Restoration of adrenal steroidogenesis by adenovirus-mediated transfer of human cytochromeP450 21-hydroxylase into the adrenal gland of21-hydroxylase-deficient mice.

21-Hydroxylase deficiency, a potentially fatal disease due to deletions or mutations of the cytochrome P450 21-hydroxylase gene (CYP21), causes congenital adrenal hyperplasia (CAH) with low or absent glucocorticoid and mineralocorticoid production. The feasibility of gene therapy for CAH was studied using 21OH-deficient mice (21OH-) and a replication-deficient adenovirus containing the genomic sequence of human CYP21 (hAdCYP21). Intra-adrenal injection of hAdCYP21 in 21OH- mice induced hCYP21 mRNA with the highest expression from 2 to 7 days before a gradual decline. 21OH activity measured in adrenal tissue increased from undetectable to levels found in wild-type mice 2 to 7 days after AdhCYP21 injection. Adrenal morphology of 21OH- mice showed lack of zonation, and hypertrophy and hyperplasia of adrenocortical mitochondria with few tubulovesicular christae. These morphological abnormalities were markedly improved 7 days after hAdCYP21 gene therapy. Plasma corticosterone increased from undetectable levels to values similar in wild-type mice by 7 and 14 days, declining over the next 40 days. This is the first demonstration that a single intra-adrenal injection of an adenoviral vector encoding CYP21 can compensate for the biochemical, endocrine and histological alterations in 21OH-deficient mice, and shows that gene therapy could be a feasible option for treatment of CAH.

Ascaris pneumonitis: case presentation.

This is a case of a 74-year-old lady with ascaris pneumonitis whose presenting complaint was a respiratory infection. X-ray suggested neoplasia. The diagnosis was more complex because of other unrelated findings suggestive of thyroid and breast lesions. She did not display the expected eosinophilia, but this is transient and may have been missed. She resides in an urban area and has good hygiene habits. No source of the infection could be found. Diagnosis is most commonly made by finding eggs in the stool; occasionally an adult worm is passed in the stool. In this case the diagnosis was considered when an adult worm was found during sigmoidoscopy. There is a brief discussion of ascaris infection.

Optimal palliation of malignant bile duct obstruction: experience with endoscopic 12 French prostheses.

Large-caliber prostheses, 11.5-12 French, were placed in 167 patients (183 attempts, 91% success) presenting with obstructive jaundice. Thirty-three patients had additional prostheses placed to selectively decompress intrahepatic ducts obstructed by cholangiocarcinoma. In this prospective unrandomized series, there were 43 lesions of the common hepatic duct, 123 of the common bile duct (96 pancreas, 27 cholangiocarcinoma) and 17 ampullary. Transient fever responding to parenteral antibiotics occurred in 11 patients who did not receive prophylaxis, whereas only 2 patients who received antibiotics prior to the procedure developed fever subsequently. Four patients bled subsequent to sphincterotomy, 1 requiring a 2-unit transfusion. Pancreatitis occurred in only 1 patient. The mean hospital stay was only 3 days, range 1-10 days, with most patients being discharged within 48 hours. No procedural deaths occurred. The patency rate of this new, larger 12 Fr. prosthesis is significantly longer than that for the 10 Fr. stent, 190 days for 12 Fr., 150 days for 10 Fr. Given the advantages of the larger prosthesis, i. e., increased patency and function and decreased rehospitalization rate, the authors recommend this method of palliation for obstructive jaundice.

Accuracy of prehospital sphygmomanometers.

The first requisite for valid blood pressure determination is a correctly calibrated manometer. A total of 125 aneroid sphygmomanometers in prehospital use were calibrated against a mercury manometer standard. A total of 43 (34.4%) aneroid instruments deviated by more than +/- 4 mm Hg tolerance (average of four readings). A deviation of greater than +/- 8 mm Hg was shown by 10.4% (13) of the maeters. These aneroid gauges are easily calibrated by prehospital personnel using a mercury standard and Y-tube connection. Departments with routine maintenance policies had fewer inaccurate instruments. Emergency medical services advisors should ensure periodic maintenance and calibration of sphygmomanometers used in emergency care situations.

The Australian Medical Support Force in Rwanda.

In the aftermath of the Rwandan civil war, Australia's defence forces deployed a medical force to support the United Nations Assistance Mission. In this article, Wayne Ramsey, Commander of the Australian contingent, Lindsay R Bridgford, Officer Commanding Bravo Section, Robert J Lusby, Lt.-Colonel (Surgeon), Australian Medical Support Force Hospital, and John H Pearn, Colonel, and Director of Intensive Care in the Australian Medical Support Force Hospital, describe the Australian effort in the rebuilding of a shattered people and, in particular, of Kigali Central Hospital, the country's major medical facility.

A comparative study of several polymers for use as intravenous catheters.

The widespread use of long-term intravenous catheters has produced some concern on the part of the clinician with respect to morbidity and occasional mortality associated with their use. Fourteen polymer surfaces were evaluated in experimental animals in this study. Several of the materials are commercially available; the others are experimental. In all, 160 catheters were placed in the femoral and jugular veins through side branches. The catheters were evaluated in terms of thrombosis, phlebitis, and periphlebitis, using an arbitary grading system. Scanning electron microscopy and light microscopy were employed to document the results. The most successful catheter materials were next evaluated in man, using the same experimental parameters. It is our belief that the observed thrombosis associated with morbidity is largely due to the surface characteristics of the catheter materials. These characteristics are largely surface phenomena related to the interactions between the tissue and catheter material. In this study, we have attempted to determine whether there are any significant differences in thrombogenicity and/or long-term function between the several currently available catheters. Based on this study, ethylene acrylic acid appears to be the most nonthrombogenic surface of the material tested.
