[Expert assessment of pelvic injuries].

Progress in diagnostics and therapy of injuries of the pelvis made in the last few years also brought advantages in the assessment of longterm outcome. Despite correct anatomical reconstruction of the pelvic-ring lumbal and sacral pain is still a problem. The intensity of pain doesn't correlate with the grade of injury. So effects of pelvic fractures on static and dynamic of the musculoskeletal system and especially the function of the hip-joint can be assessed clearly. Furthermore neurological examinations done by specialist to find out specific deficits are of utmost importance. Advanced radiological and technical diagnostics like computer tomography are significant for the assessment of pelvic injuries as well as the clinical examination.

[Psychosocial problems in patients with chronic osteomyelitis].

52 patients suffering from a chronical bone infection were followed up with a special interest in psychosocial problems. In all cases psychosocial disturbances of different importance were evaluated. Possible causes have to be seen in long time of hospitalisation (average = 304 days), and apparent inadequate enlightenment of patients and in functionally and cosmetically insufficiencies. Conclusions are shown for the management in treatment of chronical bone infections.

[Penetrating heart injury].

A report was made on a 25 year old patient with a penetrating heart injury with basic diagnosis, and treatment. A successful management was a result of good performance of the resuscitation chain, exactness of the clinical diagnosis and interdisciplinary cooperation between surgeons and anaesthetists.

[Early results with Zespol osteosyntheses of the leg].

First experiences with the Polish osteosynthesis method "ZESPOL" are reported. Considering 28 osteosyntheses this method proved a success under selected indications. For the treatment of tibial shaft fractures the assembling in the sense of a monofixing agent is efficient. Besides a high but not rigid stability, minimum impairment, a limited but multilateral applicable apparatus and simple operating technique the high variability in consequense of external, subcutaneous and paraosseous placement possibility has to be mentioned. The early results of patients treated from february 1987 to august 1988 are good.

[Clinical diagnosis of fresh injuries of the knee joint].

Clinical diagnosis has to registrate fresh injuries. These have to undergo immediate operation or further instrumental or radiological procedures respectively. This may be effected in almost all cases by systematic evaluation of informations on the anamnesis and general or special clinical diagnosis. Main symptom of the frequent, complete tear of the anterior cruciate ligament is the Lachman-test. In addition to the clinical tests for instability out-of-order-proprioception may refer valuably to limited ligamentons tears. Results of early clinical diagnosis should be improved by close hospital and out-patient department cooperation and both collective workshops and interpretation of results as well.

[Efficacy of interlocking nailing in fractures of the leg].

Closed and Grade-I open multi-fragment, comminuted, and segmental fractures of the femoral and tibial shafts are well established indications for surgical stabilisation, using interlocking nailing. Eighty-two fractures of those categories in 78 patients were handled by the authors, between 1976 and 1987. High efficiency of the approach has been confirmed by early functional treatment, soon consolidation of fragments, within half a year, less complications as compared to other techniques as well as good functional end results which had been recorded in follow-up checks from 88 per cent of the patients involved (n = 66).

[Acute intestinal obstruction caused by congenital intestinal abnormalities].

By means of two examples--one in pregnancy, the other one after an abdominal hysterectomy--we report on a rare cause of an acute mechanical ileus. In both cases an incomplete intestinal turn with distinct mobility of the colon caused a volvulus. It is pointed to the importance of an early diagnosis inspite of aggravating circumstances in late pregnancy, parturition and postoperatively after gynecologic operations. Effective operative treatment is based on careful exploration of the abdomen in order not to overlook rare causes of an ileus, among those also fetal developmental disturbances of the intestine and other abdominal viscera. Only by operative removing of the causes of an ileus in cooperation with surgeons--and that in good time--the results in gynecology can be improved especially during pregnancy but also postoperatively.

[Advantages and problems in acute management of injuries of the capsular ligament system of the knee joint].

Between 1983 and 1985, immediate surgical treatment was received by 34 of 119 patients (28.5 per cent) admitted for acute injuries to capsular ligaments of the knee joint. Good results were achieved in 90 per cent of these cases. Immediate suspicion of acute capsular ligament injury on admission to the outpatient section or casualty ward, nodelay definite diagnosis, surgical action in acute condition as well as subtle and anatomically adequate restoration of all injured structures are the major conditions that must be satisfied for good success of this approved principle. Intensive physiotherapeutic after-care must be another integral component of a clearly defined therapeutic concept. Causes for delays in diagnosis and therapy are analysed, and conclusions are drawn.

[Osteomyelitis following surgical treatment of femoral shaft fractures. Results of a community study].

A retrospective analysis was made of 3,280 diaphyseal femur fracture surgically treated in 73 medical centres of the GDR. The conclusion drawn from this quality check was that the high osteomyelitis rate of 5.6 per cent had been primarily attributable to shortcomings in osteosynthesis techniques but also to inadequate choice of methods. The following conclusions may be derived from the study reduction of bone infections: The osteomyelitis rate in the wake of Küntscher's nailing was as low as 2.2 per cent. This was the best result in the context of simple fractures with closed soft-part trauma. The advantages of covered intramedullary nailing were particularly felt in cases in which optimum protection against infection was impossible for the given surrounding conditions. Plate osteosynthesis has proved to be the most effective approach to open fractures, provided careful wound dressing, non-invasive surgical techniques, and generous indication in favour of autogenic spongiosa transplantation. Interlocked nailing is considered to be the optional technique of osteosynthesis to cope with comminuted fractures. External stabilisation is urgently indicated in cases of massive traumatisation of the femoral diaphysis in conjunction with severe open lesions of soft parts.

[Treatment of fractures of the forearm in adults; present state (author's transl)].

Conservative treatment of fractures of the forearm is no longer in all cases the method of choice. It is only still acceptable in stable diaphyseal fractures of the radius or the ulna. In all other cases plate osteosynthesis should be performed. The interpretation of a questionnaire recording 261 forearm fractures collected from 18 Traumatological Depts of the GDR permits the conclusion that compression plating will yield the best results in cases of instable or open fractures of the forearm.
