Intracavernous injection of prostaglandin E1 in impotent men.

Intracavernous injection of prostaglandin E1 was used in 210 men as a screening test for the differential diagnosis of vasculogenic impotence. Of these 210 patients 112 entered an autoinjection protocol for treatment of erectile dysfunction. Prostaglandin E1 appears to be effective in the diagnosis and treatment of nonvasculogenic impotence because it is a physiological agent that is metabolized locally within the cavernous tissue. Additionally, in our series neither systemic reactions nor priapism occurred, nor was fibrosis of cavernous tissue or scar formation observed after up to 90 injections.

Extracorporeal shock wave lithotripsy performed on woman with a cardiac pacemaker.

A 73-year-old woman with a sick sinus syndrome and a 3-year-old ventricular demand pacemaker underwent extracorporeal shock wave lithotripsy without incident. There was no damage or malfunction of the pacemaker during or acutely after this therapy. The lithotriptor had no difficulty in sensing all paced or spontaneous ventricular complexes.

Late sequelae of the management of ureteral calculi with the ureterorenoscope.

We examined 42 patients 19 to 20 months after ureteral calculi had been removed with the ureterorenoscope. All 42 patients were asymptomatic and showed no evidence of an increased rate of urinary tract infection or hypertension. An excretory urogram in 38 patients demonstrated no abnormality related to the procedure, in particular no ureteral stenosis and renal scarring. Renal sonography in the remaining 4 patients likewise was normal. Reflux cystography revealed low grade (sterile) vesicoureteral reflux in 2 patients. In view of a 92 per cent success rate in 236 renoureteral units, with serious complications in only 2 per cent, the technique can be considered safe and effective.

[Nephrotomy closure with fibrin glue].

In 45 patients staghorn calculi were removed in hypothermic ischaemia by one or several nephrotomies. The renal parenchyma was without parenchymal sutures atraumatically treated with the fibrin adhesive and a continuous renal capsule suture. After this simple technique of the adhesion of renal parenchyma no complications which needed operation occurred.

Long-term results after sclerotherapy of the spermatic vein in patients with varicocele.

Ninety-seven patients received transfemoral sclerotherapy of the spermatic vein for varicocele. After observation for 2 to 5 years, follow-up was available in 69 patients (71.2%), in 11 of whom the procedure failed (16.0%). Complications during angiography (N = 12, 12.4%) or sclerotherapy (N = 31, 32.0%) did not require hospital treatment. At least one parameter of the spermiogram was improved in 25 of 32 patients (78.1%). Forty-four of 69 patients (63.8%) treated because of a childless marriage, and whose wives were not known to be infertile, had a pregnancy rate of 47.7%. Thi method may be considered equal to surgical procedures in achieving venous occlusion; furthermore it is simple and can be used on an outpatient basis without anesthesia.

Late sequelae of ultrasonic lithotripsy of renal calculi.

We examined 82 patients 12 to 43 (mean 22) months after renal calculi had been removed by ultrasonic lithotripsy. Recurrence was noted in 2 patients but this correlated to the previous stone-forming rate. Smaller fragments that had remained in 9 patients were passed in 2, removed percutaneously in 2 and unchanged in the remainder. There was no evidence of hypertension or urinary tract infection related to the procedure. There were 62 patients available for computerized tomography and 11 per cent of the kidneys showed minute calcifications not visible on plain films. Barely visible scarring was noted on the fibrous capsule posterior to the lower pole in 22 per cent of the cases and on the abdominal wall along the former nephrostomy tract in 46 per cent. However, a cortical scar was noted in only 1 kidney and no arteriovenous fistulas were found. On dimercapto-succinic acid scans it was impossible to localize the puncture site and serial 131iodine-hippurate renograms available in 18 patients showed a 7.6 plus or minus 2.8 per cent (mean plus or minus standard error of mean) increase in function. We conclude that percutaneous nephrolithotripsy with ultrasound is a safe and effective procedure with minimal late morbidity.

[Tissue damage during percutaneous ultrasonic lithotripsy].

The ultrasound lithotripsy for the percutaneous desintegration of renal calculi does not cause any irreversible injuries. The intensity of the ultrasound lithotriptor is clearly below the cavitation threshold. By aspiration the ultrasound probe injures the mucous membrane. A superficial oedema and submucous haemorrhages appear. Despite an amplitude of 50 mu no perforation could be obtained at the urinary bladder of the rabbit. In irrigation of the ultrasound probe with more than 20 ml disinfectant per minute a tissue-injuring heat development is prevented. In the Sister-chromatide-exchange test no mutagenic effect by influence of the ultrasound could be proved. Metal splinters are excreted with the urine. Infolded into the wall of the urinary bladder they are unchangedly to be proved after nine weeks, in contrast to the suture the histological reaction is, however, minimal.

Is the solitary kidney a privileged kidney?

In rabbits the renal function of solitary kidneys subjected to ischaemic damage was compared with that of kidneys damaged identically in the presence of a normal contralateral organ. In spite of compensatory hypertrophy of the solitary kidney, no improvement in its tolerance to ischaemia was observed. The solitary kidney is thus not a privileged kidney.

Serum concentrations of gonadotrophins in hypergonadotrophic patients following stimulation by a long-acting analogue of luteinizing hormone-releasing hormone.

The effect of D-Ser(TBU)6-EA10-LRH, a long-acting analogue of luteinizing hormone-releasing hormone (LRH), was studied in patients with hypergonadotrophism due to orchidectomy (n = 8) or due to Klinefelter's syndrome (n = 6). Patients orchidectomized less than 7 days prior to the administration of the compound presented with maximum concentrations of LH (63.8 +/- 29.9 mIU/ml) within 60 min following iv injection of the LRH-analogue (10 micrograms). This behaviour of LH was qualitatively similar to that seen in healthy men. In patients orchidectomized more than 40 days prior to the administration of the LRH-analogue and in patients with Klinefelter's syndrome the occurrence of maximum serum LH-concentrations (115.0 +/- 39.4 and 149.4 +/- 134.5 mIU/ml, respectively) was delayed up to 240-360 min following iv LRH-analogue. This pattern of LH secretion is similar to that of healthy women. No qualitative differences in stimulated FSH-concentrations were observed between the described groups of hypergonadotrophic patients. These findings demonstrate a time-dependent increase in the 'second pool' of LH following orchidectomy. The similar behaviour of stimulated LH-release in healthy women and in male patients with long-term hypergonadotrophic hypogonadism could indicate an augmented production of endogenous LRH in these individuals as compared to healthy men, providing an explanation for the sexually related differences in the LH-response upon the administration of the LRH-analogue.

Partial rat kidney resection using autologous fibrinogen thrombin adhesive system.

The Fibrinogen Thrombin Adhesive System (FTAS) enables local haemostasis to occur in parenchymatous organs without tissue damage. The aim of this study was to investigate the degradation of FTAS and the process of wound healing after partial kidney resection in rats using FTAS for induction of local haemostasis. In 28 rats partial kidney resection was performed bilaterally. Haemostasis was achieved with Fibrinogen Thrombin Adhesive System. Four experimental groups were formed. Group A (n = 3): Haemostasis with unlabelled FTAS, subcutaneous injection of 0.1 ml = 60 mu Ci Na 125I. Group B (n = 3): Haemostasis with unlabelled FTAS, subcutaneous injection of 0.1 ml = 60 mu Ci 125I FTAS. Group C (n = 6): Haemostasis with 125I labelled FTAS. Group D (n = 16): treated like Group C. In Groups A - C 125I-elimination in 24 h urine samples was determined with a gamma-scintillation counter. Pairs of animals in Group D were killed after 2, 6, 12 and 24 h and 3, 7, 14 and 21 days. Kidneys were examined under the light and electron microscope and by autoradiography. In animals of Groups B and C two peaks of 125I excretion were observed: one peak within the first 48 h postoperatively which corresponded to the amount of free iodine injected with FTAS (FTAS contains 15% free iodine); a second peak after 120 h which was most probably due to the degradation of FTAS. Fibrinolysis was not observed. FTAS was resorbed mainly by macrophages. The time course of wound healing paralleled that of physiological fibrinogen concentration. Renal parenchymal damage was not observed.

A new method of transfemoral testicular vein obliteration for varicocele using a balloon catheter.

Selective obliteration of the testicular vein as a treatment for varicocele is described in 51 patients. Twenty-five patients had a history of infertility. A coaxial catheter-balloon catheter system was used. The procedure was done on an outpatient basis. The complications and success rates are comparable to postoperative results. Four of the 25 patients whose primary indication for the procedure was infertility have fathered children.

[Treatment of infertile couples--an interdisciplinary task (author's transl)].

The results obtained by closest cooperation between gynaecologist and andrologist in the treatment of sterile marriages are reported. 127 couples were treated over a period of 20 months. Almost 100% of the males and 60% of the females were sub- or infertile. Treatment of both partners was meticulously coordinated in order to increase the success ratio. Interdisciplinary cooperation seemed to be advantageous particularly in homologous insemination. 23 pregnancies were achieved, i.e. 18.1%, despite the unpromising nature of this negatively selected material.
