[Cauda equina syndrome with total myelographic block (author's transl)].

Twenty-seven cases of complete subarachnoid block discovered through myelography are analysed with regard to the causes of obstruction, the neurologic symptoms, and the cerebrospinal fluid. The neurologic signs in cases of cauda equina compression, most often caused by massive disc protrusions (20 patients), vary considerably and rarely correspond to the gross myelographic findings. Bilateral neurologic symptoms of any degree with or without sphincter disturbance were observed in 21 among 27 cases and are signs of cauda compression. In contrast to cases of sciatica with unilateral root compression and radicular symptoms neurological symptoms are not reliable in revealing the site of the lesion in cases of massive central disc protrusions. Therefore radiculography (myelography of the cauda equina) is necessary for diagnosis. In addition to the well-known elevation of the spinal fluid protein and pleocytosis, the presence of neutrophile granulocytes is frequently found (11 of 20 cases) in cytological analysis.

[Objectivation of tremor and motion disorders via computerisation (author's transl)].

The quantitative analysis of motion disorders of the upper extremities can be performed by means of a joystick connected with a minicomputer. Therefore a random squarewave curve is drawn on a computer display and the patient must follow it with the joystick. The deviation of the two curves are recorded, analysed and documented numerically and graphically.

[Cytology, lactate dehydrogenase, and creatine kinase in CSF in mild head injuries (author's transl)].

Cytology, lactate dehydrogenase, and creatine kinase in cerebrospinal fluid were investigated 6-72h after mild head injuries in 59 patients aged 16-49 years. Significant differences were not found in creatine kinase. Lactate dehydrogenase, however, was significantly more elevated in mild cerebral contusions compared with concussions 24h after head injury. Monocytoid reaction in cerebrospinal fluid was significantly more distinct in mild contusions compared with concussions 24h after impact. Large phagocytes were rarely found in concussions and were not traceable 48h after impact. Large phagocytes, however, were found in 50-55% of the mild contusions 48-72h after head injury. Siderophages were found only in contusions (16.5-35%), 48-72h after impact). The optimal time for lumbar punction is 24h after head injury.

Dedifferentiation and invasive growth of an eosinophil pituitary adenoma.

This report concerns the course of an eosinophil pituitary adenoma in an acromegalic female (16 years of age when first symptoms appeared) over a period of ten years. The case was complicated by craniocerebral trauma and CSF rhinorrhoea. After several operations, dedifferentiation, and invasive growth into the orbit and the petrous bone were observed with walling in of the VIIIth cranial nerve. The possible causes of the dedifferentiation of the adenoma are discussed.

[Autoradiographic investigations in repeated experimental brain concussion (author's transl)].

Single brain concussion in rabbits causes an increased proliferation of glial and mesenchymal cells. Repeated experimental concussions in rabbits (3 times at intervals of 24 h) led to an increased incorporation of H3-thymidine in glial and mesenchymal cells with a maximum at 48 h after the third concussion. This is interpreted as an indication of increased cell proliferation. The first and the second concussion did not cause a comparable reaction, thus suggesting that concussions may inhibit DNA synthesis under the conditions of our experimental setup. When the concussions were induced at an interval of 48 h the result was different: 48 h after each concussion we found an increase of labeled cells compared with the controls. After the second concussion the reaction was still more enhanced compared with the reaction following the first concussion. In contrast to this the number of labeled cells after the third concussion was significantly decreased compared with those after the second one. Parallels with pugilistic encephalopathy are discussed.

[The effect of L-tryptophan on L-dopa absorption (author's transl)].

Dopa blood levels were measured in healthy subjects after oral intake of 0.5 g L-dopa (seven subjects) as well as of 0.5 g L-dopa and 1.0 g L-tryptophan (six subjects). Combined L-dopa and L-tryptophan administration produced significantly less of a rise in dopa blood levels than L-dopa alone.

[Is physiotherapy useful following surgery for carpal tunnel syndrome?].

A prospective follow-up-study of 97 patients operated because of a carpal tunnel syndrome was carried out to investigate postoperative use of physiotherapy. Postoperative 32 patients were treated at least 12 times and 65 patients were not treated physiotherapeuticly. Physiotherapists postoperatively treated by activating joints of fingers and hand. We examined the patients one day preoperative and on an average of 9 months postoperative. Complaints of the patients, local findings, vigorimetrically measured strength and electromyography were registered. The vigorimetrically measured strength of the operated hand improved significantly less in patients treated physiotherapeutically compared to not treated patients. All other complaints and findings improved equally. We interpreted this difference as a result of spare caused by awareness of a sick hand.

[Effect of meteorologic parameters on acute admission of patients with lumbar intervertebral disk displacement].

The correlation between local meteorological parameters and acute admittance to hospital of patients with lumbar disk prolapses was investigated. The study shows a significant correlation between drop of temperature and increase of humidity for male on the day of admittance and for female on changes from the day before to the day of admittance. This seems to be in contrast with other investigators, who couldn't find a correlation between local meteorological parameters and pain. The reasons for this are discussed.
