Streptococcal toxic shock syndrome caused by Streptococcus suis serotype 2.

BACKGROUND: Streptococcus suis serotype 2 (S. suis 2, SS2) is a major zoonotic pathogen that causes only sporadic cases of meningitis and sepsis in humans. Most if not all cases of Streptococcal toxic shock syndrome (STSS) that have been well-documented to date were associated with the non-SS2 group A streptococcus (GAS). However, a recent large-scale outbreak of SS2 in Sichuan Province, China, appeared to be caused by more invasive deep-tissue infection with STSS, characterized by acute high fever, vascular collapse, hypotension, shock, and multiple organ failure. METHODS AND FINDINGS: We investigated this outbreak of SS2 infections in both human and pigs, which took place from July to August, 2005, through clinical observation and laboratory experiments. Clinical and pathological characterization of the human patients revealed the hallmarks of typical STSS, which to date had only been associated with GAS infection. Retrospectively, we found that this outbreak was very similar to an earlier outbreak in Jiangsu Province, China, in 1998. We isolated and analyzed 37 bacterial strains from human specimens and eight from pig specimens of the recent outbreak, as well as three human isolates and two pig isolates from the 1998 outbreak we had kept in our laboratory. The bacterial isolates were examined using light microscopy observation, pig infection experiments, multiplex-PCR assay, as well as restriction fragment length polymorphisms (RFLP) and multiple sequence alignment analyses. Multiple lines of evidence confirmed that highly virulent strains of SS2 were the causative agents of both outbreaks. CONCLUSIONS: We report, to our knowledge for the first time, two outbreaks of STSS caused by SS2, a non-GAS streptococcus. The 2005 outbreak was associated with 38 deaths out of 204 documented human cases; the 1998 outbreak with 14 deaths out of 25 reported human cases. Most of the fatal cases were characterized by STSS; some of them by meningitis or severe septicemia. The molecular mechanisms underlying these human STSS outbreaks in human beings remain unclear and an objective for further study.

Human Streptococcus suis outbreak, Sichuan, China.

From mid-July to the end of August 2005, a total of 215 cases of human Streptococcus suis infections, 66 of which were laboratory confirmed, were reported in Sichuan, China. All infections occurred in backyard farmers who were directly exposed to infection during the slaughtering process of pigs that had died of unknown causes or been killed for food because they were ill. Sixty-one (28%) of the farmers had streptococcal toxic shock syndrome; 38 (62%) of them died. The other illnesses reported were sepsis (24%) and meningitis (48%) or both. All isolates tested positive for genes for tuf, species-specific 16S rRNA, cps2J, mrp, ef, and sly. A single strain of S. suis caused the outbreak, as shown by the identification of a single ribotype. The high death ratio was of concern; prohibiting backyard slaughtering ended the outbreak.

[A study on alpha-tricalcium phosphate bone cement carbon fiber-reinforced].

In order to improve the mechanical properties of alpha-tricalcium phosphate (alpha-TCP), we prepared surface-modified carbon fibers (CF) reinforced alpha-TCP composite bone cement. Bone cement was soaked in Ringer's body solution to test its capacity of fast formation of hydroxyapatite crystals and self-solidification. Scan electronic microscope (SEM) observation and compressive strength measurement were taken to analyze the mechanical properties and the micro- morphological structure of CF reinforced alpha-TCP bone cement. The results showed that the bone cement was transferred into hydroxyapatite plates after being soaked in Ringer's simulated body fluid for 5 days. Suitable amount of carbon fibers could well spread in and bond with the matrix of the bone cement. The mechanical properties of the bone cement have been improved by CF reinforcing; the compressive strength reaches 46.7 MPa when the amount of carbon fibers is 0.5% in weight percent, which is 22% higher than that of the non-reinforced alpha-TCP bone cement.

[Study on a new type of apatite/wollastonite porous bioactive glass-ceramic].

A novel glass-ceramic has been derived from sol-gel process. In this study XRD and FTIR analysis confirmed that the main crystalline phases of the material were hydroxyapatite/fluoroapatite [Ca10(PO4)6(OH,F)] and beta-wollastonite[beta-CaSiO3]; SEM examination showed that the microstructure contained many micro pores of 2-3 microm. After pore-forming, the material possessed good macro porous structure: the size of macro pores was 300-400 microm in diameter, and pores interconnected each other. Bioactivity of the material was preliminarily evaluated in the simulate body fluid. SEM observation revealed that a lot of apatite granules had been formed on the surface of the material after soaking within 7 days. Result shows that the novel sol-gel derived apatite-wollastonite-containing glass-ceramic has good bioactivity. Porous materials have suitable microstructure as well as macrostructure, which make it an excellent material to be used as bone-repairing materials and bone tissue engineering carrier materials.

[Research and development of A-W bioactive glass ceramic].

Studies on bioactive glass and glass-ceramic are important research high-lights in the field of biomedical materials. Due to their bioactivity, these materials can form a tight chemical bond with the living bone, when implanted. As a preeminent kind of these materials, A-W(Apatite/Wollastonite) bioactive glass ceramic has not only the excellent bioactivity and biocompatibility, but also the eminent mechanical properties, so it has been largely applied and developed in clinical practice. The development, preparation, properties, applications and the mechanism of its bond with bone are introduced in this paper. We will also put forward the prospect of the research and development of A-W bioactive glass ceramic.

[Trabeculetomy combined with implantation of silicon rubber slice for intractable glaucoma].

PURPOSE: To investigate the lowering-pressure outcome and postoperative complications of trabeculectomy with implantation of silicon rubber slice for intractable glaucoma. METHODS: Sixty eyes with intractable glaucoma, including 12 eyes with neovascular glaucoma, 14 eyes with aphakic or pseudophakic glaucoma, 4 eyes with congenital glaucoma, 18 eyes with failed filtering surgery glaucoma and 12 eyes with glaucoma secondary to trauma, were performed trabeculectomy with implantation of silicon rubber slice. The postoperative lowering-pressure outcome and complications also were observed. RESULTS: All patients were follow-up for 5 to approximately 34 months ( mean, 18.3 months) . The postoperative intraocular pressure of patients was 17.30 to approximately 37.19 mmHg [mean, (23.36 +/- 4.06) mmHg] at the last visit. Of 60 eyes, the postoperative intraocular pressure was below 21 mmHg in 38 eyes (63.3%), between 21 to 30 mmHg in 18 eyes (30%) and higher than 30 mmHg only in 4 eyes (6.7%). No serious complications and rejection related to implantation of silicon rubber slice occurred in this group during the follow-up period. CONCLUSION: Trabeculectomy combined with implantation of silicon rubber slice seems to be an ideal procedure for treatment of intractable glaucoma in developing countries, especially in vast country hospital in China, this surgery also can replace the expensive import drainage device implant (Molteno tube implant) therapy for intractable glaucoma.
