[Clinical study on different operation procedure for hysterectomy].

OBJECTIVE: To investigate the effects and outcomes of four procedures for hysterectomy. METHOD: Retrospectively, we analyzed the clinical characteristics of 756 cases who respectively received hysterectomy by total abdominohysterectomy (TAH, 260 cases), modified abdominohysterectomy (MAH, 180 cases), transvaginal hysterectomy (TVH, 106 cases), or laparoscopic assisted vaginal hysterectomy (LAVH) (210 cases) and compared their effects and outcomes. RESULTS: The average operation time of TAH (98 +/- 23) minutes, MAH (67 +/- 18) minutes, TVH (63 +/- 19) minutes and LAVH (99 +/- 35) minutes. The average operation time of LAVH and TAH groups was significantly longer than that of TVH and MAH groups (P < 0.05). The volume of hemorrhage during operation in TAH group (180 +/- 49) ml was much more than that of other three groups significantly [MAH (102 +/- 43) ml, TVH (93 +/- 31) ml, LAVH (111 +/- 39) ml]. The average time of antibiotic administration and bowel function recovery in TAH group [(5.2 +/- 2.6) days, (36 +/- 9) hours] was significantly longer than that of TVH [(3.2 +/- 1.6) days, (21 +/- 4) hours], LAVH [(3.5 +/- 1.9) days, (23 +/- 6) hours] and MAH [(3.3 +/- 1.7) days, (23 +/- 7) hours] (P < 0.05). The incidence of fever in TAH group was significantly higher than that of other three groups also. The average hospital stay after operation in TVH [(3.3 +/- 1.2) days] and LAVH [(3.6 +/- 1.1) days] groups was significantly shorter than that of MAH [(5.6 +/- 1.9) days] or TAH [(5.4 +/- 2.3) days] groups (P < 0.05). CONCLUSIONS: Different procedures for total hysterectomy have their own advantages and disadvantages. Microinvasive surgery by laparoscopy for hysterectomy will be mainstream in the future.

[Clinical study on combined multi-disciplinary laparoscopic surgery].

OBJECTIVE: To investigate the clinic characteristics and efficacy of combined multi-disciplinary laparoscopic surgery. METHODS: Combined laparoscopic surgery was performed in 46 gynecological patients who concomitantly had gastrointestinal and urinary tract diseases as study group, and single gynecologic laparoscopic surgery in 40 patients as control group. The complications and mean time of operations, bowel function restoration, and hospitalization length were observed. RESULTS: The surgery of every patient was successful. None of the cases was transferred to open laparotomy. No complications occurred. The mean time of operations was significantly increased in study group (127 +/- 91) min compared with control group (100 +/- 80) min (P < 0.05). The recovery time of bowel function, as well as the mean time of hospitalization was not different between study group (20 +/- 10) h, (3.7 +/- 1.5) d and control group (19 +/- 10) h, (3.5 +/- 1.3) d (both P > 0.05). CONCLUSIONS: Combined multi-disciplinary laparoscopic surgery can be performed safely and effectively for gynecological patients complicated with gastrointestinal or urinary tract diseases.

[Influence of HGV super-infected with HIV or HCV on the virus replication].

OBJECTIVE: To realize human immunodeficiency virus(HIV) and hepatitis C virus(HCV) super-infected with hepatitis G virus(HGV or GBV/C) and to probe into the mechanism of these virus infection in the body. METHODS: HIV and HCV load were tested by the quantitated RT-PCR in the HIV or HCV infected plasma samples respectively and the HGV RNA was detected in all of the samples. Then some of the HGV positive were sequenced. RESULTS: 123 of 317 HIV patients were positive for HGV, the positive rate was 38.8%. Among the 91 HCV patients, 19 were positive for HGV. The positive rate is 20.9% which was less than that of HIV patients. HIV load of the patients super-infected with HGV was less than that of those without HGV[(1.8+/-0.6)x10 copies/ml compared with (1.9+/-1.1)x10(2)copies/ml]; while HGV and HCV super-infection did not influence the HCV RNA load significantly [(1.5+/-0.6)x10(4) copies/ml compared with (5.4+/-1.8)x10(4)copies/ml]. The HGV sequences from HIV or HCV patients were compared and showed no difference markedly. CONCLUSION: The rate of the HIV and HGV super-infection is higher than that of HCV. HGV may inhibit HIV reproduction in the body while superinfection.

[A study on myocardial Pax-8 gene].

OBJECTIVE: Conventional deletion of ALK3, also termed as bone morphogenetic protein (BMP) receptor IA, in mice might result in early embryonic lethality. To investigate the function of ALK3 in cardiac development, the cardiac-specific deletion of ALK3 in mice was made by Dr. Schneider, using Cre recombinase driven by the alpha-MHC promoter that Dr. Fukushipe worked out. Such specific deletion of ALK3 caused death in mid-gestation with defects in the trabeculae, interventricular septum, and endocardial cushion. Since ALK3 is not a cardiac-specific gene, it is extremely important to identify ALK3 downstream genes. METHODS: Alpha-MHC Cre+/-, ALK3 F/- and alpha-MHC Cre+/-, ALK3 F/+ embryos were obtained after 20 alpha-MHC Cre+/-, ALK3 +/- mice and 20 ALK3 F/F mice were mating. The ALK3 downstream genes were screened using microarray made in Germany that could identify 25000 genes in mouse. Two populations of mRNA, one derived from the embryonic heart (11.5 days) of alpha-MHC Cre+/-, ALK3 F/- mice, and the other derived from the alpha-MHC Cre+/-, ALK3 F/+ mice, were compared. Cardiac-specific ALK3 downstream genes were identified using real time quantitative RT-PCR and in situ hybridization. RESULTS: The expression of 12 genes, such as Pax-8 and Hox-3.5 were down-regulated in alpha-MHC Cre+/-, ALK3 F/- mouse heart. The expression of 16 genes including Ras-related protein Rab-5b and EPS-8 protein was up-regulated in the group of alpha-MHC Cre+/-, ALK3 F/-. It was found that the Box protein Pax-8 gene was down-regulated by 7.1 fold (P < 0.001) in the alpha-MHC Cre+/-, ALK3 F/- mice by real time quantitative RT-PCR. It was also revealed that the Box protein Pax-8 gene was expressed stronger in alpha-MHC Cre+/-, ALK3 F/+ than alpha-MHC Cre+/-, ALK3 F/- E11.5 days mouse heart by means of in situ hybridization. CONCLUSION: The Box protein Pax-8 gene is an important and cardiac-specific ALK3 downstream gene in the BMP signaling pathway during inter-ventricular septum development.

[Transforming growth factor-beta 1 expression in epithelial ovarian neoplasms and its clinical significance].

OBJECTIVE: To detect the expression of transforming growth factor-beta 1(TGF-beta 1) in epithelial neoplasms and its clinicopathologic features and prognosis. METHODS: TGF-beta 1 expression was detected in 77 cases of epithelial ovarian neoplasms by SABC immunohistochemistry (including 21 benign tissues, 7 borderline tissues, and 49 epithelial ovarian cancer tissues), and they were compared with 14 normal ovarian tissues. RESULTS: 1. The positive rate of TGF-beta 1 expression was 71.42% in the normal ovary, 76.19% in benign tissues, 4/7 in borderline tissues and 38.78% in epithelial ovarian cancer tissues. 2. TGF-beta 1 expression was associated with the grade, the FIGO stage, and the histologic type of the patients with epithelial ovarian cancer (P < 0.05). CONCLUSION: The expression of TGF-beta 1 may play an important role in the progression of epithelial ovarian cancer and may be related to the prognosis of epithelial ovarian cancers.
