In one patient treated by peritoneal dialysis for psoriasis the skin lesions cleared completely. In two persons hemodialysis gave unsatisfactory results. In treated and another three untreated patients cellular immunity was evidently suppressed. This phenomenon is similar to the immunologic changes in persons with uremia. Solutes in middle molecular weight range (SMMWR) suppress cellular immunity. These compounds were therefore investigated in psoriatics. The level of SMMWR was lower before than after each dialysis, although their concentration increased slightly during the dialytic treatment. This suggests that solutes in middle molecular weight are fixed in the epidermis and/or in the walls of the skin capillaries and that dialysis liberates them from these places. Peritoneal dialysis seems to be a more effective method of treating psoriasis than hemodialysis. Solutes in middle molecular weight range might play an important role in the pathomechanisms of this multifactorial disease.
Neutrophil peroxidase activity was determined before delivery in 46 pregnant women. 36 of these women were also tested on the 1st day and 30 on the 3rd day after delivery. A significant increase in the average values of neutrophil peroxidase activity was found in all parturients as compared with the controls. The highest peroxidase activity was observed before parturition. The possible mechanism of these changes is discussed.

To investigate further the biologic activity of solutes in the middle-molecular-weight range, we studied the influence of these compounds on the migration of unseparated white blood cells and separated granulocytes and lymphocytes. Middle molecules (MM) inhibit the migration of unseparated leukocytes, but this effect on lymphocytes was seldom observed. With scanning electron microscopy, unseparated leukocytes were shown to adhere to one another, forming cellular clumps. This phenomenon could not be seen when separated cells were used. These results give some insight into the mechanism of inhibition exerted by MM and into the changes, observed in earlier experiments, of cellular composition of skin exudate obtained from untreated patients with uremia.
The cellular pattern of skin exudate in untreated uremic patients differed from that in healthy persons. The skin exudate composition in patients on maintenance hemodialysis approached normal values. Such normalization was, however, static: after sensitization with dinitrochlorobenzene--contrary to the control--both treated and untreated patients exhibited no changes in the cellular pattern of exudate. The activity of acid phosphatase in the cells of exudate was highly positive in control, almost normal in dialyzed, but weak in untreated uremic patients. Our investigations confirmed that cellular immunity in untreated and in maintenance hemodialysis patients is suppressed.
Two cases of unusual rupture of aortic aneurysm with extravasation in the bronchus and duodenum are described. The clinical evolution of such aneurysm ruptures is associated with considerable diagnostic difficulties due to the absence of pathognomonic typical features and the short survival time after onset of pain symptoms.
Acid phosphatase (AcP), beta-glucuronidase (GR) and N-acetyl-beta-D-glucosaminidase (NAG) activity was determined, using semiquantitative cytochemical methods, in the peritoneal fluid lymphocytes obtained from 50 patients with terminal renal failure treated by intermittent peritoneal dialysis. The control group included 30 subjects with normal renal function. The percentage of AcP and NAG-positive lymphocytes was significantly lower and that of the GR-positive cells significantly higher in dialysed patients than in the control group. A group of 22 dialysed patients with bacterial peritonitis showed a significant increase of the percentage of NAG-positive lymphocytes as compared with both the subjects in the control group and the peritonitis-free dialysed ones. Changes of the lymphocytes enzymatic activities were distinct in cells exhibiting the granular reaction type, and to a much lesser extent in those showing granular diffuse reaction.
Peroxidase activity was assessed cytochemically in the peritoneal dialysate neutrophils of 50 patients with terminal renal failure treated by intermittent peritoneal dialysis. In 22 patients cells were harvested for the second time after they developed peritonitis as a complication of the treatment. The results obtained were compared with those in the control group consisting of 30 patients with normal renal function. The patients treated by peritoneal dialysis showed both in the complication-free period and in the course of peritonitis significantly higher peroxidase activity in the neutrophils, as revealed by the semiquantitative score test. In the course of peritonitis, the neutrophils peroxidase activity was significantly lower than in the complication-free period.
In 234 patients with various malignant diseases including cancer of the larynx, cancer of the lung, cancer of the stomach, cancer of large intestine as well as patients with Hodgkin's disease, plasma cell myeloma and precancerous states of the larynx, the count of peripheral blood lymphocytes showing intact beta-glucuronidase-positive lysosomes was determined using a semiquantitative histochemical method. All the patients studied, except patient with cancer of the lung, showed a tendency toward a low count of such cells significant in patients with precancerous states of the larynx, cancer of larynx, cancer of the larynx after radiotherapy and Hodgkin's disease and nonsignificant in cancer of the stomach, cancer of the large intestine and plasma cell myeloma. In patients with cancer of the lung the count of lymphocytes with intact beta-glucuronidase-positive lysosomes was significantly increased as compared with that in healthy subjects. The possible relationship between the low immune response in the patients and the functional state of the lymphocyte lysosomal system is discussed.
In 32 patients with untreated lung cancer, activity of alkaline phosphatase, acid phosphatase, beta-glucuronidase and N-acetyl-beta-glucosaminidase were determined cytochemically in peripheral blood of neutrophils. The results obtained were compared with those in 40 healthy subjects. Total counts of alkaline phosphatase, acid phosphatase, beta-glucuronidase and N-acetyl-beta-glucosaminidase positive neutrophils were significantly higher in patients with lung cancer, as compared with the control group. Mean value of alkaline phosphatase activity index (Score) was significantly higher in cancer patients in comparison to the control group whereas mean values of acid phosphatase, beta-glucuronidase and N-acetyl-beta-glucosaminidase activities did not differ significantly.
