Metabolic syndrome variables at low levels in childhood are beneficially associated with adulthood cardiovascular risk: the Bogalusa Heart Study.

OBJECTIVE: Most epidemiologic studies have focused on the adverse impact of the metabolic syndrome on cardiovascular (CV) disease. However, information on the relationship between the clustering of metabolic syndrome variables at favorable levels in childhood and the measures of CV risk in adulthood is not known. RESEARCH DESIGN AND METHODS: The study cohort included 1,474 individuals (552 blacks and 922 whites) who were examined for CV risk factors in childhood (aged 4-17 years) and again in adulthood (aged 19-41 years) in Bogalusa, Louisiana, during 1982-2003, with an average follow-up period of 15.8 years. RESULTS: In childhood, 9.0% of the cohort displayed clustering of three- or four-criterion risk variables at the bottom quartiles of BMI, homeostasis model assessment of insulin resistance, systolic blood pressure, and total-to-HDL cholesterol ratio. The clustering was significantly higher than expected by chance alone (P < 0.01). These children, compared with those having clustering of less than three risk variables at the bottom quartiles, had a lower prevalence of metabolic syndrome in adulthood (clustering at top quartiles) (3.8 vs. 14.6%, P < 0.001). A higher prevalence of clustering of risk variables at low levels in childhood was associated with negative parental histories of coronary heart disease (9.4 vs. 5.0%, P = 0.024) and hypertension (10.5 vs. 6.6%, P = 0.012). Mean values of carotid intima-media thickness in adulthood decreased with an increasing number of risk variables clustering at the bottom quartiles in childhood (P for trend = 0.013). CONCLUSIONS: The constellation of metabolic syndrome variables at low levels in childhood is associated with lower measures of CV risk in adulthood.

[Sex differences in brain stem auditory evoked potentials and P300 examination in patients with mild cognitive impairment and Alzheimer's disease].

OBJECTIVE: To compare the sex differences on brain stem auditory evoked potentials (BAEP) and P300 in the elderly patients with mild cognitive impairment (MCI) and the patients with Alzheimer's disease (AD). METHODS: Thirty-eight elderly patients with MCI, 26 cases of AD and 20 health controls (HC) were examined with BAEP, P300 . Sex difference of the variables was compared inter-and intra-groups. RESULTS: Significant sex differences of BAEP were found in the latency period wave III, V of left side in the MCI group (P <0.01), in the latency period wave IV, V of left side in HC group (P<0.01), and no such differences were found in AD group. The females had longer latency period in P300 measurement than males in MCI group (P <0.01), but there were no significant differences within AD or HC groups. The males showed significant differences in wave I, II in left side between the MCI and AD groups. The males also showed significant difference in wave I, III approximate, equals V in left side, wave IV in right side between AD and HC groups, and so did the females in wave I approximate, equals V in both sides. The males had significant difference in the left wave III, IV and right wave I, IV, V between MCI and HC groups, and so did the females in right side wave I, II, V. In P300, longer latency waves were found in AD group than in MCI group. Both sexes showed significant differences in the latency of Fz, Cz, Pz between AD and HC groups, but no significant differences were found in the latency of Fz, Cz, Pz in the females between the MCI and HC groups. CONCLUSION: Sex differences were found in the examination of both BAEP and P300 in MCI group, but not in AD group. The people of same sex have different expression of BAEP and P300 among MCI, AD and HC groups, suggesting the sex difference should be considered in the differential diagnosis.

[Protective effects of modified recombinant human acidic fibroblast growth factor on small intestine after ischemia/reperfusion injury in rats].

OBJECTIVE: To explore the protective effect of modified recombinant human acidic fibroblast growth factor (rhaFGF) on small intestinal after ischemia/reperfusion (I/R) injury in rats. METHODS: The clamp on the superior mesenteric artery (SMA) was removed after clamping it for 45 minutes to replicate I/R injury of the intestine in the rat. Rats were then divided randomly into sham operation group, normal saline treatment group and rhaFGF treatment group, in which the rats of the normal saline treatment group were injected 0.1 ml of normal saline and that of the rhaFGF group were given 4 microg of rhaFGF immediately after reperfusion. The content of D-lactate in the plasma was determined and the changes in intestinal pathology were observed. At the same time, the rates of apoptosis of intestinal epithelial cells were assessed 2, 6, 12 and 24 hours after I/R, and compared to the sham operation group. RESULTS: The plasma content of D-lactate in the saline treatment group at 6 hours after I/R reached (0.34+/-0.09) mg/L and was significantly higher than that in the rhaFGF treatment group((0.23+/-0.07)mg/L, P<0.05). It was shown histologically that the intestinal structures were damaged more seriously in saline treatment group than in rhaFGF group. The apoptosis rates in the saline treatment group and rhaFGF group were elevated significantly, peaking at 12 hours after I/R injury((62.8+/-1.7)% in saline group and (42.5+/-2.6)% in rhaFGF treatment group), then the rate began to fall. There was statistically significant difference between the two groups at 12 hours after I/R injury. CONCLUSION: rhaFGF can reduce content of D-lactate in the plasma and rate of apoptosis of epithelial cells in the intestine after I/R injury, thus it seems to afford a protective effect on the small intestine after I/R injury in rats.

[Isolation and purification of eccrine sweat glands in human skin].

OBJECTIVE: To investigate the method of isolation and purification of epithelial cells of human eccrine sweat gland in vitro. METHODS: Through digesting human skin with collagenase type II, cells of human eccrine sweat gland were isolated. Highly purified gland cells were obtained through transferring into the conditioned medium with a micropipette for at least three times under an inverted microscope. Primary culture was started immediately after purification. Cells could be further purified by enzyme-digestion to eradicate the fibroblasts. RESULTS: Collagenase type II could digest dermal collagen with little damage to gland cells. Isolated cells from human sweat glands were adherent to the wall of culture flask, and they grew well in cultures. The problem of contamination by tissue debris and other cells such as fibroblast could be overcome. CONCLUSION: Isolation and purification of human sweat gland cells in vitro are still facing tough problems. Gland cells are successful to isolate and cultivate without contamination.

[Effects of adipose tissue extract on rat skin regeneration in vitro].

OBJECTIVE: To investigate the effects of adipose tissue extract on growth of rat skin in vitro. METHODS: Neonatal rat skin (area of 2 mm x 2 mm) was cultured in Dulbecco's modified Eagle's medium (DMEM) containing 10% adipose tissue extract and 10% phosphate buffered solution (PBS). After being cultured for 6 days, the area of skin was measured. RESULTS: Skin in both groups grew in area. With the addition of adipose tissue extract, the skin measured (8.43+/-1.40) mm2 in area after 6 days, and it was approximately 2 times of that of PBS group, in which the skin area was (4.18+/-1.11) mm2. The difference was statistically significant (P<0.05). CONCLUSION: Adipose tissue extract promote skin growth, which may be attributed to the presence of some growth factors in the extract.

[Protective effects of acidic fibroblast growth factor on intestinal ischemia/reperfusion in rats].

OBJECTIVE: To observe the change in hepatic and renal functions, change in the plasma D-lactate level, and the expression of proliferating cell nuclear antigen (PCNA) after intestinal I/R injury, so as to explore the effects of reconstructive human acid fibroblast growth factor(aFGF) on intestinal I/R injury in rats. METHODS: One hundred and twenty-six Wistar rats were divided into sham-operated, ischemia (45 minutes) plus reperfusion, reconstructive human aFGF treatment (2, 4, 8 microg aFGF) and wild type aFGF(2, 6, 12, and 24 hours, respectively) groups. Hepatic and renal functions and the levels of plasma D-lactate were determined and the expression of PCNA was assessed. RESULTS: Compared with all other groups, bowel barrier function and hepatic and renal functions showed most marked deterioration in sham-operated group. The damages were less marked in reconstructive human aFGF group compared with other groups 24 hours after ischemia/reperfusion of the intestine, and the protective effect was best shown when 4 microg of aFGF was given. The trend of expression of PCNA was similar to that of changes in D-lactate level. CONCLUSION: Wild type reconstructive human aFGF treatment significantly improves the outcome of ischemia/reperfusion injury to the intestine, and the effect is dose-dependent.

[Relationship between keloid proliferation and apoptosis of epithelial cell with destruction of structure and function of skin appendages].

OBJECTIVE: To investigate the relationship between keloid proliferation and destruction of skin appendages (SAs). METHODS: Pathological biopsies of keloids were derived from 17 patients who underwent scar resection. All samples were divided into 4 groups: infiltrating growth locus of keloids (K-I, n = 9), proliferative keloids (K-P, n = 17), atrophic keloids (K-A, n = 10), and edging normal skin (K-N, n = 6). Normal skin derived from thorax of patients was used as control (NS, n = 6). The density of SAs and the expressive characteristics of pan-cytokeratin (CKp), cytokeratin 19 (CK19), secretory component of glandular epithelium (SC), proliferating cell nuclear antigen (PCNA), and apoptosis related proteins (Bcl-2 and Bax) were observed with immunohistochemical method. RESULTS: Compared with K-N and NS, the density of SAs expressing CKP and SC in keloids was apparently decreased, and remnant of CKp protein was observed after the disappearance of SAs structures. Protein expression of Bax was increased in epithelial cells of most SAs. SAs containing positive immunostaining signals of Bcl-2, PCNA and CK19 exhibited squamous epithelization and abnormal structure. The structure of SAs underwent 3 morphological stages: infiltrating, proliferating, and maturing. In correspondence to each stage, SAs underwent proliferation, structural destruction, and fibrosis which were caused by cellular migration, inflammatory reaction, and vascular occlusion respectively. CONCLUSION: Abnormal proliferation of epithelial cells and their structural destruction of SAs may be associated with tissue fibrosis in keloid lesion.

[Effect of yifei jianpi recipe on inflammatory cells, levels of interleukin-8 and tumor necrosis factor-alpha in sputum from patients with chronic obstructive pulmonary disease].

OBJECTIVE: To explore the effect and the mechanism of yifei jianpi recipe (YFJPR) on patients with chronic obstructive pulmonary disease (COPD). METHODS: Forty patients with COPD in stable phase were randomly divided into two groups, the treated group and the control group. Indexes including the total and differential count of inflammatory cell in sputum, levels of interleukin-8 (IL-8) and tumor necrosis factor-alpha (TNF-alpha), as well as the percentage of forced expiratory volume in one second in its predicted value (FEV1%) and ratio of FEV1/forced vital capacity (FVC) in patients were measured before and after treatment, and compared with those in 20 healthy subjects. RESULTS: All the indexes measured in patients before and after treatment were significantly different from those in healthy subjects (P < 0.01). Differential count of polymorphonuclear neutrophil (PMN) and levels of IL-8 and TNF-alpha in sputum in the treated group significantly decreased after treatment (P < 0.01), while the non-PMN differential count and levels of FEV1% and FEV1/FVC significantly increased (P < 0.01). But in the control group, changes only showed in increasing of FEV1% and FEV1/FVC (P < 0.05 or P < 0.01). And the effects in the treated group were better than those in the control group (P < 0.01). CONCLUSION: YFJPR can play a therapeutic role on patients with COPD by way of reducing the airway inflammatory reaction.

[Improvement of left ventricular hypertrophy by anemic-correcting erythropoietin therapy in chronic renal insufficiency].

OBJECTIVE: Left ventricular hypertrophy (LVH) is an independent predictor of morbidity and mortality in dialysis patients. It remains unclear whether efforts to correct anemia in patients with mild-to-moderate chronic renal insufficiency (CRI) can reverse LVH. This prospective multi-center Chinese cohort study evaluates left ventricular mass index (LVMI) evolution in anemic CRI patients with or without recombinant human erythropoietin (rHuEPO) therapy. METHODS: Six centers enrolled 158 patients with serum creatinine from 147 to 400 micromol/L, and 86 of whom with hemoglobin (Hb) levels < 110 g/L received rHuEPO (Group A). Forty patients with comparable Hb levels (< 110 g/L) but did not receive rHuEPO (Group B) and those with Hb >/= 110 g/L (Group C, n = 32) were served as controls. Echocardiographic studies were performed to evaluate LVMI at baseline and every 3 months during a two-year period. RESULTS: At baseline, the prevalence of LVH was 72.1% in Group A, 72.5% in Group B and 59.4% in Group C. LVMI was inversely correlated with Hb levels (r = -0.70, P < 0.01). There was no difference in age, gender, aetiology of renal failure, blood pressure (BP) and cardiovascular risk factors between the 3 groups. The administration of rHuEPO in Group A significantly increased Hb levels from (93.8 +/- 14.6) g/L to (111.2 +/- 10.3) g/L and decreased LVMI from (142.6 +/- 25.7) g/m(2) to (132.4 +/- 18.5) g/m(2). The prevalence of LVH decreased 16.3% after a partial correction of anemia at 24 months, whereas Hb levels in controls (Group B and Group C) tended to decrease and LVMI significantly increased compared with baseline. The prevalence of LVH was significantly increased in Group B and C after 24 months. The percentage of patients whose serum creatinine level doubled during the follow-up was 3.4% in Group A, 15.0% in Group B and 9.4% in Group C, the difference between Group A and Group B being significant (P < 0.05). In addition, good BP control was obtained without any adverse effects. CONCLUSION: High prevalence of LVH was present in pre-dialysis CRI patients, which is associated with severity of anemia. Early treatment of anemia with rHuEPO can reverse LVH in CRI patients.

[Investigation on sleep status of college and high school students].

OBJECTIVE: To investigate the sleep status of college and high schools students. METHODS: Pittsburgh sleep quality index (PSQI) and self-manufactured questionnaires about siesta habits were used as tools. Three groups of students from medical college (MC), senior high school (SS) and junior high school (JS) were surveyed. RESULTS: In the group MC, SS and JS, the occurrence rates of sleep disorders were 27%, 62% and 54%, respectively, and in which the appearance rates of insomnia were 17%, 19% and 19%, longing for sleep were 10%, 43% and 35% respectively. And there were no significant difference between schoolboy and schoolgirl. The occurrence rates of slack breathing were different (5/155, 1/154) significantly between group SS and JS. The distinct differences also were found in group JS and MC, in which students felt hot (10/155, 1/122) and in all the three groups, in which students felt sleepy (55/155, 62/154, 13/122) whereas the difference of sleepy between group SS and JS was comparatively distinct (55/155, 62/154). Significant differences were also found between group JS and SS, MC in average sleep time of (7.65 +/- 0.87) hours, (7.16 +/- 0.83) hours, and (7.10 +/- 0.57) hours. The time of falling asleep (median 15 min, 10 min, 20 min) and siesta habit (8/155, 19/154, 75/122) among group MC and SS, JS were different respectively and markedly, whereas siesta habit differences between group SS and JS were comparatively distinct (8/155, 19/154). CONCLUSION: Students in high school showed higher rate of longing for sleep, and this implicated they fall short of sleep time greatly and siesta could improve their sleepy signs.

[Expression of human-mouse chimeric antibody directed against Chikungunya virus with site-specific integration system].

AIM: To obtain CHO/dhfr(-) cells line with integrated FRT sequence in the chromosome transcription active site and to express human-mouse chimeric antibody directed against Chikungunya Virus by using the cell line. METHODS: The fusion gene of FRT and HBsAg was constructed by PCR and cloned into the MCS of pCI-neo to construct pCI-FRT-HBsAg. The pCI-FRT-HBsAg was transfected into CHO/dhfr(-) cells and cell clones with high expression of HBsAg were screened by detecting the amount of HBsAg with ELISA. A CHO cell clone with the highest expression was chosen and named as CHO/dhfr(-) FRT(+). pAFRT HFLF, a expression plasmid of chimeric antibody with RFT sequence was transfected into CHO/dhfr(-) FRT(+) cells and cell clones with high expression of the chimeric antibody were screened by increasing concentration of MTX. A CHO cell clone with high expression of the chimeric antibody was cultured in large scale and supernatant was collected from which the chimeric antibody was purified. The purified chimeric antibody was analyzed by SDS-PAGE, Western blot and IFA. RESULTS: A CHO/dhfr(-) cells line with integrated FRT sequence in the chromosome transcription active site was obtained successfully. A cell clone with yield of 5 mg/L of chimeric antibody was obtained, as compared with routine CHO cell expression system with a yield of 2 mg/L. CONCLUSION: A cell line with integrated FRT sequence in the chromosome transcription active site was obtained and with it human-mouse chimeric antibody directed against Chikungunya virus was expressed. This system lays a solid foundation which can be used for expressing antibodies and other proteins.

[Obesity and diseases in the elderly inpatient].

OBJECTIVE: To explore the obesity distribution in old people and the relation between senile obesity and health. METHODS: First, a questionnaire was designed which included chronic disease history, body mass index (BMI), physiological value, biochemistry index, anti-oxidation index, diagnosis of diseases, etc. Second, the measure and detection methods were unified; and the last, the investigation was made along with daily clinical work by clinicians. RESULTS: We received 391 questionnaires. The overweight rate was 36.1% and the obesity rate was 7.9% . Total anti-oxidation activity in serum (TAS) and superoxide dismutase (SOD) decreased with body mass index (BMI), and the value in the obesity group was the lowest; Malonaldehyde (MDA) of overweight obesity was the largest. The mean blood pressure, blood fat, and blood glucose as well as the prevalence of cardiovascular disease, hyperlipemia, and glycuresis increased with BMI; and the value in the obesity group was the largest. CONCLUSION: The prevalence of the senile obesity was below the average and the senile obesity complications were various and serious, and perhaps related to imbalance of free radical's production and cleanup, so the senile obesity seriously harmed old people's health.

[Gene expression of p38 mitogen-activated protein kinase and its MAPKKs in skin at different developmental stages and its possible biological significance].

OBJECTIVE: To investigate the gene expression of p38 mitogen-activated protein kinase (p38MAPK) and its upstream signaling molecule (mkk3 and mkk6) in fetal skin at different developmental stages and postnatal skin and its potential biological significance. METHODS: The fetal skin biopsies were obtained from human embryo of spontaneous abortion at gestational ages from 13 to 32 weeks and postnatal skin specimens were collected from patients (4-16 years) undergoing plastic surgery. After the morphological characteristics of skins at different developmental stages were detected with pathological methods, the gene expressions of p38MAPK, mkk3 and mkk6 in skins were examined with reverse transcription-polymerase chain reaction analysis (RT-PCR). RESULTS: The gene expressions of p38MAPK, mkk3 and mkk6 could all be detected in fetal and postnatal skins. In fetal skins, these 3 genes were strongly expressed. Along with fetal growth and development, the gene expressions of p38MAPK and its upstream signaling molecules were faded gradually. In postnatal skin, the mRNA contents of these 3 genes were significantly decreased in comparison with those in fetal skin (P < 0.01). CONCLUSION: p38 MAPK mediated signal pathways might be involved in the skin development at embryonic stage and in the determination of cutaneous structure and function, and also in wound healing at postnatal stage. The relative increment of these gene transcription in younger fetal skin might be one of the reasons why cutaneous cells proliferate rapidly and the wounds heal without scar.

Imaging proliferation in brain tumors with 18F-FLT PET: comparison with 18F-FDG.

UNLABELLED: 3'-Deoxy-3'-(18)F-fluorothymidine ((18)F-FLT) is a recently developed PET tracer to image tumor cell proliferation. We characterized (18)F-FLT PET of brain gliomas and compared (18)F-FLT with (18)F-FDG PET in side-by-side studies of the same patients. METHODS: Twenty-five patients with newly diagnosed or previously treated glioma underwent PET with (18)F-FLT and (18)F-FDG on consecutive days. Three stable patients in long-term remission were included as negative control subjects. Tracer kinetics in normal brain and tumor were measured. Uptake of (18)F-FLT and (18)F-FDG was quantified by the standardized uptake value (SUV) and the tumor-to-normal tissue (T/N) ratio. The accuracy of (18)F-FLT and (18)F-FDG PET in evaluating newly diagnosed and recurrent gliomas was compared. More than half of the patients underwent resection after the PET study and correlations between PET uptake and the Ki-67 proliferation index were examined. Patients were monitored for a mean of 15.4 mo (range, 12-20 mo). The predictive power of PET for tumor progression and survival was analyzed using Kaplan-Meier statistics. RESULTS: (18)F-FLT uptake in tumors was rapid, peaking at 5-10 min after injection and remaining stable up to 75 min. Hence, a 30-min scan beginning at 5 min after injection was sufficient for imaging. (18)F-FLT visualized all high-grade (grade III or IV) tumors. Grade II tumor did not show appreciable (18)F-FLT uptake and neither did the stable lesions. The absolute uptake of (18)F-FLT was low (maximum-pixel SUV [SUV(max)], 1.33) but image contrast was better than with (18)F-FDG (T/N ratio, 3.85 vs. 1.49). (18)F-FDG PET studies were negative in 5 patients with recurrent high-grade glioma who subsequently suffered tumor progression within 1-3 mo. (18)F-FLT SUV(max) correlated more strongly with Ki-67 index (r = 0.84; P < 0.0001) than (18)F-FDG SUV(max) (r = 0.51; P = 0.07). (18)F-FLT uptake also had more significant predictive power with respect to tumor progression and survival (P = 0.0005 and P = 0.001, respectively). CONCLUSION: Thirty-minute (18)F-FLT PET 5 min after injection was more sensitive than (18)F-FDG to image recurrent high-grade tumors, correlated better with Ki-67 values, and was a more powerful predictor of tumor progression and survival. Thus, (18)F-FLT appears to be a promising tracer as a surrogate marker of proliferation in high-grade gliomas.

[Correlation of clusterin expression to prognosis of bladder carcinoma].

BACKGROUND & OBJECTIVE: Clinically, molecular prognostic markers for bladder carcinoma are still rare. Recently, up-regulation of clusterin protein has been suggested to relate with development and prognosis of several human cancers, but its relation with bladder cancer is unclear. This study was to analyze correlation of expression of clusterin protein to clinicopathologic parameters and prognosis of bladder cancer with tissue chip. METHODS: A tissue microarray containing 81 cases of bladder carcinoma was constructed. The expression of clusterin was detected by immunohistochemistry; its correlation with clinicophathologic parameters was analyzed. RESULTS: Of the 81 cases of bladder cancer, 69 were detectable by immunohistochemistry, 32 (46.4%) of which showed overexpression of clusterin protein. Overexpression rate of clusterin was significantly higher in poorly differentiated (G3 grade) tumors than in well differentiated (G1-G2 grades) tumors (75.0% vs. 34.7%, P=0.002), and was significantly higher in invasive (T2-T4 stages) tumors than in superficial (Ta-T1 stages) tumors (65.4% vs. 34.9%, P=0.014). Expression of clusterin was negatively correlated with prognosis of bladder cancer patients (log-rank=5.88, P=0.015); the recurrence-free survival time of patients with overexpression of clusterin was shorter than that of patients with normal expression of clusterin (37.3 months vs. 48.8 months). CONCLUSION: The overexpression of clusterin might be a molecular prognostic marker of bladder cancer.
