A beam splitter on a phoropter permits fixation of optotypes while retinoscopy is being performed. The eye being examined can see the optotypes in the focus of a lens or on the wall behind the examiner by means of a periscope system. The great distance of the retinoscopic lens from the eye increases the retinoscopic phenomenon.

The difficulty of diagnosing the Takayasu syndrome is illustrated with reference to one of the authors' own cases. This rare disease can be the cause of misinterpretations and failure of treatment. In the case described, the patient was suffering from chronic vascular disease which had led not only to cerebral changes but also to an occlusion of the ciliary and retinal vessels.
Deformities in the orbital area can also be corrected by craniofacial surgery. Early surgery within the first year of life is indicated in cases with intracranial pressure if permanent damage to the eye is to be avoided. Otherwise, corrective surgery should be performed in early childhood, although it can also be performed on adult patients. More severe strabismus cannot be treated, but disturbance of lid function can be positively influenced. From the esthetic point of view considerable improvement can be achieved. These very protracted operations require intensive interdisciplinary cooperation.
