[Autologous peripheral blood selected CD34+ cell transplantation in the treatment of multiple myeloma].

OBJECTIVE: To investigate the clinical impact of CD(34)(+) cell selected autologous peripheral blood stem cell transplantation (APBSCT) in multiple myeloma (MM). METHODS: The hematopoietic recovery time, treatment efficacy, survival rate, transplantation related morbidity and the cost were compared between 21 MM patients receiving CD(34)(+) APBSCT and 21 MM patients receiving unselected APBSCT. The age and beta(2) microglobulin level at diagnosis and disease status at the time of transplantation were similar in the two groups. The induction treatment and conditioning regimen were the same. RESULTS: Significantly lower CD(34)(+) cells were infused in the selected group as compared with the control group: 2.2 (0.5 - 14.3) x 10(6)/kg vs 9.4 (1.1 - 15.0) x 10(6)/kg, (P < 0.001). The median times to neutrophil >or= 0.5 x 10(9)/L and platelet >or= 20 x 10(9)/L were 10 and 9 days for the selected group, and 9.5 (P = 0.357) and 4.5 days (P = 0.005) for the control group, respectively. Response rates were similar in both groups (85.7% for the selected group vs 90.4% for the control group). Three year disease-free survival (32% for the selected group vs 39% for the control group) and overall survival rates (85% for the selected group vs 79% for the control group) were not significantly different. Furthermore, unselected APBSCT could reduce the cost for the transplantation. CONCLUSION: It shows that CD(34)(+) cell selected autologous transplantation is more expensive and does not improve the clinical outcome of patients with MM.

[Management of arterial occlusive diseases of abdominal aorta and iliac artery: experience with 267 cases].

OBJECTIVE: To study the surgical treatment of infrarenal aortic and iliac arterial occlusive disease. METHOD: We analyzed 267 patients with arterial occlusive diseases of the abdominal aorta and iliac artery retrospectively. RESULTS: All the patients were treated by surgery including aortic-iliac bypass in 145 patients, iliac-femoral bypass in 40, femoro-femoral bypass in 45, and axillo-femoral bypass in 37. Their total effective rate and perioperative mortality were 96.5% and 3.5%, respectively. Of the 267 patients, 178 were followed up. The patency rate of prosthesis was 82.0%. CONCLUSIONS: Extra-anatomical bypass is a procedure of choice for high-risk patients, especially for those chronic patients with coronary heart disease, hypertension, cerebral arterial atherosclerosis, and brain infarction.

[Relationship between intrahepatic HCV-specific CTL activity and response to interferon therapy in chronic hepatitis C].

BACKGROUND: To study the relationship between intrahepatic hepatitis C virus-specific cytotoxic T lymphocyte (HCV-CTL) activity and response to interferon (IFN) therapy in chronic hepatitis C. METHODS: 45 patients with chronic hepatitis C who received IFN-alpha therapy were evaluated for HCV-specific CTL in the liver before treatment with standard chromium release assay. The relationship between treatment response and HCV-CTL activity was analyzed. RESULTS: HCV-CTL activity was detected in 20 (44.4%) of the 45 IFN-treated patients. Of the 42 patients who completed a 6-month course therapy, 19 had detectable HCV-CTL activity, 23 had no detectable HCV-CTL activity. After 6 months of IFN therapy, 18 of 42 (42.9%) patients received an end-of-treatment complete response (ETR). After 6-12 months of follow-up, 10 patients (23.8%) had a sustained response (SR). Of the 19 patients with detectable HCV-CTL activity, 15 (78.9%) developed an ETR. In contrast, only 3 of the 23 patients with no detectable HCV-CTL activity developed an ETR to IFN (P <0.01). All the 10 patients who got a sustained response were those with detectable HCV-CTL activity. Conclusion The host immune response, particularly that mediated by CD8+ HCV-CTL, may be very important in determining the outcome of IFN therapy for the patients with chronic hepatitis C.

[The effects of thrombopoietin and interleukin-11 on bone marrow megakaryocytic progenitors in patients with chronic idiopathic thrombocytopenic purpura in vitro].

OBJECTIVE: To observe the effects of recombinant human thrombopoietin (rhTPO) and rhTPO in combination with recombinant human interleukin-11(rhIL-11) on the megakaryocyte (MK) colony growth and maturation in patients with chronic idiopathic thrombocytopenic purpura (CITP) in vitro. METHODS: Bone marrow (BM) mononuclear cells of 21 patients with CITP were cultured in plasma clot culture systems, to which were added rhTPO alone or rhTPO plus rhIL-11 over a range of concentrations. After 14 days, the MK colonies were stained by GP III a Mc Ab(SZ-21) and ABC-Kit and counted. The diameters and areas of the positive cells of GP III a were measured by BM-cell analysis system of MCDS-2010. RESULTS: The diameters and areas of MK in CITP group were significantly lower than that in control group, P < 0.05. The addition of rhTPO to culture systems increased CFU-MK, total colonies and diameters and areas of MK in CITP patients. And this promoting effect was not dose-dependent. The optimal concentration of rhTPO was 10 ng/ml; in the group where 10 ng/ml rhTPO was used in combination with 20 ng/ml rhIL-11, the counts of CFU-MK, total colonies and the measurements of areas and diameters of MK increased significantly, compared with those in the group where rhTPO was used alone. CONCLUSION: There are maturation dysfunctions of MK progenitors in patients with CITP. rhTPO alone or in combination with rhIL-11 can promote the proliferation and maturation of MK progenitors in patients with CITP and, moreover, the combination of the two cytokines has more significant promoting effects, compared to rhTPO alone.

[Measurement of central respiratory drive and inspiratory muscles strength in patients with uremia].

OBJECTIVE: To investigate the changes of central respiratory drive and inspiratory muscles function in patients with uremia. METHODS: We performed the measurement of forced vital capacity (FVC), maximal voluntary ventilation (MBC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF), maximal midexpiratory flow (MMEF), expiratory flow of 25 per cent of FVC (V25), lung carbon monoxide diffusing capacity (DLco), maximal inspiratory mouth pressure (MIP) and airway occlusion pressure (P0.1) in 25 patients with uremia and 20 normal subjects. RESULTS: In patients with uremia, the FVC, MBC, FEV1, PEF, MMEF and V25 which reflect the lung ventilatory function and the DLco which reflects the lung diffusing function were significantly lower than those in normal subjects. The patients' MIP which reflects inspiratory muscles strength was significantly lower and their P0.1 which reflects the central respiratory drive was significantly higher, compared with the normal subjects'. CONCLUSION: Our findings suggest that on the basis of the disorder of ventilation and diffusing function of the lungs, the inspiratory muscles function of the patients with uremia is significantly decreased and the central respiratory drive of the patients is increased.

[A retrospective clinical study of 6539 cases of malignant oral-maxillofacial tumors].

OBJECTIVE: The aim of this study is to obtain the general information of post-operation malignant oral-maxillofacial tumor patients in forty-seven years. METHODS: A total of 6539 patients with malignant oral-maxillofacial tumors in the hospital of stomatology, west china university of medical sciences, from 1953 to 2000 were retrospected. RESULTS: The average age of patients with malignant oral-maxilloficial tumor is 48.7 years old. The ratio of male to female is 2.31:1. The most common original malignant tumors in this oral-maxillofacial region are malignant oral tumors, malignant maxillary tumors, malignant mandibular tumors, and malignant sialoma successively. As to the original tissues of tumors, there were 5869 cases of epithelial tumors, and made up 89.75% of all the investigated patients. Tumors came from lymphatic and hematopoietic systems and mesoderm were much less those came from epithelia. There were 4177 cases of squamous cell carcinomas and the percentage was 63.88%. CONCLUSION: The incidence rate of malignant tumors in oral-maxillofacial region tends to increase, and the average age of patients also rose. The ratio of male to female decreases gradually. Epithelial tumors, especially squamous cell carcinomas are very common tumors happening in this region.

[Expression of VEGF-C and its correlation with cervical lymph nodes metastasis of oral cancers].

OBJECTIVE: Vascular endothelial growth factor C (VEGF-C) is known to be capable of inducing cell proliferation of lymphatic endothelia and development of lymphatic vessels. Thus, in the current study the expression of VEGF-C was investigated to understand the mechanism of lymphatic metastasis of oral cancer. METHODS: VEGF-C protein expression in benign and malignant oral lesions was investigated with an immunohistochemical staining assay, followed by light microscopic examination and image analysis. RESULTS: Immunohistostaining of VEGF-C was observed in normal, benign and malignant epithelia, as well as in some lymphatic vessels. The VEGF-C positive rate was significantly higher in squamous carcinomas than in normal tissues and benign lesions. It was also noted that in squamous carcinomas the expression level of VEGF-C correlated well with pathological grades and stages of lymph node metastasis, but not with clinical grades. CONCLUSION: These findings suggest that VEGF-C may promote lymphatic metastasis of oral cancer, and it is possible to be used as a predictor for the lymphatic metastasis and prognosis of oral cancer.

[Study on the release mechanism of fenoprofen calcium from hydrophillic sustained-release matrix].

AIM: To study the release mechanism of fenoprofen calcium (FC) from hydroxypropylmethylcellulose (HPMC) matrices. METHODS: The release of FC and the erosion properties of hydrophillic matrices containing HPMC was examined at different paddle speed. The release mechanism of FC was further confirmed by evaluating the n value in Peppas equation. RESULTS: The results indicate that the release of FC and the erosion of matrices exhibit zero order kinetic equation, and it exhibits line relationship between them. CONCLUSION: In the first 40 min, FC mainly released by diffusion and erosion from HPMC matrix, while it was controlled by the rate of tablet erosion after 50 min.

[The psychological effect of clinical examination on women with tubal infertility].

OBJECTIVE: To comprehend the psychological effects in the course of diagnosis and treatment of infertility on the sexuality, emotional feeling and marriage of women with tubal infertility. METHODS: The study involved 180 hospitalized patients who were awaiting tube operation from May to September in the year 2000. A survey was made by using questionnaire about sexuality, emotional feeling and marriage after infertility examinations. RESULTS: The sexual intercourse frequency of 46.2% infertile women decreased and no influences on sexual living satisfaction were confirmed during basal body temperature (BBT) examination. 92.3% to 93.2% of the infertile women were sad after hysterosalpingography(HSG) and laparoscopy. 78.9% of them felt remorse when the result of semen examination was normal. 56.6% of them would choose in vitro fertilization and embryo transfer(IVF-ET); 38.9% of the infertile couples' wedlock would not be stable and 10.5% of them would be divorced in the case of continuing infertility 2 years after tube operation. CONCLUSION: Sexual intercourse in almost half of the infertile couples decreased during BBT examination. Most infertile women became sad when they were informed of the abnormal results of HSG and laparoscopy and the normal results of semen analysis.

The mechanism of cerebral evoked potentials by repetitive magnetic stimulation of gastrocnemius muscle in Duchenne muscular dystrophy.

OBJECTIVE: To study the features and mechanism of the cerebral evoked potentials by repetitive stimulation of calf muscle in Duchenne muscular dystrophy (DMD) patients with obvious muscular dystrophy and psuedohypertrophy. METHODS: Cerebral evoked potentials by stimulation of calf muscles and somatosensory evoked potentials (SEPs) by the stimulation of posterior tibial nerves at ankle were measured in 10 patients with DMD and 10 normal controls matched with gender and age. The intensity of the magnetic stimulation was at 30% of maximal output (2.1 Tesla, MagPro magnetic stimulator, Dantec) and the frequency was 1 Hz. The low intensity of magnetic stimulation was just sufficient to produce a contraction of the muscle belly underneath the coil. Recording electrode was placed at 2 cm posterior to the Cz, reference to Fpz. The latencies of N33, P38, N48 and P55 and amplitude (P38 - N48) were recorded. SEPs were recorded by routine methods. RESULTS: In normal subjects, the amplitudes of cerebral evoked potentials by magnetic stimulation of calf muscle was 40% lower than that by electrical stimulation of the posterior tibial nerves at ankle. The latency of P38 was 2.9 +/- 2.1 ms longer compared with electrical stimulation of the posterior tibial nerves at ankle. In 6 patients, P38 latency from magnetic stimulation was remarkably prolonged (P < 0.01), and in 4 patients, there was no remarkable response. SEPs evoked by electrical stimulation were normal in all of the patients. CONCLUSION: DMD is an available model for the study of mechanism of cerebral evoked potentials by magnetic stimulating muscle. We can conclude that the responses from magnetic stimulation were produced by muscle input. The abnormal responses in patients may relate to decreased input of muscle by stimulating dystrophic and psedohypertrophic muscle.

The influence of automobile exhausts on mutagenicity of soils: contamination with, fractionation, separation, and preliminary identification of mutagens in the Salmonella/reversion assay and effects of solvent fractions on the sister-chromatid exchanges in human lymphocyte cultures and in the in vivo mouse bone marrow micronucleus assay.

To test the assumption that automobile exhausts contribute to soil mutagenicity, two soils with low levels of mutagenic activities were exposed to traffic exhausts at a heavily charged junction of German motorways (Autobahnen) for 3, 7, 10, 13, 17, 21, and 26 weeks. Indeed, in the presence of a metabolic activation system from rat liver (S9), an average increase of 8 and 9 (4 and 12) revertants per gram per week was found in Salmonella typhimurium TA 98 (TA 100). In the absence of S9, meaningful measurements were impossible on account of a concurrent dose dependent increase of toxicity. No correlation between the increase of mutagenicity and the contents of polycyclic aromatic hydrocarbons (PAH) could be detected. In another series, soils sampled at the roadside and at distances of 10 and 50m of five roads near Mainz expressed 10-20-fold higher mutagenicity (revertants per gram) under identical test conditions as compared with the average of agricultural soils. Toxic effects, however, again confounded the results and no correlation between the distance from roads and the levels of mutagenicity could be demonstrated. Subsequently, Soxhlet-extraction with the solvent sequence dichloromethane, acetone, and toluene/diethylketone was found to be an optimum procedure for soils at roadsides. The mass balance of solvent fractionation of such soils revealed that <2% each belonged to organic acids and bases, approximately 4% to fractions designed polar neutrals, approximately 8% to polar aromatics, approximately 7% to dichloromethane solubles, and approximately 79% to cylohexane solubles, among them approximately 63% acetone soluble compounds. The major part of mutagenicity (55-65%) was present in the fraction of polar aromatics, followed by polar neutrals and the acetone subfraction of cyclohexane solubles ( approximately 10% each) summarizing the results obtained with S. typhimurium TA 98, TA 98NR, YG 1021, YG 1024, TA 100, YG 1026, and YG 1029 with and without addition of S9. The modified tester strains, either deficient in nitroreductase (TA 98NR) or overproducing nitroreductase (YG 1021, 1026) or O-acetyl-transferase (YG 1024, 1026), indicated a major contribution of nitroarenes to soil mutagenicity. With respect to mutagenic PAH, high pressure liquid chromatography (HPLC) revealed that >90% of dibenz[a,h]anthracene (4.18mg/kg soil), benzo[a]pyrene (1.96mg), benzofluoranthenes (0.14mg), and benz[a]anthracene (0. 18mg) were present in the acetone subfraction of cyclohexane solubles. Concentrations and mutagenic activities, however, did not correlate. Additional preparative and analytical HPLC of the solvent fractions of polar neutrals and polar aromatics, resulted in the tentative identification of 2-nitrofluorene. Analysis of the vertical profile of soil revealed an increase of mutagenicity per gram from the surface to a maximum at 5-15cm depth and a subsequent decrease with very little activity remaining deeper than 35cm. In human lymphocyte cultures, the fraction of polar aromatics, 0.01-0. 3microg/ml, induced 11.27+/-4.76-20.70+/-6.19 sister-chromatid exchanges (SCE) per cell in the absence of S9 (solvent control: 10. 16+/-4.83 SCE per cell) and 12.77+/-6.53-17.87+/-4.93 SCE per cell in the presence of S9 (solvent control: 8.37+/-3.92 SCE per cell). However, no activities could be detected in the fractions of polar neutrals and non-polar neutrals. Again, negative results were obtained in the in vivo mouse bone marrow micronucleus assay at 2000mg/kg p.o. with all fractions.

Event-related potential N270 is elicited by mental conflict processing in human brain.

We recorded event-related potentials (ERPs) in 15 subjects in order to elicit a N270 of arithmetic conflict. Subjects calculated an arithmetic problem and matched their calculation result to an answer digit. They pressed a button when the presented digit is a true answer (condition 1) and pressed another button when the answer is false (condition 2). ERP components of P90, N130, P180, N200 and late positive component (LPC) were recorded in condition 1. In condition 2, N270 was elicited between N200 and LPC and it peaked at approximately 270 ms (268.6 +/- 29.0 ms at Cz). The peak latency of LPC in condition 2 (405.7 +/- 51.3 ms) is significantly delayed than condition 1 (307.5 +/- 22.7 ms). N270 reflects the endogenous conflict processing in human brain.

Hypermethylation of the death-associated protein (DAP) kinase promoter and aggressiveness in stage I non-small-cell lung cancer.

BACKGROUND: Death-associated protein (DAP) kinase is a serine/threonine kinase that is important in ligand-induced programmed cell death and plays an important role in lung cancer metastasis in animal models. Hypermethylation of the promoter represses the expression of the DAP kinase gene. Our purpose was to determine whether the hypermethylation status of the DAP kinase promoter influences the prognosis of non-small-cell lung cancer (NSCLC). METHODS: We retrospectively studied 135 patients with pathologic stage I NSCLC who had undergone curative surgery. Methylation-specific polymerase chain reaction was used to determine the methylation status of the DAP kinase promoter in resected specimens from patients with primary NSCLC. Statistical analyses, all two-sided, were performed to determine the prognostic effect of methylation status on various clinical parameters. RESULTS: Hypermethylation of the DAP kinase promoter was found in 59 (44%) of the 135 tumors. Patients whose tumors exhibited such hypermethylation had a statistically significantly poorer probability of overall survival at 5 years after surgery than those without such hypermethylation (.46 versus.68; P: =.007). Moreover, the groups with and without hypermethylation of the DAP kinase promoter showed a striking difference in the probability of disease-specific survival; i.e., among people who died of lung cancer-related causes specifically, the probability of 5-year survival was.56 for those with such hypermethylation and.92 for those without it (P:<.001). Multivariate analysis indicated that hypermethylation of the DAP kinase promoter is the only independent predictor for disease-specific survival among clinical and histologic parameters tested. CONCLUSIONS: Hypermethylation of the DAP kinase promoter is a common abnormality in early-stage NSCLC. This abnormality is strongly associated with survival, suggesting that DAP kinase plays an important role in determining the biologic aggressiveness of early-stage NSCLC.

Enhanced oncogenicity of human papillomavirus type 16 (HPV16) variants in Japanese population.

To investigate whether HPV16 E6 variants carry an elevated risk for cervical cancer in Japanese population, we investigated the E6 sequence variation in 40 cervical intraepithelial neoplasias (CINs) I-III and 43 invasive cervical cancers (ICCs), all positive for HPV16. HPV16 E6 variants were frequently found in ICCs than in CINs (88 vs. 65%, P=0.01). The E6 D25E, a rare variant in Western countries, was most frequently observed in ICC (44%). CIN I/II lesions with HPV16 variants were less likely to regress than those with HPV16 prototype (P=0.048). The finding that HPV16 E6 variants represent a significant risk factor is common between Western and Japanese women despite the different distribution of each variant.

Bgs2p, a 1,3-beta-glucan synthase subunit, is essential for maturation of ascospore wall in Schizosaccharomyces pombe.

Previously we have reported that Drc1p/Cps1p, a 1,3-beta-glucan synthase subunit, is essential for division septum assembly in Schizosaccharomyces pombe. In this report, we present evidence that S. pombe Bgs2p, a 1,3-beta-glucan synthase that shows 56% identity to Drc1p/Cps1p, is essential for maturation of ascospore wall in S. pombe, but is not required for vegetative growth. Diploid cells homozygous for the bgs2-null mutation, as well as homothallic bgs2-null mutant haploids undergo meiosis normally. However, a 1, 3-beta-glucan containing spore wall is not assembled in these cells. The spores resulting from meiosis of a bgs2-null mutant lyse upon release from the ascus and are therefore inviable. Using a green fluorescent protein-tagged Bgs2p, we demonstrate that Bgs2p is localized at the periphery of the ascospores during meiosis and sporulation. However, Bgs2p is not detected in vegetative cells. We conclude that Bgs2p is required for 1,3-beta-glucan synthesis during ascospore wall maturation.

Role of phosphatidylinositol 3-kinase and specific protein kinase B isoforms in the suppression of apoptosis mediated by the Abelson protein-tyrosine kinase.

Leukemogenic oncogenes, such as the Abelson protein-tyrosine kinases (PTK), disrupt the normal regulation of survival, proliferation, and differentiation in hemopoietic progenitor cells. In the absence of cytokines, hemopoietic progenitor cells die by apoptosis. Abl PTKs mediate suppression of this apoptotic response leading to aberrant survival. To investigate the mechanism of Abl PTK action, we have used an interleukin-3-dependent murine mast cell line that expresses a temperature-sensitive form of the v-ABL PTK, which is active at the permissive temperature of 32 degrees C and inactive at 39 degrees C. At the permissive temperature, these cells are resistant to apoptosis induced both by the withdrawal of the hemopoietic growth factor (interleukin-3) and the addition of cytotoxic drugs. We demonstrate that v-Abl associates with and stimulates activation of phosphatidylinositol 3-kinase (PI3K) and, crucially, that this activation results in enhanced cellular levels of the mass of the second messenger phosphatidylinositol-3,4,5-trisphosphate. Activation of PI3K leads to enhanced activity of PKB and increased levels of the anti-apoptotic protein Bcl-X(L). Transfection of cells with a dominant negative PKB reduces both the Abl-stimulated PKB activity and the survival effect conferred by activation of this oncogene. Thus, PI3K and PKB are required for the anti-apoptotic effects of Abl PTK.

Electronic structures of single-walled carbon nanotubes determined by NMR

Single-walled carbon nanotubes were studied by (13)C nuclear magnetic resonance (NMR). Two types of (13)C nuclear spins were identified with different spin-lattice relaxation rates. The fast-relaxing component, assigned to metallic tubes, followed the relaxation behavior expected in metals, and the density-of-states at the Fermi level increased with decreasing tube diameter. The slow-relaxing component has a significantly lower density-of-states at the Fermi level. Exposure to oxygen has a substantial effect on relaxation rates of both components.
