Several epidemiological studies have focused on influence of psychosocial factors on intensity of cardiovascular diseases in general population. In 1988 Warsaw POL-MONICA PROJECT was performed covering the population of 710 men and 723 women, aged 35-64. We found that 25.1% men and 25.8% women had type A behaviour pattern (TABP). TABP was rare represented in the older age group and in low-educated persons. Extreme sleep disturbances were observed significantly (p < 0.001) more often in women (10.7%) than in men (4.9%) and the percentage increased with age. Extreme sleep disturbances were also found more often among elementary educated persons (16%) versus the others (p < 0.001). Low social support level was observed twice often in women (29.7%) than in men (14.8%) and the percentage increased with age and with lower education level (p < 0.001). Extreme stress level was found more often in women (13.3%) than in men (5.6%) (p < 0.001) and in the oldest age-group. In both sex-groups mean anger levels decreased with age (p < 0.05). Mean anxiety levels didn't differ significantly neither in age-groups nor in education categories.
The general decline of science which took place in the first half of the 18th century was a cause of a huge quantity of calendars editing. Calendars substituted for the former scientific literature. Up to 1763 over 800 calendars appeared. A half of them was published in Cracov. Those calendars differed from the contemporary ones. Apart from dates, they included not only basic information in history, geography agriculture economy but, not seldom, medicine as well. Most often they were written by university professors with a few physicians among them. Obviously, the level of the presented medical knowledge was very low. There wenadductions to astrology, wizardry and Provinience. But some diagnoses and therapeutic advice being a kind of doctor's manual, useful to so-called domestic medicine are still worth of the attention. First of all, phytotherapy chapter based on the folk empiricism, was the most rational. Thought 18th century calendars did not have much in common with real medicine, they make an interesting source to search for history of the Polish medicine of the Saxon times.

Analysis of the efficacy and tolerability of gemfibrozil (Gevilon-Parke Davis) was performed including 29 patients aged 19-69 years with primary hyperlipoproteinemia (HLP) type IIb-16 persons, IV-13 persons. All patients got dietary recommendations and received gemfibrozil 450-900 mg/day for 3 months. In both types of HLP a significant reduction of serum cholesterol (TCh)--15.5% triglycerides (TG)--32.1% VLDL-Ch--34.9% and VLDL-TG concentration--36.6% was observed as well as an increase of HDL3 fraction-16.3%. The greatest reduction of serum TCh concentration and VLDL-Ch were observed in type IIb, while that of TG and VLDL-TG in type IV HLP. The best therapeutic effect was obtained during the first month of treatment. The percent of TCh, TG, VLDL-Ch, VLDL-TG reduction correlated with their initial level. Tolerability of gemfibrozil was very good. Only in 2 patients transient abdominal pain was observed.
This prospective clinical trial (1983-1989) compares life-quality scores according to Karnofsky in patients with intra-cranial who had been operated upon. Karnofsky's performance status was evaluated during a follow-up period of 36 months. The patients were between 20 to 77 years of age; 132 patients had an astrocytoma grade III or IV; 42 patients with brain metastases were investigated; 72 had an astrocytoma grade I + II, and 144 had a meningioma. The median survival in patients with astrocytomas grade III or IV was 12 months; a short period with nearly normal activity (Karnofsky score 75) was followed by rapid deterioration. In patients with brain metastases, postoperatively the median functional ability reached its highest value three weeks after treatment and then decreased continuously (average score from 50-25). In patients with astrocytomas grade I and II, life-quality was reasonably good (mean score up to 75). The score was observed to increase slightly during the first year after treatment. A performance status improvement up to 80 could be obtained in patients with meningiomas; however, 35% were left unfit for work.

In the paper presented, the relationship was analysed between the educational level and the level of risk of ischaemic heart disease (IHD) in a random sample of the Warsaw population aged 35-64 years. Men with a lower educational level (elementary or basic vocational) were found to have a significantly higher means for systolic blood pressure level, for plasma high-density lipoprotein (HDL) cholesterol concentration, for the numbers of cigarettes smoked daily, and for the probability of IHD development according to the multivariate logistic function of Farchi and Menotti, and also significantly greater prevalences of cigarette smoking, hypertension and overweight. And for men, a negative relationship was noted between educational level and plasma low-density lipoprotein (LDL) cholesterol concentration. Women with a lower educational level had a higher mean plasma triglyceride concentration, higher values of systolic and systolic blood pressures, a greater probability of IHD development, and higher prevalences of hypertension, obesity and ischaemic heart disease symptoms. And for women, a negative relationship with educational level was noted for plasma LDL-cholesterol concentration and for the mean value of the overall risk score according to Rose.
The coefficients and trends of mortality in years 1976-1987, based on data of Central Statistical Office, has been estimated in population of men and women aged 25 to 64 years, inhabitants of the Warsaw city region or the county region of Tarnobrzeg province . The trend of decrease of general mortality due to diseases of circulatory system has been statistically significant one similarly as that of mortality due to other heart diseases or ischaemic heart disease in men (group aged 25-34 or 35-44 years) in the Tarnobrzeg province . The analogously significant trend of mortality due to diseases of circulatory system or ischemic heart diseases in men aged 35-44 years has been stated in the Warsaw inhabitants. In the group of women an increase of general mortality has been noted in the youngest age group in Warsaw. In Warsaw and the Tarnobrzeg province in 1987 deaths due to diseases of circulatory system represented 33.2% and 39.9%, respectively, of total death pool among subjects aged 25 to 64 years. The respective percentages in the group of women were 29.1% and 39.4%.
Basing on results of first screening of the Pol-MONICA Project in 1984 in two centres that is the Tarnobrzeg province and two districts of Polish capital Warsaw (Praga South and Praga North) the level of 11 main coronary disease risk factors was compared in 2469 men and 2729 women. The following 5 risk factors were statistically significant for both men and women: the Durnin index of body fat content, the total cholesterol concentration in the plasma, triglyceride level, numbers of daily meals. The mean level of two factors that is the HDL-cholesterol concentration and the HDL-cholesterol/total cholesterol ratio was lower in the Warsaw sample what speaks in favor of concept that the ischemic heart disease threat is greater in that population. Among men the Warsaw population exhibited moreover the significantly higher value of diastolic blood pressure as well as of Quetelet body mass index; age and numbers of cigarettes smoked daily has also been higher in that population. In women from the Tarnobrzeg province ++ the diastolic blood pressure was higher than that in the Warsaw population. The two populations studied showed also highly significant differences so far as the complex of all 11 features is concerned what enabled the qualification of 72.3% of men, and 68.4% of women as belonging to the big city population or to the agricultural-industrial population using the discriminant Fisher function.

The method of Durnin and Womerslay of fatty tissue content in human body was analysed for determining the reproducibility of measurements and agreement of the results depending on the number of measured folds and number of measurement repetitions. The analysis was based on the results of the first cross-sectional study POL-MONICA Warsaw. An agreement was demonstrated between the results of one and two measurements of the folds and four measurements, and between the body fat content determined by means of these measurements. This made possible reduction of the number of measurements in the second cross-sectional study. A small but statistically significant difference was noted between the mean values of body fat content determined from various combinations of skin-fat folds.
The effects of reserpine (catecholamine depletor) and LHRH analogues on gonadotropin secretion, spermiation and ovulation of common carp were investigated. Injections of reserpine alone at a dose of 1 or 7 mg/kg of body weight stimulated spermiation, and reserpine at a dose of 1 mg/kg of body weight in combination with (D-Arg6, Trp7, Leu8, Pro9, NEt)-LHRH (s-GnRH-A) or with (D-Ala6, Pro9)-LHRH (LHRH-A) at a dose of 50 micrograms/kg of body weight caused an increase of plasma gonadotropin levels, spermiation and ovulation in 80-90% of the females. Simultaneous injection of reserpine and LHRH analogues was as effective as injection of reserpine followed by injection of LHRH analogues 6 h later.

The ability of the SP fragments SP2-11 and SP3-11 to release histamine from rat peritoneal mast cells has been compared with that of the whole peptide. SP1-11 was found to be about 3.4 times more active than SP2-11 and about 10.4 times more active than SP3-11. The substance P antagonist [D-Pro4, D-Trp7,9,10] SP4-11 was equally effective at antagonizing the histamine releasing action of SP1-11, SP2-11 and SP3-11. Benzalkonium chloride was found to be a competitive antagonist of SP and SP3-11: the dissociation constants for the benzalkonium chloride-receptor interaction being about the same when either SP1-11 or SP3-11 was used as the agonist.
The substance Arg-Pro-Lys-Pro-(CH2)11CH3 [SP1-4C12] was synthesized by forming a peptide bond between Arg-Pro-Lys-Pro, the N-terminal sequence of substance P and dodecylamine. The aim was to examine the roles of the N- and C-terminal sequences of substance P in stimulating histamine release from mast cells of the rat peritoneal cavity. SP1-4 C12 induces concentration-dependent histamine release in the range 8 to 200 nM. SP1-4C12 was 50 times more potent than substance P and 300 times more potent than dodecylamine. Unlike dodecylamine itself, SP1-4C12 induced noncytolytic histamine release which was inhibited by benzalkonium chloride and by the substance P antagonist [D-Pro4,D-Trp7,9,10]SP4-11. Histamine release induced by SP1-4C12 was inhibited at temperatures below 16 degrees C and did not require the presence of extracellular calcium ions. It is suggested that substance P and some other basic histamine liberators initiate histamine secretion by a mechanism that involves the insertion of a hydrophobic region into the membrane lipid which is necessary to present positively charged moieties to a receptor site involved in activating the secretory mechanism.
Calcitonin gene-related peptide (CGRP) produced a dose-related wheal and flare reaction in human skin at doses of 12.5 to 50 pmol. The flare response but not the wheal response to CGRP and substance P were inhibited by prior treatment of the subject with oral chlorpheniramine, 16 mg. CGRP, but not substance P, was potent in producing a delayed erythema and surrounding pallor in human skin, which peaked at 1 h and persisted for more than 3 h after injection, when wheal and flare responses had subsided. The delayed response was accompanied by infiltration of polymorphonuclear leukocytes. The delayed erythema and pallor produced in response to CGRP were not inhibited by oral chlorpheniramine, or by 4% prilocaine injected locally. CGRP released histamine from rat peritoneal mast cells over the concentration range 2.5-10 microM. CGRP was about fourfold less potent than substance P in releasing histamine. The substance P analogue, [D-Pro4, D-Trp7,9,10]SP4-11 10 microM, and benzalkonium chloride 10 microM inhibited histamine release from rat mast cells stimulated by either CGRP or substance P.
The laser beam enables tissue cutting and care of the cut edges, which prevents an increased scar tissue formation. For this reason we have used the laser beam in carpal tunnel surgery for transection or sealing of the transverse carpal ligament since 1981 because scar tissue formation is for the most part the cause of recurrence. Scanning-electron microscopic studies of the carpal ligament show the difference between the laser and traditional cutting techniques. After using the laser beam, there is no normal tissue structure; the cut surface appears as uneven and rough in contrast to the cut surface of the cold knife.
