Social work at the emergency department.

We have evaluated the level of satisfaction of patients assigned to a social worker whose role was to give information, support, advice, grief and shock counselling; and develop resource strategies for marginal peoples. Six hundred patients were admitted to the study; 300 of them were supervised by a social worker (group A) and the other 300 were not (group B). After the completion of the study, a questionnaire was sent to be filled in by group A and group B for later evaluation. We collected 200 completed questionnaires (33.3%). The most important areas were: 105 patients (94.5%) in group A vs. 73 patients (82%) (p < 0.005) consider the medical care good or very good. The waiting time was assessed as short or acceptable in 91 patients (81.9%) in group A vs. 55 patients (61.8%) in group B (p < 0.001). The psychosocial care was assessed as very good or good in 101 patients group A (90.9%) vs. 73 patients in group B (82%) (p < 0.5).

Computer-assisted CT-guided stereotactic biopsy and brachytherapy of brain tumors.

From March 1991 to September 1993, 26 patients (aged 4-78 years) with brain tumors (4 glioblastoma multiforme, 10 nonglioblastoma multiforme, 1 mixed oligoastrocytoma, 2 carniopharyngiomas, 2 meningiomas and 7 metastases) were treated with stereotactic techniques at the Centro Internacional de Restauración Neurológica, La Habana, Cuba. A total of 28 stereotactic surgical procedures were performed with no operative mortality; they included biopsies in all cases, 1 stereotactic microsurgical resection and 12 permanent implants of 192Ir, followed by external beam fractionated radiation therapy (40-60 Gy). The present paper shows that the combined use of a stereotactic approach, a comprehensive and reliable stereotactic dosimetric planning system, stereotactic brachytherapy with 192Ir and complementary percutaneous radiation treatment constitutes a promising strategy for brain tumor management.

[Auricular electrodermic reflex in humans: experimental study].

Several sets of experiments have been performed for the following purposes: 1) to show that spontaneous pain as well as thermalgesic stimuli provoke the auriculoelectrodermic reflex (AER); 2) to investigate the action of local, general and rachianaesthesia on the AER, and 3) finally, to know the afferent, efferent and spinal pathways of the AER. As painful stimulus was utilized noxius heat (45 degrees C during 30 seconds) and the AER wss registered by means of a Servoscope (Sedatelec). Local and rachianaesthesia suppressed the AER but the general anaesthesia decreased only its intensity. The study of the AER in paraplejic patients revealed that the thermalgesic impulses responsible for the AER, enter in the spinal cord not only through the dorsal root corresponding to the stimulated dermatome, but also through the 4-5 cranealer roots. On the other hand, the intraspinal pathway of the AER impulses is bilateral, with a slight ipsilateral predominance. The polyradicular input and spinal bilaterality difference the thermalgesic from the other nociceptive impulses. The results obtained in tetralejic patients allow to conclude that the integration of the AER is spinal, corresponding to the supraspinal centres a modulatory influence.
