[Value of the vectorcardiogram in the evaluation of right heart hypertrophy in chronic obstructive lung diseases].

In 41 patients with severe chronic obstructive lung disease vector cardiographic and electrocardiographic results and the level of pulmonary arterial pressure (pap) are compared. We observed a positive correlation between normal results from vector cardiography, electrocardiography, and pulmonary arterial pressure. In cases of hypertrophy of the right heart, type I and type II according the method of Lichtlen, pulmonary arterial hypertension was found in 72% of the cases.

[Calculation of normal values in lung compliance].

Within a study of standard values, investigations of the static and dynamic lung compliance were performed in 162 test persons in a good state of health of heart and lungs. Age, size, constitution, and intrathoracical gas volume are necessary parameters for calculation of an individual standard value of the dynamic compliance, whereas age and intrathoracical gas volume only are needed for calculation of a standard value of the static compliance. The new standard values are compared with the values known from the literature.

[Obstruction parameters and pulmonary artery pressure in asthmatics after administration of aminophylline].

In 30 asthmatics after the application of aminophylline the measurement of the pressure of the pulmonary arteries as well as of the parameters of the respiratory function FEV1.0 and Rt was performed. It is proved a significant decrease of pressure in the lesser circulatory system. The reduction of the bronchial obstruction cannot be proved by means of the respiratory impulse test (FEV1.0), but by the determination of the resistance to respiration (Rt).

[Pituitary gland extract insufflation lung].

In two patients with diabetes insipidus the development of an allergic alveolitis with transition into pulmonary fibrosis could be observed. The alveolitis is evoked by snuffing of posterior pituitary extracts. The pathomechanism of this side effect (disease) is similar to those of other allergic alveolitides (e.g. farmer's lung, bird-fancier's lung, malt-worker's lung etc). This disease can be avoided by changing the therapy of diabetes insipidus to fully synthetic preparations free of protein. When irreversible changes are already present at the stroma of the lung (X-ray picture, function of the lung, histology), then the therapeutic possibilities are restricted and the prognosis is unfavourable.

[Alveolitis--its effect on lung circulation].

29 patients with exogenous allergic alveolitis and 57 patients with chronic fibrosing alveolitis were examined. Lung function and blood gas analytical parameters and the pressure of the pulmonary artery in rest and during load were investigated in all persons. The measured results of the two groups of patients were compared. In all patients a restrictive disturbance of ventilation and a hypoxemia were established. 34 patients showed an increased pressure of the pulmonary artery in rest, in 22 patients an elevated pressure in the pulmonary circulation on loading conditions was measured. By reason of different loading pressures all 85 patients were divided into 3 groups, where their function patterns were discussed. The special importance of evidence of loading investigations in pressure measurements in the pulmonary circulation is discussed.

[Measurement of blood pressure in the pulmonary artery during an attack of asthma].

22 patients with asthma attacks were examined by microcatheter and the pressure in the pulmonary artery (PAP) was measured. The bronchial obstruction is characterized by decrease of FEV 1.0 (average mean 44% of the vital capacity). The average pressures in the pulmonary artery change from 12 to 32 Torr (1.5 kPa to 4.2 kPa) during asthma attacks. Average of the PAP mean 20 Torr (2.7 kPa). The examination was completed by application of 0.24 g aminophylline via the catheter. Decrease of the PAP of 4.1 Torr (0.5 kPa) led also to a decrease of wheezing. In 9 patients an additional decrease of the PAP was effected by the application of 0.8 mg nitroglycerin.

[Contribution to the assessment of physical fitness in patients with pulmonary diseases on the basis of cardiopulmonary and ergometric parameters (author's transl)].

In 270 patients with COLD, bronchial asthma, exogenic allergic alveolitis or fibrosis of the lung measurements of VC, FEV 1,0, Rt, RV, DL CO, CL, PO2 and PAP (by means of floating catheter) were made. Results of measurements at rest and during exercise were compared after statistical treatment in the above groups and their value with regard to the determination of physical fitness was appreciated.

Continuous ambulatory peritoneal dialysis and pulmonary function.

Ten end-stage renal disease patients treated by continuous peritoneal dialysis were investigated by means of spirography, compliance tests, body plethysmography, and blood oxygen analysis. The ventilatory function was compared with abdomen empty and abdomen filled with 2L of peritoneal dialysis solution. Only a mild ventilatory restriction occurred (vital capacity -3.5%, thoracic gas volume -7.9%) as well as a mild decrease of static absolute compliance (-8.8%) and of dynamic absolute compliance (-6.2%). There was no significant alteration of the specific compliance tests, or of the Tiffeneau test as a parameter of bronchial obstruction, and no reduction of blood oxygen. These results were obtained from probands without bronchopulmonary illnesses. But problems with CAPD and ventilatory function may arise in patients with diseases of the respiratory tract including fluid lung, or in patients with 'space problems' in the abdomen (polycystic renal disease).

[Postural drainage and breathing gymnastics--conditiones sine qua non-before lung resections for bronchiectasis (author's transl)].

Report on 150 lung resections in 143 patients with bronchiectasis in the period of 1958-1974. The intensive preparation of the patients before surgery is of highest importance for a small risk and rare complications after operation. Postural drainage and breathing gymnastics are able to increase significantly FEV1 and VC and to reduce RV in the preoperative phase. The complication rate amounted to 8%, lethality after surgery to 1.3%.

[The effects of preoperative treatment and physical training to operability in patients with bronchial carcinomas (author's transl)].

Training effects of preoperative treatment, especially of breathing exercises in 52 patients with bronchial carcinoma were controlled by bodyplethysmography, measurements of the thoracic and abdominal circumferences in inspiration and expiration before and after the training program. In patients with obstructive ventilation disorders the expiratory resistance came to be reduced significantly. The mobility of the thoracic wall and of diaphragm increased in all patient-groups, but not in patients with restrictive ventilation disorders. After introducing the complex program the complication rate seems to drop to half of the previous rate.
