[Selected aspects of theophylline antiinflammatory action on the respiratory tract].

Theophylline is now considered not only as the a bronchodilatator, but it is increasingly recognised, that it has other anti-asthma activities. The clinical pharmacological observations and in vitro data showed, that theophylline has several anti-inflammatory activities relevant to asthma. These include, even at low plasma concentrations, the inhibition at cytokine synthesis and release, the inhibition of inflammatory cell activation and microvascular leakage, the prevention of airway hyperresponsiveness induced by airway inflammation. Theophylline appears to have immunomodulatory effects. Theophylline can be regarded as a useful alternative to other anti-inflammatory drugs for the chronic treatment of mild and moderate asthma. It should be used at the lower doses to achieve plasma concentrations of 5-10 mg/l, with will avoid the risk of side effects. Future studies are required to evaluate the role of low-dose theophylline and its place in the management of asthma.

[Nocturnal heart rate variation in patients with sleep apnea syndrome].

We observed a group of 10 patients with OSA symptoms. In all of them we investigated: the first examination-MESAM 4, EKG, UKG, lung function test, polysomnography. Analyzed the results we regard to appear of the heart rate changes in relation to apnea type, saturation and sleep phases. The results showed, that: 1. The altitude of snoring index is not adequate to the changes of heart rate and altitude of saturation (MESAM 4). 2. Apnea index and desaturation are conformed to heart rate changes (polisomnography). 3. Heart rate changes and desaturations are independent to sleep phases. 4. Heart rate changes and desaturations are independent to apnea type.

[Evaluation of thyroid anti-membrane and anti-thyroglobulin antibody determination in patients with thyroid cysts treated with external drainage under ultrasonographic control].

In patients (43 women and 12 men) external open drainage under ultrasonography control was performed. In 25 cases contents of serous nature, in 27 cases of colloid and in 3 cases of suppurative character were obtained. Anti-membrane thyroid antibodies (ATMA) and anti-thyroglobulin antibodies (Tg-Ab tire) were determined in the cyst contents as well as in the serum. The antibodies were present in the serum of 48% of the patients with serous cyst and in 52% with colloidal cysts. In serous cyst contents, ATMA and Tg-Ab antibodies were present in 52% of cases, while in the colloidal contents ATMA antibodies were found in 60% of patients and Tg-Ab antibodies in 68%. In 3 patients with cysts containing pus no antibodies were found. When antibodies were detected in the contents of serous and colloidal cysts their presence were also found in the serum. Among 55 cases with thyroid cysts, the antibodies were found in the serum of 47% of patients. The results indicate involvement of immunological factors the role of which has not been explained.

[Elastic recoil pressure in radiation induced injury of the lung].

The study was carried out on 34 females, age range 31-62 years, mean 46 years. It was conducted in the early phase, 3 months after introduction of the radiotherapy with X-rays. The results were compared with the initial values measured immediately after surgical removal of the tumour. The irradiation dose was 40 Gy to the sternum area and 35 Gy to the middle part of the axilla. Volume and capacity indices, bronchial resistance, small airway function parameters and mechanical properties of the lungs were studied. In the group of younger females (age < 46 years) a significant increase of the retraction index was noted (RC = P.TLC: TLC) following radiotherapy and a coexistent decrease of thoracic gas volume (TGV). In the whole group 13 patients reacted to radiotherapy with a left shift of the V/Pel curve, demonstrating an increase of elastic recoil. This group of patients also showed a marked decrease of TGV. These observations demonstrate that in a part of the patients treated with X radiotherapy a new equilibrium instated between elastic recoil pressure and lung capacity.

[The use of a computer program to study gallbladder motor function].

Gallbladder motility was studied in 30 women by means of an ultrasonographic method. Gallbladder volume was examined during intravenous infusion of ceruletide (15 ng Takus/min in 0.9% NaCl solution). It was several dozen less than the dose given intramuscularly. The volume and ejection fraction were defined by the ellipsoid Dodds method in the computer programme Cardio 500. During the infusion of ceruletide it was possible to identify exactly the dose of the drug in every moment and to correlate it with the gallbladder volume. Contour's analysis in Simpson's method made possible to evaluate the contractility of individual parts of the gallbladder.

[Ventilatory function of the lungs in galenite ore miners].

The study was carried out on a group of 54 galenite miners (composed of PbS slats) (age 37.9 +/- 7.5 years). The following parameters were assessed: VC, TLC, RV, RV%TLC, FEV1, FEV1%VC, Raw, Gaw, FEF50%, FEF75%. Also the ventilatory homogeneous index was calculated V2:V1 basing on the analysis of the flow volume curve. The control consisted of subjects living in the vicinity of the mine and smelting works. In 3.1% of the miners various stages of increased flow obstruction was found. In 8.3% signs of emphysematous changes, in 23.9% minor disturbances of bronchial resistance. In comparison with the control the miners demonstrated expiratory flow mainly in the small airways. Duration of exposure and co-existent chronic bronchitis increased the above mentioned observations and decreased the homogeneity index.

[Heterogeneity of lung ventilation as an indicator of early occupational exposure effects on the respiratory system].

The homogeneity of lung ventilation was evaluated in 148 male workers without any evident symptoms of impaired lung ventilation who were employed as miners (94 persons) and processors (52 persons) of PbS ore. The V2:V1 factor was calculated from the resultant of the maximum flow and volume. In the group of miners, significant increase of homogeneity was found to occur in elder workers (over 35 years of age) and in those employed for a longer period of time (above 10 years). Ventilation heterogeneity in that group was also intensified by simultaneous occurrence of chronic bronchitis. In the group of the floatation plant workers, there was a similar relationship between the ventilation heterogeneity and their age as well as employment period. As no effect of tobacco smoking on the discussed parameter was detected, it can be presumed that the effect of the working environment factors was predominant.

[Lung compliance in relation to frequency-dependent compliance (FDC) in men exposed to mixed air pollution].

53 males (age 36.8 +/- 7.2 years) with occupational exposure to atmospheric pollutants (low content of silica, sulfur oxides, carbon oxide and heavy metals) for a mean duration of 12.1 +/- 4.8 years were studied. The majority smoked. Dynamic compliance was calculated from esophageal pressure measurements using an esophageal balloon with an electronic transducer and plethysmographic thoracic volume values (Siregnost FD 88 and FD 91S--Siemens). Dynamic compliance (Cdyn) was registered at a ventilatory rate of 15, 30 and 60/min. A significant correlation was found between Cdyn and ventilatory rate in the examined patients in comparison with the control. Cdyn15 = 73.3 +/- 18.8% Cst, Cdyn30 = 54.6 +/- 18.8% Cst, Cdyn60 = 37.4 +/- 14.9% Cst (control Cdyn15 = 85.17% Cst, p < 0.005, Cdyn30 = 82.6 +/- 11.4% Cst, p < 0.001, Cdyn60 = 67.4 +/- 15.8% Cst, p < 0.001). The percentage of abnormal individual Cdyn60 values in the studied group was significantly higher in comparison with the control (p < 0.005). The decrease of Cdyn60 was related to duration of occupational exposure, to mixed atmospheric pollution and history of smoking.

[The role of the extra-bronchial factor in controlling maximal expiratory flow in workers exposed to mixed air pollution].

Narrowing of the expiratory air stream, not only depends on the intrabronchial factor (significant increase of airway resistance) but can also be a derivative of decreased elastic recoil. In patients exposed to air pollution the author demonstrated such a mechanism basing on observation of decreased air flow indices (MEFV) accompanied by significantly increased statistical compliance (Cst, sCst), and the decrease of elastic recoil pressure at different lung volumes (PTLC, Pel 60%). This was accompanied by significantly increased RV%TLC indices. The author believes that an important role in affecting maximal air flow indices is played by lung elasticity.

[Lung elasticity as the factor affecting ventilatory capacity in workers exposed to mixed air pollution].

The author assesses the role of pulmonary mechanics in decreasing ventilation in subjects exposed to mixed air pollution. He demonstrates the role of lung elasticity in such processes. The consequences of this is a decrease in Delta P and an increase of critical transmural pressure (Ptm'), which decreases maximal expiratory flow (V'e max) without affecting conductance of the terminal airways (Gus). To receive such information the author analyzed individual MFSR indices.

[Whole blood and plasma viscosity and erythrocyte aggregation in power plant repairmen].

Forty-nine workers employed at power plants repairs underwent examinations and tests of: circulatory system, whole blood and plasma viscosity, red cell aggregation, concentration and electrophoretic picture of serum proteins, levels of glucose in blood and cholesterol and beta lipoproteins in serum. Ten per cent of the test workers exhibited slight deviations of the circulatory system. More pronounced changes, as compared to controls, were found in rheological factors: corrected blood and plasma viscosity and red cell aggregation were increased but hematocrit was decreased. Biochemical investigations showed a fall in serum total proteins and albumins and a rise in serum cholesterol and beta lipoproteins.
