Isolated subaxial cervical spine stenosis in achondroplasia.

Although isolated subaxial cervical spine stenosis in achondroplasia is less common than narrowing at the foramen magnum, thoracolumbar junction, or lumbar spine, it should be recognized as a distinct entity. These congenital changes usually become symptomatic when combined with degenerative changes in early adult life. Yearly monitoring of adult achondroplastic patients for signs and symptoms of cervical myelopathy or radiculopathy is recommended. When this develops, early surgical decompression should be considered, as these patients generally do not respond well to nonoperative treatment. The altered anatomy and small size make these cases particularly challenging. A thorough history and examination and complete diagnostic testing are necessary to gain an understanding of the exact etiology of the symptoms. Wide laminectomies and foraminotomies are then usually required to decompress the spinal cord and nerve roots. Fusion using internal fixation may be required to stabilize the spine if excessive bone is removed. (Curr Surg 57:354-356)

Primary malignant melanoma of the oesophagus.

Primary malignant melanoma of the oesophagus is a rare tumour, whereas metastatic disease originating from melanomas is occasionally encountered in the upper GI-tract. Two cases of endoscopically diagnosed primary esophageal melanomas are presented which were treated by surgery and radiotherapy combined with immunostimulation, respectively.

New methods in gastroenterology.

Major advances in gastroenterology are due in part to the rapid development of fiberendoscopes. Originally intended to improve gastroenterological diagnostics, the field of application was broadened by a variety of therapeutic procedures which now concur with the corresponding surgical approach. Endoscopic electro- and photocoagulation has considerably improved the outcome of acute gastrointestinal hemorrhage: endoscopic polypectomy is the procedure of choice today in benign and occasionally in malignant bowel tumors. Biliary tract surgery was revolutionized by endoscopic sphincterotomy, offering a low-risk procedure in high-risk patients with common bile duct stones. Endoscopic treatment of chronic pancreatitis by duct occlusion is just the beginning, and the old dream of dissolving gallstones rapidly by perfusing the biliary system with litholytic agents is now reality. Finally, the transhepatic or internal drainage in obstructive jaundice leads to transitory preoperative or permanent relief in malignant blockade of bile flow.

Functional lymphangiectasia of the duodenal mucosa.

Tiny millet-like prominent villi with dilated lymphatics may be part of a food-induced functional lymphangiectasia. Peroral olive oil load causes diffuse transient lymphangiectasia, the gross aspect, histological findings and electron-microscopy of which is described in detail.
