Treatment of severe acute respiratory syndrome with glucosteroids: the Guangzhou experience.

STUDY OBJECTIVE: To investigate the efficacy and safety profiles of corticosteroid therapy in severe acute respiratory syndrome (SARS) patients. DESIGN: Four hundred one of 1,278 SARS cases treated in Guangzhou China between December 2002 and June 2003 fulfilled the diagnostic criteria issued by the World Health Organization for confirmed identification of SARS. Among them, the diagnosis of critical SARS was defined by criteria of SARS guidelines incorporated with a low oxygenation index (OI) [< 300 mm Hg]. Data of these patients retrieved from a database were retrospectively analyzed by logistic regression and Cox regression for the effect of corticosteroid therapy on death, hospitalization days, and complication presentation. RESULTS: Among the 401 SARS patients studied, 147 of 249 noncritical patients (59.0%) received corticosteroids (mean daily dose, 105.3 +/- 86.1 mg) [+/- SD], and all survived the disease; 121 of 152 critical patients (79.6%) received corticosteroids at a mean daily dose of 133.5 +/- 102.3 mg, and 25 died. Analysis of these 401 confirmed cases did not show any benefits of corticosteroid on the death rate and hospitalization days. However, when focused on 152 critical SARS cases, factors correlated with these end points indicated by univariate analysis included use of corticosteroid, age, rigor at onset, secondary respiratory infections, pulmonary rales, grading of OI, and use of invasive ventilation. After adjustment for possible confounders, treatment with corticosteroid was shown contributing to lower overall mortality, instant mortality, and shorter hospitalization stay (p < 0.05). Incidence of complications was significantly associated with the need for invasive ventilation but not with use of corticosteroids. CONCLUSION: This Guangzhou retrospective study revealed that proper use of corticosteroid in confirmed critical SARS resulted in lowered mortality and shorter hospitalization stay, and was not associated with significant secondary lower respiratory infection and other complications.

A 13C NMR study of the molecular dynamics and phase transition of confined benzene inside titanate nanotubes.

This work investigated the nanoconfinement effect on the molecular dynamics and phase transition of confined benzene inside titanate nanotubes with a uniform inner diameter of approximately 5.3 nm. For 13C-enriched organics, the 13C nuclear spin-spin relaxation was demonstrated as a sensitive tool to differentiate molecular translational motion and reorientation and, thus, was shown to be advantageous over the commonly employed 1H and 2H NMR for studying complex phase diagram, specifically, for separating the phase behavior of translational motion and the phase behavior of molecular reorientation. In such an approach, the melting of translational motion of confined benzene was explicitly observed to take place in a broad temperature range below the bulk melting temperature. The abrupt change of the 13C nuclear spin-spin relaxation time of the confined liquid benzene at about 260 K suggested that nanoconfinement induced two topologically distinct liquid phases.

Simultaneous measurement of diffusion along multiple directions.

This paper introduces the first NMR approach for simultaneously measuring the full diffusion tensor. Using magnetic field gradients of different directions to generate multiple modulations of nuclear spin magnetization, multiple echoes of different modulations are acquired in a single scan to simultaneously measure diffusion along different directions. The experimental demonstrations were conducted in both isotropic and anisotropic systems. Since the diffusion anisotropy holds structural and dynamical information, this approach may be useful for monitoring liquid crystals and electrolytes in metastable states and for studying fluidity in situ in porous networks and in vivo in biological systems.

Induction of HSF1 expression is associated with sporadic colorectal cancer.

AIM: To explore the activation of signal transduction pathways related with the carcinogenesis of sporadic colon cancers. METHODS: A gene array monitoring the activation of 8 signal transduction pathways (PathwayFinder GEArray) was used to screen the differentially expressed genes between colorectal cancer and normal colon tissues. The differentially expressed genes were further analyzed by RT-PCR, using RNA derived from colorectal cancer and normal colon tissue of 35 patients. RESULTS: The expression of HSF1, HSF27, HSP90 and iNOS was increased in colon cancer tissues compared to normal colon tissue using PathwayFinder GEArray. The RT-PCR results showed that the expression of HSF1 was increased in 86% (30/35) patients and the expression of iNOS was increased in 63% (22/35) patients. CONCLUSION: The induction of HSF1 gene expression is associated with sporadic colon cancer. HSF1 induces heat shock stress signaling pathway, which might play a role in the carcinogenesis of sporadic colorectal cancer.

SARS: hospital infection control and admission strategies.

Nosocomial clustering with transmission to health care workers, patients and visitors is a prominent feature of severe acute respiratory syndrome (SARS). Hospital outbreaks of SARS typically occurred within the first week after admission of the very first SARS cases when the disease was not recognized and before isolation measures were implemented. In the majority of nosocomial infections, there was a history of close contact with a SARS patient, and transmission occurred via large droplets, direct contact with infectious material or by contact with fomites contaminated by infectious material. In a few instances, potential airborne transmission was reported in association with endotracheal intubation, nebulised medications and non-invasive positive pressure ventilation of SARS patients. In all SARS-affected countries, nosocomial transmission of the disease was effectively halted by enforcement of routine standard, contact and droplet precautions in all clinical areas and additional airborne precautions in the high-risk areas. In Hong Kong, where there are few private rooms for patient isolation, some hospitals have obtained good outcome by having designated SARS teams and separate wards for patient triage, confirmed SARS cases and step-down of patients in whom SARS had been ruled out. In conclusion, SARS represents one of the new challenges for those who are involved in hospital infection control. As SARS might re-emerge, all hospitals should take advantage of the current SARS-free interval to review their infection control programmes, alert mechanisms, response capability and to repair any identified inadequacies.
