Magnetic resonance microscopy of water diffusion and edema during hypothermic preservation of rat kidneys.
Kidney degradation during hypothermic storage was studied on rat kidneys, using magnetic resonance microscopy. Poor storage, modeled by storage with lactated Ringer's solution, resulted in rapid kidney swelling, together with increased signal intensity and water diffusion coefficient in the cortical and medullary regions. Storage of kidneys in Euro-Collins solution resulted in slower swelling and no significant change in signal intensity and in the water diffusion coefficient. Storage with Belzer's solution resulted in shrinkage of the kidneys and no significant change in the diffusion coefficient of water over time. These changes correlated well with kidney degradation observed by histology, and show the potential of magnetic resonance imaging in quality assessment of kidney transplants.
Dose effects of temazepam in transient insomnia.
A transient insomnia model, the "first night" effect in the sleep laboratory, was used to assess the dose range for the hypnotic and sleep stage effects of temazepam. 201 healthy, normal subjects (97 men and 104 women), 21 to 49 years old, with no sleep complaints were studied. Each was randomly assigned to receive either placebo, 7.5, 15, or 30 mg temazepam hard gelatin capsules (Restoril) administered double-blind 30 min before bedtime on their first night in the sleep laboratory. Over the 8-h polysomnogram total sleep time and sleep efficiency increased significantly in a linear fashion with increasing doses of temazepam. Sleep tendency was significantly reduced by increasing doses again in a linear manner. Wake time during the sleep period was reduced significantly only by the higher dose. The percentage of stage 1 sleep was reduced and the percentage of stage 2 sleep was increased, each linearly with increasing doses. These data are the first to demonstrate the hypnotic effects of a 7.5 mg dose of temazepam and also support previous studies of 15 and 30 mg temazepam administered to chronic insomniacs. They also illustrate the utility of the "first night" effect as a model of transient insomnia.

Renal transplant obstruction by B cell lymphoproliferative disorder: case report and review of the literature.
We report a case of renal transplant ureteral obstruction caused by a cyclosporine-associated B cell lymphoproliferative disorder. The interaction of cyclosporine with Epstein-Barr virus infections and the predisposition towards B cell lymphoproliferative disorders are reviewed. The characterization of these tumors by deoxyribonucleic acid hybridization studies and a review of the clinical experience treating these disorders also are presented.
Transplantation using inverted renal unit and donor vena cava-iliac vein conduit to bypass recipient distal vena cava and iliac venous systems.
We describe a transplantation procedure using donor vena cava and iliac vein to bypass venous abnormalities in a recipient in whom conventional renal transplantation had failed. Considerations for choosing this procedure are reviewed.
Is serological evidence of infection with syphilis a contraindication to kidney donation? Case report and review of the literature.
We report successful transplantation of kidneys from a cadaveric donor with serological evidence of syphilis and no transmission of the disease. Existing guidelines prohibiting the use of serologically positive donors and experience with other forms of iatrogenic transmission of syphilis are reviewed. Specific guidelines for the use of serologically positive donors are proposed.
Validity of the laser refraction technique for determining cylindrical error.
Employing a laser refraction technique, spherical error was determined for each of three meridans in 50 eyes. The measurements were used to calculate spherical error and cylindrical axis and power, and the findings were compared with standard methods of measurement. Correlations between the laser and non-laser methods indicated that the laser technique is a valid method for determining spherical error. Correlations between the findings for laser and non-laser cylindrical measurements were not high enough to warrant use of this laser technique in clinical practice. Differential laser validity findings for against-the-rule astigmatism are also discussed.
