[Evaluation of the public on the psychosocial effects of fluorosis].

OBJECTIVE: To explore the attitude of the public to the psychosocial effects of fluorosis through investigating the public in different areas. METHODS: Twenty typical fluorosis photos were selected and evaluated by the public in Xingtai, Beijing and Guangzhou, which included 101 dentists, 118 dental students, 103 college students in non-dental schools, 102 clerks and 92 leaders. Each evaluator was required to answer seven five-point questions according to each photo. RESULTS: The mean of the public evaluation was going up with the increase of Dean's Index (DI) score, ranging from 2.21 to 4.74, meanwhile, the prevalence of reporting that fluorosis would affect their psychosocial status also ascended. Less than 25% people reported that very mild fluorosis (DI < or = 1) would affect the psychosocial status of patients, and mild fluorosis (DI = 2) would affect 25% - 56%, while the prevalence would increase to 48% - 97% in severe fluorosis group (DI > or = 3). "Bivariate Correlation Analysis" showed that significant correlation existed in the evaluation of the public and the degree of fluorosis. The scores of dentists were significantly lower than those of dental students, college students in non-dental schools and clerks. The evaluation scores of the public in Xingtai were generally higher than those in Beijing and Guangzhou. CONCLUSIONS: Mild or less fluorosis could lead to little psychosocial effects.

[Buffering effects of plaque solid, Streptococcus mutans and glucans].

OBJECTIVE: To investigate the buffer capacity of insoluble plaque mass including bacteria and glucans matrix. METHODS: Plaque samples (starved or incubated 1 h with 10% sucrose), Streptococcus mutans (S. mutans) mass, and insoluble/soluble glucans were titrated with 1 mmol/L HCl. Meanwhile, the density of bacteria was counted and related to buffer capacity. RESULTS: The bacteria density had significant effects on acid buffer capacity in dental plaque, but the effect reduced with consuming sugar. S. mutans mass with no sucrose incubating contributed to the buffer capacity of (0.609 +/- 0.202) mmol/L, and S. mutans mass with 2% sucrose had only the buffer capacity of (0.099 +/- 0.047) mmol/L. In the mean time, glucans had no effect on acid buffering (0.028 - 0.032 mmol/L). CONCLUSIONS: The bacteria including S. mutans may be the main contributor to acid buffering in plaque solid.

[A short-term clinical study of one-visit endodontic treatment for infected root canals].

OBJECTIVE: To clinically evaluate the postoperative pain level and short-term healing of one-visit endodontic treatment for infected root canals. METHODS: 100 infected cases were included in one-visit root canal therapy group and 42 infected cases in two-visit group using calcium hydroxide as an interappointment medicament. A comparison of the clinical results was done between the two groups. The main assessments included postoperative pain evaluated by patients and by clinical examination, and the six months' healing response. RESULTS: There was no statistically significance between the groups regarding pain 7 days after operation (one visit 60.0%, two visit 57.1%, P > 0.05) and six months' healing response (one visit 67.9%, two visit 61.5%, P > 0.05). The pain in the one-visit group was quickly relaxed postoperatively, as the pain in the two-visit group was given a rise because of the second treatment. CONCLUSIONS: One-visit endodontic treatment for infected root canals has the same clinical efficacy as two-visit in regarding to postoperative pain level and short-term healing.

[Analysis of volatile fatty acids in gingival crevicular fluid of patients with chronic periodontitis].

OBJECTIVE: To investigate the volatile fatty acids in gingival crevicular fluid (GCF) and to analyze the relationship between the levels of the volatile fatty acids and chronic periodontitis. METHODS: GCF samples taken from 37 patients with chronic periodontitis and 16 volunteers with healthy periodontal status were analyzed by capillary electrophoresis. RESULTS: The detection frequencies and concentrations of succinic acid, butyric acid and valeric acid were significantly higher in GCF of chronic periodontitis than in that of healthy group. The detection frequencies of propionic acid had no statistic difference between the two groups, but the concentrations of it was significantly higher in inflammation group. We also found that the concentrations of succinic acid, propionic acid and butyric acid were significantly lower in shallow pockets than that in deep pockets. CONCLUSIONS: The volatile fatty acids, especially succinic acid, propionic acid, butyric acid and valeric acid were associated significantly with the severity and inflammation of periodontal disease. The levels of succinic acid, propionic acid and butyric acid in GCF were related to pocket depth.

[Evaluation of the psychosocial status of fluorosis patients in fluorosis epidemic areas].

OBJECTIVE: To explore the psychosocial effect of fluorosis on patients in fluorosis epidemic areas. METHODS: Multistage, stratified, clustered and random sampling method was used in this survey. Totally 416 inhabitants of 12, 15 and 35-44 years of age from rural fluorosis epidemic areas in Hebei province were selected. Dental fluorosis was classified by Dean's index (DI). A total of 178 patients with different types of fluorosis were involved in a psychosocial questionnaire investigation which included six five-point subscales, including attitude to teeth, subjective well-being, life satisfaction, interpersonal or social anxiety, effect of self-esteem, impact on behavior. RESULTS: Fluorosis prevalence in fluorosis epidemic areas of Xingtai in Hebei province was 71.20%, and significant difference existed among three age groups (Pearson Chi-square value = 167.51, P < 0.01). With the increase of DI score, the average value of subscale "attitude to teeth" rising, ranging from 2.58 to 3.51, and value of "subjective well-being" was also increasing, ranging from 2.35 to 2.9. Statistical difference of subscale average value was found only in these two subscales ("attitude to teeth": F = 4.787, P < 0.01; "subjective well-being": F = 2.538, P < 0.05). "Attitude to teeth" was significantly different between control group and moderate group (F = 0.52, P < 0.01) and between control group and severe group (F = 0.72, P < 0.01). While significant difference of "subjective well-being" could only be found between control group and severe group (F = 0.56, P < 0.01). According to the bivariate correlation analysis, strong correlation could be found between the degree of fluorosis and the average value of sub-scales. CONCLUSIONS: The psychosocial impact of fluorosis on patients in fluorosis epidemic areas mainly appeared in two aspects, i.e. attitude to teeth and the perception of well-being, especially in severe group.

[Pilot study of dental erosion and associated factors in university student volunteers].

OBJECTIVE: To investigate dental erosion and associated etiologic factors among students from one university. METHODS: A total of 179 student volunteers from one university were examined clinically for their dental erosion. Each was required to complete a questionnaire regarding potentially associated etiologic factors. A modified criteria was used to grade the severity of dental erosion. RESULTS: The prevalence of dental erosion of this group was 45.8%, mostly in enamel. Facial surfaces of anterior teeth were most vulnerable to erosion. Statistic analysis indicated that acidic diets, such as carbonated drink, fruit juice, fruit drink and banana were risk factors to dental erosion. CONCLUSIONS: The findings strongly suggest that much attention should be given to dental erosion which was probably associated with acidic diet.

[Predominant aciduric microflora related to fissure caries of the first permanent molars].

OBJECTIVE: To investigate the aciduric bacteria in different stages of the fissure caries in order to determine potential roles of the microflora in the development of dental caries. METHODS: Plaque samples were taken from 10 incipient carious fissures of the first permanent molars. Plaque samples were also collected from sound fissures of the first permanent molars in 10 other subjects who kept caries free over the past 2 years and from the above 10 subjects. The predominant bacteria were isolated by using both the most probable method in media at pH7.0 and 5.2, and the conventional plating methods. Streptococcus spp. was identified by means of API 20 Strep commercial kit. RESULTS: S. mutans was the predominant aciduric bacteria infissures of caries-free children and more frequently recovered at neutral pH. Whilst, S. sanguis was predominant at pH5.2 in sound and carious of fissures of caries-active children. The proportion of Gram-positive rods at pH5.2 was 82%, significantly greater than that of 61% at pH7.0. CONCLUSION: The ecology of plaque is complex and the composition of microflora at each stage of caries could be different. The composition of bacteria was consistent with the changes of local pH in the plaque.

[Enamelin transcriptional expression in developing postnatal rat tooth germ].

OBJECTIVE: To observe the transcriptional expression of enamelin in developing postnatal rat first mandibular molar germs, for further studies of functions of enamelin in enamel development and mineralization. METHODS: Tissue slices of first mandibular molar germ of rat 1, 3, 7, 10, 14 days after birth were prepared. The enamelin mRNA expression was identified by in situ hybridization. RESULTS: Enamelin mRNA was observed in both ameloblast and odontoblast in 1-10 day old rat postnatal first mandibular molar germs. Enamelin mRNA appeared very weakly at 1st day, and increased through 3rd day, reached the maximum at 7th day, and reduced at 10th day and became negative at 14th day postnatally; while the expression of enamelin mRNA in odontoblast maintained lower from 1st to 10th day and negative at 14th day postnatally. CONCLUSION: Enamelin gene transcriptional expression lasts from preameloblast to maturation ameloblast, which suggests that enamelin may participate in the development of enamel and mantle dentin.

[Inhibition of remineralization by EDTA-soluble phosphate protein in dentin].

OBJECTIVE: To investigate the effect of removing EDTA-soluble phosphate protein in dentin on the later remineralization for the purpose of better understanding of mechanism of dentin phosphate proteins on dentin mineralization. METHODS: To remove soluble phosphate protein by EDTA dissolution, then the remineralization rate was monitored by a constant composition crystal growth technique. The results were compared with those from the normal dentin and the dentin partially demineralized by acetic acid. RESULTS: Faster remineralization rates were found with dentin demineralized by EDTA (0.5 and 2 h) compared with normal dentin powder, while a slower rate was found with dentin demineralized by acetic acid. The increase of remineralization rate by removing phosphate protein from dentin was 100% more at 200 min after the start of the reaction. CONCLUSION: EDTA-soluble phosphate protein in dentin has a great potential to inhibit remineralization.
