Postnatal involution of the human adrenal fetal zone: stereologic description and apoptosis.

During the first weeks of postnatal development the fetal zone (FZ) of the human adrenal cortex rapidly disappears. Present studies aimed to describe cellular aspects of this involution and to correlate them with apoptosis. Adrenals of 38 newborns and infants (aged 1-240 days) were obtained from autopsies due to accidental death. Postnatal decrease in the adrenal gland weight may be divided into 2 distinct phases: rapid one lasting from the birth till the end of the second week of life, and followed by a slower one. This decrease is highly correlated with the decrease of both: the volume of FZ and quantity of its cells. During the whole investigated period, volume of the FZ decreases from 8,017 to 248 mm3 (from 70 to 3% of total adrenal volume) and the quantity of its parenchymal cells from 3 x 10(9) to 0.15 x 10(9) (from 40 to 5% of all parenchymal cells). The average volume of the FZ cell decreases from 2,200 to 1,400 microm3. The volume of FZ stroma (connective tissue and blood vessels) expands notably during the first day and decreases from day 5th of life. The apoptotic index of the FZ parenchymal cells is the highest during the second week of life (ca 30%), and subsequently declines (ca 20%). Thus, the rapid involution of the FZ is connected with marked hemorrhagic changes and increased apoptotic index of parenchymal cells, while during the slow phase (after 2nd week on) apoptotic index is lower and rather constant.

[Histopathologic differentiation of tumors derived from neuroblastic cells in children depending on age].

The histopathology of 224 tumours of neuroblastic origin was analysed in relation to the age of patients. Maturing neuroblastomas (NBS) dominated in infants, where as in the most numerous group of children 1-5 years of age, prevalence of neuroblastomas without any signs of maturation was noted. In children over 5 years of age a high incidence of tumours with evident maturation was noted (ganglioneuroblastomas and ganglioneuromas). The prevalence of maturing neuroblastomas in infants correlate well with an advantageous clinical course of neuroblastomas at this age.

[Local recurrences after salvage operations in the therapy of osteogenic sarcoma cases - an analysis of adverse effects based on studied cases].

Therapeutic approach in osteogenic sarcoma until 1982 was connected mainly with surgery. Introduction of chemotherapy to the therapeutic protocols of osteogenic sarcoma improved the 5 years survival of patients with osteogenic sarcoma from 20% to 60-70%. The approach to surgical treatment was also changed. The principle of this treatment is usually to perform an operation to spare the limb with an intraoperative frozen section examination of bone marrow. In our Institute during the last 14 years about 300 cases were diagnosed, out of which about 200 were treated surgically, among these about 50% underwent treatment by salvage operations. Only in 7 cases local recurrences of disease were confirmed. Five of these cases were tumours of distal metaphysis of the femur, one case of proximal metaphysis of the tibia and one case of proximal metaphysis of the humeral bone. The sex range M/F was: 5/2, the average age of onset about 14 years. Various factors playing a role in therapeutic adversities were analysed. Among these were: radical surgery, grade of differentiation of the tumour, histological subtype, soft tissue infiltration, reaction to preoperative therapy and the type of chemotherapy. The conducted analysis indicates the role played by sparing surgery in adversities in therapy (3 cases). Attention was also given to the distinct tendency of osteogenic sarcoma to produce vascular embolism which is a source of haematogenously spreading metastases. In 7 cases vascular tumour embolism caused the appaerance of metastatic focuses adjacent to the primary tumour. A certain role in therapeutic adverities is played by the lack of response to preoperative chemotherapy (4/7 cases). The influence of other factors needs further investigation.

[Metastases to the lungs in patients with osteogenic sarcoma--treatment results].

At the National Research Institute of Mother and Child, 158 patients, aged 3 to 21 years were treated for osteosarcoma, in the period 1985 - 1994. Metastases were found in 34 persons. Localisation of metastases: lungs - 32 cases, lungs and additionally bones - 6 cases, CNS - 1 case, bones without pulmonary changes - 2 cases. Chemotheraphy (acc. to different programmes) was the initial treatment. When stabilization or partial regression was observed, operative procedures were undertaken. In the case of progression, conservative treatment was used; changing chemothrapy protocol, or using palliative treatment. Following surgical procedures were made: thoracotomy - 8 cases, sternotomy - 5 cases, excision of bone tumour - 2 cases, laminectomy - 2 cases, excision of CNS tumour - 1 case. In the group of 18 patients where conservative treatment only was used, 2 children are alive. In the group of 16 patients where surgery was performed, 9 are alive. The authors suggest that in case of osteosarcoma it is profitable to extract metastatic foci in the period of stabilization after chemotherapy. It appears important to excise the metastatic foci even 2-3 times as this considerably prolongs survival in these children.

[Sacrococcygeal tumours--diagnosis-treatment strategies based on personal examinations].

Sacrococcygeal tumours very often appear soon after birth when they are usually benign. In older children more frequently malignant tumours are observed. The authors present diagnostic and therapeutic strategy based on clinical material consisting of 34 patients (15 benign tumours, 19 malignant tumours) treated at the National Research Institute of Mother and Child in the period 1980-1999.

[Keratoconus and coexisting diseases in monozygotic twins].

The study presents conditions mostly described as co-existing with the keratoconus in 7 patients (from among 300 examined) born from an uni-ovular twin pregnancy. Only one pair of twins showed keratoconus of both eyes and developing identically in both eyes.
