[Infected chorionic hematoma as a cause of infection in the 2nd trimester].

Superinfected subchorionic haematomas are a rare septic focus in the 2nd trimenon. Symptoms being unspecific, the diagnosis has to be made by exclusion, in most cases. As the changes of a successful treatment of the manifest infection is poor, antibiotic prophylaxis as well as close laboratory controls and early antibiotic therapy should be discussed after sonographic diagnosis of an intrauterine haematoma. Two of our three patients reported on having suffered a miscarriage; only one pregnancy could be maintained after spontaneous depletion of the infected haemorrhage.

[The computer-assisted processing of CT data in planning endosseous implantation interventions].

The success of dental implantologic measures depends considerably on the correct positioning of the endosseous implants. A sufficient amount of bony substance is mandatory to provide overall covering of the implant with at least 1 mm of cortical bone. By the aid of a custom-developed software system, computed tomography image data can be handled on a conventional MS-DOS personal computer. The user is enabled to simulate size and positioning of the implant by interactive graphic animation. Quick and exact presentation is achieved by optimizing the computation algorithms. Storage needs are minimized by a segmentation procedure and additional compression of the image. Integrated patient data management and user friendly self-documenting program surfaces provide easy application.

[Suitability of metric structural criteria in vaginal ultrasound assessment of the postmenopausal endometrium].

In 135 postmenopausal women ultrasonic findings of the endometrium were compared with the final histopathology. Object of this study was to evaluate the ability of metric structural criterions for a vaginosonographic screening of benign and malignant endometrial neoplasms. A sonographic assessment of the endometrium was possible in 97.8% of the patients; anatomic peculiarities such as calcified fibromyomata proved to be the main-impediments. By means of the criterions examined the sensitivity for the differentiation of inconspicuous and pathological findings was 100% with a 98.2% specificity; the rate of wrong-positive results was 1.6%. In 9.2% ultrasound was classified as conspicuous requiring further controls. According to these results the vaginosonographic assessment of the endometrium regarding its thickness and structure (homogeneity, echogeneity, median echo) can be recommended for a non-invasive screening for endometrial neoplasms.

[Prognostic significance of intrauterine hematomas in the 1st and 2d trimester for the course of pregnancy and labor].

The courses of 66 pregnancies with sonographic evidence of an intrauterine haematoma within the egg-membranes during threatened abortion, between 7th and 24th week of gestation, were compared to the courses of 55 patients at the same gestational age admitted to our hospital, during the same period, with vaginal bleeding only. The aim of our study was to determine the prognostic significance of intrauterine haematomata in the I. and II. trimester for the course of pregnancy and delivery. The abortion-rate after intrauterine haematomata was significantly increased to 17.5%, a high percentage of them taking a febrile or septic course. The rate of preterm labour in haematoma patients was significantly higher (34.5 compared to 12.7%, respectively), than that in patients, who were only bleeding; this, furthermore, was reflected by a strong trend of preterm delivery (21.9 compared to 12.9% resp.). The caesarean section rate of 55.8% in the haematoma group was found to be significantly higher than in the control group (16.7%), mainly due to a significantly increased proportion (23.1%) of abnormal foetal presentations. Abnormal placental stages had to be treated significantly more often after intrauterine haematomata (32.6%) than in the control group (9.3%); however, no differences could be found between the two groups in respect of metabolic or respiratory placental dysfunction. The parameters examined in this study were not influenced by the volume of the haematoma, although the spectrum of complications seemed to be correlated to the gestational age at the time of diagnosis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Peroral and transhepatic cholangioscopy].

Cholangioscopy is possible via the peroral or the transhepatic route. Cholangioscopy is of diagnostic value in tumour strictures and stenoses of the common bile duct. More important than diagnostic cholangioscopy is therapeutic cholangioscopy: with the technique of intracorporeal shockwave lithotripsy nearly all difficult CBD-stones may be successfully treated. 17 patients with 40 CBD-stones were managed in mother-babytechnique in applicating intraductal shockwaves under direct visual control (electrohydraulic principle). Mechanical lithotripsy during ERCP had formerly failed in all patients. In 6 patients transhepatic cholangioscopy with intracorporeal shockwave lithotripsy was successfully performed. Patients had an operated stomach, so conventional ERCP with lithotripsy was not possible. 18 stones could be treated. As a total 23 patients with 58 CBD-Stones underwent cholangioscopy (median stone diameter 20 mm) with intracorporeal shockwave lithotripsy. Complete stone clearance was achieved in all cases, no serious complication occurred.

[Differential typology among patients with hard and soft suicide methods].

Two samples of patients admitted during one year after suicide attempts were compared: patients of the University Surgical Department in Heidelberg and a random sample of those who were admitted to the detoxication ward of the University Department for Internal Medicine. The frequency of patients who used "hard" methods was 10% of those using "soft" methods. Patients with hard suicide methods displayed more social desintegration, were more often psychiatrically ill, had a shorter presuicidal phase, a shorter time span from decision to execution of the suicidal attempt, more negative self-esteem and more chronic suicidality. Aggressiveness was low in both groups. Although the number of patients with "hard" suicidal methods is small and does not justify a special service in the Surgical Hospital, these patients need particularly careful supervision because of their psychopathological and psychosocial profile.

[Percutaneous transhepatic cholangioscopy as a single-step technique].

The PTCS is a method for diagnostic and therapeutic purpose in patients with common bile duct diseases not suitable for transpapillary approach. The present paper describes a technique which shortens the usual time of treatment and reduces injury. Lithotripsy of common bile duct stones, endoscopic controlled biopsy, introduction and removal of bile duct endoprosthesis as well as percutaneous papillotomy is possible.

Single-step percutaneous transhepatic cholecystoscopy.

A 58-year-old female patient with several concrements in the gallbladder submitted herself to pump-controlled percutaneous transhepatic chemolitholysis with methyl-tert-butyl-ether (MTBE). Shortly before the treatment terminated a calcified residual concrement caused an acute blockage of the cystic duct. After opening the puncture track with a bougie, followed by endoscopic lithotripsy, the cystic duct was made passable again.

[The value of various parameters for ultrasound assessment of the postmenopausal endometrium with reference to benign and malignant neoplasms].

In 101 postmenopausal patients, ultrasonic findings of endometrium were compared with the histopathology. Object of this study was the evaluation of several sonomorphological parameters for detection of benign and malignant endometrial neoplasms in order to derive criteria for an ultrasonic screening of these lesions in postmenopausal women. The maximum endometrial sonographic thickness was found to be the main parameter, yet was itself too insensitive and unspecific as a single parameter. Homogeneity, echogeneity and presence of a median echo, represent important additional parameters. The sonographic longitudinal diameter of the uterus cannot serve as a screening parameter similar to the amplified ultrasonic intensity through the endometrium and the low resonant subendometrial zone, but defines a collective risk referring to endometrial neoplasms and hyperplasias. The criteria derived from this are very sensitive and specific in our sample. That is the reason, why we think, that ultrasonic examination of the endometrium is an important step in early diagnosis of endometrial carcinoma.

[The influence of premedication with dipotassium chlorazepate on preoperative stress and postoperative pain].

Despite the fact that dipotassium chlorazepate (DC) is widely used for oral premedication there are few studies demonstrating the effects of this drug reliably in the preoperative patient. Therefore, we investigated the influence of DC on preoperative sleep, stress and postoperative pain, which is presumed to be influenced by the long-acting anxiolytic property of DC compared to a placebo in a double-blind study. Sixty patients undergoing dental surgery received either 20 mg DC or a placebo in the evening and at 7 o'clock in the morning before the operation, both administered orally. The quality of sleep, preoperative anxiety, depression, asthenia and postoperative pain were assessed, using nominal and visual analogous scales. The degree of sedation and the quality of premedication were assessed by the anesthesiologist. Blood pressure and heart rate were registered and interpreted as physiological stress parameters. The 2 groups were comparable with respect to the anthropometric data as well as the initial values of all other parameters. Patients receiving DC reported significantly better quality of sleep in the night before the operation. There was no difference between DC and P concerning preoperative stress either emotionally or physiologically. The postoperative pain intensity did not differ between the two groups. The quality of premedication, assessed by the observer, was comparable in both groups. Whereas DC has a positive effect on the quality of sleep in preoperative patients, it does not influence the preoperative stress on the day of operation. This might be dose-dependent, but 20 mg is recommended for premedication.(ABSTRACT TRUNCATED AT 250 WORDS)

["Aachen 3-D-finger". Development of a 3-D-digitizer for use in dental, oral and maxillary treatment].

The "AACHEN 3D Finger" is a three-dimensional measuring system for use in all fields of dentistry. The system can equally as well be installed on a plane table, as fixed to the head of a patient. The measuring device is computer-assisted, and is able to localize, register and calculate any combination of points in the oral and maxillofacial area. The reference system can be changed at any time. The "AACHEN 3D Finger" can be used as a computer-a ded system in dentistry as well as in implantology or dental and maxillofacial surgery.
