Marek's disease virus-1 unique gene LORF1 is involved in viral replication and MDV-1/Md5-induced atrophy of the bursa of Fabricius.

Marek's disease virus (MDV), an alphaherpesvirus, causes severe immunosuppression and T cell lymphomas in chickens, known as Marek's disease (MD), an economically important poultry disease primarily controlled by vaccination. Importantly, it also serves as a comparative model for studying herpesvirus-induced tumor formation in humans. MDV encodes more than 100 genes, most of which have unknown functions. MDV LORF1 is unique to serotype I MDV (MDV-1), lacking homologs in other herpesviruses, and has not been explored yet. To this end, an infectious bacterial artificial chromosome (BAC) harboring the complete genome of the MDV-1 very virulent strain Md5 was generated, and the rescued rMd5 maintained biological properties similar to the parental virus both in vitro and in vivo. Subsequently, rMd5ΔLORF1, a recombinant Md5 virus deficient in pLORF1 expression, was generated by a frameshift mutation in the LORF1 gene. Chickens infected with rMd5ΔLORF1 exhibited a lower mortality rate and delayed bursal atrophy than those infected with the parental rMd5 and the revertant virus (rMd5-reLORF1). Consistently, viral loads of rMd5ΔLORF1 were obviously lower than those of rMd5 or rMd5-reLORF1 in the bursa, but not in the spleen. Importantly, we found that pLORF1 deficiency impairs viral replication in bursal B cells. Furthermore, we showed that pLORF1 associated with the cellular membrane, interacted with MDV structural proteins, and exhibited punctate colocalization with tegument or capsid proteins in the cytoplasm. Taken together, this study demonstrates for the first time that the MDV-1 unique gene LORF1 is involved in MDV-induced bursal atrophy but not in tumor formation.

Preparation and evaluation of a Carbopol/HPMC-based in situ gelling ophthalmic system for puerarin.

The purpose of this study was to develop a pH-triggered in situ gelling vehicle for ophthalmic delivery of puerarin. The effect of hydroxypropyl-beta-cyclodextrin (HP-beta-CD) on the aqueous solubility and in vitro corneal permeation of puerarin was also investigated. The puerarin solubility increased linearly and proportionally to the HP-beta-CD concentrations and 5% (w/v) HP-beta-CD enhanced its ocular permeability significantly. Carbopol 980NF was used as the gelling agent in combination with HPMC (Methocel E4M) which acted as a viscosity-enhancing agent. The optimum concentrations of Carbopol 980NF and HPMC E4M for the in situ gel-forming delivery systems were 0.1% (w/v) and 0.4% (w/v), respectively. When these two vehicles were combined, an in situ gel that had the appropriate gel strength and gelling capacity under physiological condition could be obtained. This combined solution could flow freely under non- physiological condition and showed the character of pseudoplastic fluid under both conditions. Both in vitro release studies and in vivo pharmacokinetics studies indicated that the combined polymer systems performed better in retaining puerarin than puerarin eye drops did. These results demonstrate that the Carbopol 980NF/HPMC E4M can be a viable alternative to conventional puerarin eye drops to enhance ocular bioavailability and patient compliance.

[Results of chorda tympani nerve application or no in tympanoplasty].

OBJECTIVE: To demonstrate the effect of chorda tympani nerve used in tympanoplasty. METHOD: One hundred and forty-one cases (154 ears) were divided into 3 groups: (1) chorda applying group (CA) 35 cases (41 ears): had perforation in pars tensa but chorda nerve still exist, the nerve were applied to spring and press a PORP or TORP prosthesis which were made by auto-osseous tissue and had a fine groove for nerve inlay. (2) residual pars tensa applying group (RTP) 67 cases (72 ears): residual pars tensa was used to spring and press prosthesis. (3) no applying group. (NA) 39 cases (41 ears). The effect measurement: ABG over 30 dB as excellent over 15 dB as effective; less than 10 dB as noneffective. Three groups were analyzed by S- t test. RESULT: Excellent, effective and noneffective ears (%): CA group: 29.3%, 51.2%, 19.5%; RPT group: 22.2%, 50.0%, 27.8%; NA group: 19.5%, 36.6%, 43.9%; the previous two groups contrast with NA group, P < 0.01. Pre- and post-operation ABG odds and paired contract: CA and NA, P < 0.05; RTP and NA, P < 0.05. In CA group only one ear had taste letdown temporarily and another ear had delayed and mild facial paralysis, both were auto-recovered within two weeks. CONCLUSION: Chorda tympani nerve applied in tympanoplasty for spring and press prosthesis can make PORP or TORP prosthesis more steady and improve hearing.

[Observation of tympanosclerosis operation result].

OBJECTIVE: To assess effects of multi-technique operation for tympanosclerosis. METHOD: A retrospective analysis of 157 cases (167 ears)of tympanosclerosis (the cases with only tympanic membrane calcification were excluded) was undertaken. All patients received the multi-technique if needed: (1) Open the attic, mastoid antrum etc; (2) Rebuild scute with auto-bone or infill the mastoid cavity with auto bone pieces; (3) Preserve or reconstruct the ossicular chain; (4) Utilize the absorbable Merogel to prevent re-adhering; (5) Press prostheses with tympanic chorda nerve as a spring; (6) Clear out hyperplastic bone on facial canal with Er-laser or small chisel. Postoperative audiometric measurements were undertaken within one month. And the follow up audiometric measurements were performed at three months after surgery, and then every half or one year later again. The last evaluations were considered as the results of follow up (taking the average hearing threshold at 0.5, 1.0, 2.0 kHz HL). RESULT: The mean air-bone gap (ABG) of 167 ears was (39.74 +/- 12.54) dB before operation, while it was elevated to (20.68 +/- 11.80)dB after operation (P < 0.01). Surgical treatment was found to be successful in seventy-one ears (42.5%) and effective in forty-eight ears (28.7%). The total effectiveness was 71.3% (119 ears). Followup result was (20.67 +/- 13.52) dB, compare with pre-operative hearing threshold (P < 0.01). However, it had no difference comparing with post-operative hearing threshold (P > 0.05). Of 96 ears which were followed up over 1 year, the average ABG was (21.94 +/- 11.16) dB comparing with that of the total 167 ears follow up result (P > 0.05). CONCLUSION: Using multi-technique can obtain better and consistent hearing results. Surgery for total ossicular chain fixation of tympanosclerosis is still difficult. Stapedectomy still needs to apply in tympanosclerosis cautiously.

[Study of hoses esophagoscopy].

OBJECTIVE: To explore a method of extracting the esophageal foreign body more effectively and safely, so as to reduce its complication and mortality. METHOD: The hoses, the endoscope (gastrofibroscope or electron-icgastroscope) and the tongs were used to make up the hoses esophagoscopy, using the technique of hoses esophagoscopy to extract the esophageal foreign body. RESULT: Hoses esophagoscopy was successful in removing the impaction in 30 of 33 adults. The other three patients with longer and sharp foreign body were treated by hoses and rigid esophagoscopy. No perforation or complication occurred. CONCLUSION: The technique of hoses esophagoscopy have the advantage of flexible endoscope and rigid esophagoscopy. It is a more effective and safe method for removing esophageal impactions.

[The recurrent nasal polyp analysed by mRNA difference to demonstrate].

OBJECTIVE: To look for the correlated gene of nasal polyp. METHOD: The nasal polyp was analysed by mRNA difference to demonstrate, and the fragment of writing was analysed and appraised. RESULT: The amplification products of normal conchae tissue were compared with the nasal polyp tissue under the condition of background with one accord. The total 11 difference fragments were discovered during the 150-700 bp. One of the fragments (X-1) was completely homology with the section series of the eight chromosome long arm. There was a zine finger protein in the upper stream and the down stream. The fragment expressed in nasal polyp tissue. CONCLUSION: Based on polymerase chain reaction (pcr), using differential display R. T. PCR (DD-RTPCR) technique, the difference express gene of nasal polyp could be effectively compared with normal nasal conchae tissue or cells, the correlated gene of nasal polyp tissue could screened out. The fragment (X-1) that was completely homology with the section series of the eight chromosome long arm may be a new identified gene which cause the nasal polyp.

[Tympanoplasty in the only hearing ear].

OBJECTIVE: To evaluate result of tympanoplasty in only hearing ear. METHOD: 15 cases were analyzed retrospectively. All cases were operated by tympanoplasty for middle ear diseases with conductive deafness. The contralateral ears of all cases had sensorineural deafness. RESULT: All ears obtained dry ear and 80% ears obtained hearing improvement. CONCLUSION: Tympanoplasty is availability in only hearing ear with middle ear diseases, but operation must be manipulated by senior expert.

[Bleeding control in sinonasal microsurgery].

OBJECTIVE: To explore the results of bleeding control in sinonasal surgery. METHODS: Between Jan 1998 and Jun 2000, 82 patients (144 sides) were observed and studied for the relationship between mental tension and bleeding. The effect of blood pressure control and operative time reduction was also observed. 58 sides were of type III, 71 sides of type II (among them, 56 sides belonged to stage II, 15 sides belonged to stage III), 15 sides were type I (among them, 4 sides belonged to stage I, 11 sides belonged to stage II). Of them, 60 patients had been operated for two times. All patients with sinusitis received micro-intranasal ethmoidectomy and 140 sides had had polyps removed. Sixteen patients received correction of deviated septum at the same time. 31 patients received resection of the inferior turbinate bone. Twenty-one patients were given Dridol' 5 mg and Dolantin 50 mg intravenously to control hypertension. Bleeding amount was compared between 21 hypertensive patients who used intravenous drugs and 57 hypertensive patients who did not use intravenous drugs during operation. To reduce operation time, the operation was simplified. RESULTS: The bleeding was limited to 10 ml in 99 sides(69%), the mean bleeding amount was controlled to (18.7 +/- 24.4) ml in 78(54%) sides of patients with hypertension. In 66 sides (46%) of patients with normal blood pressure, the mean bleeding amount was (13.2 +/- 16.8) ml (P < 0.05). In 21 patients in whom the hypertension was controlled, the mean bleeding amount was (13.0 +/- 6.7) ml. In 10 patients, the bleeding amount was (30.5 +/- 21.8) ml at the first operation with hypertension uncontrolled, the bleeding amount was only (11.0 +/- 8.2) ml at second operation when the blood pressure was controlled to normal (P < 0.05). After 6 months follow-up, 104 sides (72%) were cured, 32 sides (22%) improved, and 8 sides (6%) without improvement. CONCLUSION: Mental tension in operation can induce hypertension and increase bleeding, suitable sedation is necessary. With correct use of topical anesthesia and reduced operation time, the bleeding amount can be controlled to less than 10 ml in 69% of patients.

[Use of NiTi shape memory alloys in the eustachian tube to prevent and treat adhesive middle ear].

OBJECTIVE: To explore and assess the use of NiTi shape memory alloys in the eustachian tube to prevent and treat adhesive middle ear. METHODS: All preliminary study cases were divided into three groups (A: 56 cases of adhesive otitis media, B: 27 cases of otitis media with effusion, C: 52 cases without ear disease as control). The eustachian orifice in all cases was observed by endoscopy. Autopsy of Eustachian tube was done in two specimens. The NiTi shape memory alloys were then installed into the Eustachian tube to prevent and treat adhesive middle ear in 4 cases. The installer of NiTi shape memory alloys was designed by ourselves. RESULTS: The Eustachian orifice can be classified into three types: wide, narrow and fissured. They comprised of 20, 31 and 5 cases in group A, 9, 13 and 5 cases in group B and 45, 7 and 0 cases in group C, respectively. The differences among three groups were statistically significant (A, B vs C, chi 2 = 29.66, 25.44 respectively, P < 0.001), but the difference between A and B was not statistically significant (chi 2 = 1.59, P > 0.05). Two cases with adhesive otitis media gained good ventilation with Valsalva method and the audiometric improvement of 15-30 dB after the NiTi shape memory alloys installation. One of the two cases with otitis media with effusion gained A-B gap closure over 8 months, and the other still improved with ventilative tube removed. In all 4 cases, there was no any side-effect or signs of patulous Eustachian tube. CONCLUSION: Adhesive otitis media can be prevented and treated by using NiTi shape memory alloys without any side-effect, but more study is necessary.

[Missed diagnostic cholesteattomaous otitismedia].

OBJECTIVE: To explore the missed diagnostic cholesteattomaous otitismedia lacked of clinical symptoms in order to minimize the mistake in clinical diagnose. METHODS: A retrospective study of 31 cases with the missed diagnostic cholesteattomaous otitismedia, confirmed by surgery and pathology, was conducted. RESULTS: Twenty-one cases (67.7%) had not obvious otorrhea. There were only slight finding such as attic retraction, apophysis, small granulation tissue, ear wax covered or tiny, even non, perforation in the pars flaccida or the superior and posterior pars tensa of the ear-drum. In general, the hearing loss is slight or medium conductive deafness. The type B curve was showed in tympanogram. The diagnosis rate of X-ray was only 41.9%, and 80.7% for the computed tomography. The middle ear structures aggressed by cholesteames was epitympanum, ossicular chain, tympanic antrum, mastoid process and mesotympanum. The complications occurred in 11 cases (35.5%) were the expose of facial nerve or and the meninges, or, and the labyrinth fistula. CONCLUSION: The cases with the cholesteatoma lack obvious clinic symptoms would result in the missed diagnosis because of slow and long term during cholesteatoma development. Sometimes misdiagnosed or missed diagnosis happened to in such cases of cholesteatoma with little clinical manifestation, and it is so dangerous for the patients in such case. Therefore, otolaryngologist must pay more attention to the missed diagnostic cholesteattomaous otitismedia.
