Cord blood transplantation in adult patients.

Early clinical reports outlining outcomes for primarily pediatric patients undergoing UCB transplantation point to delayed time to hematopoietic recovery and favorable incidence and severity of GvHD. Recently, clinical reports in adult patients identified the feasibility of UCB transplantation for those patients lacking an available histocompatible-related or unrelated adult donor Intensive clinical and laboratory research is ongoing focused on strategies to foster UCB allogeneic donor engraftment thereby allowing wider application of this stem cell source for patients requiring allogeneic transplantation.

Radiotherapy for non-function pituitary tumours.

OBJECTIVE: Pituitary radiotherapy (RT) is often used as adjuvant treatment in the post-operative period for patients with clinically non-functioning pituitary tumours (NFTs). There is a distinct lack of objective data, however, describing the efficacy of RT in preventing the regrowth of these tumours. We have therefore determined whether the recurrence rate for NFTs is significantly lower in patients treated with post-operative RT compared with that observed in patients not treated with RT. PATIENTS AND METHODS: A retrospective case notes review was performed on 126 patients with NFTs treated at two institutions in the UK. One hospital routinely administered RT within 12 months of initial pituitary surgery whereas the other used post-operative RT only rarely. The main outcome measure was regrowth of pituitary tumours following surgery in patients who did or did not receive post-operative RT. RESULTS: There was no significant difference between patients who received RT versus those who did not in terms of age, sex, initial tumour size or mode of operation. The actuarial progression-free survival was 93% at both 10 years and at 15 years for the RT treated group, and was 68% and 33%, respectively, for the non-RT-treated group. Using Cox's model for proportional hazard analysis, we found the only prognostic factor for NFT regrowth was the administration of pituitary RT (P < 0.00005). CONCLUSIONS: Radiotherapy administered within 12 months of initial pituitary surgery for non-functioning pituitary tumours significantly reduces the risk of tumour regrowth. It remains to be determined whether sequential MRI scanning can help delineate those patients who should receive radiotherapy following pituitary surgery for non-functioning pituitary tumours.

Spontaneous growth in Chinese patients with Turner's syndrome and influence of karyotype.

The pretreatment mixed cross-sectional and longitudinal height measurements of 203 patients with Turner's syndrome (TS) were analysed. Only one observation was included per year per child and a total of 858 observations formed the basis of the growth study. The mean and SD values were fitted separately by a second-degree polynomial function, giving smoothed growth curves. The spontaneous growth pattern of Chinese girls with TS runs parallel to published Caucasian TS growth curves. The mean final height of Chinese patients with TS was 142 cm compared to 147 cm and 139 cm observed in Northern European and Japanese patients, respectively. Patients with the 46, X, i(Xq) karyotype were found to be significantly shorter, whereas children with the 45, X/46, XY and 46, X, del(Xq) karyotypes were significantly taller among this group of patients. Patients with TS who were disomic for Xp are significantly taller than patients who were monosomic for Xp. Our results suggest that karyotype can have a significant effect on the growth of children with TS.

Activity of neutral endopeptidase and aminopeptidase N in mouse thymic stromal cells which bind double-positive thymocytes.

The activity of two peptidases was determined in immortalized lines of thymic stromal cells. A line of total stromal cells (T-TG-St) was grown from transgenic mouse expressing temperature-sensitive SV40 T antigen under the control of the regulatory elements of the mouse major histocompatibility complex class I gene. From these cells we isolated a subset (DP-TG-St) that binds thymocytes which are mainly CD4+8+. We also assayed a clone of fetal thymic epithelial cells (BA/10) that binds CD4+8+ thymocytes. Both lines of double -positive cell-binding stroma exhibited strong activity of two peptidases, neutral endopeptidase (NEP; EC 3.4.24.11) and aminopeptidase N (APN; EC 3.4.11.2). In contrast, the activity of both enzymes was very low in the total thymic stromal line. Use of the specific inhibitors confirmed that these two enzymes were responsible for the activity observed but also suggested the presence of additional unidentified aminopeptidase(s) in the same stromal cells. The high activity of the two peptidases on stromal cells that bind thymocytes at the double-positive stage raises the possibility that they might contribute to the microenvironment of the developing thymocytes.

A novel gene, AF1q, fused to MLL in t(1;11) (q21;q23), is specifically expressed in leukemic and immature hematopoietic cells.

Translocations involving chromosomal band 11q23 are associated with leukemias. These translocations fuse the MLL, a gene with sequence homology to the Drosophila trithorax, to genes from a number of other chromosomal loci. We have characterized two t(1;11)(q21;q23) translocations that fuse the MLL gene to a novel gene, AF1q on chromosomal band 1q21, in two infants with acute myelomonocytic leukemia (AMMOL). In one of these patients, der(11) represents an inframe fusion of the N-terminal portion of MLL gene to the complete AF1q open reading frame, whereas der(1) does not give rise to an open reading frame. This observation suggests that the N-terminal portion of MLL gene is critical for leukemogenesis in translocations involving band 11q23. The predicted wild-type AF-1q product is a 9-kD protein with no similarity to any other protein in the data banks. The AF1q mRNA is highly expressed in the thymus but not in peripheral lymphoid tissues. In contrast to its restricted distribution in normal hematopoietic tissue, AF1q was expressed in all leukemic cell lines tested.

Increased levels of the soluble adhesion molecule E-selectin in essential hypertension.

OBJECTIVE: To determine whether soluble E-selectin levels were raised in hypertension and whether such levels correlated with those of von Willebrand factor. DESIGN: Forty-five consecutive patients with uncontrolled hypertension (blood pressure > 140/90 mmHg) and 33 consecutive patients with controlled hypertension (blood pressure < 140/90 mmHg, i.e. normotension) were examined and compared with 40 normotensive age- and sex-matched controls. METHODS: Levels of von Willebrand factor and soluble E-selectin were measured by enzyme-linked immunosorbent assay in venous blood serum. Resting systolic (SBP) and diastolic (DBP) blood pressures were measured before venepuncture. RESULTS: Soluble E-selectin levels were raised in hypertension compared with controls and normotension. The levels of von Willebrand factor were raised in hypertension and normotension compared with controls. In multivariate analysis of all subjects, von Willebrand factor levels correlated with SBP (P < 0.001) and soluble E-selectin level correlated with DBP (P < 0.001), but levels of von Willebrand factor and soluble E-selectin did not correlate. There was no clear relationship between levels of either endothelial cell index or the class of drug therapy used or its dosage. CONCLUSIONS: We suggest that raised levels of soluble E-selectin in hypertension do not indicate endothelial cell injury but may be related to endothelial cell activation. The stimulus for release of soluble E-selectin might be different from that for von Willebrand factor.
