Primary tracheal neoplasms: recognition, diagnosis and evaluation.

Primary neoplasms of the trachea are much less common than malignancies of the larynx and lungs. Tracheal neoplasms account for less than 0.1 percent of all neoplasms. Their importance lies in the fact that they may initially be misdiagnosed, resulting in a delay in diagnosis ranging from months to years. The most common benign tracheal neoplasms are hemangiomas, squamous papillomas and fibromas. The most common tracheal malignancy is squamous cell carcinoma. Symptoms of these lesions are usually related to airway obstruction and include wheezing, dyspnea and cough.

Relationship between tryptophan biosynthesis and indole-3-acetic acid production in Azospirillum: identification and sequencing of a trpGDC cluster.

Screening the tryptophan (Trp)-dependent indole-3-acetic acid (IAA) production of different Azospirillum species revealed that A. irakense KA3 released 10 times less IAA into the medium than A. brasilense Sp7. A cosmid library of strain Sp7 was transferred into A. irakense KA3 with the aim of characterizing genes involved in IAA biosynthesis. Trp-dependent IAA production was increased in two transconjugants which both contained an identical 18.5 kb HindIII fragment from Sp7. After Tn5 mutagenesis, cosmids carrying Tn5 insertions at 36 different positions of the 18.5 kb fragment were isolated and transferred into strain KA3. IAA production by the recipient strains was screened by HPLC. The Tn5 insertions of 4 clones with decreased IAA production were mapped on a 2 kb SalI-SphI fragment. Recombination of Tn5 insertions at this locus into the genome of strain Sp7 led to Trp auxotrophic mutants. A 5.2 kb EcoRI-SalI fragment including the 2 kb SalI-SphI fragment was sequenced and six open reading frames were identified. Three of them were clustered and their deduced amino acid sequences showed significant similarity to TrpG, TrpD and TrpC, which are enzymes involved in tryptophan biosynthesis. One of the remaining open reading frames probably encodes an acetyltransferase. The region responsible for the enhanced Trp-dependent IAA production in strain KA3 corresponded to trpD, coding for the phosphoribosyl anthranilate transferase.

Physical map and properties of a 90-MDa plasmid of Azospirillum brasilense Sp7.

Homology was previously detected between the DNA restriction fragments containing Rhizobium meliloti nodulation genes and the 90-MDa plasmid, p90, of Azospirillum brasilense Sp7. Two DNA loci from Sp7 genome that complement mutations in the exopolysaccharide synthesis genes, exoB and exoC, of R. meliloti were also shown to be present on the plasmid. A more detailed characterization of the plasmid was undertaken to establish its physical map and to localize the nod homologies and other specific regions. Six loci were mapped, the region homologous to the nodulation genes, nodPQ, of R. meliloti, the exoB and exoC mutation-correcting loci, a locus for Ap resistance, a bla homology region different from the Ap resistance locus, and a region necessary for the maintenance of p90 as an independent replicon. Mobilization into Agrobacterium tumefaciens of p90-Tn5-Mob was obtained at a frequency of 10(-4), with the plasmid helper pJB3JI. Self-transfer of p90 was not demonstrated. Fragments of p90 hybridized with a plasmid of 90 MDa present in most A. brasilense and some A. lipoferum strains, suggesting a plasmid family in Azospirillum.

Ampicillin Resistance

[Reconstruction of tangential and circular infected bone defects].

In the treatment of bone infections the reconstruction and rehabilitation of bone defects is a problem that often requires treatment secondary to curative treatment of the infection. For the reconstruction of smaller and more extensive defects we used predominantly (92.7%) autogenous (autologous) untreated spongiosa and in only 7% of the cases allogenic (homologous) spongiosa from an organ bank, this being added if necessary. Recently the additionally introduced vascularized bone chip has become a useful extension of the therapy concept. The problems and complications of defect reconstruction are demonstrated for 705 cases of bone infection with 472 defects of different sizes, based on a comprehensive classification with defect calculation. Surgical technical approach and special aspects of after-treatment are described, as well as the results for every group of cases. We achieved stability and freedom from infection in a total of 93.7% of the patients. As was to be expected, the problems grow with the size of the defect. Particularly problematic are joint infections with adjacent extensive circular defect.

[Clinical application and results with fixateur externe in septic and aseptic osteosyntheses at the lower extremity (author's transl)].

Osteosynthesis with fixateur externe is indicated in unstable, infected fractures, infected pseudarthroses and defect pseudarthroses, second-degree and third-degree open fractures as well as closed or unstable old fractures and pseudarthroses with marked damage of the soft parts, as well as in all arthrodeses of the knee joint or ankle joint (foot arthrodeses). The main field of application is the shaft of the lower leg with its adjacent joints. To achieve satisfactory stability, mounting should always be three-dimensional and numerous additional details must be taken care of, all of which are described in this article. Whereas in the case of an infection no change of osteosynthesis is permissible after external fixation, it should be considered under certain conditions in the aseptic range. Results and complications after 123 aseptic and 88 septic osteosyntheses with fixateur externe during the last 5 years are described in detail and discussed.

Fracture Fixation, Internal

[Therapy, especially arthrolysis and its results, in stiffened elbow joints after fractures].

Arthrolysis plays an important part in the treatment of stiffened elbow joints after fractures. The article reports on the technical details of surgery and postoperative positional-change and exercise therapy, basing on the experience collected with a group of 30 patients. The intra-operatively detected causes of stiffening, postoperative complications and, finally, the mobility growth rates obtained in this manner, are shown and discussed while comparing these results with those achieved by other authors. This comparison also serves as a basis for a detailed discussion. The authors suggest as a standard measure for assessing the success of arthrolysis, an extent of mobility amounting to 0-30-110 degrees; in any case, a bending capacity (flexion) of 110 degrees should be accepted as a standard measure for such an assessment.
