Isolation of an amikacin-resistant Escherichia coli strain after tobramycin treatment of previous recurrent episodes of respiratory tract infections caused by Pseudomonas aeruginosa.

Amikacin-resistant Escherichia coli strains are isolated rarely from clinical samples. In the present study, investigation of an amikacin-resistant clinical isolate of E. coli demonstrated the presence of two class 1 integrons carrying the aacA4 gene plus the aacA7 gene, and the dfrA17 gene plus the aadA5 gene, respectively. Resistance to amikacin in this E. coli isolate was related to the presence of both aacA4 and aacA7.

[Use of teleradiology in the evaluation and management of head-injured patients. Results of a pilot study of a link between a district general hospital and a neurosurgical referral center].

UNLABELLED: Because of the centralization of neurosurgical services, many head-injured patients who are initially evaluated in district general hospitals need to be transferred to a high technology centre for neurosurgical assessment. However, after assessment, many of these patients are sent back to the original hospital. Establishing a teleradiological system between the two hospitals would eliminate these unnecessary transfers. OBJECTIVES: 1) to describe our initial experience and the results of a pilot study of the teleradiological link between a district general hospital and a tertiary hospital for neurosurgical assessment of head-injured patients, 2) to describe the infrastructure and the technological support required for this project, 3) to analyse the effects of the teleradiological link in both centers (referring and receiving), 4) to evaluate the effectiveness of the system in avoiding unnecessary transfers, and 5) to assess its effectiveness in improving the speed and the quality of transfers in head-injured patients. MATERIAL AND METHODS: In January 1998, the Neurotraumatology Unit of Vall d'Hebron University Hospital established a teleradiological link with the General Hospital of Vic for the neurosurgical evaluation of headinjured patients. The General Hospital of Vic sent the patients' clinical information by fax. CT scan images were digitalized, compressed and prepared for transmission with the StatView program, and were then transmitted by modem to the receiving center. The duty neurosurgeon viewed the images on a PC screen using MutiView software. After evaluating this clinical and radiological information the neurosurgeon sent a report back to the referring center recommending transfer or management (admission, observation, etc.). RESULTS: We analyse the results of our experience 5 years after the implantation of the teleradiological link. CONCLUSIONS: The use of teleradiology in the daily management of head-injured patients provides clear benefits and leads to a more rational use of resources, thus significantly reduces costs. The effectiveness of the system in reducing the interval between the injury and treatment in severe cases depends more on the infrastructure of the health system in each geographical area than on sophisticated telemedicine systems. These methods should be accompanied by other measures designed to hasten the transfer of selected patients.

Use of the ammonia-oxidizing bacterial-specific phylogenetic probe Nso1225 as a primer for fingerprint analysis of ammonia-oxidizer communities.

Autotrophic ammonia-oxidizing bacteria (AOB) are an essential component of nitrifying wastewater treatment systems. The molecular tools used in group-specific studies are mostly based on the sequence of the 16S rRNA gene, but they have not proved to be fully specific. In this study, the sequence of the FISH probe Nso1225R was used as a reverse primer in order to analyze the AOB composition of several environmental samples by denaturing gradient gel electrophoresis (DGGE). For this purpose, samples from several environments, including aerated reactors, water treatment wetlands, and pilot plants, both aerobic and anaerobic, were analyzed. PCR fragments displayed a DGGE pattern consisting of bands melting between 30 and 40% denaturant, and a series of unresolved bands above 45%, mostly corresponding to AOB and beta-non-AOB, respectively. This second set of bands corresponded to environments subjected to severe oxygen restrictions. AOB sequences showed similarity percentages higher than 92% with those of known beta-AOB. Nso1225R, therefore, proved to be a good molecular phylogenetic marker for AOB samples from well-aerated systems, showing a higher specificity than the group-specific primers used previously.

Evaluation of a portable respiratory recording device for detecting apnoeas and hypopnoeas in subjects from a general population.

This study was designed to validate a new home portable respiratory recording device (PRRD) to identify sleep apnoea and hypopnoea in a group of subjects (n=116), from a sample of the general population. Full night polysomnography (PSG) was used as the gold standard and simultaneously performed with PRRD. PRRD measurements included oronasal airflow (thermistry), chest wall impedance, oxygen saturation, snoring and body position. The sensors were unique for each recording system. Data obtained was blindly reviewed and analysed. A high level of agreement between both methods apnoea/hypopnoea index by PSG and the respiratory disturbance index (RDI) by PRRD was observed. Accuracy of the PRRD was evaluated in terms of sensitivity and specificity for different RDI-PRRD cut-off points with respect to AHI-PSG >10 and AHI-PSG >30. A logistic regression model was performed to estimate the chance per unit of RDI of apnoeas. A received operating characteristic (ROC) curve was drawn to obtain the sensitivity/specificity profile for each observed RDI value obtained. From the ROC curve the authors identified the better cut-off points, which represent a balanced sensitivity/specificity. Through a classification table defined by the cut-off point, the post-odds to exhibit the disease was calculated. For a full PSG cut-off point of 10 a PRRD of six showed a balanced sensitivity of 95% and a specificity of 92%. For a full PSG cut-off point of 30 a PRRD of 16 shows a balanced sensitivity/specificity (100% and 97%, respectively). Post odds of apnoea were calculated for each cut-off point. In conclusion, these data suggest that the portable respiratory recording device is an effective device to identify apnoeas and hypopnoeas in a general population and is therefore a suitable device to be used in epidemiological studies.

Two new motile phototrophic consortia: "Chlorochromatium lunatum" and "Pelochromatium selenoides"

Two new phototrophic consortia, "Chlorochromatium lunatum" and "Pelochromatium selenoides", were observed and collected in the hypolimnion of several dimictic lakes in Wisconsin and Michigan (USA). The two consortia had the same morphology but different pigment composition. The cells of the photosynthetic components of the consortia were half-moon-shaped. This morphology was used to differentiate them from the previously described motile phototrophic consortia "Chlorochromatium aggregatum" and "Pelochromatium roseum". These phototrophic cells did not resemble any described unicellular green sulfur bacteria. The predominant pigments detected were bacteriochlorophyll d and chlorobactene for the green-colored "Clc. lunatum", and bacteriochlorophyll e and isorenieratene for the brown-colored "Plc. selenoides". Their pigment compositions and the presence of chlorosomes attached to the inner face of the cytoplasmic membrane in both kinds of photosynthetic cells confirmed this new half-moon-shaped morphotype as a green sulfur bacterium. Both consortia were found thriving in lakes with low concentrations of sulfide (< 60 &mgr;M), below the layers of "Clc. aggregatum" and "Plc. roseum". The green consortia were observed in lakes where the oxic-anoxic interface was located at shallow depths (2-7 m), while the brown consortia were found at greater depths (8-16 m). The two newly described consortia were never detected together at the same depth in any lake.

[Comparative study of acute and chronic forms of pleural tuberculosis].

To analyze whether a characteristic pattern distinguishes acute forms (symptoms starting no more than one week earlier) and subacute or chronic forms (symptoms lasting more than one week) of pleural tuberculosis (PT) in our practice. Retrospective analysis of 102 cases of PT diagnosed between 1986 and 1992, comparing the findings of case histories, imaging, pleural biochemistry and cytology, microbiology of sputum, pleural biopsy and fluid, anatomy and pathology studies of the biopsy specimens, course of disease, response to treatment and sequelae after one year and a half. Thirty-two patients (31.4%) had acute PT and 70 (68.6%) had chronic forms. LDH levels and the percentage of pleural nuclear polymorphism were higher in acute cases, while the concurrence of systemic involvement and lymphocytic predominance was more often seen in chronic cases. Other data analyzed were similar in both groups. No specific clinical or X-ray profiles or differences in course of disease and response to treatment distinguishes between acute and chronic forms of PT after 7 days. Only LDH levels and cytology results differentiated acute forms and, occasionally, such cases were indistinguishable from pneumonia-like effusion.

[Multiple pulmonary hyalinizing granuloma].

The causes of multiple pulmonary nodules are many, with metastasis being the most feared. A rare but possible etiology, however, is hyalinizing multiple granuloma. We present a case that allows us to review this condition and its course, as well as a variety of associated immunological changes and possible complications.
