[The treatment of middle and lower thirds fractures of humerus by anterior plate fixation].

OBJECTIVE: To study the effect of anterior plate fixation on the treatment of middle and lower thirds fractures of humerus and the possibility of operating without injuring the radial nerve. METHODS: Forty-nine patients with the middle and lower thirds fractures of the humerus were treated with anterior plate fixation from March 1998 to December 2002. Of the 49 patients, 27 were with new fractures, 12 with old fractures, and 10 with nonunion fractures. According to AO classification, of the 49 patients, there were 19 type A, 14 type B1, 9 type B2, 5 type B3, 2 type C1. Thirty-seven patients had closed fractures and 12 had open fractures. Of the 12 patients, 8 were type Gustilo I , 4 were type Gustilo I . Four out of the 49 patients were associated with radial nerve palsy. All patients were treated with anterior plate fixation through the anterior approach to the humerus. The radial nerves injured were explored. RESULTS: Of the 49 patients, 48 were followed up 6 to 48 months (28.7 months on average). All fractures were healed within 3 to 9 months(4.7 months on average). Fixed plates of 37 patients out of the 49 were removed. No iatrogenic radial nerve injury occurred in the 82 times of the operations among the 49 patients. CONCLUSION: The treatment of middle and lower thirds fractures of humerus by anterior plate fixation through the anterior approach to the humerus does not interfere with the fracture healing and can prevent the iatrogenic radial nerve injury.

Correlation of gastric liquid emptying with various thresholds of sensation in healthy controls and patients with functional dyspepsia.

Our objective was to investigate the correlation between gastric emptying and sensation in patients with functional dyspepsia (FD). Thirty patients with FD and 15 controls were studied. Proximal gastric compliance and sensation thresholds were measured with a barostat, and gastric emptying was assessed for 70 min after the ingestion of 500 ml of water using real-time ultrasonography. Results were as follows. (1) Patients showed a significantly longer half-time of gastric emptying than controls (30.03 +/- 8.20 vs. 23.07 +/- 4.67 min; P < 0.001). The sensation thresholds were significantly lower in patients than in controls based not only on pressure but on volume. However, there was no difference in gastric compliance between patients and controls. (2) The half-time of gastric emptying was correlated with various thresholds of sensation based on the volume (perception, r = -0.52, P < 0.005; discomfort, r = -0.54, P < 0.005; pain, r = -0.58, P < 0.005; maximal tolerance, r = -0.42, P < 0.005). But there was no significant correlation between gastric emptying and sensation thresholds based on pressure. Delayed gastric emptying and/or gastric hypersensitivity are present in more than 50% of FD patients. The coexistence of these two abnormalities is more common than the single impairment. There is a correlation between gastric emptying and visceral sensation to gastric distention in patients with FD.

Analysis on the causes for refractory GERD.

To analyze the causes of failure in conventional treatment to refractory gastroesophageal reflux diseases (GERD) patients, 16 refractory GERD patients (group R) and 16 cases of GERD primarily diagnosed (group P) were studied. Endoscopy, pathologic examination and 14C urea breath test were conducted in every patient. 24 h ambulatory pH and bilirubin monitoring were performed with Digitrapper MK III and Synetics Bilitec 2000. It was found that esophagitis in group R was more severe than in group P. The rate of Helicobacter pylori infection in group R was significantly lower than in group P. Fraction time pH below 4.00 was not longer while the bile reflux represented by fraction time abs above 0.14 was greater for patients in the group R as compared with those in the group P. The mixed refluxes and pure bile refluxes between the two groups had significant difference. The reflux episodes in the group R mainly occurred during nights. These results indicated that severe esophagitis, especially Barrett's esophagus with complications makes it difficult to control GERD. Severe duodenogastroesophageal refluxes (DGER) are often accompanied by refractory GERD. Mixed refluxes aggravate the esophageal injuries. Pure bile refluxes and nocturnal refluxes may cause failure of administration of proton pump inhibitors (PPI) in the morning. Helicobacter pylori infection and acid refluxes may not be the direct cause of refractoriness. Individual refractory GERD patient without abnormal results on pH or bile reflux recently should be diagnosed again.
