A total of 161 cases of choroidal metastases are reviewed. Breast or bronchial carcinomas are the most frequent primary tumors. One-third of the patients develop bilateral choroidal metastases. In 67% of all cases metastatic choroidal lesions were found in the posterior pole. Eighty-nine percent of all patients had a history of malignant tumor, in 11% of all patients choroidal metastasis was the first sign of a primary carcinoma elsewhere in the body. Sixty percent of all patients with choroidal metastases had additional other distant metastases at the time of diagnosis of choroidal involvement. The survival rate is poor: 50% of all patients with choroidal metastasis had died 1 year later. In 35% of all patients, choroidal metastases occurred despite prior systemic therapy with cytotoxic drugs or hormone therapy. In many cases the spontaneously course leads to irreversible loss of central visual acuity due to tumor growth, tumor necrosis or the complications of an exudative retinal detachment. Early fractionated external beam in radiation may prevent this outcome. The clinical course in 71 eyes following irradiation is reported. In 80% radiation therapy led to regression or eradication of the tumor, and a vision of at least 0.4 was maintained in 61% of the treated eyes.
It exists the possibility in the neutron therapy of specific tumours to increase the local energy dose in the tumour by treatment with fast neutrons under use of B-10. Thermal neutrons cause the neutron capture reaction B-10(n, alpha)Li-7 in tissue. A sufficiently high number of thermic neutrons is necessary combined with a possibly selective concentration of B-10 in the tumour as target volume for the clinical application. Gold foils were activated in a water phantom at several depths for the quantitative determination of the thermal neutron fluence. A result of this measurements is a maximal thermal neutron fluence of 1.44 X 10(10) cm-2 at depth of 5 cm with applied total energy dose of 0.8 Gy.

Advanced malignant melanomas of the uvea unsuited for an eye salvaging approach require enucleation of the tumor containing eye. A series of 68 patients is reported who underwent enucleation combined with insertion of a spherical dura-encased implant after 30 Gray pre-irradiation therapy of the orbit. Postoperative results with special attention to cosmetic outcome and motility of the prosthesis suggest that the insertion of an orbital implant should be preferred to the enucleation with no implant.
The results of animal experiments and clinical studies concerning preoperative radiation of malignant extraocular tumors suggest that pre-enucleation radiation may be helpful in reducing the incidence of tumor-related death after enucleation of an eye containing a melanoma. Based on a retrospective study of 80 patients with malignant melanomas of the choroid or ciliary body, in which pre-enucleation irradiation with 8.5 Gray or 20 Gray was performed, no beneficial effect could be found in terms of better survival. The need for a prospective, randomized and possibly multicentered study is emphasized.

From 1973 through 1986, twelve patients suffering from carcinomas of the outer auditory canal were irradiated at the Radiologic Hospital of Essen. The five-year survival was 64% after postoperative irradiation and 0% after irradiation alone. Considering the data communicated by other authors, the following therapy recommendations can be deduced: early stages where bones or cartilages are not involved represent an indication for sole operation. These tumors can be successfully treated by sole radiotherapy, too. In case of greater tumors, combined surgery and postoperative radiotherapy is the most favorable method. Besides a high total dose (50 to 60 Gy for microscopic tumor residues, about 70 Gy for macroscopic tumor residues), a sufficiently great target volume has to be chosen which includes the ear, the temporal bone, the preauricular lymph nodes and the angle of mandible. To prevent bone and cartilage necroses, it is recommended to restrict the individual doses to 1.8 to 2 Gy each.

High-resolution computerized tomography of the orbit was performed in patients with histologically proven retinoblastoma to assess the efficacy of CT in delineating the retro-orbital extent of the tumor and determining if the optical nerve was involved. High-resolution CT proved able to accurately delineate tumor size, tumor localization, tumor calcification, retro-orbital and intracranial tumor growth. Microscopical tumor spread past the lamina cribrosa of the optic nerve, however, could not be detected by CT.

From 1978 to 1982, 45 postoperative cases of breast cancer with lymphatic spread received radiation to the chestwall and axillary, supra- and infraclavicular lymph nodes up to 40 Gy. In addition, we gave 600 mg cyclophosphamide, 50 mg methotrexate and 750 mg 5-fluorouracil intravenously on day 1 of altogether nine 21-day cycles. The median follow-up is 37 months, and we calculate a 5-year over-all actuarial survival-rate which was 83% in the premenopausal and 77% in the postmenopausal patients. The disease-free 5-year survival-rate was 74% for the premenopausal and 61% for the postmenopausal women. There were twice as many patients with stage N2 in the postmenopausal group. No severe side-effects were observed. These promising preliminary results and the good tolerance of the above-mentioned combined therapy recommend it for future randomized studies.
Secondary malignancies are a rare complication after irradiation. 31 observations of such tumours are described, which appeared 4 to 56 years after radiotherapy. The NSD values varied between 400 and 3500 ret. 19 patients developed sarcomas and 12 others carcinomas. The hypotheses of radiation induced tumours and their clinical relevance are discussed. Three patient groups at risk may be distinguished: 1. children, 2. patients who received several treatment series, 3. patients who received combined radio- and chemotherapy.
During performing a lumbar myelography the contrast medium, which was injected correctly, suddenly disappeared. By measuring the blood-iodine concentration a flow of contrasting medium into the venous system was found. This phenomenon may explain an acute allergic reaction.
From 1970 through 1981 232 women with breast cancer (190 with T1 N0 M0 and 242 with T2 N0 M0 disease) received postoperative radiotherapy . The usual surgical procedure was simple mastectomy with axillary lymph node dissection. The chest wall was irradiated using 137Caesium up to 40 Gy, while supraclavicular, infraclavicular and axillary lymph node areas were irradiated using 5,7 MeV photons up to 45 Gy. The rate of local recurrence was 2,4% and 3,3% in stage T1 and T2 respectively. The corresponding projected ten-year-survival was 88% and 78%, respectively. These data and results obtained by others may strengthen the grounds for recommending conservative surgical approaches in early stage breast cancer, in combination with tumoricidal radiotherapy, individualized according to the anatomic tumor site.
A 4.5 year analysis of neutron and neutron-boost therapy is presented for 139 patients with soft tissue sarcomas. The actuarial survival rate of all patients is 64.3%, the disease-free survival rate is 61.6%. Significant differences in local control rates were found as a function of tumour stage (100% for T1, 72% for T2 and 46% for T3; p: 0.018) and as a function of the presence (50.5%) or absence of clinical evidence of tumour at the time of irradiation (76%; p: 0.002). The corresponding actuarial survival rates are 90.1% for T1 tumours, 78.5% for T2 tumours and 51.2% for T3 tumours, and on the other hand, 50.5% for patients with gross tumour and 76% for patients with no clinical evidence of tumour. A rather high morbidity rate (31/111, i.e. 28%) was observed after full neutron treatment. In order to reduce this complication rate, a combination of photons and a neutron boost was adopted. On 28 patients treated up to now, only two severe complications were observed. The effectiveness of this treatment regimen is presently being evaluated in a prospective European Organization for Research on Treatment of Cancer (EORTC) trial.
Report of an investigative study concerning 33 patients suffering from tumors and inflammatory lesions of the pelvis examined by computed tomography. These findings have been compared with the results of conventional radiologic techniques. For many reasons CT of the pelvis shows considerable advantages to all other diagnostic radiologic methods. Therefore CT evaluation is now a standard method before and after surgery.
