Reactions to insect allergens. The incidence of response to testing among allergic and nonallergic persons.

Positive reactions to skin tests with extracts of various insects were found to be significantly higher in 200 patients with allergic disease than in 150 nonallergic controls.Forty-seven per cent of the allergic group had positive reaction to bee, 46 per cent to house fly, 39.5 per cent to moth, 29.5 to ant and 28.5 to aphid. Ant and house fly extract caused the greatest number of severe reactions (4+), with moth second and bee third. Because extracts of nonstinging insects caused reaction almost as often and as strong as stinging insects, it may be inferred that sensitization is due to inhaled insect particles. There appeared to be no necessary connection between a history of having been stung and a strong skin test reaction to bee protein. Testing for insect sensitivity appears a good way to get diagnostic information that can enhance the effectiveness of treatment.

Acute and chronic asthma. Treatment with theophylline in hydro-alcoholic solution: clinical evaluation and pulmonary function studies.

A flavored solution containing 80 mg. of theophylline and 3 cc. of ethyl alcohol per 15 cc. was given orally to 31 patients with acute asthma to terminate the attack. Thirty patients with moderate to severe chronic asthma were alternated for three or four weeks on daily multiple doses of either the theophylline solution or a placebo. In the acute cases three-second Vital Capacity increased by 33.8 per cent and Maximal Breathing Capacity by 30.2 per cent in one hour after taking 60 cc. to 75 cc. of the theophylline solution. When placebos were given, both measures of lung function declined during the first half hour.Seventy-one and a half per cent of patients with acute cases felt moderate to complete relief of symptoms. In persons with chronic asthma the regular use of the theophylline solution did not change the frequency of asthma in most cases, but it decreased the severity in 59 per cent of cases. The values for three-second Vital Capacity and Maximal Breathing Capacity rose only a little. Gastric irritation was noted in one-third of the chronic cases and one-fourth of the acute cases. This could be reduced by appropriate measures.

Hypersensitivity to house dust; a preliminary report on treatment of children with antigens at two different dosage levels.

Two comparable groups of children with allergic sensitivity primarily to house dust were treated respectively with low (1:1,000,000) and high (1:100) dilutions of Endo house-dust extract. In the two groups there were 47 patients with asthma, 5 with chronic allergic rhinitis, and 10 with both. After an average of 16 to 18 months of injection treatment, good to excellent results were obtained by 75 per cent of the patients on low dosage and 80 per cent on high dosage. Change of dosage from low to high in five cases, and from high to low in five others, made no perceptible difference in results. Continuous and regular treatment seemed to be more important than the size of dose. The apparent effectiveness of small doses of dust extract may be due to the polysaccharide nature of the material which persists as an active antigen in the tissues.
