[Diagnosis and surgical management of petrous apex cholesteatoma].

OBJECTIVE: To explore the etiology, diagnosis and surgical management of petrous apex cholesteatoma. METHODS: Twelve cases of petrous apex cholesteatoma (primary 3, secondary 9) were retrospectively studied. RESULTS: Primary petrous apex cholesteatoma was characterized by non otorrhea history, normal tympanic membrane appearance and the initially occurring symptoms of facial paralysis and hearing loss. In contrast, secondary petrous apex cholesteatoma was characterized by otitis media history, perforated drum, hearing loss and facial paralysis. The treatment principle for either primary or secondary cholesteatoma is to remove all the cholesteatoma. Based on the status of hearing and location and extent of cholesteatoma within temporal bone, 4 surgical approaches were taken in our study, which were translabyrinth, middle cranial fossa, combination of translabyrinth and middle cranial fossa, craniocervical combination approach (intralabyrinth approach). The complications of these surgical procedures and their management were discussed. According to result of 4 months to 15 years follow-up, there were no recurrence cases up to now, except one, which was operated 4 times as recurrence. Facial nerve anastomosis (3/12) or decompression (3/12) was performed simultaneously in the operations. The function of facial nerve partially recovered from V to IV of House and Brackmann grading in 2 out of 3 anastomosis cases and from IV to III in 2 out of 3 cases of decompression. CONCLUSION: Although there are some differences in symptoms and etiology between primary and secondary petrous apex cholesteatoma, complete surgical removal of lesion is necessary for both. Surgical approaches are decided according to location and extent of the lesion and hearing status. Our study indicated that open cavity operation had more advantage in terms of morbidity compared with the closed cavity operation, which closes the auditory canal as a blind sac.

[Characteristics of the response of vestibular evoked myogenic potentials to different stimulus modes].

OBJECTIVE: Vestibular evoked myogenic potential (VEMP) has been utilized in clinic to test the function of saccule. In the present study, 3 stimulus modes were applied to 21 normal adults and intended to draw in a clinical test protocol. METHODS: The normal latency and amplitude of VEMPs of 21 normal subjects were recorded and the ratio of bilateral amplitudes and asymmetry were calculated. RESULTS: The response to binaural clicks were similar to the response of the monaural clicks. The interindividual variation in amplitude were large, but the latency varied little. CONCLUSIONS: VEMPs is a stable myogenic potentials. Monaural and binaural clicks stimulus can be used in clinical practice. The latter is appropriate for normal hearing subjects. Amplitude measurement is probable to evaluate the asymmetry of bilateral saccular function and may reveal saccule lesion.

[Inferior vestibular nerve impairment in auditory neuropathy].

OBJECTIVE: To investigate if auditory neuropathy have inferior vestibular nerve (IVN) lesion and to explore the relation between AN and the IVN lesion by vestibular evoked myogenic potentials (VEMPs). METHODS: VEMPs were observed in 13 patients with auditory neuropathy. And the relation among the duration, hearing threshold of lower frequency and speech discrimination score with VEMPs were observed. RESULTS: Fifty-four percent patients in auditory neuropathy had abnormal VEMPs. They took the form of lower amplitude and no response. The statistical analysis showed that the abnormality of VEMPs had no correlation with lower frequency hearing loss, the duration and speech discrimination score. CONCLUSIONS: The IVN dysfunction may coexist with auditory neuropathy, having lesion in the IVN. However, there was no significant relation between the severity of AN and VEMPs, which meant that AN and inferior vestibular neuropathy had their independence to some extent.

[Clinical analysis of 430 cases of skull base lesions].

OBJECTIVE: Through analysis of surgical treatment and repair methods for the skull base lesions, to summarize the type, sites, characters of the lesions and the methods of surgical treatment and repair, and to provide the beneficial experience for the surgeons. METHODS: Four hundred and thirty patients were hospitalized and treated surgically between Feb. 1982 and Dec, 2002, and their clinical data were analyzed. RESULTS: Of 430 patients, 265 (61.6%) were male, 165 (38.4%) female. The mean age was 36. 1 years old (range from 2.5 to 76.0). In 173 cases (40.2%), the lesions were on the left side of the skull base, 178 cases (41.4%) on the right, 79 cases (18.4%) involving the central and both side of the skull base. In 116 cases (27.0%), the lesions were located in anterior skull base, 57 (13.3%) in central, 174 (40.5%) in lateral and posterior, and the remainder 83 cases (19.5%) in two or more regions. Of 430 cases, 283 (65.8%) were tumors, among them, 154 (35.8%) were benign, 129 (30.0%) malignant, 76 (17.7%) trauma, 42 (9.8%) congenital, 18 (4.2%) infection, 11 (2.6%) unknown. All patients were surgically treated. Of them, 43 cases were treated for 2 times, 5 cases for 3 times, 4 for 4 times, 1 for 5 times, 3 for 6 times. The total were 514 times. The bone defect was reconstructed in 95 cases (22.1%). The dura mater was repaired in 117 cases (27.2%). The postoperative complications occurred in 27 cases. Two cases died in 2 and 3 days after operation because of brain hernia with the ligation of common carotid artery and internal carotid artery for artery rupture. No death occurred during operation. CONCLUSIONS: The majority of the skull base lesions were tumors. The male patients were more than female. Appropriate surgical approach and better perioperative treatment could harvest the satisfactory effect.

[Design and preliminary use of the device of subjective visual vertical].

OBJECTIVE: The device of subjective visual vertical (SVV) was made to test the gravity orientation of subjects visual. METHODS: The normative value was established firstly in the right, left and both eyes respectively. Then the test was applied to evaluate the function of otolith function (utricular function) in normal adult subjects and examined the SVV in some patients suffered from acute unilateral vestibulopathy. RESULTS: The difference of the SVV among right, left and both eyes was not significant and the normative value was lower than +/- 2 degrees. No matter one eye or both eyes, SVV had the tendency to lateraliseze to the right. CONCLUSIONS: The SVV is a reliable choice to test the otolith vestibulopathy, which is at least suitable for acute phase. The bilateral eyes open mode is recommended to test the subjects suspected of utricuar dysfunction.

[Construction of bicistronic eukaryotic vector containing basic fibroblast growth factor and study of their functions in gene therapy for hearing impairment].

OBJECTIVE: To construct basic fibroblast growth factor(bFGR) and enhance green fluorescence protein(EGFP) fusion gene eukaryotic expression vector internal ribosome entry site (pIRES)-bFGF-GFP and to evaluate the effect of transduction bFGF gene on noise induced hearing-loss in inner ear hair cells of guinea pigs. METHODS: Human bFGF cDNA was inserted into mammalian expressed plasmid pIRES-EGFP. The recombinant expression plasmid pIRES-bFGF-EGFP was transfected into inner ear of guinea pigs, using lipofectin method. The transduced bFGF gene was mediated by SA lipidsome. IRES- bFGF-GFP was administered into the round window as rescue agent at the same time of noise exposure or as a protective agent 7 days before. RESULTS: SA liposome-mediated bFGF expressed at a high level in the cochlea of guinea pigs, and in the rescue group, a significant lower hearing thresholds was displayed. pIRES- bFGF-EGFP could protect hair cells. It demonstrated that pIRES- bFGF-GFP could protect the inner ear both structurally and functionally. bFGF/EGFP gene could be transcripted and translated into inner ear hair cells of guinea pig. bFGF/EGFP gene could express a specific protein. The recombinant bFGF/EGFP had significant protective effect as well as manifestation of autonomous fluorescence. CONCLUSION: bFGF/EGFP fusion protein not only expressed in hair cells of guinea pig but also showed significant bFGF activity and autonomous fluorescence. IRES induced exogenous gene could enter the hair cells.

[Electrically evoked compound action potential thresholds of pediatric cochlear implant recipients and its clinical application].

OBJECTIVE: To investigate the possibility of using pediatric cochlear implant mapping protocol, to estimate the psychophysical levels based on the electrically evoked compound action potential (ECAP) threshold measured with the neural response telemetry (NRT) capabilities of cochlear corporation's CI24M device. METHODS: ECAP amplitude growth functions were regressed via NRT 3.0 software to determine ECAP threshold. 6 pediatric cochlear implant recipients' ECAP thresholds 1, 2 and 3 months after surgery were compared, meanwhile, intraoperative and postoperative ECAP thresholds in 7 pediatric recipients were compared. RESULTS: The ECAP amplitude growth functions were not linear when ECAP was close to threshold or saturation. There were no significant difference among the ECAP thresholds 1, 2 and 3 months after surgery. The average intraoperative ECAP threshold of 22 electrodes was 15 CL greater than postoperative threshold. It showed high relationship between them (R2 = 0.9154). CONCLUSION: ECAP threshold should be determined by regression of data from linear part of amplitude growth function. Only one NRT trial should be assessed when mapping protocol based on ECAP threshold to estimate pediatric map parameter. The intraoperative ECAP threshold can act as C-level at the initial mapping. [Clinical analysis of 95 patients with large vestibular aqueduct syndrome].

OBJECTIVE: To investigate the occurrence, genetic inheritance, and hearing loss conditions of large vestibular aqueduct syndrome (LVAS), and to measure the width of external aperture of vestibular aqueduct (VA) of LVAS, and to analyze the relationship between the hearing loss and the width of external aperture. METHODS: A retrospective review was conducted by the history, auditory, and vestibular function examinations in 95 patients (190 ears) of LVAS. The width of VA was measured by CT scanning criterion of each LVAS patients in the past 10 years (from 1992 to 2002). RESULTS: In 95 patients, there were male 65 cases and female 30 cases. The male cases was more than the females by a ratio of 2.2:1. Twelve cases (12.6%) would have familial genetic history. The average of the consult age was 7.6 years (range from 1.2 to 39 years). The most patients were misdiagnosed, the hearing deterioration originated from mild head trauma in 21 cases (22.1%), the hearing loss after common cold in 13 cases (13.6%). Nine cases (9.5%) misdiagnosed as sudden hearing loss, 5 cases (5.3%) as ototoxic hearing loss of Aminoglycoside antibiotic. The profound hearing loss was found in 172 ears (92.4%). The average width of external aperture of LVAS was 7.5 mm +/- 1.2 mm (mean +/- s, 77 cases, 154 ears). The level of hearing loss did not proportional to the width of VA. The hearing impairment could be derived from protein-rich endolymph refluxed into the cochlear and vestibule. CONCLUSION: The LVAS is distinct clinical entity characterized by fluctuate and progressive SNHL. The diagnosis depends upon the CT scan and MR image. If the hearing fluctuated and subsequently improved, the children were kept under conservation treatment. If the significantly decreased of hearing effected school performance, the cochlear implant might be considered.
