Effectiveness of loaded sit-to-stand resistance exercise for children with mild spastic diplegia: a randomized clinical trial.

OBJECTIVE: To investigate effectiveness of a functional strengthening program, the loaded sit-to-stand (STS) resistance exercise, for children with cerebral palsy (CP). DESIGN: A single-blind, randomized block design. SETTING: STS exercises were carried out at the children's homes. PARTICIPANTS: Twenty children (12 boys, 8 girls; age range, 5-12y) with spastic diplegia CP and classified by the Gross Motor Function Classification System as level I or II were stratified by their severity and age and randomly allocated into either the experimental or control group. INTERVENTION: Both groups received their regular physical therapy. The experimental group underwent loaded STS exercise 3 times a week for 6 weeks. MAIN OUTCOME MEASURES: Goal dimension scores of the Gross Motor Function Measure (GMFM), gait speed, 1 repetition maximum (1-RM) of the loaded STS, isometric strength of knee extensor, and Physiological Cost Index (PCI). The outcome measures were conducted at the beginning and end of the 6-week study. RESULTS: After loaded STS exercise, the experimental group showed statistically significant differences in GMFM goal dimension scores, 1-RM STS, and PCI from the control group. The changes in gait speed and isometric strength of the knee extensor did not differ significantly between the 2 groups. CONCLUSIONS: After the loaded STS exercise, children with mild spastic diplegia improved their basic motor abilities, functional muscle strength, and walking efficiency.

Skeleton pruning by contour partitioning with discrete curve evolution.

In this paper, we introduce a new skeleton pruning method based on contour partitioning. Any contour partition can be used, but the partitions obtained by Discrete Curve Evolution (DCE) yield excellent results. The theoretical properties and the experiments presented demonstrate that obtained skeletons are in accord with human visual perception and stable, even in the presence of significant noise and shape variations, and have the same topology as the original skeletons. In particular, we have proven that the proposed approach never produces spurious branches, which are common when using the known skeleton pruning methods. Moreover, the proposed pruning method does not displace the skeleton points. Consequently, all skeleton points are centers of maximal disks. Again, many existing methods displace skeleton points in order to produces pruned skeletons.

Structural characterization and transcriptional regulation of the gene encoding diapause hormone and pheromone biosynthesis activating neuropeptide in the cotton bollworm, Helicoverpa armigera.

We have cloned the gene encoding the diapause hormone and the pheromone biosynthesis activating neuropeptide in Helicoverpa armigera (Har-DH-PBAN). The Har-DH-PBAN gene contains six exons and five introns that fall in the same positions as in the Bombyx mori DH-PBAN gene (Bom-DH-PBAN). The transcription initiation site lays 29 bp upstream of the translation initiation site. Southern blot analysis suggests that a single copy of this gene is present per haploid genome. A structural comparison of DH-PBAN promoters between H. armigera and B. mori show similarities in the TATA box and in a potential binding site for a POU family transcription factor, POU-M2. However, testing of these DNA regions for factor binding in vitro and transcription assays in cell culture highlight significant differences in their regulation particularly in reference to the POU-M2 sites. Our results uncover common and different regulatory mechanisms at work in the control of DH-PBAN gene expression in H. armigera and B. mori.

Application of a novel integrated pointing device apparatus for children with cerebral palsy.

BACKGROUND: To improve the computer operation of children suffering from cerebral palsy (CP) with severe disabilities, more flexible pointing devices are required. This study investigates the effectiveness of a newly developed Integrated Pointing Device Apparatus (IPDA) that can integrate numerous commercial pointing devices. METHODS: We enrolled 27 children with quadriplegic CP and 15 healthy children. All children were required to perform three specific mouse operation tasks. Children with CP were classified into two groups based on hand operation: one hand (group A) and both hands (group B). The efficiency of children with CP in each mouse operation task was expressed as a percentage of that for normal children (% NL). RESULTS: Group A operated a standard mouse with their dominant hand; group B had to use both hands to operate a mouse via IPDA. Group A demonstrated better efficiency of continuous-clicking tasks than group B (p < 0.05). Group B had a similar level of efficiency in the target-acquisition task (30% NL) and drag-and-drop task (20% NL) as that of group A, although group B could not operate a standard mouse with one hand. All children in group A were spastic quadriplegia, while 30% of children in group B were spastic-athetoid (p < 0.05). All children in group B were at level 3 of the gross motor functional classification system (GMFCS) but only 57% of children in group A were at level 3 (p < 0.05). CONCLUSION: The IPDA can help some children with CP, who cannot utilize a commercial mouse alone, to achieve acceptable operational efficiency. The operation methods for children with CP were determined by their underlying motor control.
