[Use of high-risk human papillomavirus DNA testing to deal with atypical squamous cells undetermined significance and low squamous intraepithelial lesion].

OBJECTIVE: To explore the role of high-risk human papillomavirus (HPV) DNA testing in improvement of recognition of cervical intraepithelial lesions (CIN) 2, 3 or cervical cancer confirmed by biopsy in women with abnormal cervical cytology including atypical squamous cells undetermined significance (ASC-US) and low squamous intraepithelial lesion (LSIL). METHODS: Total 940 patients with abnormal cervical liquid-based cytology including ASC-US and LSIL were included in this study. Hybrid capture II assay was applied in the high-risk HPV detection. The cervical pathologic diagnoses were obtained under colposcopy guided biopsy. RESULTS: Of the 940 cases, 497 were ASC-US, and 443 LSIL. With pathological diagnosis of cervical biopsy, 417 cases were chronic inflammation, 315 condyloma, 124 CIN 1, 65 CIN 2, 19 CIN 3 or cervical cancer. The positive rate of high-risk HPV DNA in groups of ASC-US and LSIL were 59.6% and 84.9%, respectively. In each group of abnormal cytology, detection rates of CIN 2, 3 or cervical cancer in patients with positive HPV DNA were 8.8% and 14.1%, respectively, which were higher than those with negative HPV DNA (0.2% and 6.0%, P < 0.05). In group of ASC-US, high-risk HPV DNA test had a negative predictive value of 99.5%; detection rates of CIN 2, 3 or cervical cancer in patients with HPV DNA virus load 1-9.99 and > or = 10 was 1.6% and 10.7%, respectively. CONCLUSION: High-risk HPV DNA test were helpful to improve the detection rates of CIN 2, 3 or cervical cancer in patients with ASC-US.

[Monocyte chemoattractant protein-1 and its correlation with lipoprotein in polycystic ovary syndrome].

OBJECTIVE: To measure serum monocyte chemoattractant protein-1 (MCP-1) levels and study its associations with lipoproteins in patients with polycystic ovary syndrome (PCOS). METHODS: Sixty-five PCOS women and 20 ovulating normal women with body mass index (BMI) < 25 kg/m2 as controls were recruited. PCOS women were divided to two groups: 27 BMI >or = 25 kg/m2 patients as obese group; 38 BMI < 25 kg/m2 as non-obese group. Serum MCP-1 was assayed by enzyme-linked immunosorbent assays (ELISA). Serum prolactin (PRL), follicle stimulating hormone (FSH), luteinizing (LH), estradiol (E2) and testosterone (T) were assayed by chemoluminescence method. Serum androstenedione (A) was assayed by radioimmunity method in patients. And triglycerides (TG), total cholesterol (TC), apoprotein A (ApoA), apoprotein B (ApoB) , lipoprotein (a) [LP(a)], high density lipoprotein (HDL) and low density lipoprotein (LDL) cholesterol (HDL-C and LDL-C) were measured. RESULTS: MCP-1 (P = 0.001) and ApoB (P = 0.018) levels were found to be significantly increased in PCOS groups compared with that of controls, but the ratio of ApoA/ ApoB was significantly decreased in groups PCOS (P = 0.015). PCOS obese group had markedly higher MCP-1 serum levels than non-obese group (P = 0.012), and MCP-1 serum levels in PCOS non-obese group higher than controls (P = 0.03). Univariate analysis revealed that serum MCP-1 levels were significantly and positively correlated with BMI (r = 0.350, P = 0.001), LH(r = 0.262, P = 0.016), TG (r = 0.480, P = 0.000) and ApoB (r = 0.289, P =0.008); but significantly and negatively correlated with the ratio of ApoA/ ApoB (r = -0.282, P = 0.009). Partial correlation showed that serum MCP-1 levels were correlation with LH (r = 0.2577, P = 0.020) and TG (r = 0.4611,P = 0.000). Multiple regression analysis showed that MCP-1 levels was influenced by BMI and TG. Furthermore, TG showed more effect on MCP-1 levels. CONCLUSION: PCOS obese and non-obese patients had higher serum MCP-1 levels than controls. MCP-1 was correlated with BMI, LH ,TG, ApoB and the ratio of ApoA/ ApoB. BMI and TG were two major determining factors of MCP-1 in patients with PCOS. Furthermore,TG had more effect on MCP-1 levels. Based on the above findings, we presume that MCP-1 is likely to participate in the pathophysiology and long-term complication of PCOS.

[Genetic advances in hydatidiform mole].

Hydatidiform mole is the most common gestational trophoblastic disease and is characterized by gross trophoblastic hyperplasia resulting from abnormal fertilization of the oocyte. It is of great interest to researchers because of its propensity for local invasion and spreading. Using cytogenetic and molecular genetic techniques researchers have performed various studies on hydatidiform mole. There are advances in the understanding of karyotype, DNA ploidy, fertilization types, expression of imprinted genes, the differences between genetic classification and pathologic classification, and the correlation to its invasion and spreading.

[Value of serum procalcitonin in diagnosing bacterial lower respiratory tract infections in people with exacerbation of Chronic Obstructive Pulmonary Disease].

OBJECTIVE: To investigate the changes and clinical implications of serum procalcitonin in exacerbation of chronic obstructive pulmonary disease (COPD). METHODS: We have evaluated PCT measurement in 45 patients with an exacerbation of COPD (group A) and 25 patients with stable COPD (group B), quantitative sputum culture was performed, too. PPMs were only regarded as significant if they reached a growth of > or =10(7) cfu/mL, indicating the presence of bacterial infection. RESULTS: In patients with an exacerbation, 15 patients, sputum yielded a high (> or =10(7) cfu/mL) bacterial load (group A1), 30 patients, sputum yielded a low (<10(7) cfu/mL) bacterial load or a negative bacterial culture (group A2). The levels of procalcitonin in sera from patients of group A1 were significantly higher than those from group A2 and group B [0.24 (0.17, 0.28) microg/L vs. 0.125 (0.10, 0.18) microg/L vs. 0.12 (0.10, 0.145) microg/L, P= 0.000, 0.000]. The levels of procalcitonin in sera from patients of group A2 were similar to those from group B (P>0.05). Using a cut-off point of 0.155 microg/L for PCT, the sensitivities and specificities for bacterial infection in patients with an exacerbation of COPD were 93.3% and 60% respectively. CONCLUSION: Serum procalcitonin measurements in patients of an exacerbation of chronic obstructive pulmonary disease play a role in the diagnosis of bacterial infection.

Prophylaxis against carcinogenesis in three kinds of unestablished tumor models via IL12-gene-engineered MSCs.

Mesenchymal stem cells (MSCs) were adenovirally engineered to secrete interleukin-12 (AdIL-12-MSCs) and evaluated for their anticarcinogenesis efficacy against three kinds of unestablished tumor models including B16 melanoma, LLC Lewis lung cancer and HCC hepatoma. Injection of AdIL-12-MSCs into protected mice before tumor inoculation prevented all of 12 mice in B16 preventive groups, 10 out of 12 in LLC lung cancer model and 11 out of 12 mice in HCC hepatoma model from developing tumors, whereas the control groups pre-receiving PBS were validated for 100% carcinogenesis; the tumor formation rates in free-AdIL-12 and vacant MSC groups were unveiled between approximately 83 and 100% even with plentiful angiogenesis and newborn lymphatic vessels, as well as distant metastases. As a novel approach, AdIL-12-MSC has revealed expected preventive effects on carcinogenesis (P<0.01) with low-toxic, broad-spectrum and long-range superiorities. In conclusion, our data indicate that AdIL-12-MSC possess the potential for tropism to preclinical tumor lesions and deprives surviving or hibernating tumor cells, which have escaped from conventional treatments, of revival and recurrence.

[The changes and clinical implications of serum procalcitonin in acute exacerbations of chronic obstructive pulmonary disease].

OBJECTIVE: To investigate the changes and clinical implications of serum procalcitonin (PCT) in acute exacerbations of chronic obstructive pulmonary disease (COPD). METHODS: A total of 45 patients with an acute exacerbation of COPD were studied. On presentation, serum PCT concentrations were measured, and quantitative sputum culture was also performed. The patients were reevaluated when they had returned to their stable clinical state. Potentially pathogenic microorganism (PPM) was only regarded as significant if they reached a growth of >or= 10(7) CFU/ml, indicating the presence of bacterial exacerbation of COPD. RESULTS: (1) On presentation, sputum samples from 21 (46.7%) patients yielded PPM. When reevaluated in stable clinical state, sputum samples from 9 (20%) patients had a positive PPM culture [2.8 x 10(6) (1.3 x 10(6), 1.9 x 10(7)) CFU/ml], but with a significantly lower bacterial load than on presentation [7.0 x 10(7) (4.5 x 10(7), 7.1 x 10(8)) CFU/ml, Z = -2.666, P = 0.008]. (2) The patients were classified into two groups: group A included patients with bacterial exacerbation of COPD (n = 15), group B included patients with nonbacterial exacerbation of COPD (n = 30). The levels of PCT for patients of group A [0.24 (0.17, 0.28) microg/L] were significantly higher than group B [0.13 (0.10, 0.18) microg/L, Z = -3.531, P = 0.000]. When they had returned to their stable state, the levels of PCT for patients of group A decreased to 0.12 (0.10, 0.14) microg/L, which was significantly lower than in exacerbation [0.24 (0.17, 0.28) microg/L, Z = -3.298, P = 0.001]; But compared with exacerbation [0.13 (0.10, 0.18) microg/L], the levels of PCT for patients of group B did not changed [0.13 (0.10, 0.15) microg/L, Z = -1.614, P = 0.107]. In the stable state, there were no differences in the PCT measurement between the two groups (Z = -0.382, P = 0.703). CONCLUSION: In patients presented with an acute exacerbation of COPD, the elevation of serum PCT is associated with bacterial infection.

[Genotype distribution of extended-spectrum beta-lactamases and AmpC beta-lactamases produced in Escherichia coli isolated from men with urinary infection].

OBJECTIVE: To study the genotype distribution of extended-spectrum p-lactamases (ESBLs) and AmpC p-lactamases produced in E. coli isolated from men with urinary infection in Nanjing. METHODS: Organisms of clinical infection were identified by automatic microbial system (Vitek-32). ESBLs were detected by disk diffusion confirmatory test, and ESBLs and AmpC p-lactamases by three-dimensional extract test (TDET) , the presence of plasmid-mediated ESBLs and ampC genes determined by PCR, and conjugal transfer assays of the ampC resistance determinants carried out by a broth mating procedure. RESULTS: ESBLs were produced in 24. 6% (46/187) of the E. coli and the 46 E. coli isolates showed p-lactamase activity in TDET, 3 positive for both ESBLs and AmpC beta-lactamases and 43 for ESBLs only. Forty-four of the 46 isolates were shown by PCR to contain at least one of the genes blaTEM, blaOXA, bla(CTX-M), but no blaSHA. AmpC specific amplication products were observed in 3 of the 46 isolates, of which 2 were of CIT type, and 1 of DHA type. All of the 3 transconjugants transferred the plasmids harbouring ampC genes to recipients. CONCLUSION: CTX-M is the most common genotype in plasmid-mediated ESBLs produced by E. coli isolated from men with urinary infection in Nanjing. Present findings indicate that AmpC-producing E. coli are present in this hospital, and ampC-encoding plasmids are transferable.

Human T lymphocyte responses against lung cancer induced by recombinant truncated mouse EGFR.

The induction of active cellular responses against EGFR should be a promising approach for the treatment of those receptor-positive tumors. However, the immunity against EGFR is presumably difficult to elicit by vaccine based on self or syngeneic EGFR due to the immune tolerance acquired during the development in immune system. We proposed a model to break immune tolerance against self-EGFR through an altered immunogen source based on xenogeneic homologous EGFR. We have previously shown human EGFR as a xenoantigen could induce specific immune responses in mouse and cross-react with mouse EGFR, and resulted in therapeutic benefits for EGFR-positive mouse tumor. Here, we show a recombinant form of extracellular domain of mouse EGFR, in the presence of DCs, could activate human peripheral T cells to proliferate, secret IFN-gamma, the induced responses could cross-react with human EGFR and kill autologous EGFR-positive lung cancer cells which could be blocked by anti-CD8 and anti-MHC class I antibody. There is no detectable cytotoxical activity against lung tissue, liver tissue and kidney tissue derived from paracancerous normal tissue. These observations suggest that antitumor immunity induced by the truncated mouse EGFR may be provoked in a cross-reaction between mouse EGFR and self-EGFR, and may provide insight into treatment of EGFR-positive tumors through induction of the autoimmune responses against EGFR.

[Study of gene differential display of hydatidiform moles].

OBJECTIVE: To screen the specific molecular maker of invasive hydatidiform moles (HM) by differential display analysis. METHODS: For dot hybridization, about 1.0 microg of each cDNA sample of invasive and non-invasive HM were labeled as probes using the Dig DNA labeling and Detection Kit (Boehringer Mannheim). The specific expression fragments of invasive HM recovered from PAGE gel were re-amplified by PCR, and the PCR products were dotted onto nylon membrane and hybridized by two probes of invasive and non-invasive HM cDNA respectively. Some fragments with a strong positive hybridization signal were cloned into the polylinker of lasmid PUC19 and were sequenced. The fragments' sequences were searched for homology in the NCBI data using the BLAST (Database: GenBank Human EST entries; Posted date:Aug 31, 2004; Number of letters in database: 1,697,659,032; Number of sequences in database: 3,677,722). RESULTS: The 20 fragments in 28 bands with specific expression in invasive HM were re-amplified, of which 13 showed positive hybridization signals, and 3 were cloned into the polylinker of lasmid PUC19. A fragment in the 3 was a new expressed sequence tag (EST) and the sequence was submitted to NCBI data (dbEST_Id: 10875704; GenBank_Accn: BM403211). CONCLUSION: There are more differences in gene expression between invasive and non-invasive HMs, and differential display analyses are of a potential significance to early diagnosis of invasive HM.

[Association between insulin resistance and cholesteryl ester transfer protein gene polymorphism in type 2 diabetes mellitus].

OBJECTIVE: To study the association between cholesteryl ester transfer protein (CETP) gene polymorphism and insulin resistance in type 2 diabetes. METHODS: CETP-TaqIB gene was genotyped with polymerase chain reaction-restriction enzyme fragment polymorphism analysis in 108 patients with type 2 diabetes and in 60 normal controls. Plasma lipids, fasting insulin, insulin sensitivity index and insulin resistance index were determined in 108 patients with type 2 diabetes. RESULTS: The distribution of CETP-TaqIB genotypes and B1B2 allele frequency in the patients with type 2 diabetes were similar to that in general population. High density lipoprotein cholesterol (HDL-C), apolipoprotein A1(apoA1) and insulin sensitivity index (ISI) levels were significantly higher in B2B2 genotype than in B1B1 genotype. Fasting insulin (FINS) and Homeostasis model assessment-insulin resistance (HOMA-IR) levels were significantly lower in B2B2 genotype than in B1B1 genotype. No significant differences in triglyceride (TG), total cholesterol(TC),low density lipoprotein cholesterol (LDL-C), apolipoprotein B (apoB) levels were observed among different CETP-TaqIB genotype groups. By multivariate analysis, the determinants of ISI and HOMA-IR were body mass index (BMI), TC, HDL-C and CETP-TaqIB genotype. CONCLUSION: The CETP-TaqIB genotype is independently associated with insulin resistance and lipid metabolism. It may be an important risk factor of insulin resistance in type 2 diabetes.

[Clinical evaluation on surgical implant anchorage in orthodontic and prosthodontic therapy].

PURPOSE: To evaluate the clinical effect of implant anchorage in the correction of malocclusion and restoration of dentition defects. METHODS: 14 cases who had Angle II malocclusion as well as dentition defects and whose molars could not serve as anchorage or who had no molars at all were selected. Implants in the maxilla or mandible were served first as anchorage to correct the malocclusion and then to restore the dentition defects. RESULTS: The implant anchorage in the palate didn't move medially after the anterior teeth had been adducted by 6-8 mm. 3 cases of zygo-alveolar ridge implantation failed. 5 cases of adult Angle II division I, II malocclusions as well as Kennedy II dentition defects were orthodontically treated for 8-10 months, the lower anterior teeth were adducted by 6-8mm, aligned and the midline was corrected with no medial movement of the implant anchorage. Then the implants were used to restore the missing teeth. CONCLUSION: It is feasible and effective to use implant as anchorage in orthodontic and prosthodontic treatment in cases without available anchorage.

[The relationship between malignant transformation and fertilization types of hydatidiform moles].

OBJECTIVE: To study the relationship between malignant transformation and fertilization types of hydatidiform moles (HM). METHODS: Fifty four HM specimens were analyzed by using multiplex STR-PCR (9 loci) to determine the fertilization types and all patients were followed-up for the human chorionic gonadotropin (hCG) over 1 year. RESULTS: Total malignant transformation cases were 10 in all the 54 HM. Ggenetics complete hydatidiform moles (g-CHM) with DNA from only paternal origin, were observed in 38 cases including 28 homozygote and 10 heterozygote cases. In homozygote and heterozygote cases, malignant transformation occurred in 8 cases of the empty eggs fertilized by single sperms and 2 by double sperms respectively. In all the 54 HMs, 16 cases of DNA from parents were genetic partial hydatidiform moles (g-PHM), and no malignant transformation occurred in each haploidy egg fertilized by double sperms. CONCLUSION: (1)Genetic complete hydatidiform moles (g-CHM) showed a higher malignant transformation risk than genetic partial hydatidiform moles (g-PHM) (P=0.024 2); (2)There was no significant difference in malignant transformation between homozygote and heterozygote of g-CHM (P=0.699).

[The differential expression profile of polycystic ovary syndrome associated genes].

OBJECTIVE: To explore the gene differential expression pattern of polycystic ovary syndrome. METHODS: We carried out microarray analysis to define the gene networks by the PCOS granulosa cells in order to identify differentially expressed genes in PCOS patients. These granulosa cells of five PCOS cases and five control cases which were derived during oocyte retrieval from women undergoing IVF. RESULTS: As compared with control human ovarian granulosa cells, 46 genes were screened out, 25 genes were up-regulated, and 21 genes were down-regulated in PCOS. These differentially expressed genes were involved in various biologic functions, such as regulation of fatty acid metabolism, cell-cell signal transduction, immune and inflammatory response, reflecting the complexity of clinical manifestations of PCOS. CONCLUSION: Microarray analysis technology is an effective method to identify novel PCOS associated candidate genes.
