Maldigestion phenomena in patients subjected to a Billroth II (B II) operation are in part attributed to an atrophy of the pancreas due to inactivity. In order to investigate this, a Lagerlöf tube was introduced under endoscopic control into the afferent loop, and the function of the exocrine pancreas studied after endogenous and exogenous stimulation. During endogenous stimulation of the exocrine pancreas by means of a Lundh test meal, no significant differences in the secretory pattern of the Billroth II patients as compared with healthy test subjects were found. In contrast, during exogenous stimulation, the output of trypsin and chymotrypsin in healthy subjects was significantly greater than in the B II patients. Since, in comparison with maximal stimulation, the endogenous secretin released by the jejunal mucosa in response to the intake of food, effects only approximately 25% of the bicarbonate production and endogenous pancreozymin effects some 50% of the enzyme output, the maldigestion phenomena observed in B II patients are inpart understandable.
Except for impaired resorption of vitamin D and calcium, the small bowel does not exhibit significant functional impairment during aging. Vascular problems in the small and large bowel are rare, but are of dramatic importance. Chronic constipation and diverticulosis are classical disorders of aging which should be treated by adding fiber to the normal diet. Surgery is only indicated in the case of complicated diverticulitis. Adenoma and colorectal carcinoma show an age-related incidence. Angiodysplasia, preferentially localized in the caecum, is the major cause of lower gastrointestinal tract hemorrhage. Anal incontinence may be multifactorial, behavior modification and biofeedback may help to avoid surgery.
A case report is presented here on the rare osteoclastoma of the pancreases which was observed in a patient with pancreas divisum. The leading symptom was recurrent bleeding from Santorin's duct (Santorinorrhage).
We report on a 68-year-old patient who developed an obstructive jaundice 13 years after surgery of a malignant melanoma of the right ear. The initial sonographic and CT diagnosis of a benign pseudocyst of the head of the pancreas had to be revised based on ERCP, sonographically guided puncture and papillary biopsy. By Whipple's procedure, the solitary intrapancreatic metastasis could be successfully removed.
