Penalised regression improves imputation of cell-type specific expression using RNA-seq data from mixed cell populations compared to domain-specific methods.

Gene expression studies often use bulk RNA sequencing of mixed cell populations because single cell or sorted cell sequencing may be prohibitively expensive. However, mixed cell studies may miss expression patterns that are restricted to specific cell populations. Computational deconvolution can be used to estimate cell fractions from bulk expression data and infer average cell-type expression in a set of samples (e.g., cases or controls), but imputing sample-level cell-type expression is required for more detailed analyses, such as relating expression to quantitative traits, and is less commonly addressed. Here, we assessed the accuracy of imputing sample-level cell-type expression using a real dataset where mixed peripheral blood mononuclear cells (PBMC) and sorted (CD4, CD8, CD14, CD19) RNA sequencing data were generated from the same subjects (N=158), and pseudobulk datasets synthesised from eQTLgen single cell RNA-seq data. We compared three domain-specific methods, CIBERSORTx, bMIND and debCAM/swCAM, and two cross-domain machine learning methods, multiple response LASSO and ridge, that had not been used for this task before. We also assessed the methods according to their ability to recover differential gene expression (DGE) results. LASSO/ridge showed higher sensitivity but lower specificity for recovering DGE signals seen in observed data compared to deconvolution methods, although LASSO/ridge had higher area under curves than deconvolution methods. Machine learning methods have the potential to outperform domain-specific methods when suitable training data are available.

On the rigidity of polynorbornenes with dipolar pendant groups.

A range of polynorbornenes (PNBs) with fused dipolar pendant groups at C-5,6 positions was synthesized by ring-opening metathesis polymerization catalyzed by a ruthenium carbene complex (Grubbs I). Photophysical studies, EFISH measurements, and atomic force microscopy images have been used to investigate the structures and morphology of these polymers. These results suggest that the polymers may adopt rigid rod-like structures. The presence of the double bonds in PNBs appeared to be indispensable for the rigidity of the polymers. Interaction between unsaturated pendant groups may result in coherent alignment leading to a rod-like structure.

Polymorphic catechol-O-methyltransferase gene, duration of estrogen exposure, and breast cancer risk: a nested case-control study in Taiwan.

BACKGROUND: Polymorphic catechol-O-methyltransferase (COMT) catalyzes the O-methylation of catechol estrogens, which are hypothesized to participate in estrogen-induced carcinogenesis. METHODS: We examined 87 cases and 341 population controls in Taiwan to determine the association between the functional genetic Val158Met polymorphism in membrane-bound form of COMT gene and female breast cancer risk. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated by conditional logistic regression. RESULTS: There was no overall association between COMT genotype and individual susceptibility to breast cancer. However, COMT-L variant genotypes appear to pose increased risk of breast cancer in women with greater duration from menarche to first full-term pregnancy (>8 years) (OR, 2.67; 95% CI, 1.00-7.36). CONCLUSIONS: This study based on limited sample sizes suggests that there may be no overall association of COMT genotype with breast cancer, but the COMT-L allele could pose enhanced risk of breast cancer in the presence of relevant environmental exposures, as most low penetrance gene are expected to act through gene-environment interactions.

Therapeutic options for proximal ureter stone: extracorporeal shock wave lithotripsy versus semirigid ureterorenoscope with holmium:yttrium-aluminum-garnet laser lithotripsy.

OBJECTIVES: To compare the safety and cost-effectiveness of ureterorenoscopic holmium:yttrium-aluminum-garnet (YAG) laser lithotripsy (URSL) with extracorporeal shock wave lithotripsy (ESWL) for proximal ureteral stones. METHODS: This investigation assessed 220 patients with upper ureteral stones. Those in the ESWL group were treated on an outpatient basis using the Medispec Econolith 2000 (Medispec, Germantown, MD) under intravenous sedation. URSL was performed with a 6/7.5F semirigid tapered ureterorenoscope and holmium:YAG laser under spinal anesthesia on an inpatient basis. A successful outcome was defined as the patient being stone free on radiography 1 month after treatment. The stone size, success rate, postoperative complications, and cost were evaluated in each group. RESULTS: A total of 220 patients were enrolled in this study. Hematuria and flank pain were the most common complaints in each group. The mean stone burden +/- SD was 58.7 +/- 3.1 mm2 in the ESWL group and 108.4 +/- 10.0 mm2 in the URSL group (P = 0.000). The accessibility of the semirigid ureterorenoscope for upper ureteral stones was 98.1% (101 of 103), and the stone-free rate achieved after one treatment was 83.2% (84 of 101). The initial stone-free rate of in situ ESWL was 63.9% (76 of 119). Significantly, the initial stone-free rate of the URSL group was superior to that of the ESWL group (P = 0.001). The average cost in the URSL group appeared to be lower than that in the ESWL group (P = 0.000). CONCLUSIONS: The results of this study have demonstrated that URSL achieved excellent results for upper ureter calculi. In terms of cost and effectiveness, this procedure should be the first-line therapy for proximal ureter stones.

Genetic polymorphisms of FAS and FASL (CD95/CD95L) genes in cervical carcinogenesis: An analysis of haplotype and gene-gene interaction.

OBJECTIVE: Whereas human papillomavirus (HPV) infection is necessary but not sufficient for cervical carcinogenesis, host genetic variations may confer individual susceptibility. Resistance to apoptosis is a hallmark of cancer in which FAS/FAS ligand signaling plays an important role. The present study examines the hypothesis that genetic polymorphisms in FAS and FAS ligand genes, alone or in combination, are associated with cervical carcinogenesis. METHODS: The genotypes of FAS -670A/G, FAS -1377G/A, and FASL -844C/T were assessed in 143 patients with high-grade squamous intraepithelial lesions (HSIL), 175 patients with invasive squamous cell carcinomas (SCC), and in age-matched controls by real-time PCR with allele-specific TaqMan probes. The status of cervical high-risk HPV infection was determined and adjusted to test the independence of genotype in the risk assessment. RESULTS: The A-allele and AA-genotype frequencies of FASA -670G were significantly higher in HSIL/SCC than in controls (60% vs. 54%, P = 0.04, OR 1.26 [95% CI 1.01-1.57]; 38.0% vs. 28.6%, P = 0.02, OR 1.70 [95% CI 1.07-2.70]). No association between FAS -1377 or FASL -844 polymorphisms and HSIL/SCC could be identified. The FAS -1377A/-670A haplotype conferred a higher risk for HSIL/SCC (OR 3.05, 95% CI 1.28-7.30) than FAS -670A alone (OR 1.26, 95% CI 1.28-7.30). The interaction between FAS -670AA and FASL -844CC genotypes was associated with a risk of HSIL/SCC (OR 2.13, 95% CI 1.06-4.29) higher than that of the FAS -670AA genotype alone (OR 1.70, 95% CI 1.07-2.70). CONCLUSIONS: The FAS -1377A/-670A haplotype in combination with FASL -844C is associated with cervical carcinogenesis.

Clinical observations of the effect of antidiuretic hormone on nocturia in elderly men.

OBJECTIVE: To evaluate the effect of desmopressin on nocturia, based on patients' subjective scoring of nocturia, as desmopressin is widely used to treat nocturnal enuresis and nocturnal polyuria. PATIENTS AND METHODS: We investigated a specific subgroup of 28 men with benign prostate obstruction and nocturia who were treated with desmopressin. Patients with nocturnal polyuria were excluded. All the patients were refractory to treatment with antimuscarinics or anticholinergics, e.g. oxybutynin, tolterodine, and propantheline bromide. We assessed the effect of desmopressin using a quantitative nocturia score and analysed its synergistic effect with alpha-blockers. RESULTS: The mean frequency of nocturia was 6.1 before desmopressin and most (86%) patients had an improvement in nocturia within 1-12 weeks of treatment with desmopressin. There was a 43% reduction in nocturia after using desmopressin (P < 0.001). The correlation coefficient between the number of nocturnal voids and the reduction in nocturia after treatment with desmopressin was 0.756, indicating that the more severe the nocturia, the more effective was desmopressin. CONCLUSIONS: Desmopressin is effective for refractory nocturia in elderly men with no nocturnal polyuria, and has limited side-effects.

A new aristolactam alkaloid and anti-platelet aggregation constituents from Piper taiwanense.

A new alkaloid, piperolactam E, and fourteen known compounds have been isolated from the stem of Piper taiwanense. Bioassay-guided fractionation of the methanolic extract led to the isolation, from the chloroform-soluble part, of 4-allylcatechol, eugenol, trans-caffeic aldehyde, 2-hydroxy-1-methoxy-4 H-dibenzo[ de,g]quinoline-4,5-(6 H)-dione, piperolactam B, piperolactam C, and piperolactam E as the active principles of anti-platelet aggregation in vitro. The derived 1,2-diacetoxy-4-allyl-benzene and eugenol acetate exhibit stronger anti-platelet aggregation activities induced by arachidonic acid than 4-allylcatechol and eugenol, respectively.

Comparison between extracorporeal shock wave lithotripsy and semirigid ureterorenoscope with holmium:YAG laser lithotripsy for treating large proximal ureteral stones.

PURPOSE: Management of large impacted upper ureteral calculi remains challenging for urologists. These calculi are frequently associated with obstructive uropathy and deteriorated renal function. Extracorporeal shock wave lithotripsy (SWL) is the least invasive treatment but its success rate is decreased for large impacted upper ureteral calculi. According to the American Urological Association guidelines on ureteral stones published in 1997, the appropriateness of ureteroscopy decreases when stone size exceeds 1 cm. However, the application of advanced ureteroscopy and techniques has increased the success rate of treating proximal ureter calculi. In this study we compared the safety and efficacy of ureterorenoscopic holmium:YAG laser lithotripsy (URSL) with SWL for large impacted proximal ureteral stones. MATERIALS AND METHODS: This study evaluated 82 patients with large impacted upper ureteral stones. Two patients were excluded from study owing to conversion to open surgery. SWL and URSL were performed in 41 and 39 patients, respectively. Those in the SWL group were treated on an outpatient basis with Medispec Econolith 2000 (Medispec, Germantown, Maryland) under intravenous sedation. The URSL was performed with a 6/7.5Fr semirigid tapered ureterorenoscope and holmium:YAG laser with the patient under spinal anesthesia on an inpatient basis. Successful outcome was defined as the patient being stone-free on radiography 1 month after treatment. Stone size, success rate, postoperative complications and cost were analyzed in each group. RESULTS: A total of 80 patients were enrolled in this study. Hematuria and flank pain were the most common complaints in each group. Mean stone size +/- SD was 1.28 +/- 0.04 cm in the SWL group and 1.51 +/- 0.05 cm in URSL group (p = 0.0009). Accessibility of the semirigid ureterorenoscope for impacted upper ureteral stones was 95.1% (39 of 41) and the stone-free rate achieved after 1 sitting was 92% (36 of 39). The initial stone-free rate of in situ SWL was 61% (25 of 41). Notably, the initial stone-free rate in the URSL group was better than that of the SWL group (p = 0.003). The efficiency quotient was 0.53 for URSL and 0.59 for SWL. The average cost in the URSL group appears to be lower than in the SWL group. Both groups were free of major complications. CONCLUSIONS: This study demonstrated that URSL achieved excellent results for upper ureteral calculi greater than 1 cm. Thus, this procedure should be considered first line therapy for large proximal ureteral stones.

Anti-platelet aggregation and chemical constituents from the rhizome of Gynura japonica.

A novel quinonoid terpenoid, (-)- alpha-tocospirone, a new chromanone, (-)-gynuraone, as well as three new steroids, (22 E,24 S)-7 alpha-hydroperoxystigmasta-5,22-dien-3 beta-ol, (22 E,24 S)-stigmasta-1,4,22-trien-3-one, and (24 R)-stigmasta-1,4-dien-3-one, together with 15 known components, were isolated from the rhizome of Formosan Gynura japonica. Structures of the new compounds were determined through spectral analyses and chemical evidence. Several of the isolated compounds, caryophyllene oxide, 6-acetyl-2,2-dimethylchroman-4-one, vanillin, 2,6-dimethoxy-1,4-benzoquinone, and benzoic acid exhibited significant anti-platelet aggregation activity in vitro.

Renal angiomyolipoma with lymph node involvement: a case report and literature review.

Renal angiomyolipomas (AMLs) are relatively common tumors containing fatty tissue, blood vessels, and muscular cells in various proportions. Typical AMLs can be diagnosed without histological confirmation by a combination of ultrasound and computed tomography imaging in up to 95% of cases. In contrast, simultaneous involvement of the kidney, renal vein, or lymph nodes is uncommon and might be confused with a metastasizing malignant tumor. We present a pathologically proven case of the very uncommon simultaneous involvement of the kidney and the lymph nodes in AML.

The methylenetetrahydrofolate reductase 677C-->T polymorphism and lung cancer risk in a Chinese population.

BACKGROUND: We examined the relationship of the MTHFR C677T genotype with the risk of lung cancer in a nested case-control study conducted in a cancer screening follow-up study in Taiwan. MATERIALS AND METHODS: A total of 59 histologically confirmed lung cancer cases and 232 cancer-free controls were recruited between July 1990 and December 2000. The MTHFR C677T genotype was determined by a PCR-restriction fragment length polymorphism assay. RESULTS: The MTHFR 677T variant genotype, including 677CT and 677TT, was observed in 39% of cases and 47% of controls. The resultant adjusted odds ratio (OR) was 0.71 (95% confidence interval [CI] = 0.39-1.30). The inverse association of the MTHFR 677T variant genotype with lung cancer risk was apparent in both smokers and nonsmokers. CONCLUSION: These results suggest that the MTHFR 677T variant genotype reduces the risk of lung cancer. Larger studies incorporating dietary folate intake and the variant genotypes are needed to clarify our results.

The MTHFR C677T polymorphism, estrogen exposure and breast cancer risk: a nested case-control study in Taiwan.

BACKGROUND: We evaluated the effects of the MTHFR C677T polymorphism and its interaction with estrogen exposure on breast cancer risk in a nested case-control study conducted in Taiwan. MATERIALS AND METHODS: A total of 88 histologically confirmed breast cancer cases and 344 cancer-free controls were recruited between July 1992 and December 2000. The MTHFR C677T genotype was determined by a PCR-RFLP-based assay. All subjects completed in-person interviews. RESULTS: There was a significant trend of breast cancer in relation to prolonged exposure to estrogens prior to the first full-term pregnancy (FFTP) (p for trend = 0.0015). In contrast, there was no statistically significant association between the risk of breast cancer and the MTHFR C677T genotype. However, a significantly elevated risk of breast cancer predisposed by the MTHFR 677T variant genotype (CT and TT) was observed in women with prolonged exposure to estrogens prior to FFTP (adjusted OR = 4.98, 95% CI = 2.00-12.43). CONCLUSION: The results of this study suggest that the MTHFR 677T variant genotype per se may have no overall association with breast cancer risk, but a sizable association could be observed in the presence of relevant environmental exposure.

Modified bowel preparation to reduce infection after prostate biopsy.

BACKGROUND: Infectious complications after ultrasound guided prostate biopsy are an important issue of concern. We found a higher infection rate with traditional bowel preparation, the phosphate enema, for prostate biopsy and so we modified our technique. In addition, we tried to assess the efficacy of this modified method for aged patients in an agricultural area who have poor compliance or inaccuracy when self-administering bowel preparations. METHODS: Between April 2002 and May 2005, all patients who received prostate biopsy were reviewed retrospectively. Exclusion criteria included patients who had an indwelling Foley catheter, symptomatic urinary tract infection or suspected prostatitis before prostate biopsy. Group I consisted of patients who self-administered a phosphate enema at home. Group II had a phosphate enema combined with povidone-iodine administered by a doctor at the hospital. All patients took oral fluoroquinolone (500 mg) twice daily for a period of one day before the procedure. Both groups received trimethoprim (160 mg) with sulfamethoxazole (800 mg) twice daily for three days after the biopsy. Postoperative infection was defined as an oral temperature higher than 37.7 centigrade or any episodes of chills with painful digital rectal examination. RESULTS: There were 65 patients in Group I and 157 patients in Group II. Within Group I, six patients (9.23%) were found to have a symptomatic infection with leukocytosis or chills; none were found in Group II. Between Group I and II, different bowel preparation was the only parameter shown to have statistical significance on the infection rate. CONCLUSIONS: Bowel preparation before prostate biopsy is not standardized among urologists. Phosphate enema with povidone-iodine administered at the hospital is an effective way to reduce the infection rate for agricultural people who have poor compliance or inaccuracy when self-administering bowel preparations.
