After a four-week replacement pause folic acid concentration in RBC had not decreased in those patients on long-term dialysis who had previously received folic acid. The decreased folic-acid concentration in plasma of patients in compensated chronic renal failure is not due to the renal failure. Folic acid concentration in RBC is decisive in the assessment of folic acid deficiency.

Non-uniformity of regional vasomotor activity indicating the existence of 2 different systems in the sympathetic cardiovascular outflow.

In rabbits, 2 populations of sympathetic postganglionic fibres innervating the skin, heart, muscle and kidney could be classified by their different spike heights and their different susceptibility to noradrenaline and angiotensin amide. The ability of the 2 populations to respond to physiological stimuli in a highly differentiated manner leads to the assumption that 2 different systems in the cardiovascular sympathetic outflow exist.

[Partial replacement of the heart].

A ventricular-aortic blood pump was constructed for the functional replacement of the left heart. Hemodynamic experiments showed that the device was able to pump the total cardiac output in calves with a body weight under 85 kg. In these cases the myocardial oxygen consumption decreased to 40%. Laboratory findings in 5 longterm experiments revealed a complete adaptation of the body to the artificial organ up to 73 days.

The correlation between microscopical examination and erythrocyte band 3 (AE1) gene deletion in South-east Asian ovalocytosis.

South-east Asian ovalocytosis status was determined by microscopical examination of peripheral blood samples collected from 137 individuals in Papua New Guinea. The examination was performed separately by 2 microscopists, one of whom was very experienced in examining peripheral blood films for the diagnosis of south-east Asian ovalycytosis and the other was recently trained. The samples were also analysed by polymerase chain reaction (PCR) to determine ovalocytosis status by demonstrating a 27 base pair deletion in erythrocyte band 3 protein of the affected individuals. The microscopists were unaware of each other's results and of those obtained by PCR. Generally, there was very good agreement between the results obtained by both microscopists and the PCR. Although there was considerable inter-observer variation in the final ovalocyte count between the 2 microscopists, this did not affect their ability to discriminate between ovalocytic and normocytic individuals. Taking the PCR results as the standard, for the first, more experienced observer, the most efficient ovalocyte count cut-off point was around 50%. At this ovalocyte count the sensitivity and specificity of microscopical examination were 93.6% and 92.2%, and the positive and negative predictive values 86.3% and 96.5%, respectively. The second microscopist generally underscored the ovalocyte counts and his most efficient cut-off point was 20%, with sensitivity and specificity of 85.1% and 93.3% and positive and negative predictive values of 87.0% and 92.3%, respectively.
