Antimicrobial susceptibility of anaerobic and capnophilic bacteria isolated from odontogenic abscesses and rapidly progressive periodontitis.

In dentistry antimicrobials are used in the treatment of progressive periodontitis and odontogenic abscesses, therefore the susceptibility to commonly used antibiotics of capnophilic and anaerobic species causing these diseases should be investigated. The activity of penicillin, amoxycillin, cefoxitin, clindamycin, doxycycline, metronidazole and ciprofloxacin was investigated. One hundred and sixty four isolates from subgingival plaque samples of 66 patients with progressive periodontitis and 192 bacterial strains from pus of 74 patients with odontogenic abscesses were included in this study. The majority of species tested were gram-negative anaerobes (Prevotella spp., Porphyromonas spp., Fusobacterium spp.), and were highly susceptible to clindamycin and metronidazole. Nearly 6% of the periodontal isolates and 22% of the bacteria obtained from pus samples produced beta-lactamases. With the exception of the periodontopathogenic species Actinobacillus actinomycetemcomitans and Eikenella corrodens, clindamycin seemed to be a useful antibiotic and could be recommended for empirical antimicrobial treatment.

Influence of hormonal contraceptives on microbial flora of gingival sulcus.

To determine a possible influence of two different hormonal contraceptives on bacterial microflora of gingival sulcus, subgingival plaque samples of 29 healthy women aged between 20 and 32 years were investigated bacteriologically before subjects took a contraceptive and 10 and 20 days after subjects started the medication. In 14 women, and oral contraceptive containing 0.02 mg ethinyl estradiol and 0.15 mg desogestrel (preparation A) was used, and 15 women took a contraceptive containing 0.03 mg ethinyl estradiol and 2.00 mg dienogest (preparation B) daily over 21 days. There were no changes in clinical parameters of the teeth investigated during 3 weeks of the study. The periodontopathogenic bacteria Porphyromonas gingivalis and Actinobacillus actinomycetemcomitans were never detected throughout the study. On the other hand, the periodontopathogenic species Prevotella intermedia was found in plaque samples of 22 women. The content of this microorganism showed only a little change between the pretreatment period and plaque sampling after 10 days of contraceptive treatment, but a striking increase occurred after 20 days of contraceptive treatment, especially in the preparation A group. In this respect, there was a significant difference between preparations A and B.

[Individual plaque flora determination in periodontal diseases. Methods and standardization].

The dental plaque of 15 patients with gingivitis and of 15 patients with marginal periodontitis was won by means of two different methods of isolation and sampling. Either the plaque was sampled by a scaler with and without covering the crown. The estimation of plaque-microflora was done by microscopical and cultural bacteriological methods. Differences in the composition of plaque-microflora could be found in dependence of method used in isolation and sampling. Also differences in bacterial colonisation of different regions were found using the same technique of sampling. The subgingival plaque in marginal periodontitis was different of the plaque in gingivitis. In plaque-microflora of periodontitis the percentage of gram-positive cocci and rods was decreasing, the percentage of gram-negative and mobile bacteria was increasing compared with the bacterial flora in gingivitis. In the subgingival plaque in gingivitis streptococci and actinomyces were the predominant bacteria.

[Microbiological parameters in periodontitis marginalis during local treatment].

Repeated professional removal of plaque and tooth-stone by means of deep scaling and root planning is able to avoid a further progression of marginal periodontitis. This therapeutical measures lead to changes in the bacterial composition of subgingival plaque. Important bacteria for the etiology of periodontitis are reduced. 8-12 weeks after therapy the recolonization of the complex periodontitis flora is finished. Therefore a repeated subgingival scaling is necessary to make the treatment successful.

[The evaluation of the adhesive properties of dental materials by using radioactive-labelled bacteria].

Dental materials with different surfaces were incubated with 111In-oxine-labelled bacteria Streptococcus mutans 20 min respectively 2 h, 6 h and 24 h. To determine the amount of the bacteria accumulated on the specimens we measured the radioactivity with a scintillation counter. The same measuring was done after decontamination by the help of ultrasonication. The comparison of activities shows that different dental material surfaces adhere different amounts of bacteria. The adhesive properties are material and time dependent, but the surface processing has a lower influence on the bacterial adherence. The lowest degree of bacterial adherence has the cobalt base alloy Gisadent KCM 83. On the other hand the highest remaining activity was found after decontamination of cobalt base alloy and PMMA resin Kallocryl A.

[Microbiological and morphological findings in reactive lymphadenopathy].

An attempt was made by means of microbiological and histomorphological methods for diagnostic elucidation of reactive lymphadenopathies in 52 patients. The serological findings differed from those recorded from clinically intact individuals, in that they included not only increased prevalence of antibodies but also increased antibody titres to cytomegalovirus, influenza virus, and toxoplasma gondii. The dynamic of antibody titres against antigens specific of herpesvirus, influenza virus, and toxoplasma gondii were followed up and enabled positive detection in 11 of 18 patients of active infections with the above pathogens involved. Histologically, the lymph nodes affected were characterised by alterations reminiscent to Piringer lymphadenitis. While, in the course of bacteriological investigations, various bacterial species were isolated from lymph node tissue of 35 patients, direct morphological identification from lymph node sections failed to work. Immature sinus histiocytosis and an increase in phagocytosis activity were among the typical tissue findings.

[Wet root canal instrumentation and microbiologic monitoring of treatment results].

The time and frequency of wet and dry root canal instrumentation of teeth with apical periodontitis were compared. The importance of negative microbiological culture for the success rate and for the number of treatment appointments was determined. According to the microbiological and clinical results wet instrumentation is the more effective treatment. Microbiological testing is not requested for routine use.

[Nuclear magnetic resonance tomographic appearance of disseminated encephalomyelitis in relation to clinical diagnosis].

The MRI findings in 149 patients with a clinical diagnosis of disseminated encephalomyelitis (D.E.) were related to the diagnostic criteria of McAlpine (definite, probable, possible). The most common pattern in all three groups was a mixed peri- and para-ventricular distribution of foci. The findings in 'possible' D.E., compared with 'definite' D.E., showed fewer confluent lesions, but were characterised by peri- and para-ventricular as well as solitary foci. The results of the present study permit better classification and evaluation of the MRI findings if the clinical diagnosis of D.E. is only suspected (probable or possible).
