Excessive epithelioid cell granulomatous reaction associated with a lymphoepithelial carcinoma (Schmincke-Regaud).

We report on a 56-year-old man in whom histologic examination of "granulation tissue" from the right lower meatus of the nose revealed the presence of an undifferentiated nasopharyngeal carcinoma of the lymphoepithelial type (Schmincke-Regaud). The diagnosis was supported by serologic detection of antibodies against the Epstein-Barr virus. Approximately 3 weeks after the first excision, biopsy material was taken from the epipharynx. Histologically a marked epithelioid cell granulomatous reaction was found. As an unusual feature, granulomas were not located between tumor cell complexes, but rather contained tumor remnants in their center. At tissue site all transitions from well preserved to completely necrotic tumor cells were discernible. This unusual reaction probably represents a strong, favorable immunologic response against the neoplasm.

[Congenital pulmonary lymphangiectasis].

3 cases of congenital pulmonary lymphangiectasis are described. A girl, now 3 years old, underwent at the age of 4 weeks a resection of the left superior lobe of the lung which was singularly involved. A male newborn died 4 h after birth and showed the typical affection of both lungs. A second male infant died 19 d after birth and had an isolated involvement of the left lung. Post-mortem examination additionally revealed in both boys a cardiovascular malformation. Histologically, we especially observed a papillar endothelial hyperplasia in a dilated lymph vessel in case 1 and multinuclear giant cells of the foreign-body type in case 3. In the literature, there are reported 99 cases of congenital pulmonary lymphangiectasis. Including our case, a unilobar or unilateral involvement is described in only 8 cases. The congenital pulmonary lymphangiectasis occurs more often in males than in females (1.8 : 1). 90 out of 93 life-born children suffering from this malformation died, 57 of them during the perinatal period. However, cardiovascular malformations, which were observed in 53 cases, are playing an important role as the cause of death. The aetiology of the congenital pulmonary lymphangiectasis remains unknown. There are 4 different theories concerning the pathogenesis, namely, a persistence of early-fetal lymphatic vessels, a missing connection between primitive lymphatic vessels, a hyperplasia of lymphatic ducts and, last but not least, a passive dilatation of the lymph vessels because of venous or lymphatic congestion.

Antibodies against herpesviruses in patients with Hodgkin's disease.

The antibody titres to Epstein-Barr virus (EBV), cytomegalovirus (CMV), herpes simplex virus (HSV) and varicella-zoster virus (VZV) were determined in 57 adult patients with Hodgkin's disease and a group of age- and sex-matched controls. In the group of patients, the geometric mean antibody titres to EBV capsid antigen, CMV early and late antigens, and also VZV, were significantly higher than in the healthy controls. Antibodies to early antigens of EBV and CMV were found more commonly in patients than in controls. The raised antibody titres to herpesviruses were taken to be the result of the disturbed cellular immunity in Hodgkin's disease.

[Contribution to the true non-African Burkitt's lymphoma (author's transl)].

It is reported on a 6-year-old Middle European boy suffering from a nasopharynx tumor with involvement of a submandibular lymph node. The tumor was classified by morphologic, serologic and virologic data as a true or Epstein-Barr virus-associated Burkitt's lymphoma, resp., which has been, up to now, seldom seen outside Africa. The patient died 3 months after diagnosis. Autopsy has shown tumor involvement of nasopharynx and soft tissues in the left neck region, several lymph nodes, kidneys, liver, spleen, and bone marrow. Cytologic, light and electron microscopic data are reported and the structural change of the tumor is referred to.

[Hodgkin's lymphoma in the biopsy material. A retrospective analysis (author's transl)].

112 cases of Hodgkin's disease diagnosed between 1967 and 1976 at the Institute of Pathology and the Department of ENT, Medical Academy of Erfurt, GDR, were reclassified according to the Rye classification. Age and sex distribution as well as frequency and survival are presented. The most frequency subtype is "mixed cellularity" (43.8%) followed by "nodular sclerosis" (40.2%). The age distribution of all cases shows 3 peaks with only small differences between the subtypes. The cases are distributed among 71 males and 41 females. The most frequent subtype in males is "mixed cellularity" (50.7%) whereas in females it is "nodular sclerosis" (61.0%) The survival of cases with "lymphocyte predominance" and "nodular sclerosis" is significantly better than that of case with "mixed cellularity" and "lymphocyte depletion".

[Frequency of pulmonary causes of death in perinatal post-mortem material (author's transl)].

Morphological lung changes were found to be the cause of death in 304 of 618 live births that died during the perinatal period in the years 1975-1979 (49.2%:369 males, 249 females). Causes were hyaline membrane disease (168 = 27.2%), massive lung bleeding (64 = 10.4%), pneumonia (36 = 5.8%), atelectases (29 = 4.7%), and amniotic fluid aspiration (7 = 1.1%). There were minor sex differences in the frequency of the causes, which were often present in combination. Among the pulmonary causes of death, hyaline membrane disease was the most common (55.3%), followed by massive bleeding (22.1%), pneumonia (11.8), atelectases (9.5%), and amniotic fluid aspiration (2.3%), with marked differences among premature and mature births.

A variant of the Wiedemann-Beckwith syndrome.

The following combination of findings were established by histological examination of biopsy or autopsy material in an 11-day-old male baby: congenital mesoblastic nephroma, nodular renal blastema, dysgenetic pancreatic cyst, "cytomegaly" of the adrenal cortex, hyperplasia and hypertrophy of the islets of Langerhans, hypoplasia of the thymus and the lymphatic tissue. Since some of these changes constitute the characteristic histological findings in the Wiedemann-Beckwith syndrome and the remainder can be readily reconciled with it, the assignment of this case to this syndrome is discussed. Although the three cardinal symptoms are lacking, the combination of findings are interpreted as a variant of the wiedemann-Beckwith syndrome.

[Frequency of malignant non-Hodgkin's lymphomas in biopsy material. A retrospective study].

The biopsy material of the Institute of Pathology of the Medical Academy of Erfurt, GDR, from January 1967 to February 1978 has been reinvestigated for malignant non-Hodgkin's lymphomas which were reclassified according to the Kiel classification (Gérard-Marchant et al. 1974). We observed 122 (72.6%) low grade malignant lymphomas and 46 (27.4%) high grade malignant lymphomas. 17 cases remained unclassifiable. The median age ranged between 47.6 years (lymphoblastic malignant lymphoma) and 65.8 years (centrocytic malignant lymphoma). The sex ratio shows a predominance of males for all types of low grade malignant lymphomas. Only the immunoblastic malignant lymphoma in the group of high grade malignant lymphomas has a predominance in females. The survival of patients with high grade malignant lymphomas shows a good correspondence to the data given in the literature, whereas patients suffering from low grade malignant lymphomas have a distinctly worse prognosis in our material in comparison to literature data.

[Demonstration of Epstein-Barr virus genome in european Burkitt lymphoma (author's transl)].

A 6-year-old European boy with a tumour in the head and neck region died 86 days after the first occurrence of symptoms despite chemo- and radiotherapy. An epipharyngeal biopsy at the beginning of the disease and a cytological investigation of biopsy and autopsy material showed non-Hodgkin lymphoma with a high degree of malignancy of the Burkitt type. High antibody titres against Epstein-Barr virus (EBV)-specific virus capsid antigen, nuclear antigen and the R component of the EBV-early-antigen-complex as well as the approximately 27 EBV genome equivalents per tumour cell, demonstrated by DNA-DNA hybridisation, characterised the tumour as true Burkitt lymphoma which is observed extremely seldom outside Africa.

[Morphological classification and problems of lymphogranulomatosis (Hodgkin's disease) (author's transl)].

According to the Rye classification, four subtypes of Hodgkin's disease are distinguished: lymphocytic predominance, nodular sclerosis, mixed cellularity, and lymphocytic depletion type. In this order of array prognosis is increasingly unfavourable. The importance of the diagnostically relevant Sternberg-Reed cell is discussed. The fact is emphasized that the diagnosis of Hodgkin's disease cannot be made in the absence of diagnostic Sternberg-Reed cells, but the diagnosis must depend on the picture of the section as a whole. The histological feature of the four subtypes is demonstrated. Finally, problems of subclassification, the different data on frequency of the subtypes, the problematic nosological position of Hodgkin's disease, and the obscure histogenetic origin of Sternberg-Reed cells are discussed.

[Metastasising squamous-cell carcinoma of the skin in a ten-year-old girl].

Because of recurrent paronychia of the right thumb "granulation" tissue was removed from the nail-bed of a ten-year-old girl. Histological examination revealed a cornified squamous-cell carcinoma. About five months later a lymph-node metastasis was found in the right axilla. After radical dissection of the axillary region further lymph-node metastases were found, as well as a lymphangitic carcinosis with tumour infiltration in the veins. Despite telecobalt radiation there were recurrent retastases which could no longer be removed by operation. Two years after the diagnosis the girl died. Necrospy revealed diffuse tumour infiltration of the soft tissue of the axilla, upper arm, shoulder, neck and thoracic wall on the right, including the breast tissue, with thoracic para-aortic-abdominal and left-sided axillary lymph-node metastases, and lung and pleural metastases bilaterally.

Late neuropathological findings after acute rubella encephalitis.

Neurophathological findings are reported in a 15-year-old boy who died 8 months after acute rubella encephalitis. There are widespread bilateral necroses in large areas of the cerebral cortex with a diffuse macroglial and microglial reaction. In basal ganglia, degenerative changes and a more or less pronounced rarefication of nerve cells as well as very small cystic areas and glial scars are observed. In the white matter, neither foci of demyelination nor perivascular cuffings nor focal gliosis occurred. The results are compared with other observations in the literature. It is concluded that rubells encephalitis is a disease of a variety of pathogenetic mechanisms and has a variety of clinical and morhpological features. It may evidently be caused by direct viral invasion of the CNS or by hypersensitivity reaction.

[On the histology of malignant tumours in infants and children (author's transl)].

Autopsy and biopsy material obtained between 1967 and 1976 at the Institute of General Pathology and Pathological Anatomy of the Medical Academy Erfurt included 260 malignant tumours in infants and children. Male to female ratio was 54.6:45.4. There were slight peaks for the fourth and sixth year of life. The most frequent site was the CNS (44.2%), followed by the haematopoetic-lymphatic system (28.1%), skeletal system (7.7%), sympathetic nervous system including adrenal medulla (6.2%) and kidneys (4.6%). Among tumour types the neurogenic ones were most frequent at 50.8%, followed by leukaemias (18.1%) sarcomas (12.3%), malignant non-Hodgkin lymphomas (7.3%) and mixed tumours (6.5%). Comparing these results with those quoted in the literature and earlier personal studies indicates that the higher frequency of CNS tumours is probably due to regional factors. Linked to this is a percentage increase of neurogenic tumours and a relative decrease in leukaemias. Additional changes in distribution among the different forms of tumour are due to the introduction of the group of malignant non-Hodgkin lymphomas.
