We were able to re-examine already known facts in tests of pressure areas in seats by using a new analytic system. It was shown that only in backrest seat angles of 90 and 110 degrees pressure areas below the ischial tuberosities were clearly defined. The transition area of zones of high to medium pressure was sharply defined with these angles. In the anterior position and with large backrest seat angles high pressures were not found and the transition areas between zones of medium and low pressure became fluent. Only the pressure distribution pattern in the 90 and 110 degrees position is physiologic and corresponds to the pressure tolerance of different parts of the human body in seats. This pattern has to be maintained principally in a contoured and upholstered seat.
Posttraumatic bone infection very often signifies a life threatening complication. However, it must no longer be accepted that amputation of the afflicted limb be the logical treatment of such a complication. A surgeon who offers the advantages of operative treatment of a fracture to his patient must also be able to control its complications. Osteitis can be treated with a good chance of success. In our clinic 29 bone infections were treated, 13 of them healing without substantial restrictions, 7 with distinct discomfort and 4 with arthrodesis, i.e. 83% healed. However, the treatment may require months and may necessitate further operative interventions. The transfer of such a patient from an acute care hospital to a high altitude clinic offers -- besides the medical and psychological advantage of a continuous long-term treatment plan -- the climatic conditions of sun and high altitude air which apparently act very favorably on the healing process of chronic infections. Of course, the possibility of competent surgical interventions must exist. Thus, with proper therapy the disastrous chain of errors beginning mainly with poor initial care through insufficient treatment can be broken.

A statistical collection of squash injuries in 8000 players from 30 squash centers in the Federal Republic of Germany over a 12 months period is presented and evaluated. The comparison with other types of sport shows squash to be sooner likely to cause injuries than tennis, but to entail essentially less injuries than e.g. football or handball. With a view to prevention, certain demands must be made on the player as regards his manner of playing and equipment, but also as regards the ground conditions of squash halls.
The problems of the treatment of femoral neck fractures in young patients are discussed on 22 cases as well as the operative technique, post-operative treatment, late complications and their treatment. We feel that femoral neck fractures in children and young ones should be treated operatively. The three flanged nail should be used for internal fixation, giving a better fixation than K-wires. We have not seen any damage caused by the crossing of the epiphyseal plate with a nail or K-wires. Postoperatively the patient should walk non weight bearing on the fractured hip for 9 months, especially in transcervical fractures. The danger of aseptic necrosis of the femoral head is clearly diminished by this precaution. The late results of femoral neck fractures in juveniles are clearly worse than in children. The femoral neck fracture of the young is to be treated as an emergency. If the therapy is too late or not adequate the consequences for the hip joints of the juveniles can be drastic.
The problems of femoral neck fractures in young children are discussed. Results of 12 own cases are reviewed with an average age of the patients of 7.5 years at the time of accident. The average time between accident and last examination was 12 years. The operative technique, pre- and postoperative care, indications for conservative treatment and late complications are discussed. All types of fractures should be treated operatively. K-wire fixation is sufficient in most cases and does not damage the epiphyseal plate. Immobilization in POP spika is advisable for 6 weeks. With delayed operative treatment of intracapsulare fractures the intraarticular haematoma should be aspirated. Traumatic epiphyseolysis should be reduced by manipulation or operatively and stabilized by K-wires. The treatment of pseudarthrosis is by internal fixation and bone grafting. Late valgus and especially varus deformations should be corrected by intertrochanteric osteotomy.
The sitting posture of persons with arthrodesis of the hip joint and with lumbosacral fusion are analyzed. It is shown that any disorder in the hip joint - pelvis - lumbar spine system results in a change of posture. In particular, a defective hip joint produces an unfavorable posture by folding over the lumbar spine. The effect of fusion of the lumbosacral area on sitting posture is less marked.
Follow up studies of total hip replacement using polyester as a component of the alloplastic joint have shown that this material causes soft tissue reaction and osteolysis. The problem is demonstrated in this paper.
High-performance liquid chromatography of collagen degradation products in human urine for diagnosis of osteoarthrosis was introduced by Macek and Adam (6) in 1987. The aim of the present study was to demonstrate the clinical usefulness of this method in a group of 20 patients with osteoarthrosis of the hip and 10 healthy volunteers. There were no significant differences in number or intensity of the detected signals between the two groups. The results of this study show that the determination of collagen degradation products is not useful for diagnosis or for the control of therapy in patients with osteoarthrosis.
