Feasibility and safety of vaginal myomectomy: analysis of 90 cases.

BACKGROUND: Vaginal myomectomy was firstly reported in 1994, however, it is a relatively new technique in China. The feasibility and safety of the procedure is still controversial in this country. The aim of this study was to analyze the outcomes of vaginal myomectomy in 90 patients and to investigate the feasibility and safety of the surgery. METHODS: From June 2001 to June 2004, 90 patients with uterine leiomyoma were treated with vaginal myomectomy in our hospital (vaginal group). The indications, operative performance, postoperative complications, and recovery of the patients were analyzed and compared with those of 93 patients with uterine leiomyoma treated by laparotomic myomectomy from January 2000 to January 2001 (laparotomy group). The Student's t test was used to compare the continuous variables between the two groups, and the chi-square test was used to compare the categorical variables. The vaginal and laparotomy groups were followed up for 10 - 34 months (median, 21) and 15 - 24 months (median, 30), respectively. RESULTS: Vaginal myomectomy was performed successfully in 87 of the 90 patients (96.7%). In the other 3 patients, the procedure failed and laparotomy was carried out. In both groups, the uterus was enlarged to 8 - 16 weeks gestational size (median, 10 gestational weeks). The number of resected tumors was 1 - 12 (median, 2) in the vaginal group and 1 - 15 (median, 4) in the laparotomy group, respectively (P > 0.05). The mean operating time, intraoperative blood loss, and postoperative hospital stay were (52 +/- 21) minutes, (230 +/- 44) ml, and (7.0 +/- 1.2) days in the vaginal group, and (65 +/- 32) minutes, (200 +/- 56) ml, and (7.0 +/- 1.5) days in the laparotomy group (P > 0.05). The mean top postoperative temperature was (38.4 +/- 1.1) degrees C and (37.8 +/- 0.6) degrees C in the two groups respectively (P < 0.05). Both groups had one recurrent case during the follow-up (P > 0.05). CONCLUSIONS: Vaginal myomectomy is feasible and safe in treating uterine leiomyoma. To some extent, it is superior to laparotomic myomectomy by avoiding severe trauma during the surgery.

[Clinical analysis of 942 cases of Kawasaki disease].

OBJECTIVE: The study was designed to investigate the clinical characteristics and the effects of therapeutic proposal on Kawasaki disease (KD). METHODS: Clinical features, diagnosis and treatment for totally 942 patients with KD hospitalized during Jan, 2000 to Dec, 2004 were reviewed. Clinical features of typical and incomplete KD were compared. Also, influential factors for KD resistant to intravenous immune globulin (IVIG) therapy were analyzed. Five hundred and ten cases were followed up for analyzing the prognosis of coronary artery lesion (CAL). RESULTS: (1) 774 cases were diagnosed as typical KD, and 168 cases as incomplete KD. The incidence of infants with incomplete KD was higher than that of infants with typical KD (18.5% vs. 10.1%, P < 0.01). As compared with typical KD, the cases of incomplete KD had a long duration of fever before final diagnosis [(7.7 +/- 2.9) d vs. (7.0 +/- 2.4) d, P < 0.01], high hemoglobin level [Hb, (106.6 +/- 13.4) g/L vs. (103.5 +/- 12.3) g/L, P < 0.01], high hematocrit [Hct, (32.0 +/- 4.3)% vs. (31.0 +/- 4.0)%, P < 0.01], and high prevalence of CAL (23.8% vs. 16.8%, P < 0.05), respectively. The occurrence rate and emerging time of clinical manifestations in incomplete KD and in typical KD were presented, respectively: non-exudative conjunctivitis [occurrence rate, 64.9% vs. 93.5%; emerging time, (4.4 +/- 1.4) d vs. (4.0 +/- 1.6) d, respectively (P < 0.05 or P < 0.01)], erythema and cracking of lips [occurrence rate, 50.6% vs. 94.8%; emerging time, (4.9 +/- 1.4) d vs. (4.5 +/- 1.6) d, respectively (P < 0.05 or P < 0.01)], rash [occurrence rate, 35.1% vs. 87.7%; emerging time, (3.9 +/- 1.9) d vs. (3.4 +/- 1.7) d, respectively (P < 0.05 or P < 0.01)], erythema and edema of extremity [occurrence rate, 26.8% vs. 71.4%; emerging time, (6.7 +/- 1.5) d vs. (5.3 +/- 1.7) d, respectively (P < 0.01)], cervical lymphadenopathy [occurrence rate, 34.5% vs. 68.0%; emerging time, (4.3 +/- 2.5) d vs. (3.6 +/- 2.2) d, respectively (P < 0.05 or P < 0.01)], strawberry tongue [occurrence rate, 31.0% vs. 59.8%; emerging time, (5.6 +/- 2.2) d vs. (4.9 +/- 1.8) d, respectively (P < 0.05 or P < 0.01)], membranous desquamation of fingertips [occurrence rate, 34.5% vs. 56.3%; emerging time, (11.7 +/- 3.3) d vs. (10.3 +/- 2.7) d, respectively (P < 0.01)], and desquamation peri-anus [occurrence rate, 42.9% vs. 50.0%; emerging time, (6.7 +/- 2.7) d vs. (6.9 +/- 2.5) d, respectively (P > 0.05)]. Except for peri-anus desquamation, other clinical manifestations in incomplete KD were sporadical as compared to typical KD. (2) Six per cent (51/857) of cases were resistant to the IVIG therapy. As compared to the group responding to IVIG therapy, high prevalence of CAL (31.4% vs. 17.1%, P < 0.05), long fever duration [(10.6 +/- 3.9) d vs. (7.5 +/- 2.3) d, P < 0.01], low Hb level [(99.9 +/- 14.1) g/L vs. (104.3 +/- 12.4) g/L, P < 0.01], low Hct [(30.1 +/- 4.5)% vs. (31.2 +/- 4.0)%, P < 0.05], low platelet [PLT, (256.9 +/- 142.4) x 10(9)/L vs. (309.7 +/- 131.5) x 10(9)/L, P < 0.05], and low albumin level [ALB, (27.8 +/- 8.4) g/L vs. (33.5 +/- 6.7) g/L, P < 0.01] were found in the group resistant to IVIG therapy, respectively. (3) In patients who received IVIG 1 g/kg and 2 g/kg, the recovery rates from CAL were 83.1% and 89.7% (P > 0.05), respectively. The prevalence of CAL in those without CAL in acute and subacute stages was 0.9% and 3.5% (P > 0.05), respectively, during 2 year-follow-up period. CONCLUSION: (1) Infants appeared to have more chances to suffer from incomplete KD. Incomplete KD had high prevalence of CAL. The peri-anus desquamation might be an important clue for early diagnosis of incomplete KD. (2) In acute stage, the influential factors for KD resistance to IVIG therapy included prolonged fever, non-elevated PLT, and persistent decrease in Hb, Hct and ALB levels. (3) Children receiving IVIG 1 g/kg and 2 g/kg had the similar effects on recovery and prevention from CAL within the first two years after KD onset.

[Expression and its significance of aquaporin 5 in rat nasal mucosa of experimental allergic rhinitis].

OBJECTIVE: To confirm the expression and distribution of aquaporin 5 (AQP5) in rat nasal mucosa of experimental allergic rhinitis and to investigate the relationship between AQP5 and allergic rhinitis. METHODS: Twenty-four healthy Wistar rats both male and female weighting 200-300 g were divided into two groups randomly, one was testing group (n = 12), the other was comparing group (n = 12). Generally sensitized rats in testing group were given repeated local booster sensitization into nasal cavity. Twelve normal rat nasal mucosa and twelve nasal mucosa from rat with allergic rhinitis were used. The distribution of AQP5 in normal rat nasal mucosa and nasal mucosa in rat with allergic rhinitis were observed by immunofluorescence technique. Furthermore, the expression of AQP5 in normal rat nasal mucosa and nasal mucosa from rat with allergic rhinitis were studied by immunohistochemical staining. RESULTS: Hematoxylin and eosin staining showed that there was obvious inflammation reaction, a large quantity of glands hyperplasia and acidophils soaked in nasal mucosa from rat with allergic rhinitis. (2) Both immunofluorescence technique and immunohistochemical staining showed that the distribution of AQP5 in normal rat nasal mucosa was in accordance with that in nasal mucosa from rat with allergic rhinitis on the whole. AQP5 expressed mainly in the membrane and cytoplasm of the epithelium in the glands, ducts and cilia. (3) The statistical analysis of the immunohistochemical staining showed that the quantity of AQP5 in rat nasal mucosa with allergic rhinitis (156.37 +/- 1.93) was obviously higher than that in the normal rat nasal mucosa (178.52 +/- 1.94). There was statistical significance between the two groups (t = 28.08, P < 0.01). CONCLUSIONS: The hypersecretion of glands has a close relationship with the high expression of AQP5 in allergic rhinitis.

[Effect of intensive insulin therapy on serum proinflammatory cytokine levels in patients with severe trauma].

OBJECTIVE: To investigate the effect of intensive insulin therapy on serum levels of tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL-6) and C reaction protein (CRP), all of which reflected the inflammatory status in patients with severe trauma. METHODS: Forty patients with severe trauma [injury severity score (ISS)>or=20] were randomly divided into intensive insulin therapy group and control group. Enzyme-linked immunoadsorbent assay (ELISA) method was used to determine the TNF-alpha and IL-6 levels within 72 hours after admission. RESULTS: Serum levels of TNF-alpha, IL-6 and CRP in patients received intensive insulin therapy were significantly lower than those in patients without the therapy (P<0.05 or P<0.01). CONCLUSION: Intensive insulin therapy can attenuate the systemic inflammatory response to trauma. The anti-inflammatory actions of insulin, as well as its glycemia controlling effects, might contribute to the improved outcomes of patients with severe trauma.

[Suggestive guideline for respiratory support in emergency--stepwise breath management].

OBJECTIVE: To propose a guideline of respiratory support for emergency patients. METHODS: To summarize the experiences gained in giving stepwise respiratory support (SRS) in 2,548 patients for emergency care (including 1,823 trauma patients and 725 non-trauma patients) between 1992 to 2002. RESULTS: There were 1,876 male and 672 female patients with the ratio of male to female 2.79:1. The age of patients ranged between 4 to 86 years, and the mean age of traumatic patients was (32.3+/-19.8) years, and the dominant ages were between 20 to 39. The mean age of non-traumatic patients was (65.2+/-17.3) years, and the dominant ages were between 50 to 69. The traumatic and non-traumatic patients accounted for 60.2% and 32.0% of all patients, respectively. The respiratory management included resuscitation positioning in 816 (32.0%), open airway and suctioning in 314 patients (12.3%), oxygenation through nostril or nasal intubation in 2,311 patients (90.7%), oxygenation by mask in 124 patients (4.9%), endotracheal intubation in 254 patients (10.0%), thyrocricocentesis or cricothyrotomy in 25 patients (1.0%), tracheotomy in 195 patients (7.7%), percutaneous tracheotomy in 58 patients (2.3%). SRW management included two types, four steps and ten ways. The two types included traumatic and non-traumatic; the four steps and ten ways included first step of manual treatment (including (1) resuscitation positioning, (2) open airway and suction and (3) chest-back press), second step oxygenation (including (4) oxygenation through nostril or nasal intubation, (5) oxygenation by mask, (6) endotracheal intubation), third step invasive airway support (including (7) thyrocricocentesis or cricothyrotomy and (8) tracheotomy or percutaneous tracheostomy), fourth step mechanic ventilation (covering (9) manual ventilation and (10) mechanical ventilator). CONCLUSION: The series of SRS management plan and principles can improve the respiratory support in rescuing emergency patients efficiently.

[Research on the mechanisms of PTEN gene inactivation in ovarian cancer].

OBJECTIVE: To investigate the mechanisms of PTEN gene inactivation starting from DNA, mRNA and protein levels in ovarian cancers. METHODS: Tumor tissue samples were obtained from 48 patients with epithelial ovarian cancers. Using four polymorphic markers (D10s541, D10s583, D10s1687 and D10s2491) within and flanking the PTEN gene located in chromosome 10q 23.3, polymerase chain reaction (PCR) and loss of heterozygosity (LOH) were introduced to examine LOH of PTEN gene; PCR-single strand conformation polymorphism (PCR-SSCP) was introduced to examine mutations of the fifth, sixth, seventh, and eighth exons of PTEN. Reverse transcriptase-polymerase chain reaction (RT-PCR) and immunohistochemistry (SP method) were applied to detect PTEN mRNA and PTEN protein expressions, respectively. RESULTS: LOH of PTEN gene was observed in 19 of 48 (39.6%) ovarian cancers. PTEN mutations were found only in 2 (4.2%) of the cases. Absence of PTEN mRNA expression was 18.8% (9 of 48). Immunostaining of 48 cancer samples revealed that 13 (27.1%) were PTEN immunostain negative. Of these 13 samples, only 2 (15.4%) had structural, biallelic inactivation by intragenic PTEN mutations and loss of the remaining wild-type allele; 7 (53.8%) showed evidence of LOH, 5 of these 7 samples showed deletion of PTEN mRNA expression, another 2 samples showed positive expression of PTEN mRNA; 4 (30.8%) tumors had neither PTEN gene mutation nor LOH but exhibited no PTEN protein expression, 2 of these 4 cases showed deletion of PTEN mRNA expression, another 2 showed positive expression of PTEN mRNA. For the cases of PTEN protein absent staining, the rate of LOH was 69.2% (9 of 13), higher than 28.6% (10 of 35) for the positive staining (P < 0.05). CONCLUSIONS: PTEN gene inactivation may contribute to epithelial ovarian carcinogenesis. There may be several mechanisms of PTEN gene inactivation in ovarian cancers. Protein expression deletions may be a significant mechanism.

[Producing human lactoferrin by high-density fermentation recombinant Pichia pastoris].

BACKGROUND: To evaluate expression of human lactoferrin gene by high-density fermentation in recombinant Pichia pastoris on the premise of maintaining its biological activities. METHODS: The neutrophil was isolated from human peripheral blood and its total RNA was prepared. Full-length cDNA of human lactoferrin gene was then obtained by RT-PCR, cloned into expression vector pPIC 3.5 K and transformed into Pichia pastoris strain KM71. With two-layer filter method, the transformants with high-productivity of human lactoferrin were screened out into fed-batch high-density fermentation. And later, the physical, chemical and biological activities of fermentation product were detected preliminarily. RESULTS: The strain p3.5-k-7 with better productivity of human lactoferrin was screened out into fed-batch high-density fermentation. The fermentation lasted nearly for nine days, with A-600 of culture once above 260 and the highest productivity of human lactoferrin being 115 mg/L, 7.67 times the amount of that in shake flask cultivation. CONCLUSION: The authors successfully realized high-density fermentation expression of human lactoferrin gene in recombinant Pichia pastoris.

[Effect of morphine on expression of gene of enzymes related to purine nucleotide metabolism in c6 glioma].

OBJECTIVE: To investigate the effect of morphine on catabolism and anabolism of purine nucleotide in c6 glioma cells. METHODS: C6 glioma cells were cultured and divided into 3 groups: 1) morphine group: morphine (10 micro g/ml culture) was added for 6 h, 12 h, 24 h, 48 h, and 72 h; 2) morphine + naloxone group: naloxone (1 micro mol/L) was added for 1 hour and then morphine (10 micro g/ml) was added for 24 hours; and 3) control group: normal saline was used for 6, 12, 24, 48, and 72 hours. The C6 glioma cells were centrifuged. RT-PCR was used to examine the gene transcripts of key enzymes of purine salvage way, hypoxanthine-guanine-phosphoribosyl transferase (HGPRT) and adenylate kinase (AK). RT-PCR-Southern blotting was used to examine the gene transcripts of key enzymes of purine salvage way, xanthine dehydrogenase (XD)/xanthine oxidase (XO) mRNA. RESULTS: Compared with that in the control group, the transcript of AK mRNA was significantly lower in the C6 cells treated with morphine for 24 hours, and began to re-increase 48 hours after morphine treatment. The transcript of AK mRNA remained at a low level after treatment of naloxone for 1 hour and treatment of morphine for 24 hours. The levels of transcript of HGPRT mRNA were lower in the morphine group than in the control group at all time points after treatment. However, the level of transcript of HGPRT mRNA 72 hours after treatment was higher in the morphine group than in the control group. The level of transcript of HGPRT mRNA 24 hours after exposure to morphine in the naloxon2 + morphine group was still lower than in the control group. The levels of transcripts of XD/XO mRNA were significantly higher after exposure to morphine in comparison with those in the control group at all time points after treatment. However, the levels of XD/XO mRNA 24 hours after exposure to morphine in the naloxone + morphine group recovered to the normal level. CONCLUSION: The downregulation effect of morphine on the gene expression of AK and HGPRT may not be mediated by mu receptor. The upregulation effect of morphine on the gene expression of XD/XO may be mediated by mu receptor. Naloxone reverses the effect of morphine on enhancement of XD/XO gene expression and cannot reverse the inhibitory effect of morphine on HGPRT and AK gene expression.

[Clinical study of plasma substitute (Gelofusion) on fluid resuscitation in patients with burned shock].

OBJECTIVE: To investigate the effects of plasma substitute(Gelofusion) on fluid resuscitation in patients with burned shock. METHODS: Twenty burn shock patients with total body surface area (TBSA) more than 40% were enrolled for clinical study on the effect of resuscitation with plasma substitute (Gelofusion). The patients were randomly divided into two groups: Gelofusion resuscitation group (n=11) and plasma resuscitation group (n=9). The cardiac output (CO),oxygen delivery (DO2), packed cell volume (PCV), blood and plasma viscosity, lactate(LA) and base deficit (BD) levels were detected at shock stage (postburn from 1 to 48 hours). RESULTS: Two hours after rapid fluid replacement, the levels of CO and DO2 were gradually increased, while the levels of PCV, blood and plasma viscosity, LA and BD were markedly decreased (P<0.05 or P<0.01). After resuscitation, plasma viscosity in Gelofusion resuscitation group were obviously lower than that in plasma resuscitation group within 24 hours postburn (all P<0.05).With an exception of plasma viscosity, there were no significant differences in other parameters between two groups at various intervals (all P>0.05). CONCLUSION: In the resuscitation of burn shock, the clinical effect of Gelofusion treatment is similar to that of plasma treatment. Gelofusion appears to be a fairly good plasma substitute for extensive application on the management of burn shock during the early stage.

Phosphorus limitation in biofiltration for drinking water treatment.

Bacterial growth potential (BGP) method and two parallel pilot-scale biofilters were used to investigate phosphorus limitation and its effect on the removal of organic matters in biofiltration for drinking water treatment. Addition of phosphorus can substantially increase the BGPs of the samples. Its effect was equivalent to that of addition of a mixture of various inorganic nutrients including phosphorus. The biofilter with phosphate added into its influent performed a higher biological stability of the effluent and a higher COD(Mn) removal than the control filter. These results suggested that phosphorus was the limiting nutrient in the biofiltration and the removal efficiency of organic matters could be improved by adding phosphate into the influent.

[Inhibition of respiratory syncytial virus replication in cultured cells by RNA-cleaving DNAzyme].

OBJECTIVE: DNAzyme/Deoxyribozyme is another novel molecular biological tool following the ribozyme. DNAzyme consists of a 15-nucleotide (nt) internal loop as its catalytic domain and two flanking substrate-recognition domains of 7 to 8 nt each which is complementary to substrate. The RNA substrate is cleaved at a particular phosphodiester located between an unpaired purine and a paired pyrimidine residue. DNAzyme has been applied in fields such as viral infectious disease, tumor, cardiovascular disease and genetic disease. But there is no report about using DNAzyme for anti-respiratory syncytial virus purpose. To observe the inhibitory effects of RNA-cleaving DNAzymes on respiratory syncytial virus (RSV) replication in cultured cells. METHODS: Anti-RSV RNA-cleaving DNAzyme DZ604 was designed to target the RSV genome at the start of the NS2 gene in an effort to inhibit the RNA replication. Microscope and electron microscope were used to observe the effects of anti-RSV genomic RNA DNAzyme on cytopathogenic effect (CPE) and ultrastructural change of 9HTE cell induced by RSV infection. Viral plaque forming reduction assay and MTT assay were used to detect the anti-RSV activity and protective function for RSV infected 9HTE cells of DNAzyme. RESULTS: Anti-RSV genomic RNA DNAzyme (DZ604) significantly improved CPE of RSV-infected 9HTE cells. The time to appearance of CPE and of total CPE was delayed by using DZ604 in a dose-dependent manner. At a 5 micro mol/L concentration of DZ604, CPE of 9HTE cells induced by RSV infection at 10 and 1 multiplicity of infection (MOI) was not improved. At smaller MOI (0.1, 0.01, 0.001, 0.0001) of RSV infection, CPE of 9HTE cells was significantly lightened by DZ604 at the same concentration. DZ604 also significantly improved ultrastructural change of 9HTE cells at early stage of RSV infection. Reduction in RSV yield was 85.56% and 8.33% at concentrations of 5 micro mol/L and 0.25 micro mol/L of DZ604. DZ604 inhibited RSV yield in a dose-dependent manner (P < 0.05). Non-specific DNAzyme did not have anti-RSV activity (P > 0.05). CONCLUSION: Anti-RSV genomic RNA DNAzyme designed and synthesized in our laboratory was capable of inhibiting respiratory syncytial virus replication specifically in cultured cells. Our data indicated that DNAzymes could be useful for the prevention against respiratory syncytial virus infection.
