[Basic studies of ultrasonic surgery. IV. Osteosynthesis in bone welding using ultrasound].

A conglomerate of homologous bone-chips and n-butyl-cyanoacrylate fixes the fragments in the ultrasonic osteosynthesis. The sound pressure leads to the forced penetration of the monomer into preformed gaps of the bone surface (Volkmann and Haversian canals) up to maximum 300 microns during the polymerisation forced by ultrasound. A toothing effect between the conglomerate and the cortex takes place in this way, this one leads to a higher mechanical resistance and to specialities in the regenerative process. The term welding is founded scientifically by the mechanism of the connecting formation. On the other hand the application of the ultrasound is sensible in order to force the polymerisation.

[Novel modification of a 2-row intestinal anastomosis technic].

A novel modification of a two-row intestinal anastomosis technique is introduced. A step-like anastomosis was carried out by a short stepwise resection of the several intestinal wall laminae with intramural adaptation on the specific juncture. In 44 end-to-end intestinal anastomoses in dogs and 5 in children there were not observed any insufficiency and stenosis.

[Basic studies of ultrasonic surgery. III. Histomorphologic changes in various tissues following exposure to ultrasound].

Absorption of ultrasound leads to reversible or irreversible changes in biological tissues in dependence on the sound intensity. The ultrasound causes under appointed conditions in skin, muscles, vessels, nerve tissue, and bone histopathological changes in a different scale which must be observed in the ultrasonic surgery. 200 tissue specimens from 40 guinea pigs were taken from defined localisations after an ample ultrasound exposition and were evaluated microscopically by hematoxylin-eosin staining technique. Skin and vessels showed very vehement reactions. Intact osseous tissue reacted with periosteal callus formation. Practical conclusions can be draws for the ultrasonic surgery by the discussion on histopathological changes.

[Basic studies on ultrasonic surgery. I. Principles, status and perspectives of ultrasonic surgery].

The ultrasonic power with regard to its medical use for resection, treatment, and joining of soft and hard biologic tissues is investigated after the formation of the ultrasonic diagnostics and therapy. The situation of the ultrasonic surgery as an unconventional method for the ingenious complementation of tested operative techniques is valued after the representation of the mode of operation and the present equipment. The trend of development and prospective problems of the research are deduced from them.

[Fat embolism; a survey (author's transl)].

As far as diagnostics, prophylaxis and therapy are concerned, this clinical entity has not lost its acuteness. However it can no longer be considered only as a mechanical problem (impression of fat corpuscles out of the broken bone into the capillaries), instead it has to be regarded as an epiphenomenon of shock in combination with the respiratory distress syndrome and disorders of the microcirculation. A successful anti-shock regimen including effective stabilisation of fractures are at present the best means to prevent fat embolism. The effect of medicaments after manifestation of fat embolism is dubious.

[Basic studies on ultrasonic surgery. II. Studies on the stability after ultrasonic osteosynthesis in various forms of fractures in vitro].

Investigations on the load capacity of an ultrasonic weld in transverse fractures, oblique fractures, and multiple fragment fractures by means of homologous bone fragments and Fimomed were taken up in vitro on 84 human femur diaphyses. It was possible to demonstrate the essential influence of the form of the fracture on the stability of the weld. The mechanical density was the highest in transverse fractures, small in oblique fractures, and the smallest in multiple fragment fractures. Conclusions were drawn up for the clinical use.
