The lack of effect of a prophylactic dose of enoxaparin on thrombin generation in patients subjected to nephrectomy because of kidney cancer.

It is assumed that major surgery, connected with extensive tissue dissection, brings about the release of tissue factor into circulation and subsequent activation of coagulation system. This activation results in the thrombin generation, which is supposed to be suppressed by the low-molecular-mass heparins (LMMH), administered to the surgical patients as the prophylaxis against postoperative venous thromboembolism. We have estimated the concentration of circulating thrombin-antithrombin (TAT) complex in patients subjected to transperitoneal nephrectomy and randomized into controls and who received 40-mg enoxaparin 12 h before and 12 h after the operation and then once daily for 7 days. We have observed a sharp rise of TAT concentration at the end of surgery and it corresponded to the simultaneous drop of antithrombin (AT) activity. TAT concentration gradually decreased and AT activity increased up to the end of observations on the seventh postoperative day, but there were no differences observed between the groups of patients. We have also observed a biphasic increase of plasmin-plasmin inhibitor (PPI) complex concentration in our patients. Again, there were no differences in PPI between the groups of patients. It is our conclusion that under the conditions of this study, the well-known prophylactic effect of enoxaparin against the venous thromboembolic complications was not mediated by the inhibition of intraoperative thrombin generation. The anti-inflammatory or biophysical influence of LMMH may be rather taken into account in surgical patients receiving prophylactic doses of these heparins.

Treatment of a benign prostatic hyperplasia by Nd:YAG laser - own experience.

OBJECTIVES: To evaluate the effectiveness and safety of visual laser ablation (VLAP) and interstitial laser coagulation (ILCP) versus transurethral resection of the prostate (TURP). PATIENTS AND METHODS: Altogether 259 consecutive patients with symptomatic benign prostatic hyperplasia (BPH) were treated by VLAP (n = 117), ILCP (n = 30) or TURP (n = 112). VLAP was carried out using both contact and noncontact techniques, whereas ILCP using the technique described by Muschter. The results were obtained at a mean follow-up of 23 months (from 6 to 48 months). IPSS, quality of life, maximum flow rate, and residual urine were evaluated preoperatively and at 3 and 6 months after operation in all patients. The results at 12, 24 and 48 postoperative months are available for 122, 86 and 72 patients, respectively. RESULTS: Mean values of all outcome parameters improved during 6 postoperative months (p<0.05) in all groups of patients and subsequently remained unchanged during the 4-year postoperative period. There were no serious postoperative complications in the laser groups. Very good and good results were obtained in 71.3% patients after VLAP, in 89.3% patients after ILCP and 81.4% patients after TURP. CONCLUSION: Both VLAP and ILCP are safe methods with low risk of complications, however, they are less effective than TURP, which remains the gold standard for BPH patients requiring surgical treatment

Usefulness of cytologic examination of urinary sediment with the millipore method in routine diagnosis based on 125 cases.

The paper evaluates the usefulness of cytologic examination of urine sediment performed by millipore method, based on 125 cases. The method permits high quality cytologic smears obtained even from cellularly poor urines. The sensitivity of this analysis is directly proportional to the histological grade of the tumour. In the case of transitional cell carcinoma G1, the sensitivity was 65%, in G2-79.3% and in G3-92.9%. High sensitivity of this examination was found in carcinoma in situ. Our study is of retrospective character, the results presented were obtained on the basis of one cytoanalysis of urine sediment.

[A case of pregnancy with polycystic kidneys].

The authors described a course of pregnancy and delivery of pregnant women who was admitted to the hospital in the third trimester with symptoms of sepsis in course of the pyelonephritis. Signs of a kidney insufficiency and pulmonary oedema were observed. During hospitalization ultrasound examination showed polycystic kidneys.

Procoagulant activity of gastric, colorectal, and renal cancer is factor VII-dependent.

The PA of GC, CC, and RC extracts was assayed by the recalcification of human normal or F VII-DP, and the PA of normal tissue was also determined. The PA of normal tissue was higher than that of the cancer tissues in all groups of specimens. Substitution of normal plasma by F VII-DP resulted in significant depression of the PA and the differences in the PA between the normal and cancer tissue samples disappeared. Preincubation of normal and cancer tissue extracts with the cysteine proteinase inhibitors, mercuric chloride and iodoacetamide, did not affect the PA of these extracts. We conclude that the PA of the investigated cancer extracts is factor VII-dependent and can be related to the presence of tissue factor within cancer tissue.

Antireflux operations without catheter.

Forty-three patients are presented in whom 80 ureters were transplanted by Cohen's method without leaving a catheter in the bladder. No complications were observed and the incidence of urinary tract infection and dysuria was lower. The late results of the operation were good in every case.

Diverticula of renal calyces.

The authors present results of the treatment of 31 patients with calyceal diverticula. Conservative therapy was applied in 19 cases, whereas in 12 patients surgical treatment was performed. Surgery was mandatory in patients with persistent pains and/or in whom the infection of the urinary system did not respond to conservative therapy, as well as in cases of nephrolithiasis. Diverticulectomy was performed in eight patients, and resection of the lower or upper renal pole ws done in four patients. In all cases the results of treatment were good.
