[Pleomorphic parotid adenomas and their recurrence].

Even today, a wide variety of techniques are performed in parotid pleomorphic adenoma surgery, ranging from enucleation to total parotidectomy. This study reports the results of our experiences. Among a total of 587 surgically treated parotid lesions at the Homburg/Saar University ENT Department from 1968 through 1991, 205 were parotid pleomorphic adenomas. Among these, 166 patients received primary surgery at our department. Twenty-one enucleations led to 3 recurrent tumors (14%), while 108 superficial parotidectomies had 3 recurrences (2.8%). Fifteen extended superficial parotidectomies also produced 3 recurrences (20%) and 22 total parotidectomies had one recurrent case (4.5%). The overall recurrence rate was 6%, but increased to 8.3% in 5 years, 9.5% in 10 years and 12.5% in 15 years follow-up. The "true" overall recurrence rate after 10 years was estimated to be between 9.5 and 20% and that for lateral parotidectomies between 4.4 and 13%. Patients younger than 30 years of age at the time of primary surgery developed a tumor recurrence in 17% of all cases. The overall recurrence rate following secondary surgery was 13%. Clinical findings and surgical techniques for resections of pleomorphic adenomas and their recurrences are discussed. Segmental parotidectomy is the preferred treatment, but may have to be changed locally to enucleation in some cases in order to preserve the facial nerve.

[Combined surgical flaps for simultaneous reconstruction of the tongue, mouth floor and pharynx in expanded glossectomy within the scope of multi-modal treatment of squamous epithelial cancers of UICC stage IV].

Multimodal approach and improved surgical techniques enable the head and neck surgeon to operate even far advanced tumours of the head and neck under curative intention. Microanastomosed distant flaps and intestinal grafts expanded the operative spectrum while having the disadvantages of being time-consuming and in most cases the necessity to call for the specialist (e.g. the abdominal, vascular surgeon). In 6 cases of total glossectomy combined with hemipharyngectomy and hemimandibulectomy the possibilities of closing large defects by combined regional and pedicled flaps in the head and neck area are shown. In all of our cases surgical therapy had been placed at the end of therapeutic interventions in a multimodal treatment protocol. Treatment started with induction chemotherapy with a regimen including cisplatin, bleomycin and vindesin sulfate and was followed by a subsequent radiotherapy reaching a total reference dose of 60 to 70 Gy. After radiotherapy 5 patients received adjuvant chemotherapy of 1-4 cycles. Despite the aggressive presurgical treatment, the postoperative complications were minor. In a median survival of 37.5 months there were 3 recurrences, 2 patients died of tumour progression. All of our patients benefited from of the "salvage-operation" especially with regard to tumour pain. Quality of the patients' life could be improved upon by percutaneous endoscopic gatrostomy (PEG) and as well good functional and cosmetic results of the surgical reconstructions. The results justify the discussion about a palliative indication of extended surgical procedures and reconstructive surgery in far advanced head and neck cancer in the course of a multimodal treatment regimen.

Neural and infranuclear region changes in outer hair cells in acoustically exposed rabbits.

Rabbits were exposed to 100 dB pure-tone sound at 2 kHz for 2 h. Electron microscopy was used to study the relationship between changes in the outer hair cells, afferent nerve endings and efferent nerve endings. There was no relationship found between the degree of changes produced in the afferent nerve endings and that the outer hair cells. However, there was a relationship demonstrable between the degree of changes seen in the efferent nerve endings and the infranuclear region of the outer hair cells. These findings show that acoustic trauma will damage most the infranuclear region of the outer hair cells, while efferent nerve endings are injured next and least affected are afferent nerve endings.

[Differential diagnostic observations on acute sialadenitis].

62 inpatients suffering from an acute suppurative sialadenitis of the parotid and submaxillary glands were treated at the ENT clinic of the University of Homburg/Saar between 1982 and 1988. In general, the anamnesis is short. Typical findings are fully elastic, immobile, painful swellings of the inflamed salivary gland. In most cases, staphylococci could be identified by microbiological examination as the causative germs. As therapy, high-dose antibiotic treatment is necessary. In case of an abscess or a phlegmon, incision and drainage has also been performed. Differential diagnosis of an acute purulent sialadenitis is discussed and listed in tabular form.

[Lymphadenitis toxoplasmotica--a zoonosis].

Between 1975 and 1985, 49 in-patients suffering from Toxoplasmosis of head and neck lymph nodes, have been treated chirurgically at the ENT clinics of the universities of Cologne and Homburg/Saar. In the collective there are as much men as women. The patients have an average age of 30 years, the anamnesis is about 3 months. The patients were complaining of swellings of lymph nodes of the angle of mandible or of the submandibular region, but without fever or other signs of infection. Typically, the lymph nodes were not fixed but mobile, hard and indolent. Blood parameters and electrolytic levels are not pathologically changed. The patients do not feel sick in general. The diagnosis is to be confirmed by serological investigations, supported by lymph node histology (Lymphadenitis Piringer-Kuchinka). In the present study, three acute cases of Toxoplasmosis with high serological titers could be diagnosed. The differential diagnosis of the lymphadenitis toxoplasmotica will be discussed.

Timentin--clinical and pharmacokinetic evaluation in otorhinolaryngology.

The efficacy and tolerance of Timentin was studied in 41 patients with various infections of the ears, nose and throat. The pharmacokinetics of Timentin were determined in pus, mucosa and polyps of the nose and paranasal sinuses, the tonsils and submandibular gland as well as in the interstitial fluid collected in Redon vacuum drainage. Experimental pharmacokinetic studies in the guinea pig were performed in the inner ear and eye fluids and in serum. Ticarcillin and clavulanic acid concentrations were determined by a microbiological micromethod. The clinical studies demonstrated that Timentin was very effective and well tolerated. The clinical and experimental pharmacokinetic studies provide a good explanation of this high degree of effectiveness.

Electron microscopical studies on the effect of lapsed time on the nerve endings of the outer hair cells in acoustically exposed rabbits.

Rabbits were exposed to pure tone (2 kHz, 100 dB for 2 h) and the changes in the nerve endings of the outer hair cells over a period of 1 month were studied using the electron microscope. In the afferent nerve endings mitochondria dilated and small vesicles decreased in number immediately after acoustic exposure. However, the afferent nerve endings recovered during the period of 1 month. In the efferent nerve endings mitochondria dilated and were distributed throughout the nerve endings, and small vesicles decreased in number immediately after acoustic exposure. The efferent nerve endings recovered more slowly than afferent nerve endings. In addition, the changes in the agglomerations of synaptic vesicles along the synaptic membrane and the gap-structures in the synaptic cleft in the efferent nerve endings after acoustic exposure are briefly discussed.

[Current status of serial sialography and sialoscintigraphy in the diagnosis of salivary gland diseases].

Sialographic investigation of enlarged salivary glands contribute to the important decision whether a secretion blockage is caused by a stone, chronic inflammation, sialoadenosis, or a tumour. The results of sialographic investigations have been much improved by the method described by Brands and Schnepper (1967) of specific serial sialography using a single-shot and fast-repeating camera, under fluoroscopic control. The success rate of the sialographic diagnosis on 141 patients in the ENT Department of Saarland University Hospital was 80%. The fact that in four cases spaces, occupying lesions were falsely interpreted or undetected leads to the conclusion that where a tumour is suspected negative sialography should be followed by further diagnostic steps. Scintigraphy of the salivary glands with 4 mCi 99mTechnetium Pertechnetate produces extra information which clearly increases diagnostic accuracy. However, experience to date shows that it is not an alternative to serial sialography but rather a supporting diagnostic method.

Electron microscopical studies on the nerve endings of the outer hair cells in acoustically exposed rabbits.

The afferent and efferent nerve endings of the outer hair cells of the rabbit after acoustic exposure were investigated under the electron microscope. After acoustic stimulation, the afferent and efferent nerve endings of normal range were respectively 57 per cent and 64 per cent. This result suggests that half of the afferent and efferent nerve endings maintain their function under the conditions of this experiment. An abnormal inclusion body forming an oblong dense body and cistern in addition to the postsynaptic cisterna was observed. The active zone and the vesicular gap-structure of the synapse are discussed.

Cefsulodin pharmacokinetics and otitis media.

The pharmacokinetics of cefsulodin in the fluids of the ear and eye as well as in serum were studied after intramuscular administration of 200 and 400 mg/kg to eight groups of six guinea pigs. A retention of cefsulodin in the inner ear-similar to that of the aminoglycoside antibiotics - was observed. As the measured perilymph concentrations of cefsulodin and gentamicin are comparable on a weight for weight basis and as the clinical dosage of cefsulodin is much higher than that of gentamicin, the perilymph concentrations of cefsulodin reached in man are probably much higher than those of gentamicin. The results obtained are compared to those of mezlocillin, azlocillin and fosfomycin, Finally, the importance of the cefsulodin perilymph concentrations in the treatment and prophylaxis of infectious inner ear complications due to the most common pathogens of chronic otitis media as well as in the treatment of certain eye infections is discussed.

[Pharmacokinetics of fosfomycin and otitis media].

The pharmacokinetics of fosfomycin in the ear and eye fluids and in serum are studied after intramuscular administration of 300 and 600 mg per kg respectively to 11 groups of 7 guniea pigs each. Maximal fosfomycin concentrations in the perilymph and aqueous humour are about 12% of the serum peak concentrations and the aqueous humour concentrations are similar to those of the aminoglycoside antibiotics, whereas the fosfomycin concentrations in the perilymph are twice as high as those in the aminoglycoside antibiotics. A retention of fosfomycin in the perilymph, similar to that of the aminoglycoside antibiotics, can be observed. The fosfomycin half-lives in serum, aqueous humour and perilymph are 1.1, 3,7 and 4.9 hours, respectively. The discussion mainly concerns the validity, after suitable adaptation, of the experimental results and their clinical importance for therapy and prophylaxis of the infectious inner ear complications due to the usual germs of chronic otitis media as well as for the treatment of certain eye infections.

Afferent and efferent nerve endings of the outer hair cells in the rabbit.

The afferent and efferent nerve endings of the outer hair cells of the rabbit were investigated electron-microscopically. The afferent nerve ending was 1.4 X 2.0 microns in size and contained small vesicles, spherical mitochondria, and occasionally multivesicular bodies. The partially thick and dense membranes on either side of the synaptic cleft were observed and this region of the synaptic cleft was high in electron density. This area was considered to be an active zone of the afferent nerve ending. The efferent nerve ending was 4.0 X 6.0 microns in size and contained many synaptic vesicles, many oblong mitochondria, and multivesicular bodies. Synaptic vesicles were concentrated at some sites opposite the synaptic cleft. This area was thought to be an active zone where vesicles discharge their contents into the synaptic cleft. Furthermore, in some places of the synaptic cleft there were gaps (0.16 X 0.68 microns) which contained small vesicles. The gaps in the synaptic cleft have been briefly discussed.
