[Surgical treatment for squamous-cell carcinoma of the posterior pharyngeal wall: reconstruction with a free forearm flap].

Squamous-cell carcinoma of the posterior pharyngeal wall is a rare finding. Surgical treatment with preservation of laryngeal function is usually associated with relatively poor functional outcome. We report the cases of 5 patients treated by pharyngectomy with preservation of the laryngeal function. The posterior pharyngeal wall was reconstructed with a free forearm flap. This reconstruction method provided satisfactory functional results. Swallowing disorders resolved within 2 to 6 weeks following the reintroduction of oral food intake. Mean follow-up of nearly 3 years did not evidence any local tumor recurrence.

Intra-laryngeal endoprosthesis: an alternative therapeutic approach to surgical procedures of laryngeal exclusion.

The uncertain results of aryepiglottopexy in our personal experience led us to develop, for patients with aspirations associated with the risk of severe pneumonitis, three types of intralaryngeal endoprostheses (ILEP) under the systematic cover of a tracheostomy. From September 1997 to May 1999, seven protheses were implanted in six patients. It was not possible to restore the full range of laryngeal functions, i.e. deglutition, phonation and respiration. However, our results, that were obtained with an intracricoidal prothesis closed at its upper end or equipped with a phonatory valve, are quite encouraging, even more so when in association with early alimentary re-education and support continuation of our trial. These ILEPs are simple to place, well tolerated, efficacious in preventing deglutition pneumonitis and easy to remove. Furthermore, the reversibility of this bloodless procedure facilitates the monitoring of a possible recovery of the laryngeal functions.

[Antiallergy drugs and respiratory diseases in children].

Second generation antihistamines (anti H1) are effective for seasonal and perennial rhinoconjunctivitis curative or preventive treatment in children. They are better tolerated than first generation drugs. They probably do not act specifically against asthma itself, but are nevertheless useful for relief of nasal obstruction, which is an asthmogenic factor frequently linked with bronchial asthma. The therapeutic relevance of oral ketotifen and inhaled chromones (sodium cromoglycate, nedocromil sodium) is unequally considered among pediatricians. However, their efficacy has been clearly demonstrated as a ground treatment in mild-to-moderate asthmatic children. Chromones are also useful in preventing exercise-induced asthma. Because of their low cost and the lack of potential side effects, and according to the guidelines established in 1997 by the National Heart, Lung and Blood Institute, they should be prescribed at first sight in these indications.

[Osteosynthesis of subcondylar fractures using a rectangular 3-dimensional stabilization plate].

The current international opinion for the treatment of displaced subcondylar fractures has become more aggressive after the rediscovery of the submandibular incision and the creation of new fixation material. We report 19 patients with displaced subcondylar fractures who were treated with reinforced rectangular plates. The pure titanium plates are slightly concave and measure 13 by 9 mm. We evaluated dental articulation, mouth opening, propulsion and duction and observed rapid recovery of mandibular kinetics and mandibular function. Use of this plate is a major progress in the treatment of fractures of the condyle.

[Hemangiopericytoma of the infratemporal fossa of nasopharyngeal manifestation. Value of the cervico-transoral approach].

Authors present one case of hemangiopericytoma of the infra-temporal fossa and the lateral wall of the nasopharynx. They describe the characteristics of this rare, recurrent and sometime malignant vascular tumor. The selected therapy is an aggressive and wide exeresis. The antero-inferior approach via the cervico-trans-oral route would seem to be of particular interest, offering good exposure of the anatomical features of the region avoiding the parotid bed ans dissection of the facial nerve.

[Cavernous hemangioma of the geniculate ganglion. Value of otoneurosurgical collaboration].

A patient with a progressive peripheral facial paralyses should be considered to have a tumor involved the facial nerve until proved otherwise. Haemangiomas of the temporal bone are uncommon and the present study discusses pathology, clinical presentation and therapeutic management of a tumor located at the geniculate ganglion. Complete surgical removal by the middle fossa approach is the treatment of choice with adequate margins and preservation of adjacent structures when it is oncologically possible. The surgeon must be familiar with all aspects of facial nerve repair.

[Ear prosthesis anchored on osseointegrated implants. Experience with 9 cases].

The use of osseointegrated titanium implants has improved the quality of auricle prosthesis fixation. Between december 1993 and july 1994, twenty two titanium fixtures have been surgically implanted in nine patients in the Ear Nose Throat department of the university hospital of Strasbourg. This method of auricle amputation repair achieves a good esthetic result with a low rate of postoperative complications.

[Cholesteatoma in children. Apropos of 110 cases].

This retrospective study of 110 middle ear cholesteatomas, concerning children up to 15 years old which have been treated between 1982 and 1992 in the ENT department of the university hospital of Strasbourg, analyzes the particularities of this disease and the therapeutic strategy adopted. The evolution of middle ear cholesteatoma is more rapid in childhood than in adulthood, due to greater growth potential of tissue in childhood, persistence of Eustachian tube dysfunction and aggressive extension in middle ear. The surgical removal is said to be difficult with numerous postoperative recurrences. Closed technique tympanoplasty was used for 47 cholesteatomas (43%), with a postoperative recurrence rate of 54%. An open technique was used for 63% cholesteatomas (57%), with a recurrence rate of 29%. The postoperative follow up has to be continued as long as possible, whatever the technique used may be.
