[Scintigraphy in the diagnosis of visceral candidiasis].

From the features of two cases, the authors stress the usefulness of the scintigraphy as regards to the diagnosis of visceral candida abscesses. Such fungal localisations are not infrequent, especially in immunodeficient patients (haematologic malignancies undergoing chemotherapy, lupus, serious visceral illness...). The positive diagnosis is uneasy because of non-specific clinical features and frequent negative blood cultures. Splenic localisation is the most likely. Citrate Gallium scintigraphy together with splenic labeled RBC scan enables us to give a precise view of the splenic involvement and even of the abdominal extension of the fungal abscess. From the literature review and these two cases, the excellent adequacy of the scintigraphy to the follow-up of systemic candidiasis is underlined and is compared to other usual morphological studies such as US scan and CT examination. In both cases, the diagnosis is fully confirmed by mycological examination.

[Evaluation by biphotonic absorptiometry of bone mineral content and determination of the fracture threshold in a population of patients 60 to 75 years old].

A measurement of the bony density of vertebrae and femur was carried out, in the course of a prospective study, in 99 patients between the ages of 60 and 75 years, living in northern France. 76 patients had no recognized problem of demineralization and 23 others were consulting for an evaluation of vertebral compression. The measurement of the bony density was done by biphotonic absorptiometry with a source of labelled Gadolinium. In women, a significant difference in the values obtained by absorptiometry in terms of the existence of vertebral compressions, symptomatic or not, is noted. A statistical study using the ROC curves method (Receiver Operating System) has enabled us to determine a threshold of high risk of vertebral fracture measured at 31 g or 0.85 g/cm2 in women, and 35 g or 0.85 g/cm2 in men. In addition, there is a significant correlation with the vertebral radiographical index as defined by Meunier. Among the various factors of influence which were studied, only the size and the morphotype seem to play an essential role in our study group. The comparative study of the densities measured in the femoral neck and the lumbar spine enabled us to find a very significant correlation in patients with as well as without compression (r = 0.714, p less than 0.001). Biphotonic absorptiometry proves therefore to be a method which may be used to define a group with a high fracture risk permitting to determine, as early as possible, therapeutic, curative and preventive measures.

Latent pulmonary involvement in Crohn's disease: biological, functional, bronchoalveolar lavage and scintigraphic studies.

We have investigated the following pulmonary related parameters in 22 patients with Crohn's disease who were free of clinical pulmonary symptoms and had normal chest roentgenograms and in 25 controls: serum angiotensin converting enzyme, pulmonary function tests, bronchoalveolar lavage (lymphocyte count and subpopulations, macrophage viability and superoxide anion release by macrophages) and pulmonary scannings. Serum angiotensin converting enzyme was lower in Crohn's disease (14.1 +/- 5.1) than in controls (25.2 +/- 4.7) (p less than 0.001). Twelve of 22 Crohn's disease (54%) had a bronchoalveolar lavage lymphocytosis (greater than 18% alveolar lymphocytes). Bronchoalveolar lavage lymphocytes subpopulations were quite variable. Twelve of 17 Crohn's disease (71%) had an increase spontaneous and/or stimulated superoxide anion production by alveolar macrophages. Six of 12 Crohn's disease (50%) had an increase physiologic dead space in the upper part of their lung against one of 11 controls (9%). These data suggest that most patients with Crohn's disease have a latent pulmonary involvement.

[The association of asthma and gastroesophageal reflux: strategy of paraclinical studies].

This study was carried out on 104 patients of whom 94 were asthmatic and 10 patients presented with a spasmodic intractable cough; all presented with symptoms evocative of an associated gastro-oesophageal reflux (RGO). The clinical symptoms revealed a nocturnal cough (67%), cough preceeding asthma (46%) and heartburn in 60%. The asthma was severe (type III and IV in 89% of cases), or dependent on corticosteroids (37% of cases). pH monitoring of the oesophagus is the most sensitive examination (88% with positive results) slightly ahead of manometry and scintigraphy (both 81%), these examinations were clearly superior to radiographic examination (49%) and oesophageal fibroscopy (36%). The combination of pH monitoring and of scintigraphy enabled 98% of RGO cases to be identified by their clinical data. Medical treatment with Tagamet, Gaviscon and Primperan (alone or in combination) produced an improvement in the respiratory symptoms in 50% of the cases. Of the 14 surgically treated, 7 obtained an improvement in their respiratory symptoms. Seven of the ten patients with spasmodic cough were improved by medical treatment. Our study shows the frequency of oesophageal reflux in patients with severe asthma. In half of them RGO intervened as an aggravating factor and the medical treatment of RGO led to a clear improvement in the respiratory symptoms.

[Scintigraphy in the evaluation of painful arthroplasties. Simultaneous use of technetium and gallium].

Fifty-nine patients with 60 painful hip arthroplasties were assessed by scintigraphy using M.D.P. 99m Te and 67 Ga bone scanning. The M.D.P. 99m Te bone scanning was very reliable with 100 p. 100 of positive results. However, it did not elucidate the cause of the pain. Sixty-seven Ga scanning, when positive, allowed the diagnosis to be orientated towards an infective cause. However in 6 out of 18, the scanning was negative despite a proved infection. The technical details are described by which quicker and safer results can be obtained.

[Scintigraphy in spleen injuries].

Scintiscans of the spleen were performed in 62 hospital patients under observation for abdominal contusion. No abnormality was detected in 82% of the cases, and the course of events showed that the spleen was intact. Lesional images were found in the remaining 18%. Injury to the spleen was excluded by other investigations in 10% (false positive results) but confirmed in 8% who underwent laparotomy. This fast and non aggressive method can be recommended outside emergencies to exclude rupture or haematoma of the spleen.

Massive digitoxin intoxication. Treatment of Amberlite XAD 4 resin hemoperfusion.

Amberlite XAD 4 resin hemoperfusion was studied in the treatment of massive digitoxin overdosage. In vitro experiments using 4 hour resin hemoperfusion showed that digitoxin is removed 100 percent from an isotonic saline reservoir, 83 +/- 14 percent from plasma, and 42 +/- 7 percent from whole blood. In dogs, serum level reduction is fast: 50 +/- 12 percent in 15 minutes, 71 +/- 6 percent in 180 minutes; mean plasma clearance is 24 +/- 6.5 ml/min for a 150 ml/min plasma flow rate. Eleven patients with massive digitoxin overdosage recovered with resin hemoperfusion. Serum level reduction was 32 +/- 14 percent with a 325 g resin column, 59 +/- 6 percent with two 325 g resin columns in series as compared with a 4.5 +/- 1.7 percent reduction without perfusion. Hemoperfusion increased the rate of spontaneous removal of digitoxin 8.6 +/- 3.3 times with a 325 g resin column, and 14 +/- 7.4 times with two 325 g resin columns.

[Brain scanning with 201 thallium. Comparative study with D.T.P.A. 99mTc brain radioangiogram].

Data of brain scans with 201Th and 99mTc - D.T.P.A. are studied in 43 patients. Some of the results sustained earlier data from Ancri (1978): lack of interest through vascular diseases, good sensitivity for the focalized brain lesions (24/25). But new features are underlined: occurrence of false - positive, intermediary patterns laying between normal scan and focal pathologic distributions but without etiologic correspondence. The quantified contrast is usually better with 99mTc - D.T.P.A. than with 201Th; these data challenge Ancri's ones. Actual indications of Thallium brain scan are discussed.

Fast Diagnosis (6h) of clinically silent pyonephrosis by combined use of 99mTc-MDP and 67Ga citrate.

A fast diagnosis (6h) of clinically latent pyonephrosis by combined use of 99mTc-MDP and 67Ga-citrate is reported. This technique combines the potential early detection of focal infectious processes by 67Ga and the "high-pulse" separation possibility of multiple isotopes. A brief prospective study (6 months) showed the interest of this technique: in six cases focal bone and/or joint infections were found three of which were absolutely latent clinically.

Lateral ventricle visualization during brain scanning with Tc-DTPA.

The visualization of the cerebral ventricles during a brain scan is a relatively rare possibility. The authors had the opportunity of observing four cases. They report them and discuss the 19 cases reported in world literature. There were, in order of importance, 12 cases of ventriculitis, 3 of non tumorous intraventricular erruptions, 2 of tumours infiltration, 1 metabolic encephalopathy and one unexplained case. It is concluded that the visualization of the ventricule during a brain scan (particularly the early visualization) is a good etiological indicator of ventriculitis.

[Paravertebral hematopoietic pseudotumor during Paget's disease (author's transl)].

Extramedullary hematopoiesis is usually secondary to a severe and prolonged reduction in medullary functions, but can be the rare consequence of an extrusion of tissue formed of weakened or fractured bone. The hematopoietic nature of this pseudotumor can be confirmed histologically or, as in this case of Paget's disease, by Indium scintigraphy which can define the specific locations of the hematopoietic marrow.
