[Experimental study of hydroxycamptothecine and theprubicine in the treatment of adenoid cystic carcinoma lung metastases].

OBJECTIVE: To investigate anticancer effect of hydroxycamptothecine and theprubicine in the treatment of established human adenoid cystic carcinoma (ACC) lung metastasis of nude mice and to determine the best therapy sequence of the two drugs. METHODS: The model of ACC lung metastasis (60 animals) was established by injecting the ACC-M cells into the nude mice from their tail veins. Animals were divided into six groups: v acuity control group, HCPT group, THP group, HCPT and THP group 1, HCPT and THP group 2, HCPT and THP group 3 (No.1 - No.6). Different treatment sequence was performed on these groups. The lung samples were collected and observed after HE staining. The area of metastases and lung tissue were analyzed by the images analysis system. RESULTS: The size of the metastatic lesion in the treatment groups were smaller than that in the control group (P < 0.01). The treatment effects between the group with simultaneous administration of HCPT and THP and the group with single drug administration were not different (P > 0.05). No.5 and No.6 groups showed better effect of treatment than the other treatment groups (P < 0.05). CONCLUSIONS: There are two recommended sequences: (1) HCPT should be used after THP; (2) THP is used 48 hours, After HCPT is administered. Because using two drugs together will decrease the anticancer effect, HCPT and THP should not be used simultaneously.

[Modulation of Chinese regimen granules of Shenyang on peripheral blood lymphocyte subsets of SD rats with SCC of tongue].

OBJECTIVE: Prospective research demonstrated that Chinese regimen granules of Shenyang could prolong survival time and improve survival rate of patients with oral squamous cell carcinoma (SCC). But the mechanism was not clear. The purpose of this study was to investigate Shenyang's effect on peripheral blood lymphocyte subsets of SD rats with SCC of tongue and explore immunological mechanism. METHODS: Among 80 SD rats fed by 0.002% 4-nitroquinoline-1-oxide (4NQO) drinking water for 36 weeks, 61 rats with SCC of tongue had been found and were randomly divided into 4 groups, namely Shenyang A, Shenyang B, positive and blank control groups. Before and after high and normal dosage of Shenyang, acanthopanax senticoside and water had been given for 15 days respectively, peripheral blood lymphocyte subsets were detected with flow cytometry. The data were statistically analyzed with paired t Test. RESULTS: Percentage of CD3+ CD4+ T cell and CD3-CD161a+ NK cell, ratio of CD4+/CD8+ were increased. Percentage of CD3+CD8+ T cell was decreased, and the effect was better than that of acanthopanax senticoside in improving the percentage of CD3-CD161a+ NK cell. CONCLUSION: Among anti-tumor mechanisms of Shenyang it is that corrects disorder of lymphocyte subsets and increases percentage of CD3-CD161a+ NK cell.

[Strengthen the scientific research on oral hereditary diseases].

There are many kinds of oral hereditary diseases. Although some attempts have been tried to find the genetics of those diseases, still fewer information are available than the other hereditary diseases at present. Much is needed to do in the future. This article reviewed some cutting-edge findings on the genetics of several oral hereditary diseases including hereditary diseases involving the teeth, periodontal tissues, soft and hard tissues of the oral and maxillofacial region, as well as cleft lip and palate.

[Modulation of Chinese prescription Shenyang on Th1/Th2 cytokines in serum of SD rats with squamous cell carcinoma of the tongue].

PURPOSE: Prospective research demonstrated that Chinese prescription Shenyang could prolong the survival time and improve the survival rate of patients with oral squamous cell carcinoma (SCC). But the mechanism was not clear. The purpose of this study was to investigate effect of Shenyang on the level of Th1/Th2 cytokines in serum of SD rats with SCC of tongue and clarify its mechanism. Secretion of Th1-type cytokines, such as IFN-gamma, IL-2, TNF-alpha and Th2-type cytokines, such as IL-4, IL-10 were detected by enzyme-linked immunosorbent assay (ELISA). Statistical analysis was made with paired t test. RESULTS: Shenyang significantly enhanced ,the serum levels of Th1-type cytokine, TNF-alpha and IL-2 (P<0.05), and decreased the expression of Th2-type cytokine, IL-4 and IL-10(P<0.05). Serum IFN-gamma had been increased to some extent. CONCLUSION: Reversing Th2 to Th1 cytokines and then enhancing the immunity may be one of the mechanisms in anti-tumor effect of Shenyang. [Immunodetections and analysis of peripheral blood for patients with oral cancer undergoing operation].

PURPOSE: To investigate the immunological function for patients with oral cancer undergoing operation. METHODS: Peripheral blood was taken from patients with oral cancer before and after operation. Proportions of CD3(+) total T, CD4(+) Th, CD8(+) Tc/Ts, CD19(+) total B, CD16(+)/CD56(+)NK were detected by flow cytometry (FCM). Detections of serum tumor necrosis factor alpha (TNF-alpha) and soluble interleukin-2 receptor (sIL-2R) were performed with enzyme-linked immunosorbent assay(ELISA). Paired t test was used for statistical analysis. RESULTS: The proportions of CD8(+)Tc/Ts, CD19(+) total B and CD4(+)/CD8(+) increased significantly after surgery. Post operative proportion of CD16(+)/CD56(+)NK decreased more significantly. The concentration of TNF-alpha was less than that before operation(P<0.05). CONCLUSIONS: Surgery could change immunological function and alleviate immunosuppression of oral cancer patients. Detections for immune conditions of oral cancer patients might have important clinical values in prognosis evaluation and guiding treatment, especially immunotherapy, after operation.

[Chemotherapy with higher or lower dose of teniposide combined with cisplatin and pingyangmycin for oral squamous cell carcinoma].

OBJECTIVE: To compare the clinical efficacy and toxicity of teniposide (VM26) of higher dose with those of lower dose, both combined with cisplatin (CDDP) and pingyangmycin (PYM), in the treatment of patients with squamous cell carcinoma of oral and maxillofacial region (SCCOMR). METHODS: Sixty-five patients with SCCOMR entered into this study prospectively. Thirty-three patients were treated with higher dose of VM26 (total dose was 320 mg) combined with CDDP and PYM (PTP1), the other thirty-two patients were treated with lower dose (total dose was 158 mg) of VM26 combined with CDDP and PYM (PTP2). RESULTS: Thirty-three patients received a total of 38 cycles of PTP1. The overall response rate was 81.82% (27/33). Thirty-two patients received a total of 36 cycles of PTP2 and showed overall response rate by 81.25% (26/32). There was no significant difference between PTP1 and PTP2 groups in response rate (P > 0.05). But the blood toxicity was more severe in PTP1 group than in PTP2 group (P < 0.01). Bone marrow depression rate (1-4 stage) was 48.48% in PTP1 group versus 25.00% in the other group. CONCLUSIONS: A high response rate of 81.25% and relatively slighter adverse events could be obtained for lower dose of VM26 combined with CDDP and PYM (PTP2). So, the chemotherapy schedule, PTP2, a novel teniposide based regimen in SCCOMR could be employed and spread in clinical practice.

[Prognosis evaluation and statistical analysis].

With the progress of medical sciences, many new methods and protocols have been developed for clinical diagnosis and treatment. And evaluation of disease prognosis has been emphasized increasingly. Thus, choosing appropriate statistical parameters and analytic methods become more and more important for objective evaluation. This article reviews the parameters used in evaluation of patients' prognosis, influencing factors on prognosis, design of research on prognosis and statistical analysis. The causes for bias and problems needing attention in evaluation of prognosis are also discussed.

[Relevant factors and management for patients with squamous carcinoma in tongue without clinical cervical lymphatic node metastasis].

OBJECTIVE: To discuss the treatment of cN(0) tongue carcinoma patients. METHODS: 185 cases of the mobile tongue carcinoma patients (male 102, female 83, aged 28 to 88) treated with surgery from 1988.5 to 1995.6 had been followed up and retrospectively analyzed. Extensive resection of the primary tumors and neck dissections were performed, and all the samples were pathological positive. RESULTS: The cervical lymphatic node metastasis rates for stage I-II, III-IV disease, grade I, II disease were 16.66%, 38.05%, 17.42% and 37.50% respectively. And the rates were 9.00%, 31.37% and 55.55% for submucous infiltration, muscle infiltration and perineural infiltration, respectively. The overall 5 year survival was 72.43%, and the 5 year specific survival rate was 44.44% and 83.96% for those having or not having cervical node metastasis. The levels of 29 patients with positive node metastasis for 148 cN(0) patients were submandibular and submental lymphatic nodes (22.64%), superior deep cervical lymphatic nodes (35.84%), middle deep cervical lymphatic nodes (26.41%), inferior deep cervical lymphatic nodes (15.09%), posterior neck lymphatic nodes (0.00%). The over all 5 year survival rates for selective neck dissection were 85.13% and 21.62% in therapeutic dissection (chi(2) = 29.73, P < 0.01). Patients performed selective neck dissection the rates were 68.96% and 89.07% respectively with or without lymphatic node metastasis. Comparably the 5-year rate was only 20.00% for the patients performed the therapeutic dissection with lymphatic node metastasis. CONCLUSIONS: (1) cN(0) patients should be observed carefully in stage I, and the selective dissection must be performed in stage II-IV. (2) Supraomohyoid ND is essential for T(2) patients, and functional ND is essential for T(3 - 4) patients. (3) There is correlation between cervical metastasis and the stage, grade or infiltration of tongue cancer (P < 0.05). The prognosis could be expected from these factors.

[CT assist in surgical design for the posterior pharyngeal flap].

OBJECTIVE: To establish a method for assist design of the posterior pharyngeal flap (PPF). METHODS: The width, length of velopharyngeal space in repose and phonation of "i" in 26 cases with VPI were measured with CT. The width and length of PPF needed, the location of the pedicle and the contact position with the soft palate were analyzed. RESULTS: The width of velopharyngeal space in repose ranged from 15.10 to 37.25 mm, with the average of 27.01 mm, the length was 12.54 to 30.39 mm, with the average of 22.10 mm. The width in phonation of "i" was 9.11 to 34.06 mm, with the average of 17.69 mm, the position of the pedicle wasall located at the level of the first cervical vertebra or above and the contact area of the flap with the soft palate was in the middle posterior 1/3 mostly. CONCLUSION: As a method to quantitatively evaluate velopharyngeal function, CT can assist in design for the PPF.

[Treatment of distraction osteogenesis in the patients of obstructive sleep apnea-hypopnea syndrome with micrognathia].

OBJECTIVE: To apply the treatment of distraction osteogenesis(DO) to obstructive sleep apnea-hypopnea syndrome(OSAHS) patients with croniomaxillofacial deformities. METHODS: All 46 OSAHS patients with micrognathia are had polysomnography(PSG) study and cephalometric analysis. Their age from 4 to 18 years old, the mean age is 11.4. The number of temporal mandibular joint (TMJ) ankylosis with micrognathia, micrognathia; 1st & 2nd bronchial arch syndrome and crouzon syndrome patients were 32, 9, 2 and 3 respectively. All were treated with DO. Maxilla or mandible was advanced from 5 to 35 micrometers; the mean advanced distance is 18.34 mm. They were all revaluated by PSG and cephalometric analysis postoperatively. RESULTS: All patients have good respond to the treatment. They have a better appearance and the narrow upper airway was enlarged remarkably, their AHI drop from 66.31 +/- 14.74 pre-operately to 3.16 +/- 1.70 pro-operately, and minimal posterior airway space(PAS) from (5.48 +/- 2.76) mm to (9.97 +/- 2.05) mm. There is remarkable difference (P < 0.001). CONCLUSION: DO is a good method for the patients of OSAHS with micrognathia.

[A clinical study on septic arthritis of temporomandibular joint: culture and identification of pathogenic bacteria].

OBJECTIVE: To explore the pathogenic organism of septic arthritis of temporomandibular joint and the culture condition of the available bacteria. METHODS: Joint fluid was aspirated from 30 patients in recent 5 years. Smear and Gram stain of joint fluid were done immediately, then cultured in blood agar, bacteria-preserving culture medium, trypticase soy agar(TSA) or trypticase soy broth(TSB) under aerobic or anaerobic condition, respectively. Pathogen identification was performed with biochemical methods. RESULTS: Microorganism were found with Gram staining in 15 out of 30 cases, but the culture of bacteria only succeeded in 5 cases. Staphylococcus saprophyticus and Staphylococcus aureus were the bacteria isolated. Bacteria was isolated in TSB mostly. CONCLUSIONS: 2 kinds of microorganism were isolated from joint fluid, and S. saprophyticus was reported for the first time. However, the positive rate was low, and the better technique for culture was expected.

[A clinical study on septic arthritis of temporomandibular joint: characteristics and diagnosis].

OBJECTIVE: To investigate the clinical characteristics and the procedure of early diagnosis of septic arthritis of temporomandibular joint (TMJ). METHODS: 30 consecutive patients with TMJ septic arthritis were included in this clinical study between 1995 and 2001. Physical examinations, joint fluid analyses, radiographic imagings and laboratory studies were performed in all cases. Sequelae were evaluated. RESULTS: The clinical data showed 29 of the 30 patients were more than 18 years old. The infections of the TMJs were mostly through hematogenous route, and the original infectious sites could not be found in many cases. The obvious inflammatory symptoms in TMJ areas and severe morbidity were not detected in most of the cases (25/30), and a high level of polymorphonuclear cells, and some fibro-protein or cartilage fragments were seen in the synovial fluid. Microorganism were found with Gram staining in 15 of 30 cases, but the culture of bacteria only succeeded in 5 cases. S. saprophyticus and S. aureus were the common kinds of bacteria isolated. The sequelae were mild, and most of the cases had only postinfectious osteoarthritis. CONCLUSIONS: Nowadays TMJ septic arthritis has shown some new characteristics, such as occult original infectious sites, atypical and mild symptoms, and slight sequelae; Joint fluid analysis is necessary for diagnosis.

[A clinical study on septic arthritis of temporomandibular joint: treatment].

OBJECTIVE: To investigate the procedure and effect on treating septic arthritis of temporomandibular joint (TMJ). METHODS: 30 patients were treated by antibiotic therapy, resting of the jaw, and local therapies. The local therapies included arthrocentesis with single needle in 20 joints, arthrocentesis with double needles in 13, arthrocentesis accompanied by submandibular drainage of the infratemporal space abscess in 1, and arthroscopy in 4. The therapeutic effects were judged by joint cavity puncture, mouth opening, jaw pain, and jaw function, and so on. Sequelae were evaluated. RESULTS: The overall success rate of the treatment was 100% (30/30). The success rates of the 4 different management were: 14/20 in arthrocentesis with single needle, 9/13 in arthrocentesis with double needles, 1/1 in arthrocentesis combined with submandibular drainage, and 4/4 in arthroscopy, respectively. The sequelae were mild, and most of the cases had only postinfectious osteoarthritis. CONCLUSIONS: Low pressure arthrocentesis, antibiotic therapy and resting of the jaw were important components of treatment in acute septic arthritis. If infratemporal space abscess was found, submandibular drainage should be done. Arthroscopy could be recommended to those who haven't got good results by arthrocentesis or those who have already had some sequelae, such as TMJ fibrous ankylosis.

[Long term follow-up results of Chinese prescription "Shen Yang" in the combined and sequential treatment of oral squamous cell carcinoma].

OBJECTIVE: To evaluate the long term therapeutic effect of Chinese prescription "Shen Yang" in the combined and sequential therapy for oral squamous cell carcinoma cases. METHODS: There are 238 cases with oral squamous cell carcinoma. They were divided into two groups randomly as "Shen Yang" experiment group and control group (placebo). 25 cases were precluded from the experiment group. 17 of them were due to unexperiment of taking "Shen Yang" within three months. 8 cases were lost of follow-up. 213 patients were included in this study. Among them, 104 cases in experiment group, and 109 cases in control one. Patients in both groups were followed-up for 5-10 years. Their life-curve was calculated by means of Logrank method. RESULTS: The survival rate of "Shen Yang" group was improved by 8.46%, 9.26%, 9.04% and 8.57% for 3-year, 5-year, 8-year and 10-year survival rates. But the differences between the two groups were not statistically significant (P=0.1936, P>0.05). CONCLUSION: Chinese prescription of "Shen Yang" had a tendency to improve the survival rate for oral squamous cell carcinoma patients.
