[HBx gene facilitates the proliferation activity of hepatoma cells in vitro and in vivo].

OBJECTIVE: To investigate the effects of HBx gene on proliferation activity of hepatoma cells in vitro and in vivo. METHODS: The plasmid pHA-HBx carrying HBx gene was transfected into HepG(2) cells, and the positive clones were screened and identified with G418 and RT-PCR, respectively. The growth curve and population doubling time were calculated, and the cell cycle was analyzed by flow cytometry (FCM). The proliferation activity of transformed cells was measured with (3)H-TdR incorporation rate and nude mice model in vitro and in vivo. RESULTS: The result of RT-PCR indicated that HBx gene was integrated into the genome DNA of HepG(2) cells and transcripted. The growth curve and population doubling time showed a high proliferation activity of transformed cells. The amount of cells at stage S and G(2)/M were significantly higher, and cells at stage G(0)/G(1) were lower than those in control group. The tumors developed from transfected cells grew much quicker than those developed from HepG(2) cells in nude mice model. CONCLUSION: HBx gene can facilitate the proliferation of hepatoma cells both in vitro and in vivo.

[Effects of rosiglitazone and metformin on insulin resistance in high-fat diet rats].

OBJECTIVE: To observe the effects of rosiglitazone and metformin in rats on insulin resistance induced by high-fat diet. METHODS: Normal 8-week old male SD rats were divided into four groups. They were normal chow group (NC, n = 11), high-fat diet (HF, n = 11), metformin-treated (HF + Met, n = 11) and rosiglitazone-treated group (HF + Ros, n = 11). Rosiglitazone 3 mg x kg(-1) x d(-1) and metformin 300 mg x kg(-1) x d(-1) were given orally to HF + Ros and HF + Met group, respectively. After feeding for 8 weeks, serum insulin, adiponectin, glucose (BG), triglyceride (TG) and free fatty acid (FFA) were measured in all the rats. Insulin sensitivity was measured with glucose infusion rate (GIR) and determined by using euglycemic-hyperinsulinemic clamp method. RESULTS: High-fat diet induced obesity in SD rats after feeding for 8 weeks. High-fat diet decreased adiponectin level by 43.7% (P < 0.01) and GIR by 51.3% (P < 0.01) as compared with the NC group. Metformin decreased body weight by 8.4% (P < 0.01) and TG level by 40.5% (P < 0.01). Metformin significantly increased GIR by 58.9% (P < 0.01) when compared with the HF group. Rosiglitazone caused an apparent reduction of FFA (-25.3%, P < 0.05) and TG level (-54.0%, P < 0.01). At the same time, rosiglitazone increased adiponectin by 60% (P < 0.01), and improved insulin sensitivity by 149.6% (P < 0.01) as compared with the HF group. CONCLUSIONS: (1) High-fat diet induces insulin resistance in SD rats; this was associated with an increase in visceral fat and a decrease in the level of adiponectin; (2) Metformin treatment improved insulin sensitivity accompanied by a decrease in body weight and TG level; (3) Rosiglitazone treatment ameliorates IR in a greater extent and is accompanied by a reduction of FFA, TG and an increase of adiponectin levels.

Effects of raloxifene hydrochloride on bone mineral density, bone metabolism and serum lipids in Chinese postmenopausal women with osteoporosis: a multi-center, randomized, placebo-controlled clinical trial.

BACKGROUND: Raloxifene has been approved for prevention and treatment of postmenopausal osteoporosis in Caucasian women. It also has some positive effects on serum lipids in Caucasians. The objective of this study was to determine the effect of raloxifene hydrochloride on lumbar spine and total hip bone mineral density (BMD), bone metabolism, and serum lipids in Chinese postmenopausal women with osteoporosis. METHODS: This was a multi-center, randomized, double-blind, placebo-controlled clinical trial in which 204 postmenopausal Chinese women with osteoporosis were assigned to receive raloxifene (60 mg) or placebo treatment daily for 12 months. BMD, serum bone metabolism markers, and serum lipids were measured before and after drug administration. BMD was measured by Dual-Energy X-Ray Absorptiometry (DEXA) and bone metabolism markers were analyzed by one-step enzyme-linked immunosorbent assay. Serum lipids were measured by enzymatic analysis. RESULTS: At the end of the 12-month study, lumbar spine BMD increased in both groups with a mean increase of (3.3 +/- 4.8)% in the raloxifene group and (1.0 +/- 4.9)% in the placebo group (P < 0.001). There was a mean increase in total hip BMD of (1.4 +/- 4.8)% in the raloxifene group and a mean decrease of (0.9 +/- 5.0)% in the placebo group (P < 0.001). No subject in the raloxifene group had a new vertebral fracture and 5 placebo subjects had new fractures (P > 0.05). In the raloxifene group, the median decreases in the biochemical markers of bone metabolism serum osteocalcin and C-telopeptide were 41.7% and 61.5%, respectively. These changes were statistically significant compared with those in the placebo group (10.6% and 35.6%, P < 0.001, respectively). Both total cholesterol and low-density lipoprotein cholesterol decreased significantly in the raloxifene group compared with those in the placebo group (P < 0.001, respectively) and there was no significant effect of raloxifene on high-density lipoprotein cholesterol and triglycerides compared with placebo. CONCLUSIONS: Raloxifene 60 mg/d for 12 months significantly increases lumbar spine and total hip BMD, significantly decreases bone turnover, and has favourable effects on serum lipids in Chinese postmenopausal women with osteoporosis.

[Tumor necrosis factor related apoptosis-inducing ligand induces apoptosis and sensitizes resistant hepatic cancer cells to chemotherapy].

OBJECTIVE: To investigate the effective of tumor necrosis factor related apoptosis-inducing ligand (TRAIL) combination with chemotherapeutic agent inducing apoptosis in resistant hepatic cancer cells. METHODS: HepG2 cells resistant to Adriamycin (HepG2/ADR) were induced stepwise. The effects of TRAIL in combination with ADM (0.1 mg/L) on promoting apoptosis in HepG2/ADR were analyzed, the proliferation was observed by MTT assay, the apoptosis of cells was also observed by flow cytometry and TUNEL method. RESULTS: HepG2/ADR was confirmed resisting to ADM. Treated with TRAIL combination with ADM (0.1 mg/L), it showed significant inhibitory effect on the growth of HepG2/ADM, the percentage of apoptosis was increased as comparison with control at 24 h. CONCLUSION: MDR1 might not take part in resistance to TRAIL-induced apoptosis. TRAIL dramatically augmented the sensitivity to chemotherapeutic agents in HepG2/ADR. Combined TRAIL with chemotherapeutic agents treatment could be a novel and attractive strategy to drug-resistant/ TRAIL-resistant tumor cells.

[Expression and clinical significance of HIF-1a protein in hepatocellular carcinoma tissues].

OBJECTIVE: To investigate the expression and clinical significance of HIF-1a protein in hepatocellular carcinoma (HCC) tissues. METHODS: Immunohistochemistry (IHC), Western blotting and RT-PCR techniques were used to detect the expression of the HIF-1a gene protein in 35 HCC, 26 cirrhotic and 15 normal liver tissues. Their relationship with the pathological characteristics of the tumors were also analyzed. RESULTS: The positive rates of HIF-1a expression in HCC tissues was 94%, which was similar to the positive rates of HIF-1a expression in liver cirrhosis tissues of 92%, but was higher than that in normal hepatic tissues of 7%, but the residual proliferatic hepatic trabeculae among the necrotic liver cells and the fibrotic tissues expressed HIF-1a strongly in comparison with the cirrhotic liver tissues. The expression intensity of HIF-1a protein of the cirrhotic liver tissues was stronger than that in HCC; the results by Western blotting and RT-PCR were in accordance to that by IHC. In addition, the expression intensity in HCC had a negative correlation in differentiation degree and a positive correlation to intrahepatic and extrahepatic metastases but no correlation was found between HIF-1a expression and the existence of portal vein tumor emboli, prognosis and the status of HBsAg. CONCLUSION: HIF-1 protein was expressed in HCC and cirrhotic liver tissues, and was only affected by the factor of hypoxia. The expression of HIF-1a protein is associated with the differentiation of the tumor and its intrahepatic and extrahepatic metastases but was not related to the existence of portal vein tumor emboli, prognosis and the status of HBsAg. This phenomenon may provide a new idea for the treatment of liver cancer.

[Antitumor effects of soluble TRAIL in human hepatocellular carcinoma].

OBJECTIVE: To investigate therapeutic potential of soluble TRAIL (sTRAIL) in hepatocellular carcinoma (HCC). METHODS: Expression of TRAILR was determined by in situ hybridization in 60 samples of resected hepatocellular carcinoma, 20 samples of normal liver tissue near the margin of benign tumor and 2 HCC cell lines of HepG2 and SMMC-7721. The clinical data of the patients were analyzed as well as cellular effects of sTRAIL in promoting apoptosis on HCC cell lines HepG2 and SMMC-7721 (p53 gene mutated) after exposure to different concentrations of recombinant protein. RESULTS: High death receptor (DR) expression and low DcR expression in HCC tissue differed from low DR expression and high DcR expression in the normal hepatic tissue with statistical significance. DR5, DR4, and DcR2 but not DcR1 were expressed in both cell lines. The expression of DR was closely correlated with HCC differentiation, with the weak expression in poor differentiation. The positive rate of DR expression in 32 cases of grade III-IV was significantly lower than that in 28 cases of grade I-II (P < 0.05). Cell apoptosis rates were 10%, 70% and 50% of HCC cells, Jurkat cells and human cholangiocarcinoma cell line QBC939 24 h after recombinant of TRAIL alone. CONCLUSION: TRAILR expression is prevalent in HCC, with different receptor types existing. HCC is resistant to TRAIL-mediated apoptosis. The treatment of TRAIL alone only has a limited effect on inducing apoptosis on HCC cell lines of HepG2 and SMMC-7721.

[Massive hemorrhage in hepatectomy: causes and management].

OBJECTIVE: To analyse the causes and the management of massive hemorrhage in hepatectomy. METHODS: With over 1 000 ml of bleeding, 4 368 patients with hepatectomy between 1955 and 2000 were analysed retrospectively. RESULTS: Among 4 368 patients receiving hepatectomy, 286 (6.5%) had massive hemorrhage because of damage to the major hepatic veins, portal hypertension, hepatic insufficiency, and the extensive adhesion around the tumor. Massive hemorrhage was managed by repair and transfixation of the damaged vessels; transfixation or devascularization of variceal bleeding; complete vessels ligation of the hepatic section with mattress suture; resection of the ruptured tumor after temporary occlusion of the porta hepatis; fibrinogen infusion; hot saline compression of the surface of the wound and/or daub biological glue; argon beam coagulation and packs placement. CONCLUSIONS: Light performance and nonforce dragging of liver can reduce massive hemorrhage caused by major vessel injury or tumor rupture. Normothetic occlusion of porta hepatis can reduce blood loss effectively when liver resection. In situ hepatectomy must be adopted if there is extensive adhesion around the tumor. Packs placement is still an effective measure to stop bleeding caused by defective coagulation and extensive blood oozing of wound surface.

[Interleukin-12 enhanced tumor necrosis factor related apoptosis-inducing ligand TRAIL-induced apoptosis in human hepatocellular carcinoma by inhibiting expression of survivin].

OBJECTIVE: To investigate therapeutic potential of TRAIL in hepatocellular carcinoma (HCC) and the mechanism of sTRAIL resistance and to reverse the resistance to sTRAIL-inducing apoptosis. METHODS: The expression profiles of TRAILR were determined 60 HCC samples, in 20 normal liver tissues and 2 HCC cell lines HepG2 and SMMC-7721 by in situ hybridization. Cellular effects of sTRAIL in promoting apoptosis on HCC cell lines HepG2 and SMMC-7721 were analyzed after exposure to recombinant protein and after transfection with a cDNA expression construct. In vivo effects of sTRAIL on tumor growth were investigated using a nude mice HCC model of hepG2. Furthermore, the expression of survivin in HCC was detected, and treatment with antisence oligonucleotide was accepted. Finally, therapeutic effect on HCC by combining sTRAIL and interleukin-12 (IL-12) was detected. RESULTS: Both DR4 and DR5 were present in all HCC tissues as well as normal hepatic tissues. In contrast, 54 HCC tissues did not express DcR1 and 25 did not express DcR2. But both DcR were detectable in all of the normal liver tissues. The expression patterns of DR and DcR in HCC samples were quite different from those in normal tissue. DR5, DR4, and DcR2 expressed in both cell lines, while no DcR1 expression was detected. Recombinant sTRAIL alone was found to have a slight activity as it killed a maximum of 15% of HCC cells within 24 h while killing over 70% of Jurkat cells. In vivo administration of the TRAIL gene couldn't inhibit tumor growth in a nude mice HCC model. Mostly, HCC tissue and both HCC cell lines expressed survivin, whereas normal liver tissue did not express survivin. Treatment with antisence oligonucleotide enhanced sTRAIL-inducing apoptosis. IL-12 significantly augmented sTRAIL-inducing apoptosis and inhibited survivin expression. CONCLUSIONS: HCC cells are insensitive towards TRAIL-mediated apoptosis. Survivin may play a role in resistance to TRAIL-induced apoptosis in HCC, and antisence oligonucleotide could partly reverse the resistance to TRAIL-inducing apoptosis. IL-12 may sensitize HCC cells to TRAIL-induced apoptosis by preventing survivin. Combining gene therapy strategy such as combining gene therapy of TRAIL with IL-12 may be a promising maneuver to HCC.

Application of organic polymeric flocculants in centrifugal dewatering of oil refinery sludge.

In order to evaluate the applicability of the organic polymeric flocculants (OPF) in the treatment of oil refinery sludge, experiments were conducted to show that OPF have better performance of flocculation than inorganic flocculants. Both the anionic and cationic OPF have satisfactory flocculation efficiency in oil sludge treatment, but the latter are more cost-efficient. Among the over 20 types of flocculants tested, 2 OPF (CPAM-2 and HPAM-2) were selected as the treatment agents, based on their good treatment performances, oil-resistance and economic feasibility. It was demonstrated in the industrial-scale centrifugal dewatering experiments that the application of either CPAM-2 or HPAM-2 could achieve high treatment efficiency of the oil sludge dewatering and reduce the COD of centrifugal liquid to less than 1000 mg/L.

[Expression and implication of multidrug resistance associated -protein gene in primary hepatocellular carcinoma].

OBJECTIVES: To investigate the relationship between the expression of mrp and both the responses to chemotherapy and the level of alpha-fetoprotein (AFP). METHODS: S-P immunohistochemical staining and in situ PCR were adopted to detect MRP and mRNA mrp in 54 cancer tissues taken from untreated HCC patients whose tumor could not be removed during the operation, 24 para-cancer tissues, and 12 posthepatitis cirrhosis paraffin-embedded tissues. The relationship between the expression of mrp and their curative effect to chemotherapy in all the patients was analyzed, so was the relationship between the expression of mrp and the level of AFP in 38 patients whose AFP was positive after operation. RESULTS: The positive rates of expressing MRP and mRNA mrp in the three kinds of tissues were 61.1%, 25.0%, 33.3% and 77.8%, 37.5%, 41.7%, respectively, with higher rates in HCC tissues than those in other tissues (chi2=9.842, P< 0.01; chi2=13.956, P<0.01). The rates of curative effect to chemotherapy in groups of negative and positive MRP and mRNA mrp expression were 61.9%, 30.3% and 75.0%, 33.3%, respectively, with significant difference between the negative and positive groups (chi2=5.242, P<0.05; chi2=6.627, P< 0.05). As the same as the percentage of curative effect to chemotherapy, the rates of AFP level decreased evidently were 62.5%, 27.3% and 87.5%, 30.0%, with remarkable difference between the two groups (chi2=4.710, P<0.05; chi2=8.566, P<0.05). CONCLUSIONS: The multidrug resistance (MDR) of HCC is related to mrp expression, which initiates the intrinsic MDR. There is an important significance by detecting mrp expression in selecting chemotherapeutic method, and the expression of mrp can act as an indicator for chemotherapeutic sensitivity in HCC patients.

[Expression of human TRAIL gene in COS-7 cells and its effects in the treatment of hepatic carcinoma].

OBJECTIVE: To study the therapeutic potential of TRAIL and in combination with subtoxic level of chemotherapeutic agents in the treatment of human hepatocellular carcinomas (HCCs). METHODS: The plasmid pcDNA 3.0-hTRAIL was transfected into COS-7 cells, and it was transiently expressed. The cytotoxic functions of the expressed product and in combination with chemotherapeutic agents were detected. RESULTS: The transiently transfected COS-7 could express the active human TRAIL. It had mild cytotoxic functions on HCC cell lines. The cytotoxicity rates in both cell lines were 9.2% and 9.5% respectively. Chemotherapeutic agents, mitomycin (M), 5-FU and actinomycin D (AcD) dramatically augmented TRAIL induced cytotoxic function. There were significant differences in the cytotoxicity between the use of single agent and in combination with TRAIL (TRAIL + M, M; 60.1%, 32.6%, TRAIL + 5-FU, 5-FU: 68.1%, 29.2%, TRAIL + AcD, AcD; 72.9%, 58.6%) (P < 0.05). CONCLUSIONS: The combination of human TRAIL and chemotherapeutic agents, such as mitomycin, 5-FU and actinomycin D, seemed to exert a synergistic effect compared with either agent alone, and it might have therapeutic potential in the treatment of human HCC.

[To set up a mdrl multidrug resistant model of orthotopic transplantation of liver carcinoma in nude mice].

OBJECTIVE: To set up a mdr1 multidrug resistant model of orthotopic transplantation of liver carcinoma in nude mice by intermittent abdominal chemotherapy. METHODS: The hepatocellular carcinoma cell line HepG2 was first cultured, then subcutaneous carcinoma was produced to form the tumor donor mice. An orthotopic mdr1 hepatoma was produced by implanting the tumor fragment subserosally to the mice liver, and intermittent chemotherapy with Pharmorubicin given to induce drug resistance. Physical examination, ultrasonography, spiral CT and laparotomy were used to examine the growth of the tumor. RT-PCR and SP method by the monoclonal antibody JSB-1 were adoped to detect the expression of mdr1-mRNA and p-gp protein. RESULTS: There was no mortality. The successful rate of tumor implantation is 88% (22/25), the successful rate of supplementary implantation was 100% (3/3), and the successful rate of induction of drug resistance was 80% (16/20). The expressions of mdr1-mRNA and p-gp in the Pharmorubicin group were 23 folds and 13 folds of the control group, respectively. CONCLUSIONS: The nude mice mdr1 multidrug resistant model of orthotopic liver carcinoma were set up successfully, with its features similar to clinical cases. It would be helpful for the further study of the diagnosis and reversal strategy of the MDR phenomenon.

[Contour changing of forehead].

OBJECTIVE: To bring forward a method to improve the prominence of the forehead contour. METHODS: 16 patients(age 29-53 years, female 11, male 5) were treated through a bicoronal incision. Following the incision, the forehead soft tissues are reflected down to the level of the superior orbital rims with the protection to the supraorbital neurovascular bundles. To adjust the forehead contour, both superior orbital rims and frontal bossing was reduced by burring down the excess bone or removing the anterior wall of the forehead. RESULTS: The operative effect were satisfactory during 3-12 months of follow-up. No recurrence or osteogenesis were found in the cases. Nasofrontal angle and forehead angle were normal. The prominence of the supraorbital ridges disappeared and forehead became nature. CONCLUSION: Cranioplasty via an osteomy or burring of the outer line of forehead bone are safe, compact and effective way to improve the prominence of the superior orbital rims or frontal bossing.
