[Clinical relevance of radioimmunologic determination of prostate specific antigen in prostate cancer].

To recognize progression of an inoperable prostatic cancer we use clinical parameters and the prostate specific phosphatase. The prostate specific antigen (PSA) is a new, sensitive and specific laboratory tumor marker. With 363 specimens of patients without prostatic cancer we defined for the normal range of this serum parameter. In 98 men with histologically proven prostatic cancers we investigated for the clinical relevancy of the serum level of PSA. We believe, that measurement of serum PSA give important information for clinical management of prostatic cancer.

[Kidney cancer with invasion of the vena cava: classification, surgical strategy and results of treatment].

A new classification of vena caval tumor thrombi is recommended from our experience with 37 cases. For advanced tumor thrombi a surgical technique with routinely performed thoracotomy, inserting a Sarns-catheter and preparing the cardiopulmonary bypass is mandatory. If metastases are absent the prognosis of the renal carcinoma is nearly as good as in cases without tumor extension into the vena cava. The total 2-year-survival rate in our series is 47%, for the cases without metastases it is 65%. The total 5-year-survival rate is 25% and for patients without metastases 57%.

[Effect of task complexity on cerebral organic reaction impairments--brain damage or patient effect?].

The contradictory results concerning the influence of task complexity on reaction time impairments following brain damage were reconsidered in this study. Patients with damage of the right or left cerebral hemispheres, as well as neurological patients without CNS lesions and normal controls were examined by four reaction tasks of varying complexity, a simple visual or auditory task and a choice reaction task. While there was no statistical interaction between the task-complexity and group effects for the brain-damaged and non-brain-damaged patients, comparison of the performance of brain-damaged patients and normal controls revealed such an interaction. Furthermore, the patients with damage of the right cerebral hemisphere showed a more distinct impairment of reaction time with the simple visual reaction task, while the patients with damage of the left cerebral hemisphere showed a more distinct impairment with the choice reaction task. These results are discussed in connection with several self-evaluations of the patients' performance and state of health.

[Soot lung as occupational disease].

Lung findings among 13 workers employed in a carbon black plant are presented. The concentrations of respirable dust at the work place exceeded the MAC for dust free of quartz. X-rays show disseminated small irregular and large shadows with slow progress. In two cases lung tissue was examinated histologically. Both accumulation of carbon black and development of collagen fibers were seen. According to x-rays and histological findings the lung disease can be estimated as a pneumoconiosis. Legal recognition of carbon black lung as an occupational disease can be achieved in a special procedure called "Sonderentscheidverfahren".

Surgical correction of rectoprostatic fistula.

A rectoprostatic fistula is a rare complication of prostatic operations, and the literature on the subject is scanty. Three men were treated for rectoprostatic fistulas representing complications of prostatic surgery. Mason's transsphincteric transrectal operation resulted in prompt, primary healing of the fistula. Subsequent colostomy closure left all 3 patients with normal urinary and fecal control. A systematic three-step sequence of operation is advocated for the treatment of rectoprostatic fistulas. The Mason technique provides excellent exposure and reliable closure of rectourinary fistulas.

Electromyographic feedback treatment of chronic hemiparesis: an attempt to quantify treatment effects.

The efficacy of electromyographic (EMG) feedback for the treatment of chronic upper extremity hemiparesis was quantitatively assessed through a series of objective and standardized tests of motor skills given to 12 stroke patients before and after treatment. In addition to standardized tests, video recordings of patient performance during simple motor tasks were made before and after EMG feedback training to permit subjective ratings of patient performance. Only six patients were able to perform the standardized tests of motor skills in a way permitting meaningful evaluation of results, and their test scores showed no significant improvement after treatment. Ratings of the video records by 37 independent physical therapists showed that some improvement of gross motor performance could be observed at the level of subjective global evaluation.

[The value of hypothermia methods in operative stone removal].

In spite of the possibility of using ultrasound for identification of stone fragments and vascular localisation, it is still necessary to clamp the renal vascular pedicle in selected cases. In situ perfusion achieves a nearly complete restoration of renal function, but requires a more sophisticated technique and preoperative planning. Surface cooling on the other hand is universally applicable and can be used during surgery, when unforeseen clamping of the renal pedicle is needed.

[Histamine in the urogenital tract].

Tissue histamine determination in the urogenital tract showed that there are significant differences concerning tissue histamine content between intraoperative and postmortem tissue samples. The difference appeared to be most significant in the case of renal pelvis samples, where the decrease was about 90% in the autopsie material compared to intraoperative tissues. In comparing tissue histamine content in special organs of different animal species we discovered considerable differences; e.g. in the ureter of the dog we measured 30 micrograms/g tissue - where as against the values in man amounted to only 6 micrograms/g tissue. The physiological meaning of the different tissue histamine levels is unknown.

[Results of a training program to improve the speed of visual perception and concentration in brain-damaged patients].

A program for the training of perceptual speed and attention with a duration of four weeks was developed and given to thirty brain damaged patients. In order to evaluate a possible generalization effect of the training all subjects were tested three times, i.e. at the beginning of the training, after four and after eight weeks with a series of 16 standardized psychometric tests. These comprised tasks which closely resembled the training procedures as well as tasks testing psychomotor functions in a broader sense and even tasks of reasoning and other intellectual functions. To test the different effects of training, spontaneous recovery and test repetition an experimental design with two groups of patients and two control groups was necessary. Only the groups of patients showed a significant and marked improvement in all tests which could neither be explained by spontaneous recovery nor by test repetition. The training effect proved to be stable after four weeks. The most marked improvement, however, was seen with those tasks most closely resembling the training procedures; in no case the performance level of the control groups was reached.

[Malignant diseases of the respiratory organs caused by asbestos exposure in municipal environment].

Exposure of population to asbestos comes from several origins. It is proved by measuring of asbestos emission and by discoveries of asbestos emission and by discoveries of asbestos fibres in lungs of adults. Asbestos induced maladies are not only founded after professional exposition, but also sometimes after domestic asbestos contacts or in neighbours of asbestos processing firms (generally upon a long time past influence of asbestos holding emissions). There is no evidence for an increase of the frequency of asbestos induced cancer in the inhabitants of large towns and industrial centres of GDR, till now. Morbific effects of asbestos must be expected in special risk groups. All efforts are necessary to reduce asbestos in known dust touched spheres.

Malignant pericardial mesotheliomas and asbestos exposure: a case report.

Three cases of malignant pericardial mesotheliomas are presented with evidence of occupational asbestos exposure. Examination results are compared with findings from experimental and epidemiological research on biological effects of asbestos dust. There are sufficient indications that time-limited effects of asbestos dust established either by measurements or assessment of the amount of concentration after a latency of more than 20 years are apt to result in the development of mesotheliomas of the pleura and peritoneum and, moreover, the pericardium. It is suggested that malignant pericardial mesothelioma also be recognized as another form of occupational disease caused by asbestos dust.

Disorders of calculation in aphasic patients--spatial and verbal components.

The aim of the study is to investigate whether the disorders of calculation observed in patients with Broca's aphasia are due to linguistic factors whereas those observed in patients with Wernicke's aphasia may be reduced in part to a disorder in the ability of spatial visualization. Two sets of arithmetic tasks were given to 20 Broca's aphasics, 20 Wernicke's aphasics, 20 patients with right hemispheric retrorolandic cerebral lesions and 40 patients with no brain damage. The solution of the tasks required different degrees of spatial visualization. The statistical evaluation of the data showed, as hypothesized, that Wernicke's aphasics perform significantly poorer than Broca's aphasics in tasks with a more pronounced spatial component than in the tasks having a predominantly verbal component.

[Contribution to importance of asbestos in the epidemiology of bronchial cancer].

Today asbestos is the most important professional carcinogenic agent of the respiratory tract. Asbestos caused bronchial cancer doesn't differ from its well-known morphology, progress and prognosis, but almost it is starting earlier. Fibrous form of asbestos represents its determinant potency. Carcinogenic fibres are about or below 1 micron in diameter and part of them can't be seen in light optical systems. Curious bodies may be missed sometimes also. A cancerogenic limit of asbestos concentration can't be quoted exactly now. Asbestos exposition during 60 contacting days or more are able to induce cancer. Cancerogenesis itself is during 20-30 years until clinical manifestation on an average. If the exposition is started in the youth, the period of latency is shortened distinctly. Tobacco smoke inhalations increase the carcinogenic effectiveness multiplicatively. In the districts of Magdeburg and Halle (GDR) more than 10% of the bronchial cancers are expected to be induced by asbestos. On the other hand only about 3% of the adults have had asbestos contacts at any former time. This is a raising of risk to at least triple rate.--It is necessary to realize all demands on technical prophylaxis of occupational asbestos exposition.
