[Surgical treatment of upper extremities of cases arthrogryposis multiplex congenita].

Between 1990-1999 at the Polish-American Pediatric Institute 36 children with arthrogryposis were treated. In 10 cases 21 corrective surgical procedures of the upper extremities were performed. Corrective procedures of the upper extremities were performed. Corrective procedures aimed at contractures of the hand, wrist and forearm, consisting of capsulotomies, lengthening of the finger flexor tendons and transposition of wrist flexor on to the wrist extensors were performed 8 times. Correction of extension contracture of elbow joint, consisting of posterior capsulotomy and lengthening of the triceps tendon was performed 9 times. In 4 cases transposition of the pectoralis major onto the biceps tendon according to Clark or Zancolli in Carrol's modification was performed. The average age of the children was 4.7 years (age ranging from 1.5 to 9 years), the follow-up period was 4.25 years (ranging from 0.5 to 10 years). Palmer contracture of the wrist was an average 80 degrees (70-90 degrees) before surgery. Passive flexion in the elbow joint was on average 15 degrees (0-35 degrees). Active flexion of the elbow was absent in all cases. After corrective procedures in the wrist, in 5 cases physiological positioning of the was achieved. After lengthening of the triceps tendon passive flexion of elbow was 80-90 degrees, in one extremity 70 degrees. After the Clark or Zancolli procedures active flexion of the elbow was 80 degrees in one case, 70 degrees in another and 50 degrees six months post-op. The author's own experiences indicate that and multi-stage interdisciplinary approach in necessary from the beginning in these cases.

[Melorheostosis: a case review and review of literature].

The authors present a review of the literature and the diagnostic difficulties in a case of melorheostosis in a 6 year old boy. Melorheostosis is characterized by longitudinal foci of subcutaneous sclerosis, limitation of motion in the joints and swelling of the knee and ankle joints. Radiologically numerous foci of hyperostosis similar to wax flowing down a burning candle have been noted.

[The experience in employing reciprocal gait orthoses].

The paper presents the experience of the authors in employing reciprocal gait orthoses in a group of 23 patients age 3-25 years (mean age 7.8 years). The orthoses were indicated in patients with flaccid paresis (17 children with myelodysplasia and 3 patients with traumatic paraplegia) and with arthrogryposis (3 patients). The follow-up period was 6 months to 5 years (mean 2.4 years). The authors discuss the principles of construction and operation of reciprocal gait orthoses and types of patients in whom they are recommended. The principles of learning walking and using the orthosis are also presented.

[Possibilities of ambulation in children with myelodysplasia].

The authors discuss the possibility of ambulation of children with myelodysplasia, employing different types of orthopedic devices. According to the personal experience of 23 myelodysplastic patients, the indications and results of using reciprocal gait orthoses have been described. The paper emphasizes the significance of interdisciplinary team in the treatment of this disease.

[Duplication of the lumbar spine--case report].

A case of a child with duplicated lumbar spine and concomitant multiple defects within alimentary tract, urogenital system, deformities and pareses of lower extremities is presented. After many operations, at the age of 5 he could walk unaided for 300 meters.

[Congenital dislocation of the patella in children and adolescents].

Eleven patients were treated at Orthopaedic Unit of Pediatric Surgery Department because of congenital dislocation of the patellae between 1988 and 1995. In 4 cases the condition was bilateral. Mean follow-up was 3 years and 7 months. Surgical procedures of Ali-Krogius-Roux-Goldwaith or Ali-Krogius-Hauser were performed. According to Hall and Michelli classification 6 results were rated excellent, 4 good, 3 fair and 2 poor. Flexion contracture of the knee resolved in all cases. Surgery employed proved efficient in treatment for congenital dislocation of the patella.

[Proximal hamstring release for knee flexion contracture treatment in children with childhood cerebral palsy].

A series of 24 children with cerebral palsy has been reviewed to asses proximal hamstrings release for treatment of flexion contracture of the knee. In 22 cases improvement of a various degree has been achieved, in 2 cases no improvement was gained. In 4 children hyperextension of the knee and lumbar hyper-lordosis was found. Observing exact indications for surgery and avoiding hyper-corrections allows for good results of treatment.

[Chronic recurrent multifocal osteomyelitis--personal experience].

Chronic recurrent multifocal osteomyelitis is a rare entity with obscure etiology. A review of literature concerning clinical appearance, diagnostics and management of this condition is presented. Own material embraces a group of 7 children who have chronic recurrent multifocal osteomyelitis. Mean follow-up was 2.5 years. On the basis of results achieved the authors believe this condition is of good prognosis and should be differentiated with chronic osteomyelitis and neoplastic changes.
