[Experiences with an antimycotic tampon in the treatment of saccharomycetic vaginal infection].

Antimycotic (miconazole) medicated tampons were used for treatment in 53 patients with vaginal saccharomycetic infections as evidenced by hanging drop and on culture. A short term 2 1/2 day treatment with 5 tampons showed a success rate of 92.5%. The fast relief of symptoms like discharge, itching and burning is an improvement of the medicated tampon treatment. The new medicated tampons are considered as improvement of local vaginal treatment.

High molecular fibrin derivatives in pre-eclamptic and eclamptic patients.

Soluble fibrin monomer complexes were assayed by agarose gel filtration and by ultracentrifugation. Increased values of SFMC were found in the normal third-trimester pregnancies. They were considerably higher in plasma of patients with pre-eclampsia. Concentrations were higher using ultracentrifugation as compared to gel chromatography. Using ultracentrifugation 4 components were identified, indicating that SFMC in this disease are not a homogeneous group.

Ultrastructural morphometric investigations on human platelets after long-term ingestion of oral contraceptives.

Platelets from healthy females, who were taking oral contraceptives over a long period of time, were investigated by ultrastructural morphometry. The size, shape, relative volumes, numerical and surface densities of the platelets and their organelles were measured. In 'long-term pill users' the platelets showed a significant increase in the area of section profile. Further changes as, for example, increased volume of platelet granules, reduced number of mitochondria, decreased volume and surface of the open canalicular system, and increased surface of the dense tubular system are to be noted.

[Treatment of colitis ulcerosa during pregnancy (author's transl)].

Aggressive pharmacotherapeutic and surgical measures are often contra-indicated in the treatment of colitis ulcerosa in pregnancy. Especially in the sensitive embryonal development phase, the use of antibiotics, anti-inflammatory drugs, chemotherapeutics and ACTH is considered questionable. The subtotal colectomies and ileostomies often demanded in toxically dramatic colitides place a considerable burden on mother and foetus. A conservative therapeutic regimen, which has been successfully used in more than 80 patients with inflammatory intestinal diseases, was modified to suit the requirements of pregnancy. This is a combination therapy involving parenteral feeding, food which can be absorbed by the intestinal walls, and a suitably adapted pharmacotherapy. In one case of a severe toxic relapse of colitis ulcerosa during early pregnancy, remission was achieved in a patient while fully maintaining the pregnancy.

[Intrauterine infections in pregnancy. The significance of premature rupture of the membranes and early lung complications].

The course of intrauterine infections are illustrated and 5 maternal deaths reported. Early lung complications were predominant, described as shock lung or septic pneumonia. Renal complications or disseminated intravascular coagulation were absent or developed late. In contrast to gram negative sepsis there was a frequent association with premature rupture of the membranes, which indicates the danger of this event. On microbiology gram positive rods were found more frequently than a mixed flora. The morphology of intrauterine infection is similar to those of puerperal sepsis although the clinical course is quite different. This indicates a change in infectious pattern during the last centuries.

[A new functional durable mold after surgical correction of vaginal aplasia (author's transl)].

Unsatisfactory long-term results following operative construction of a neo-vagina are often produced by contraction in the upper third of the blind vagina. These contraction tendencies can only be halted by use of durable molds. Standardized phalli are not, in our experience, well-suited for this purpose. Fabrication and fitting of an individual durable mold of tissue-neutral plastic, adapted to pelvic stability and function and yielding a good measure of success, are described.

[Biochemical and coagulation-physiology examinations before and after amniocentesis in early pregnancy].

In 46 women the alpha1-fetoprotein content, the soluble fibrin monomer complexes (LFMK), and the coagulation status were determined in the maternal serum at the beginning of the second trimester before and after amniocentesis. Increases of AFP by more than 100% of the original level pointed to fetomaternal microtransfusion, the quantity of which was calculated by means of a formula. Overall coagulation tests did not show any change after the puncture. A moderate LFMK increase was seen (p less than 0.05), which, however, was not clearly correlated with the AFP increase.

Ultrastructural morphometric investigations on normal human platelets.

Platelets from normal human volunteers were investigated by ultrastructural morphometry. Preparation of blood samples was done in two different ways in order to evaluate the possible influence of the preparative method on the size and shape of platelets and their organelles. No significant differences could be dmonstrated for any of the measured parameters. Relative volumes of specific granules, mitochondria and the open canalicular system were calculated according to the method of Weibel. Surface and numerical densities of some compartments were determined. Direct measurements of the long and short axes of the platelets were conducted to ascertain further information about their size and shape. Absolute data are in good agreement with the few reports in the literature.

[Problems in the evaluation of contraceptives (author's transl)].

For the evaluation of the effectiveness of contraceptives the Life Table method is at present the best method. It is a disadvantage that the original method of Tietze & Potter was restricted to the evaluation of intra-uterine contraception devices. A Belgian team is now in the process of developing a modified life table method for the evaluation of the effectiveness and the side effects of oral contraceptives. The reference to the Pearl-index for the effectiveness of contraceptives is unclear and in the way in which it is at present used scientifically untenable.

[The evaluation of cortisol in amniotic fluid and in maternal blood as parameter for foetal lung maturity (author's transl)].

The lecithin/sphingomyelin-ratio (L/S-ratio) in amniotic fluid has become established in recent years as a valuable laboratory parameter for determining foetal lung maturity and has thus helped to avoid the neonatal respiratory distress syndrome. Since glucocorticosteroids are a decisive factor in stimulating foetal lung development, we were prompted to investigate whether the determination of cortisol in amniotic fluid or maternal blood could also serve as an indicator of foetal lung maturity. The cortisol concentration was measured by radioimmunoassay in amniotic fluid (n = 91) and in maternal venous plasma (n = 70) from normal and complicated pregnancies and was compared with the L/S-ratio. A significant correlation between gestational age and cortisol in amniotic fluid (r = 0.711) and in maternal venous plasma (r = 0.625) was observed. There was also a significant correlation between the L/S-ratio and amniotic fluid cortisol (r = 0.701). However, compared to the L/S-ratio, measurements of cortisol yielded 10% false positive and 11% false negative results. A significant correlation between the L/S-ratio and the maternal plasma cortisol could also be demonstrated (r = 0.631). Here the comparison with the L/S-ratio gave even poorer results: 16% false positive and 6% false negative. The high percentage of false results, especially false positive results yielded by cortisol estimations in both fluids leads to the conclusion that the L/S-ratio remains the superior diagnostic parameter in determining foetal lung maturity.

[Cultural and serologic examination for saccharomycetic infection in pregnant women (author's transl)].

Cultures from retrolingual, vaginal, and anal smears were used to determine the presence of saccharomycetic infection in 177 pregnant women. The retrolingual smear was saccharomyces-positive in 29.1% of the cases; the vaginal smear, in 18.1%; the anal smear, in 7.3%. The fungus differentiation revealed the pathogen Candida albicans 85 times and the pathogen Candida krusei, 1 time. The Candida HA Test and the Candida JF Test (Roche) were carried out in 160 of the patients. A relationship could be determined between the clinical findings and the serologic results. The danger to the newborn when the mother has a saccharomycetic infection as well as the fundamentals for therapy were discussed.

[Serum folic acid, vitamin b12 and iron values during pregnancy (author's transl)].

Of 125 pregnant women, of whom 81 were treated and 44 not treated, 14 (12,5%) were found to be suffering from anemia, with a hemoglobin count to be diminished. In 31 cases iron deficiency was established and in 8 women vitamin B12 in serum was reduced. Iron and vitamin deficiencies were found more frequently in the non-treated group than in the treated group.

[A new needle for low risk amniocentesis in early pregnancy (author's transl)].

A new needle will be described, which has been especially useful for amniocentesis in early pregnancy. Construction is based on the principle of the Verres-needle. After passing through the abdominal wall and entering the amniotic sac, a blunt stylet prevents the needle from contacting the fetus. The depth of the puncture is controlled by a device attached to the side of the needle. The needle is easily observed using ultrasound because of good sound waves reflections of specially constructed surface.
