Arthroscopic side-to-side rotator cuff repair.

PURPOSE: To study the results of arthroscopic repair of full-thickness rotator cuff tears using a side-to-side suture technique without fixation to bone. TYPE OF STUDY: Case series study of the long-term results of patients who underwent purely arthroscopic rotator cuff repair with a side-to-side suturing technique. METHODS: A retrospective review was performed of patients who underwent arthroscopic repair of full-thickness rotator cuff defects. Patients with full-thickness rotator cuff tears repaired in a side-to-side fashion without anchoring the repair to bone were selected. Patients were evaluated using a modified UCLA shoulder scoring system. The data collected were analyzed to determine the outcome in patients with a 4- to 10-year follow-up. RESULTS: A total of 105 arthroscopic rotator cuff repairs were performed in 104 patients between February 1990 and February 1996. Forty-two patients had a full-thickness tear of the rotator cuff that was repaired using a purely side-to-side suturing technique. The mean UCLA score of all patients in this group was 33; 23 patients reported excellent results, 18 good results, and 1 poor result according to the UCLA scoring system. CONCLUSIONS: In this series, 98% of patients qualified as a good to excellent result according to the UCLA shoulder score. This study shows that patients with a full-thickness defect of the rotator cuff tendon with anatomy amenable to side-to-side closure may be effectively treated with a purely arthroscopic repair using only a side-to-side suturing technique with excellent long-term clinical results. LEVEL OF EVIDENCE: Level IV.

Arthroscopic glenoid resurfacing with meniscal allograft: a minimally invasive alternative for treating glenohumeral arthritis.

The chronically painful arthritic glenohumeral joint recalcitrant to nonsurgical treatment modalities generally has been treated with an open arthroplasty type of procedure. Certain patients may benefit from a less invasive surgical technique in which a meniscal allograft is used to resurface the glenoid, resulting in decreased pain and increased function. We describe an arthroscopic method of glenoid resurfacing with a meniscal allograft to aid in the restoration of function by providing pain relief to patients debilitated by arthritic conditions of the glenohumeral joint.

A novel achiral seco-amino-cyclopropylindoline (CI) analog of CC-1065 and the duocarmycins: design, synthesis and biological studies.

The design, synthesis and DNA binding properties of a novel achiral and amino-containing seco-cyclopropylindoline analog (seco-amino-CI-TMI, 1) of the duocarmycins are described. Thermal induced DNA cleavage studies on pUC18 DNA revealed compound 1 to preferentially bind in the minor groove and to covalently react with AT-rich sequences, particularly at the underlined adenine-N3 group of 5'-AAAAA(865)-3'. This sequence specificity is similar to adozelesin and CC-1065. Using a 4-day continuous exposure, compound 1 inhibited the growth of K562 human chronic myeloid leukemia cells in culture. Compound 1 has appreciable cytotoxicity (IC50 value of 1.30 microM) relative to compound 2 (0.15 microM), the corresponding racemic and hydroxy-seco-CI-TMI analog. These results indicate that the aminophenethyl chloride group present in compound 1 has similar sequence specific and cytotoxic properties to the hydroxy-containing seco-precursors of CC-1065 and the duocarmycins. Moreover, the results suggest that the chiral center present in the natural products is not absolutely necessary for biological activity. The novel aminophenethyl halide moiety is, therefore, a useful template from which to develop future achiral analogs of CC-1065 and the duocarmycins.

Rubidium bis(trifluoromethanesulfonyl)imidate dioxane disolvate.

The bis(trifluoromethanesulfonyl)imidate anion crystallizes with Rb as the title dioxane 1:2 solvate, Rb(+).CF(3)SO(2)NSO(2)CF(3)(-).2C(4)H(8)O(2), with the anion in a transoid conformation, as opposed to the cisoid form typically seen when there are significant cation-anion interactions. The Rb(I) cation is eight-coordinate, interacting with one anion in a chelating fashion and with two other anions through the remaining sulfonyl O atoms. The latter interactions link ion pairs through the formation of Rb(2)O(2) dimers about inversion centers at (0, 1/2, 0) and (1/2, 1/2, 0), forming extended columns which run parallel to the a axis of the unit cell. Rb-dioxane bridges crosslink these salt columns in the (010), (001) and (011) directions, resulting in a three-dimensional network solid. One dioxane solvent molecule is disordered over two half-occupancy sites.

Arthroscopic rotator cuff repair: 4- to 10-year results.

PURPOSE: The purpose of this article is to report the 4- to 10-year results of arthroscopic repair of full- thickness rotator cuff tears. TYPE OF STUDY: This is a retrospective study evaluating a series of arthroscopic rotator cuff repairs performed by a single surgeon from February 1990 to February 1996. METHODS: Retrospective chart reviews and telephone interviews were performed to evaluate the results of arthroscopic repair of rotator cuff tears. Results were evaluated using a modified University of California, Los Angeles (UCLA), shoulder scoring system. RESULTS: One-hundred five arthroscopic rotator cuff repairs were performed in 104 patients between February 1990 and February 1996. Of these, 95 patients (96 shoulders) were available for follow-up evaluation at the time of this review. The mean UCLA score of all shoulders involved was 32. Fifty-one patients showed excellent results; 39, good; 2, fair; and 4, poor according to the modified UCLA scoring system. In no case was any loss of motion noted as a result of the surgical intervention. CONCLUSIONS: This retrospective study is the largest series of arthroscopic rotator cuff repairs with the longest period of follow-up thus far reported. Of the patients available for follow-up evaluation, 94% of patients qualified as a good to excellent result according to the UCLA shoulder scoring system. This study shows that patients treated with this arthroscopic rotator cuff repair technique have maintained excellent clinical outcomes 4 to 10 years after surgery. LEVEL OF EVIDENCE: Level IV.

A ruthenium complex with 1,5-diphenyl-3-thiocarbazone (dithizone) as monodentate ligands: bis(2,2'-bipyridyl-kappa(2)N,N')bis(dithizonato-kappaS)ruthenium(II).

The title compound, [Ru(C(13)H(11)N(4)S)(2)(C(15)H(8)N(2))(2)], has C(2) symmetry, with bidentate 2,2'-bipyridyl ligands dictating a cis geometry around the Ru(II) center. The monodentate S-bonded dithizone ligands are almost planar, except for one of the phenyl rings, which is twisted by 34.2 (4) degrees from the N/N/C(S)/N/N plane. The Ru-S bond length is 2.4140 (13) A, and the Ru-N bond lengths are 2.048 (4) and 2.074 (4) A.

1,2,4,5-tetrakis(2-vinylpyridyl)-benzene-dichloromethane (1/2).

The title compound, C(34)H(26)N(4) x 2CH(2)Cl(2), lies about an inversion center. The solvent molecules interact with the benzene molecule both through C-H...N hydrogen bonding to span pyridine N atoms of adjacent vinyl groups, possibly stabilizing the rotational conformation observed, and through a pi interaction between a dichloromethane Cl atom and a pyridyl ring C-C bond of a c-glide-related molecule. The benzene molecules form stacks along the a axis such that two of the four olefin groups are properly oriented for photoreactivity (2+2 cyclodimerization).

2,6-Diiodopyridine.

The title compound, C(5)H(3)I(2)N, crystallizes in the polar space group Fmm2, with crystallographic mm2 symmetry imposed on the molecule. Molecules are linked through C-H...N hydrogen bonding to form chains which are, in turn, joined through weak I...I halogen-bonding interactions to form layers. The pyridine ring lies parallel to the polar z axis and has the N atom pointing in the +z direction. The layers stack in a polar fashion normal to the a axis and the absolute structure has been determined.

Synthesis of [n]- and [n.n]cyclophanes by using Suzuki-Miyaura coupling.

Reaction of the bis-9-BBN adduct of several dienes with 1,3-dibromobenzene via Suzuki coupling leads to a series of [n]metacyclophanes ranging in size from 10 to 17 atom members. In each case, two carbon-carbon bonds are formed in one reaction vessel. However, when the bis-9-BBN adduct of 1,5-hexadiene is coupled with a variety of aryl dihalides, larger [n.n]cyclophanes were formed in preference to the [n]cyclophanes. Four carbon-carbon bonds are formed in this instance. Single-crystal X-ray analyses of these [n.n]cyclophanes reveal interestingly shaped molecules with large cavities.

Targeting DNA bulged microenvironments with synthetic agents: lessons from a natural product.

Bulged regions of nucleic acids are important structural motifs whose function has been linked to a number of key nuclear processes. Additionally, bulged intermediates have been implicated in the etiology of several genetic diseases and as targets for viral regulation. Despite these obvious ramifications, few molecules are capable of selective binding to bulged sequences. Prompted by the remarkable affinity of a natural product metabolite, we have designed and prepared a series of readily accessible synthetic agents with selective bulge binding activity. Furthermore, by screening a library of bulge-containing oligodeoxynucelotides, correlations between structure and affinity of the agents can be drawn. In addition to potential applications in molecular biology, the availability of these spirocyclic agents now opens the door for rational drug design.
