[Role of pathologic anatomy in the diagnosis, prognostic evaluation, and surveillance of bladder tumors].

The usual histological techniques are necessary and sufficient for the routine diagnosis of bladder tumours. Many laboratory tools are designed to determine prognostic factors but require such technical conditions that they cannot be used routinely. The determination of DNA ploidy by flow cytometry of voided urine, fresh, formalin-fixed or paraffin-embedded tumour fragments is a simple, standardised and reproducible technique and the method of choice of pathologists as it allows morphological control of the element analysed. Based on a series of 720 bladder tumours collected by 3 different centres, the authors demonstrate how to use, correlate and integrate into routine practice the data of image analysis flow cytometry in relation to conventional cytology and histology. 5% of grade 1 tumours, 27% of grade 2 tumours, and 92% of grade 3 tumours were DNA aneuploid. 17% of non-invasive tumours (PTa) and 79% of invasive tumours (PT1 and more) were DNA aneuploid. These proportions are in line with the results of various series in the literature. DNA ploidy is therefore an objective and measurable element completing histological grading. The determination of DNA ploidy also helps the cytologist in difficult cases of urine cytology.

[Concentration of lipids in ascitic fluid and the concentration gradient of albumin in blood and ascites: diagnostic significance].

The aim of this study was to test the diagnostic value of ascitic fluid cholesterol and triglycerides concentrations and of serum-ascites albumin concentration gradient in the differentiation between cirrhotic and malignant ascites. These biological parameters were determined, on the one hand in 34 cirrhotic patients, 6 of them having an hepatocellular carcinoma and 6 others having a spontaneous bacterial peritonitis and, on the other hand, in 16 patients with malignant ascites, 13 of them having an abdominal extra-hepatic or pelvic cancer, and 3 others having an extra-abdominal cancer with multiple liver metastases. Ascitic carcinoembryonic antigen assay and ascitic fluid cytology were also done in the 50 patients. In differentiating the cirrhotic patients from those with malignancy, ascitic fluid cholesterol concentration (discriminating value less than 1.1 mmol/l) ascitic fluid triglycerides concentration (discriminating value 0.5 mmol/l) and serum-ascites albumin concentration gradient (discriminating value greater than 11 g/l) allowed a diagnostic efficiency of 0.92, 0.80 and 0.77, respectively. Ascitic fluid cytology showed presence of malignant cells in 3/6 patients with hepatocellular carcinoma associated with cirrhosis, in 9/16 patients having a malignant ascites, and was negative in other patients. Ascitic carcinoembryonic antigen assay was abnormal only in 3/16 patients with malignant ascites. These results suggest that measurement of ascitic fluid cholesterol concentration must be included in the initial evaluation of patients with ascites of unknown origin.

Hodgkin's disease: ultrastructural localization of intra-cytoplasmic immunoglobulins within malignant cells.

An immunoperoxidase technique has been applied to the detection of intracellular immunoglobulins at the light and ultrastructural levels in three untreated cases of Hodgkin's disease. The results are compared with those obtained in three treated cases. In both groups, 20-90% of malignant cells had intra-cytoplasmic immunoglobulins. There was no correlation between the percentage of immunoglobulin-containing cells and the histological type or the stage of the disease. At the ultrastructural level, immunoglobulins were constantly localized on cytoplasmic ribosomes, the later being either free in the cytoplasm or bound to the endoplasmic reticulum and to the external envelope of the perinuclear space. In addition, a very few malignant cells exhibited immunoglobulins within their perinuclear space and their endoplasmic reticulum. These results demonstrate that immunoglobulins in Hodgkin's malignant cells are present on the cellular sites of protein synthesis. They appear to be retained in their cytoplasm, and to be secreted only very occasionally. The significance of these findings as to the cellular origin of malignant cells in Hodgkin's disease is briefly discussed.

[Splenic lesions during benign inoculation lymphoreticulosis].

The authors describe the fibro-granulomatous lesions with scar formation on a splenectomy specimen removed for familial spherocytosis, eight weeks after the onset of cat scratch disease. This original observation, apparently unique in the literature, is in favour of the blood spread of the agent responsible for benign inoculation lymphoreticulosis, and suggests a pathogenic theory for the unusual visceral manifestations.

[Coronary thrombosis on oral contraception (author's transl)].

A case of myocardial infarction observed in a 27 years old female taking an oestrogen-progestogen combination for contraception, who died suddenly two months after the resection of a left ventricular aneurysm, provided unprecedented anatomical documents. Their histological examination allowed to state the thrombotic mechanism of the coronary occlusion and brought arguments in favour of an alteration of the coronary arterial wall in the form of thickening of the intima, likely to have been the starting point of this thrombosis.

[Thoracic and abdominal actinomycoses. Anatomo-pathologic study of 5 cases].

The authors report 5 cases and study the thoracic and abdominal lesions of actinomycosis. This is a rare bacterial disease but not exceptional, discovered usually by chance on operative specimen on autopsy. The clinical and bacteriological diagnosis is very difficult although the lesions are characteristic. Thus it is always necessary to carry out biopsy and seek sulphur granules in the actinomycotic follicle. The differential diagnosis is usually easy with granulomycoses, but it is sometimes more difficult to distinguish the disease from nocardioses.

Prognosis of myeloid metaplasia with myelofibrosis.

The study of the evolution of 168 cases of idiopathic myeloid splenomegaly allowed to point out the prognostic value of the clinical hematologic, isotopic and radiological parameters. The correlation of these data with the histological type of the osteomedullary lesions at the time of the diagnosis, allows to confirm the long survival of the hyperplastic formes (type I) and the more reserved prognosis in advanced myelofibrosis (type II and III together). Indeed, median survival is of 82 months for type I, and 60 months for type II and type III together. It seems advice to gather the two last types and so, to compare two anatomoclinical entities : one is characterized by hyperplastic marrow with reticulinic fibrosis, usually associated with enlarged spleen and sometimes even with increase red blood cells volume but without radiological bone lesions; the other one is characterized by a marrow of middle importance or marked marrow depletion, fibrosis, with or not osteosclerosis, and shows moderate or marked radiological lesions (respectively 16,9% of the cases) and, often, pancytopenia.
