[Early results of the surgical treatment of stomach cancer. Study of 752 patients during the period 1967-1977].

Investigations at 752 patients in the ten-year-period 1967/77. The early results founded out at 752 cases of cancer of the stomach are the following: Operative quote 93,5%, resection for cure 51,2%, postoperative lethality after radical and palliative surgery with elimination of tumour 18,4% and 42,4% respectively. The relations of the lethality to age and kind of operation and the reasons are discussed.

[Prognostic significance of cellular stromal reaction at the tumor margin and the reaction in the regional lymph nodes in rectal adenocarcinoma. Patient follow-up studies after curative abdominoperineal rectal amputation].

A significant correlation was found between the prognosis, expressed by the tumorfree survival time after resection of the rectum and the stage of tumor, the grading of histological differentiation, and the cellular stromal infiltration at the margin of the tumor. The stage of the tumor ranked first in influence on prognosis followed, unexpectedly, by the granulocytic stromal reaction, relative lymphocyte value in the peripheral blood, the histological grading, and the adjuvant therapy variants. The prognostic relevance of the immunologic reactions in the regional lymph nodes (follicular hyperplasia, paracortical macrophage activity, sinushistiocytosis) and of the round-cellular stromal reaction at the margin of the tumor can be seen in the correlation analysis but is no longer evident in the steplike multiple regression analysis.

[Metabolic problems of malnutrition in cancer patients and the treatment by parenteral substitution (author's transl)].

The central and primary metabolic disturbance of the host's turnover by the tumor introducing a malnutrition syndrome is the increased energy expenditure of the tumor within an uneconomical glycolysis and lactate recycling and an increased amino acid requirement in consequence of an uncontrolled protein synthesis and of an amino acid consumption by gluconeogenesis. These are the explanations of the early signs of malnutrition for instance deficit of albumin and consumption of the energy depots. In contrast further reasons of the malnutrition (anorexia, gastrointestinal passage and resorption decreasing) are to be considered as secondary. If there are objective findings of malnutrition is to postulate a nutritional correction by substitution before the tumor therapy is beginning. Therefore the parenteral infusion of amino acid solutions without carbohydrates and possible in combination with fat solutions is to be recommended. Besides tolerability and efficiency of chemotherapy or radiotherapy can be improved by such parenteral substitution.

[Reasons for the late diagnosis of rectal cancer].

At present, patients with rectal carcinoma are admitted for clinical treatment only 8-10 months following the appearance of typical symptoms. In 115 patients followed up the tumor size was on an average 5 cm. In 79 cases (68.7%) the tumor could be revealed in manual examination of the rectum, however, the correct diagnosis was established not earlier than in rectosigmoid tumors. In 81 patients (70,5%) the tumor proved to be inoperable, metastases in distant regional lymphnodes being revealed in 34 cases. Thus, only 47 of 115 patients (40.8%) were hospitalized with the lesion in prognostically favourable stages. The perspectives for improving the results consist in shortening of the period from the appearance of complaints till the onset of treatment. A prerequisite for the latter is an early diagnosis of rectal carcinoma based on a number of consecutive diagnostic procedures. In some cases, it is possible to establish the diagnosis by manual examination of the rectum. Negative results of this examination do not exclude the diagnosis of the tumor.

[Early gastric carcinoma; surgical problems (author's transl)].

In the period from 1965 to 1979 63 patients suffering from early gastric cancer underwent resection, i.e. 7,4% of all resected cases of cancer of the stomach in our hospital. The 3 year survival rate came up to 80%, 5 years survival was 75%. The postoperative mortality rate was less than 5%, since 1975 no postoperative mortality. Distal resection was performed in 74%, gastrectomy in 11%, upper partial resection in 6%, gastrotomy in combination with polypectomy in 9%.

Xenogeneic transplantation of human tumors of the gastro-intestinal tract in nude mice.

The transplantation of human adenocarcinomas of the gastrointestinal tract into nude mice is described. The human primary tumors were located in the stomach, colon or rectum. Successively transplantable lines could be established by means of implantations of tumor blocks in the left abdominal wall of nude mice. Some biological and histopathological criteria of the implanted tumors as compared to their human origins are given. Several experiments concerning the dependence of tumor growth in nude mice upon the region of implantation are reported. Moreover, first results with regard to development of tumor recidives in nude mice after resection are presented.

[Treatment of rectal cancer (author's transl)].

The rate of neglected diagnoses of rectal cancer today will not be lowered by mass screening only but especially by a more consequent use of wellknown diagnostic methods. Preoperative irradiation with 2000 rad seems to be able to improve the survival rate in stage Dukes B. cases. Anterior remaining anal sphincter is out of order or if it is impossible to guarantee radical surgery concerning the perirectal connective tissue, e. g. in stage Dukes C cases. Intraoperative local administration of cystostatic drugs can be recommended whereas a satisfactory effective adjuvant chemotherapy is unknown until now. The local excision of small tumours of stage Dukes A in the lower part of the rectum can be taken into consideration and has also good results as a palliative procedure because of functional inoperability. Postoperative control of CEA-titers can inspire the suspicion of recurrence or metastases already several months before they are clinically evident.

Antineoplastic Agents↗

[The CEA-concentration in plasma of patients with colorectal carcinoma and polyps has been determined by radioimmunoassay (RIA, Z-gel-method)].

82% of the cancer patients and 52% of the patients with polyps showed CEA levels above the upper normal limit of 2,5 microgram/1. CEA-values of more than 10 microgram/1 could only be detected in patients with carcinoma. Complete removal of the tumour by surgery has been followed by a marked decrease of prior increased plasma-CEA to values in the normal range, which could not be observed in the cases with tumour residues or metastases. In the patient's follow-up recurrence or metastases could be recognized several months before clinical detection by a renewed increased of the CEA-level in plasma.

Carcinoembryonic Antigen↗

[Incidence and prognosis of metastases in colonic and rectal carcinomas].

In colorectal carcinoma at the time of primary operation a much higher rate of occult micrometastases in the liver is to take in account as it is evident by the frequency of intraoperative found metastases (10--20%. Micrometastases likely can to need 3 to 8 years till they have increased so that the finally stage of generalization is to state. The survival rate of patients with colorectal carcinoma and hepatic metastases -- without therapy of metastases -- is 6 to 8 months and not differs from the survival rate of these patients whose primary tumor was inoperable by local reason. The today usual diagnostic procedures in the postoperative follow up control are able to detect liver metastases in most cases only in an inopportune stage for therapy. Probably the analysis of carcinoembryonic antigen is a progress in this question. The unique chance to block up or eliminate the frequently suspected micrometastases seems to be the general use of a post-operative adjuvant chemotherapy.

Antineoplastic Agents↗

[The therapeutic situation at the colorectal carcinoma (author's transl)].

With a period of 8--10 months between the beginning of troubles at the patient and the reception in hospital for treatment (= duration of anamnesis) the colorectal carcinoma patients come only still in 25--40% in the stages I and II to the therapy which then can be radical and curative. 20--30% of the patients at the time of diagnosis respectively at the start of therapy already are in stage IV with a 5-year-survival rate between 0 and 1%. An improvement of results is to expect from an earlier diagnosis, development of general arrangements for the decreasing of operative risk, improvement of anastomosis, postoperative adjuvant therapy, and an individual treatment scheduling.

[Parenteral protein substitution in tumor patients].

The substitution of protein deficiency by a standardized dosage of 48...60 g human albumin and 160 g amino acids divided over 4 days is hardly less effective as the substitution by the same dosage of amino acids alone. Insignificant differences of the preoperative und postoperative effect of the substitution therapy in patients with tumors of esophagus or stomach and colorectal cancer are discussed and seem to confirm the conclusion to perform the parenteral substitution of protein deficiency economical better only by the administration of amino acid solutions. The additional administration of non-protein calories in the meaning of parenteral nutrition seems to be dispensable if the simultaneous enteral alimentation satisfies these requirements. The relative substitution dosage of amino acids should not fall below 2,5 g . kg-1 divided over 4 days.
